Mud boots. *Wore boots with no socks, rubbed skin off feet in places.

While throwing chain under van 5-442, EE injured his left shoulder.

While cleaning frozen chunks of mud from twin screen, a piece of frozen mud fell onto employee's right leg.

EE left index finger was pinched by a sliding leg jack.

Employee was under loader greasing. When the employee got up, his hard hat fell off and he hit his head on the lifting arms. Employee was transported by his supervisor to the hospital for stitches.

Employee's finger got between the legs and the conveyor. Employee received stitches and was prescribed medication.

Employee was lifting spokane and hurt lower back.

Employee got left thumb under cone, causing laceration to left thumb. Received stitches.

Laceration to left index finger, EE rec. 5 stitches.

EE lifted the door handle and pulled back, it did not come open so he applied more force, the door swung open and the handle hit him in the face cutting from his nostril to lip.

Employee was pulling on a tie wire while cleaning debris around screen on crusher, felt a slight twinge in back then was ok. He then woke up with severe back pain the next morning. Went to the doctor and medication was prescribed.

Setting down a piece of iron, left index finger got between iron and rock which resulted in a laceration to left index finger, needing stitches.

Employee was re-hooking chain to grizzley bar, bars tipped and pinned his against the loader bucket. Right wrist fractured. Reporting due to medication given to employee.

EE was using water pump hose and it twisted so water was not reaching the tank. EE released the camlocks to adjust hose and scalding hot water escaped from the hose and gave him 2nd degree burns on his inner thigh.

Changing screens on the plant, while handling screens twisted right knee, causing knee to pop out.

The crew was working on chain feeder removing side panels. Employee was removing panel & slipped, landed on right foot above steel toed boot.

Lifting roller into conveyor, pulled muscle in lower back.

Was changing blow bars and when Injury started cleaning rotors and ran back side of right hand against rotar, cutting pinky.

Shut door on loader and thumb got caught in door. Stitches were needed.

EE was helping change the hammers and anvils in the spokane. When he picked one up, he tore a bicep muscle in his right arm.

Performing repair to machine, his hand slipped causing lacerations.

Employee was checking drive belt tension on jaw screen 13-367 (Plant was shut down). Employee pulled down on drive belt and pinched right middle and ring finger. Plant was locked out. **Lost tips of middle and ring fingers**

Removing hose from creel & the latch slipped, hose hit right arm & right side of face.

Changing screens on jaw plant, fingers were in the way when it got moved.
