Struck against stationary object

An EE and IE were working together to remove pins from a quick coupler. One of the pins was stuck and not moving. When it finally released, the pin shifted suddenly and pinched IE's right hand/knuckle against the frame of the quick coupler, causing an injury.

Fall onto or against objects

employee slipped on rocks and scraped their left arm and right hand. Initially it was a first aid, however few days later it got worse, and employee was diagnosed with cellulitis

Employee was changing rods on EE's Epiroc D-30 drill. EE felt a pop in EE's hand. EE told me that EE's hand and arm was tingly. Employee continued activity. Employee was sent to the doctor on 12/12 to be evaluated. Employee was diagnosed with wrist strain and is on modified duty wearing wrist brace as needed.

Drilling a vertical line with a plug Drill and the bit broke and caught EE's finger between the broken bit and stone EE was drilling

Breaking a slab and the slab shifted suddenly when it broke and EE fell between the slabs and it rolled back onto EE causing a crushing injury to pelvis.

Hydro bag fell and EE tried to catch it and it pinned EE's finger between the bag and the railing on the man lift

Mounting a steel collar to a pole, collar slipped and caught his index finger on his right hand.

While placing a drive shaft into the bucket of a loader his gloved hand got caught on it causing his finger to become pinched when he put it down.

Worker slipped on some ice when getting out of his vehicle and landed on his shoulder. Got up and brushed off and went to work. No accident report was filled out and no lost time for the injury. Throughout the year his shoulder was giving him trouble and was traced back to the fall he had in December. He held off Surgery until December 28 2015. No lost time until now.

Employee trying to move Slot Drill over w/a bar. The Slot Drill dropped a few inches and the bar came down on his foot.

EE WAS WALKING IN THE QUARRY AND STEPPED ON A STONE. THIS STONE ""ROLLED"" A BIT AND HE LOST HIS BALANCE AND TWISTED HIS RIGHT KNEE.

Employee was using the steam cleaner to melt ice from the drill holes. The pipe became stuck and employee tried to free it without shutting it off.

EMPLOYEE TRIED TO OPEN THE DOOR TO THE BUILDING AND IT WAS FROZEN - HE KEPT PULLING ON THE HANDLE.

Ee had one foot on the block that he was ""breaking"". When it broke, ee fell and hit his side on the block.

EE WAS CLIMBING ON THE COMPRESSOR TO HITCH A CHAIN ON THE TOP HOOK IN ORDER TO MOVE IT - AND SLIPPED AND FELL.

EE WAS BREAKING LINES IN THE QUARRY WITH A JACKHAMMER AND WEDGES. A PIECE OF STEEL BROKE OFF OF THE WEGE AND LODGED IN HIS LEFT KNEE.
