LONGVIEW OPERATION Metal/Non-Metal

Grinding on sealing of the kiln, the grinder kicked back, hitting the bottom of EE's welding helmet, knocking it up, allowing the grinder wheel to make contact with EE's face. EE was taken to the hospital where EE received stitches to EE's upper and lower lips. The grinder was equipped with a guard.

Employee was operating a haul truck in the quarry when a bracket broke under the front of the seat. This caused the seat to move up and back causing a sprain to the lower back. The employee did have an existing back issue.

Employee went to catch a slamming door at the breakroom while coming in to clock for EE's shift, when EE's index finger was caught between the door and door jaw from a gust of wind. Employee suffered a fractured finger.

Employee was working in the Kiln 3 screen room, cleaning under a screw conveyor using the vacuum truck when some lime went under EE's safety glasses. EE flushed eyes then and reported the incident the next day. EE was seen by a doctor, eyes washed, given prescription drops, and released to work.

Employee was trying to put a portable dump hopper back on track when EE felt something pop in EE's right arm. Possible torn bicep muscle that may require surgery. Employee is on restricted duty at this time.

Employee stepped back into the path of forklift that was backing up. Forklift knocked EE down to the ground and the forklift tire rolled up onto foot and ankle. Employee was on restricted duty for a few days then written out of work awaiting MRI results.

Employee was walking backwards dragging a return roller when EE stepped into mud causing EE's foot to get stuck and fall backwards. This resulted in a sprained ankle. The doctor wrote EE out of work.

Employee slipped on ice as they opened the door to enter the mill 2 area. Employee fractured an ankle.

EE was inside a kiln pre-heater removing rock, using a chute. While throwing rock into the chute, the employee's hand hit the roof of the pre-heater. The EE's 2nd digit of the right hand (index finger) was lacerated. EE was wearing leather gloves. EE was taken to the local emergency room where EE received five (sutures) and returned to full duty.

Employee was repairing a hole in the screen room tin. When the drill bit came through the tin and into employee's ring finger. Initially this was not reportable, but issues arose with the finger causing employee to have surgery to repair a tendon.

Injured reached between the frame and conveyor belt to strike a troughing roller and arm was pulled in between the pulley and conveyor belt. EE received stitches in middle finger, index finger and wrist. Is currently off work.

Employee was descending rear ladder on a rail car when EE slipped on the next to last rung (approx. 5-6') landing on EE's back. It had been raining and the area was muddy. The employee was wearing a fall protection harness and tie off to a retractable lanyard.

Employee was raking clinkers off the kiln #3 grate when ee felt something pop in left shoulder. The pain comes and goes. Employee reported the injury on 04/28/2017 and was taken to the doctor on that date.

Raking kiln, stumbled stepping down a step, landed on wrists.

Employee suffered a laceration to right thumb while replacing buckets on a hydrated lime elevator. They pinched their finger between the bucket and chain as they tightened the nut with a pneumatic driver.

Employee was walking along the rail tracks in the lime load out area and slipped on wet pavement. Employee didn't fall completely to the ground but sprained leg.

Employee was putting together a rail on a DM45 drill. EE was hitting a pin with a hammer and a piece of metal chipped off and hit left hand. It punctured the palm of hand.

Employee slipped on the wet walkway hitting left knee on the walkway as EE was attempting to close the lid on the kiln 3 reclaim hopper.

Employee was cutting zip tie when knife slipped. EE suffered a minor cut on left index finger.

Employee was throwing a rail switch and slipped. EE then fell, face first, into the handle and suffered a laceration requiring two stitches.

Contract truck driver (Evergreen) tripped on bottom step descending from loadout.

NOTE: This is a contract driver and not a carmeuse employee. Employee was stepping down from his truck and placed his left foot on what appeared to him to be small rocks or gravel. States that his foot sunk through it into a "hole" and he began to fall. The driver injured his knee. (he had surgery on this knee several months ago.

Employee suffered heat exhaustion while cleaning tank 17 screen room.

Customer bulk truck EE was preparing to climb on tank trailer to open the lids for loading. He was at the tie-off rack at Carmeuse and had hooked to the self retracting lanyard. As soon as he placed his second foot and full weight onto the step, (This step is 20-1/2 inches from the ground.) it broke and he fell to the ground, landing on his buttocks.

While rolling on v-belts for the cooler fan, employee caught his left index finger in between the motor sheave and the belt pinching the end of his finger. The injury was to his left pointer finger.

EE dropped a long handled scraper into hydrated lime. The hydrated lime splashed into his eye. He was not wearing his safety glasses properly, they were being worn on the tip of his nose.

While loading shells into the kiln gun a piece of one of the slugs ricochet back out of the kiln after it had been shot and struck EE on his right forearm. It scraped a small section of skin off and the area swelled up. EE was taken to doctor for evaluation. No fractures or stitches.

