Struck against stationary object

Screen plant 'middle hopper'. Employee using tooling to remove material blockage, as the material stopped flowing. As the material dislodged, momentum forced employee's forearm, through tooling', into frame of hopper, resulting in laceration.

The EE was desending a ladder on the side of the cooler above the cooler outlet as another EE was removing material from the cooler outlet below in a skid steer. as the injured EE was on the ladder the EE in the skid steer pushed hot material into water creating steam that rose to the level of the ladder causing burns to the injured employees arms.

Employee was inspecting kiln for potential repairs to be made, while walking in area employee was paying more attention to footing, not noticing overhead obstruction. Employee struck 'material lifter' with hard hat, knocking it off and shoving safety glasses into facial/forehead region and breaking the skin with glasses.

While cleaning a buildup of material in a receiving chute, EE smashed left little finger between the hand tool and chute.

DURING PRE-START UP INSPECTION OF SECO BOTTOM SHAKER SCREEN, EMPLOYEE WAS ON DECK CONDUCTING INSPECTION. THE EMPLOYEE LOST FOOTING DUE TO DAMPNESS OF SCREENS, RESULTING IN A FALL, WHICH INJURED THE EMPLOYEES RIGHT KNEE.

The employee was doing inventory on material piles. He lost his footing on the loose material and twisted his knee. He suffered severe pain and was transported to the local hospital.

Employee was poking dust out of a hole in a dust hopper and material/debris fell into his eyes. There was no ill effect until the next day. Employee went to the doctor and had debris removed.

Employee left a hand written note saying he hurt his knee climbing on stiffleg crane bucket to grease it. The employee stated the next day that it was an existing old injury and that he must of aggravated it. He was in the process of having the knee looked at with his doctors before the work incident.

Our customers contract hauler slipped and fell while climbing on his trailer to remove tarp bows. He fell onto a bracket on the side of the trailer cutting his leg at the thigh area. The cut was about 3in. long X 0.75in. deep. 911 was called and the victim was taken to emergency room. The victim was an employee for Go-Ko trucking federal I.D. #35-1824252.

Employee was helping install a 1200 foot boom cable. We needed about another 6 feet and we decided to pull it by hand and I must have strained my back or pulled a muscle.

Employee was trying to pry a clinker out of #5 kiln cooler when he felt a pull in his lower abdominal area. He didn't say anything about it. Employee went to his personal doctor on Tuesday 7/6/10 but said nothing to any supervision until 6:30pm on 7/9/10.

Employee was welding on coal plant structure and when he started to chip off the slag, a piece went under his welding helmet and under his safety glasses and into his left eye.

Screenplant crew was changing the closing cable on stiffleg. Employee was standing on the stiffleg bucket guiding the cable through the sheave when it flipped out and hit him in the face.

Employee was checking the heater in the #2 Euclid haul truck to see if it was blowing out heat. The employee's foot slipped and to catch himself he grabbed the heater unit with his hand and his finger went into the rotating blower fan and cut his right ring finger.

Fall from piled material

Employee was disconnecting frozen water lines from stockpile. When he was coming back down the face of the stockpile the employee slipped and slid approxmately 20 feet to the bottom. He called the office with his cell phone to tell that he couldn""t get up he had hurt his back.

EE was running the dozer and stopped to go to the restroom. When he returned there were some rocks laying next to the dozer and EE tried to move them and strained his back.

Employee was going to the pond pumps to restart after power outage. He drove the Volvo loader out to where the pumps were located and when he got off of the loader he got a sharp pain in his groin area. When he returned to the bathhouse he notified the plant manager of what happened and the manager sent a co-worker to take him to the emergency room.

Subject was welding on the stiffleg bucket when a piece of slag hit him in the eye while he was wire brushing it off. Plant supervisor was notified by telephone at approxmately 8:30A.M. on Saturday 5/31/08.

Employee was changing a screen on the Seco shaker. The catwalk on which he was standing collapsed falling through causing injury to his right hand and arm, head, back and ribs.

Subject was doing service on back pier of #4 kiln. When he came down stairs after servicing back pier he slipped down stairs hurting back.

on aug 13 06 around 10:50pm i was out getting a sample of material to check weights, as i got sample i turned to head back in on the burner floor. when i started to walk back in something caught around my foot and caused me to trip. when i fell i tried to catch myself and hurt my wrist.
