I WAS INFORMED ON 1-19-98 OF THIS LOST TIMEACCIDENT, THE INCIDENT REPORT WAS COMPLETED BY EE ON 1-27-98.

EMPLOYEE WAS STANDING NEXT TO ANOTHER EMPLOYEE WHO WAS ATTEMPTING TO REVERSE #1 SCREEN MOTOR BY SWITCHING WIRES IN COMBINATION STARTER. CO-WORKER DISCONNECTED ONE WIRE AND TOUCHED ENERGIZED WI RE TO PORTION OF BOX CAUSING INSTANT EXPLOSION.
WHILE EMPLOYEE WAS ON CATWALK, HE FAILED OT SEE AN OPENING IN THE CATWALK PLATFORM. HE ACCIDENTLY FELL THROUGH THE OPENING AND SUSTAINED A PUNCTURE WOUND TO HIS RIGHT LOWER LEG.
EMPLOYEE WAS WALKING DOWN STAIRWAY, SLIPPED AND FELL SPRAINING HIS RIGHT ANKLE. EMPLOYEE FAILED TO JUDGE CONDITION OF STAIRWAY. WEATHER WAS RAINY AND STAIRWELL WAS WET.
EE WAS DRIVING STEEL WEDGE BEHIND CONCAVE BLOCK IN CRUSHER WHEN WEDGECAME OUT STRIKING EE ABOVE THE RIGHT EYE.