Employee mashed his finger while swinging a sledge hammer. It appears when he was hitting the side of the slurry bin his hand slid down the handle of the hammer. He continued to swing and his left index finger got mashed in between the handle and a section of plate that wraps around the bin. Employee received 5 stitches.

Maintenance EE's were striking the end of a screw conveyor with an 8lb sledge hammer and a piece of the shaft flew off and struck the injured employee in the forearm causing a laceration and embedding of metal.

Employee was raking clinkers from kiln #3 and mashed his finger between the kiln rake and the handrail. He suffered a laceration and broken fingernail to his right, middle finger and received 5 stitches.

While using a large pipe wrench to turn the shaft on a quarry pump, employee pulled muscle in neck/shoulder area. The wrench was above his head while pushing, which contributed to the injury. Employee requested medical aid on August 9th.

EE was swapping a material flow gate using a come-along and had to apply a lot of pressure. when the gate broke free, the EE's arm came back towards his body. Injury became reportable when EE was taken to the doctor on 9/30/13 and X-Ray revealed chip on elbow.

Employee was tightening loose screw connections inside the K4 Manlift Circuit Breaker MCC Box. The disconnect was pulled. The employees's screwdriver contacted the live handle that is used to disengage the breaker box from the bus bar and arced. The employee had 1st degree burns on his left side. He was given a prescription for his eye as a precaution.

EE was welding the flange on the top hatch of the kiln 4 baghouse and a piece of hot slag fell onto the tongue of his boot (nylon), burned through, and caused a 3rd degree burn the size of a dime on his foot.

Employee was walking down stairs, from K4 Firing Floor, came to the mid level landing and turned left ankle while placing foot on the landing. Employee reported that he was walking down stairs and was holding the handrails.

Employee was assisting in cleaning the inside of #3 rail scale. The manhole was uncovered. Employee picked-up the manhole cover to put it on the manhole. The cover weighed approximately 50 pounds. The manhole started slipping out of his hands. EE tried to catch the cover, when he did, he fell down and he felt a sharp pain in his right arm/shoulder area.

EE was exiting a manlift basket (type with the sliding midrail) and as he was stooping to exit he slipped in wet lime and began falling. He injured his left shoulder as he caught himself. This did not become reportable until 1/5/12 when work restrictions were implemented.

Employee was grinding a handrail with a hand-held grinder to prepare for painting. The grinder jerked to the side when he contacted the metal and slammed his hand into the vertical part of the handrail.

Employee was removing the mounting plate for kiln 4's O2 analyser. He was using his left hand to hold the socket end of the wrench in place and his right to turn the socket wrench in an upward motion. As he attempted to turn the socket wrench, he felt a pop in his left shoulder and immediately reported the accident.

Employee was working on replacing the bags in Kiln 4 Baghouse Module 3 when his right foot got caught in one of the baghouse holes where the top of the bags are and when he went to get up from there he sprained his right knee. This did not become reportable until February 14th and light duty was given.

Employee was attempting to remove a pipe to the suction/holder when the pipet broke and lodged in his right pointer and middle finger. Per ee, the pipe was stuck in the holder and he had to use extra force to remove the pipe. The pipe broke, and the section that remained in the holder struck his fingers.

Employee was inspecting and cleaning the ice from the rollers on K4 stone belt when he stepped on a pry-bar on the catwalk. He was using a sledge hammer to beat the ice from the rollers. The bar slipped out from under him and he fell. As he fell His right hand became lodged in between a return roller and the belt. He was able to pull the e-stop and call for help.

Employee claims that his neck has been hurting because of driving a haul truck on the quarry roads. There was nothing specific noted for the cause. The pain has gradually gotten worse over the past few weeks. This was initially reported on 2/8/10 and employee asked to see a dr on 3/4/10.

Employee was climbing the permanent ladder from the kiln 4 cooler pit and felt a sharp pain in his right hip. No other contributing factors other than climbing a ladder. Went to the doctor on 2/2/10 after no improvement of condition.

Employee was cleaning the bull gearbox on the kiln drive pier. He was using 5 gallon buckets to put the grease in and then lifting the bucket over the handrail and lowering to the ground with a rope. He strained his left tricep while lifting the bucket over the handrail. Became recordable when Dr. prescribed physical therapy.

Employee was pulling a 1.5" dia. water hose and felt a "pop" in his side as he was placing the hose (90 feet long) into the holder that ran along side the conveyor. He was not lifting or pulling the entire length. Employee is being scheduled for surgery to repair hernia and will lose time as a result.

Injury was working on kiln 5 baghouse (E module). This is a negative draft baghouse and while working around the entrance, a piece of metal blew into his left eye. He was wearing prescription safety glasses with sides shields at the time of the accident.

