EE was using a wheelbarrow to move spilled sand and felt a pop in their arm.

After greasing, employee turned to walk down the cat walk & received a minor cut on the finger. The transfer belt is a fully enclosed structure and employee says they cut it on the outside of the housing. They went to hospital & received 2 stitches, was discharged and returned to finish the shift

EE was performing various tasks, welding and grinding. EE was not sure of the exact time or source but eye became irritated during lunch break and assumed happened prior.

EE was getting out of loader when he slipped and fell to the ground, fall around 5'8"".

EMPLOYEE WAS CLOSING A SLIDE GATE WITH A WRENCH, THE WRENCH SLIPPED AND HIS ARM HIT CORNER OF GATE FRAME WHICH CUT HIS ARM.
