Employee was carrying a bucket of stone down the pit ramp and slipped and landed on their elbow, which resulted a bump on their elbow. The bucket of stone was to prevent slipping in other locations.
An EE and IE were working together to remove pins from a quick coupler. One of the pins was stuck and not moving. When it finally released, the pin shifted suddenly and pinched IE's right hand/knuckle against the frame of the quick coupler, causing an injury.
ee had just finished breaking a slab into blocks. ee was putting a Retractable back in the storage box it hit the top of the box causing it to come down and punch their finger.
Employee was attempting to pull a hydraulic splitter up from a block. It was stuck in the drill hole; they pulled with more force to free it and felt a pulling sensation in their abdominal area.
Employee was helping put a saw bar back on the saw as EE was adjusting the bolts to fit back in the bolt hole, a team member moved the bar by hand resulting in the bar shifting positions and pinching the employees hand in a pinch point. The employees palm and thumb were caught between the saw and bar briefly.

Employee was helping move power cords and pulled a muscle in left arm.
EE was sliding a small and thin piece of grout (around 10 lbs) on the conveyor in order to grab it. While EE was doing so, it flipped while falling between the two sections of conveyor and the sharp edge cut EE's wrist (above glove line).
Employee was pulling a piece of curb (with an end piece attached, coming out of the straight joint saw). While they were pulling, the end piece broke and fell right on the top of their right foot. The end piece was weighing around 90 lbs.
Employee was changing rods on EE's Epiroc D-30 drill. EE felt a pop in EE's hand. EE told me that EE's hand and arm was tingly. Employee continued activity. Employee was sent to the doctor on 12/12 to be evaluated. Employee was diagnosed with wrist strain and is on modified duty wearing wrist brace as needed.
Employee was moving stone on the saw when EE felt a pain in EE's right side, this is during the week of 11/18/2024. The pain got worse as time went on and EE decided to be seen on 11/22/2024. EE was diagnosed with musculoskeletal strain/sprain.
Employee was handling curb with it tipped over and EE's finger was caught between. EE was wearing gloves, while pulling hand out EE's finger got scratched. Friction of the glove and pressure under stone is what caused the scratch on finger. EE wanted to go to the doctor. Doctor administered tetanus shot and antibiotics. First aid was administered at site.
On Friday 9/6/2024, EE was working on the joint saw, which involves going up and down a couple of steps quite often. By mid-day, EE reported to co-worker that their knee was starting to hurt because of the steps. Co-worker moved EE to another position. On 9/11/2024, EE told another co-worker that knee pain was still there and asked to go get it checked out at the clinic.
Operator was picking up a water bag when EE felt a sharp pain in EE's right shoulder.
Employee was clearing grout and received a puncture wound on left 3rd digit finger.
Injury was declared on 8/6/24 @ 7:15 AM. EE was cleaning the radius splitter pit & while dragging some small granite shavings out of the trays, a splinter made its way through the glove into middle finger. EE didn't think it was anything serious & didn't declare it right away, but realized a few days later that it might be infected & asked to go get it checked out.

On 5/29/2024 Lifting deadman link on JL6600S manlift and felt a bee sting in EE's arm, Charlie horse feeling in right arm and bicep. On 5/30/2024 the area where EE felt discomfort turned black and blue.
Employee was placing a water bag in a cut, the bag fell from the cut hitting the employee's right hand causing a small laceration.

THE EE CAUCHT THE SHELL WITH EE'S RIGHT HAND TO KEEP IT FROM FALLING AND HIT EE'S WRIST ON THE SHARP EDGE CAUSING A SMALL LACERATION. EE WAS RELEASED BACK TO WORK FULL DUTY SAME DAY

Cleaning the floor of the saw shed with the bobcat (small tractor). EE wanted to put a piece of grout - by hand - in the bucket of the bobcat and hurt back by trying to lift/drag this piece.