A group of six maintenance employees were removing bags from a dust collector. The injured employee was removing a bag and hdyrated lime fell into his left eye. To remove the bag requires a 1/4 twist and then pulled down. He was wearing prescription safety glasses with sideshields. The bags in this dust collector are roughly changed every 5 years.

Employee was directing forklift unloading material and had his hand under the pallet.

Employee was cutting a hose used for the vacuum truck with an utility knife and pulled the knife towards his right hand. He suffered a laceration to his right, middle finger approximately 7mm in length. He was initially treated on site with basic first aid. This became reportable on 4/20/09 when he received surgery to repair a damaged tendon.

Employee was moving coal mill liners (approximately 60 lbs) from an elevated pallet to the coal mill. He would have to lift them up around 10 inches and place them on the lip of the coal mill for support. He was moving the liner with a twisting action and felt a pinch in his side. He did not report the accident until 3/9/2009 and comp carrier determined coverage on 3/19/09.

while exiting Kiln 3 preheater feed leg area his left foot became lodged between grating and lost footing with right leg, which caused employee to fall and turn while left foot and leg stayed stationary.

Employee was moving a 4x8 sheet of 3/4 inch plywood (approximately 75 lbs) into the kiln and dropped the edge onto his left middle finger. His finger was caught between the door sill and the plywood.

Employee was walking next to rail road tracks during rain storm. He stepped into a pot hole and sprained his right knee.

Employee suffered moderate heat exhaustion. Didn't intake enough fluids and required IV fluids.

Quarry loader operator was filling in as a quarry haul truck driver was jarred around when another loader operator dropped a boulder into the bed of his haul truck.

Employee was walking up flight of stairs and experienced sudden pain in his 2nd and 3rd toes of his right foot. He could not explain any contributing cause or incident to intiate the pain.

Employee was cleaning out rail cars and was using a rope ladder for access. As he was climbing out of the rail car, the left side of the ladder broke and the employee swung to the right.

Employee was working on a screw conveyor. The screw connection required an awkward position for the employee to work in with his arm reaching around the bottom of the screw. While using an impact wrench his arm was twisted because of the awkward position and pulled the pectoralis muscle.

The employee was an oiler and appeared to have his grease gun in hand while coming down a 9 foot permanent ladder. He fell from between 0 and 9 feet onto a concrete pad. The employee suffered a laceration above his left eye, and broken left lower forearm, and a break to the left side of his hip.

Employee was in a screen room on top of a lime silo. He was positioning a vac truck hose and was pulling. The hose had a cut and broke. The employee tripped backwards and his shoulder hit spare screeens that were leaning against the wall.

Employee was working inside of a kiln pre-heater. He was removing mud build up (large heavy pieces). Due to the construction of the space, he did not have much room to work and was working in an awkward position. While lifting one of th heavy pieces of mud he pulled a muscle in the left thorasic region of his back.

Employee was walking along a trail next to a culvert and slipped into it. He slid at an angle 2-3 feet and suffered an injury to his elbow. Employee had been given an alternative route which was slightly longer to travel that did not take him near the hazard.

Employee was shoveling limestone fines and he twisted to throw the fines on the other side of himself. This awkward movement caused a pulled muscle in his lower back.

Employee was operating a CAT 966. After a shot, he was moving rock. He traveled into the rock pile and the bucket hit a toe. The loader stopped and jarred him. He was wearing a seatbelt. Diagnosed with Lumbar Radiculopathy (strain) on 4/7/06.

Employee was shoveling lime dust from the top of a silo. Lime was blown into his eye. the wind was strong and gusting. The employee was wearing his appropriate PPE.

Employee was installing a V-belt to machine pullies. While rolling on the belt he caught his right ring finger and right pinky between the belt and the pulley. Ice was applied and employee went to the emergency room. he was diagnosed with a fracture to the right index finder. Employee returned the next day with light duty restrictions.

Employee stepped off of firing floor while giving crane signals. Fell roughly 15 ft. Normal procedure not followed and caution tape tied across opening instead of rope which is normally used. Employee suffered fracture to L3 vertebrae. MSHA notified at 3:30 pm.

EE, an engineer with Refractamerica, was assising with the installation of a prototype kiln brick and a brick fell on his left pinky. He suffered a laceration that required 10 stitches and a cracked bone.

Employee was raking clinkers from the cooler grate on kiln #3, and felt a pull in his right elbow. He reported to his supervisor, but felt he could continue. Employee continued to work but the condition did not get better. He was sent to Dr. on 10/6/05. No lost time.

Employee was pulling clinker from kiln. As he pulled the rake back, his left pinkie was mashed in between the rake handle and the handrail that was located behind him.

Employee was stepping off of our track mobile. While coming down the ladder, he skipped the bottom step onto the ground. this extra distance resulted in the employee spraining his left knee.

