EE strained his right knee dismounting a front-end loader. He did not report his injury immediately. Two days later he requested to go to the doctor. He did not improve as expected and was put on restricted duty.

While changing screens, one screen was too tight, tried to push a little harder to get screen in place and lost balance and fell forward. EE tried to catch himself and hurt left wrist.

EE was installing repaired lift cylinder on a 750 John Deere trackhoe he positioned a ladder against the boom of the excavator, as the cylinder was lifted up and close enough for him to reach, the ladder started to slide along the boom of the excavator. He then lost his balance, fell & struck the rear bumper of the service truck resulting in pain to left shoulder & neck.
Slipped after his foot got caught on step, causing a bruise on his left shin.
Employee was checking on a broken travel chain on dragline when he slipped and fell in the engine compartment, hurting his right knee (contusion). He went to doctor and was given light duty until Jan. 22, 2005 at which time he may resume regular duty.

EE WAS AT THE SCALE HOUSE. A TRUCK WAS COMING UPTO THE WINDOW TOO CLOSE TO HIM SO HE MOVED TO GET OUT OF THE WAY. HE TRIPPED & FELL ON METAL BARRAILING ON CHEST & CHIN.

EE WAS REPAIRING DRAGLINE. DUG THREE FOOT HOLE, WHILE CLIMBING INTO HOLE, HE SLIPPED AND TWISTED LEFT KNEE.
EE WAS GETTING OFF OF HIS BELLY DUMP TO CHECK HIS TIRES WHEN HE SLIPPED AND FELL. TRYING TO BRACE HIMSELF WITH HIS LEFT HAND AND HURTING IT. EE WENT TO DR AND FOUND OUT HE BROKE HIS HAND. CAST WAS PUT ON HIS HAND. DR SAID FOR HIM TO STAY HOME UNTIL 11-10-00 AND RETURN TO WORK ON MONDAY 11-13-00.
EE SLIPPED AND HIT HIS LEFT SHIN AND CUT IT WHILE WASHING SAND OUT OF SCREW. EE WENT TO HOSPITAL AND HAD SEVEN STITCHES TO CLOSE THE CUT AND WAS SENT HOME.
CARRYING SCREEN UP STAIRS TO 8 BY 20 PRIMARY SCREEN, TRIPPED ON STEP, STRAINING RIGHT KNEE.
EMPLOYEE WAS CHANGING A ELECTRIC MOTOR OUT ON CRANE ST8 WHEN HE SLIPPED OFF BUMPER AND HIT RIGHTSIDE OF HEAD.
