Employee was using a manual post pounder to install a grounding rod. Pounder came off rod and went in between pounder and employee's hand. Causing laceration to employee's right hand. employee received 6 stitches.

Injured employee was supervising/assisting the loading of a scoop onto a scoop dolly. As the battery end of the scoop was being lowered onto the dolly, it pushed the dolly out from under the scoop and the dolly rolled back on the track rails and struck the employee's right ankle & foot, knocking employee down. Employee has been diagnosed with a strain/sprain, contusions, swelling.
Employee was entering a trailer porter-john when the trailer tipped towards them. Employee fell backwards onto loose rock, landing on knees and wrists. Employee sustained left knee contusion and right wrist strain.
Employee was using a T-post driver to drive 3/4" electric metal tubing (EMT) into the ground. As EE was raising the driver, EE went too far up and the tubing came out of the driver cylinder. On the downward motion, EE missed the EMT striking EE's right index finger on the EMT while holding the driver handle. This caused an open wound and broken bone in the tip of EE's finger.

Crusher Operator was moving a wheelbarrow full of material and as it was tipping over they tried to catch it and strained a shoulder and bicep. The EE is still able to perform normal task and has not missed work. MRI results came back in November showing torn rotator cuff and EE will need surgery.
The Aggregate Foreman was attempting to attach a trailer carrying a compressor to EE's F-150 work truck. While trying to set the trailer hitch onto the hitch ball of the truck, the trailer hitch came off the ball and moved forward. The employee got EE's finger caught between the hitch and the bumper of the truck, causing a laceration to EE's left index finger.
Miner Injured moving a 55-gallon oil drum on dolly. Drum was not set properly on dolly and miner adjusted drum. While adjusting drum right pointer finger was pinched between drum and dolly resulting in a hairline fracture and laceration. Gloves were worn.
On April 5, 2024 at approximately 2:15PM the tank 7 agitator shaft was being moved into position to be installed when one of the wheels on the dollies used to move the shaft caught a weld point on the floor, spinning the dolly, causing the shaft to roll off of the cart and onto the top of the employee's right foot. Resulting in a fractured foot.
An employee was moving a FK pump on a dolly with assistance from coworkers. After completing the work, the employee had pain in their back. On 4/5/2024, the employee sought medical treatment for continued pain and was diagnosed with a back strain. They were placed on restricted duties.

Employee was moving a cart down a ramp when the items on the cart slid and lacerated left hand.
Employee was attempting to move a wheelbarrow, when they felt a pull in their back. EE received medical attention at a panel physician the day of the incident and was cleared to return to work with no restrictions but on follow up apt on 7/31/23, EE was put out of work until 8/2/23 when EE had their next follow up apt and was allowed to return to work but with restrictions.
While the employee was positioning the wheelbarrow for a co-worker to start loading material, their finger got caught between the structure of 2140CON001 and the handle of the wheelbarrow. Resulted in a fracture to the tip of ring finger.
On 5/11/23, operator was using a dolly to move ten-50lb bags of material. Operator pulled dolly back, lost their footing and caused the momentum of the material to push the operator down and they fell on their left knee. The operator was treated with first aid for knee abrasion. Operator did not want to see a doctor when injury occurred. Operator requested to see a doctor 5/19/23.