Two well experienced miners were working in a bag house. They didn't take the air monitor with them and one was overcome by carbon monoxide. Also, the reverse air valve was not reopened after returning from lunch to bring in fresh air.

Maintenance was called in to fix a problem with #3 screen. Roof opened to let air in and began work. Around 11:30 employee suffered heat exhaustion. Fellow employee found him laying down and unconscious. He was revived on the scene by co workers and given water. Due to recent health problems, 911 was called and he was taken to the ER. No health problems were found.

Employee closed finger in standard entry door at #3 screen room. Employee suffered a cut to right index finger which she did not require stitches. No fractures. Employee received a prescription for pain.

2 EE'S WERE WORKING IN SACKING HOUSE, EE #1 SAID THAT HE WAS GETTING HOT, THEY BROKE FOR LUNCH, WHEN THEY ARRIVED AT THE LUNCHROOM EE #1 SAID HE WAS DIZZY, EE #2 CALLED SUPERVISOR & EE #1 WAS TAKEN TO THE HOSPITAL WHERE DR.'S SAID HE HAD SUFFERED A MINI STROKE & THAT HE WOULD FULLY RECOVER IN A SHORT TIME.

EE WAS ATTEMPTING TO PUT A WINDOW AIRCONDITIONERBACK INTO ITS HOUSING WITH A FELLOW WORKER, WHENIT SLIPPED & CUT 2 FINGERS OF THE EE'S FINGER

EE WAS PULLING ON A BAR OPENING THE BOTTOM DOORS ON A RAIL CAR WHEN HE STRAINED HIS LOWER BACK.

Employee was getting off of track mobile and tripped over a portable ladder that was lying on the ground. Employee has worked regular job since accident. Surgery performed on 9/9/04 to correct disc. expected time out is 2-3 weeks.

THE EE WAS AT THE SCREENING PLANT WASHING MATERIAL OFF THE #1 LEVEL WHEN HE HEARD A ROCK FALLING FROM ABOVE, HE LOOKED UP TO SEE WHERE IS WAS COMING FROM WHEN IT STRUCK HIM IN THE FACE.

EE WAS HOLDING A 8LB BLEDGE HAMMER BEHIND A ELEVATOR CHAIN AS ANOTHER EE USED A 10 LB SLEDGE HAMMER TO DRIVE INT HE PIN THAT HOLDS THE CHAIN TOEGETHER, HE MISSED THE PIN AND STRUCK THE EE HOLD ING THE 8LB HAMMER IN THE LEFT HAND BREAKING HISLEFT HAND BREAKING HIS INDEX FINGER.

EE WAS WORKING IN THE #2 SCRUBBER CLEANING IT OUT. HE WAS STANDING ON LOOSE MATERIAL & WHEN ITWAS CLEANED UP WITH BOBCAT FROM THE BOTTOM OF THE TANK ON THE OUTSIDE THE MATERIAL GAVE WAY & EE STRUCK HIS LEFT ELBOW AGAINST THE TANK. HE SAID HE WAS FINE & DID NOT NEED MED ATTENTION WE TOOKHIM TO HOSP ANYWAY TO BE CHECKED OUT HE WAS FINEON 4/26/02 HE HAD SURGERY ON THE ELBOW

EE CAME INTO WORK APPROX 2 HOURS EARLY (UNSCHEDULED) TO CONFRONT A FELLOW EMPLOYEE THAT HE HAD A DISAGREEMENT WITH THE DAY BEFORE. AN ARGUMENT ENSUED AND HE WAS SHOT & KILLED WITH A 38 CALIBAR HANDGUN. ONE SHOT THROUGH THE CHEST GOING FROM LEFT TO RIGHT GOING THROUGH HIS LUNGS & HEART.

DRILLER WAS IN THE DRILL. DRILL STARTED DOWN THE SLOPE AND THE DRILL STARTED DOWN THE SLOPE, DRILLER SET THE BRAKE WHEN THE DRILL STARTED SLIDING DOWN THE SLOPE, DRILL KEPT SLIDING DOWN THE SL OPE, DRILLER JUMPED OUT. THE DRILLWENT HALF WAY DOWN THE SLOPE AND FLIPPED ON DRILL'S LEFT SIDE.

EE WAS USING A 90LB JACKHAMMER TO REMOVE BRICKS FROM #3 KILN ANOTHER EE WAS HELPING HIM THE 2 WERE TRYING TO REMOVE THE BRICKS ON THE SIDE UP ABOUT 3-4 FT FROM THE BOTTOM. THE JACKHAMMER WEN T THRU THE BRICKS AND HIT THE OUTER SHELL (STEELAND SKIDED DOWN EE TRIED TO HOLD ONTO THE HAMMERRATHER THAN LETTING IT DROP IN THE PROCESS HIS FINGER STRUCK A BRICK

OPERATOR RAN HAUL TRUCK INTO A STONE WALL.
