Fall to the walkway or working surface

Employee was carrying a bucket of stone down the pit ramp and slipped and landed on their elbow, which resulted a bump on their elbow. The bucket of stone was to prevent slipping in other locations.

while sawing the saw kicked back and inflicted lacerations to employee's face, EE received stitches. The kick back safety function worked and shut the saw off preventing further injury.

Miner was moving a water line for the saw, and stepped on wet track to move and slipped causing a bruise to EE's left arm.

Knife slipped, lacerating knee while cutting wire

Miner was taking hose off of exhausted hydro bag, when second water bag fell and rolled out of cut, causing injury to top of left hand. Cut and tendon damage, and some broken bones.

Employee was opening a skid of plaster. He was using a knife, when he missed the plastic wrap over the plaster, and ran the knife into his lower right from of leg, resulting in stitches.

Knife laceration to bottom of left index finger, required sutures. Was cutting wire. Had safety gloves on. Returned to work at 2:00pm same day

Struck against stationary object

Laceration to the left hand 5th digit (pinky finger). Employee was plastering around granite block for placement on wire saw. Employee's pinky finger hit edge of granite rock, and received a cut. First aid was performed at office, and employee then went to clinic to see if he needed stitches. Stitches were placed, and employee returned to work.

Struck by flying object

Breaking granite with shims and wedges, when a piece of wedge broke off, and embedded in left forearm. Had to go to Urgent Care clinic to remove object, and get stitches. Could not lift 20 lbs for 5 working days. Had sutures removed after 7 days, and is now not limited.

Over-exertion in lifting objects

Employee was helping lift an electrical cable over pins on a wire saw, and felt a pull in his groin. He finished his on Friday May 3rd, came in Monday May 6th, and decided his groin hurt more, so we sent him for medical attention. It was determined he had a groin strain.

Employee was wearing safety glasses when a piece of rust/dirt fell under the top of the glasses and went under his top eyelid. After attempting to clear it out with eyewash solution, was unable to. We sent him to Urgent Care Outpatient Facility. They numbed his eye, and removed the foreign material without incident. Employee returned to work, with no restrictions.

Abrasion to right thumb, while drilling, from 2 inch diameter rock piece flew from wall 100 feet away.

EE told his Quarry Manager that he felt he had strained his back the previous Friday, June 11th, while pulling 2inch electrical cable. The quarry manager immediately set up an appointment to go to a care center to be evaluated at that time. He had a no loss of work injury, according to documents from the Care Plex, occupational Health Facility, in PA.
