Removing wear plate. Wear plate bounced off wall of chute and contacted employee leg

Employee couldn't see above the sides of the truck for correct location under chute, so EE straddled between the truck and the concrete wall to see the correct chute location. Employee slipped off the step and fell between the wall and the truck on the ground and bent right knee out on a 45 degree angle. It was diagnosed on 11/3/16 that employee had a broken tibia.
Employee was walking and holding a pipe union in left hand. Employee stopped and headed back to their truck to grab a wire brush when they tripped causing them to fall forward to their hands and knees. The two pieces of the pipe union the employee was holding cut their finger when they fell.
Employee was changing a large acetylene tank on the welding cart. When ee picked up the large tank, ee felt pain in lower back and couldn't feel left leg.
Employee was using a torch to cut bolts off of the feeder in tunnel #3. A piece of slag landed inside left ear burning close to ear drum.

Employee worked all day on top of the bag house, and he was drinking water regularly. It was hot on top of the steel structure. Employee felt fine until the last hour of the day, when he felt a little dizzy. Employee came back down with manlift and went to the break room to get cool air. Employee was taken to the emergency room where he was treated with IV fluids and released.

After getting into cab of truck, driver was stung by a bee and had an allergic reaction.
Employee servicing a CAT 922 loader. While descending loader, lost his grip and fell backwards. Fall was broken when his left upper arm struck a U-shaped piece of steel on a nearby welder. Employee crushed an artery. Had surgery to remove blood clot and sew arteries together. Full circulation restored. Out of work with restrictions.
Entering tunnel for daily housekeeping and found water running in the tunnel from the evening before. Stepped off stairs and hit a slick spot causing him to slip and twist left knee. Felt injury superficial and gave it some time to heal. On 5/12 pain still persisted. Sought medical care. Left knee sprain. Placed on restricted duty and prescribed PT and prescription.

EE WAS MOVING A LARGE ANGLE IRON WITH ANOTHER. THE ANGLE IRON WAS BEING SUPPORTED BY A SLING. THE SLING SLIPPED CAUGHT EE'S HAND TO BECOME CAUGHT BETWEEN THE ANGLE IRON AND A CHUTE, EE SUSTAIN ED A FRACTURE TO HIS LEFT HAND CURRENTLY ON RESTRICTED DUTY.
EE WAS FABRICATING A GATE POST. WHILE LOADING THE GATE POST ONTO A TRUCK, EE FELT A POP IN HIS LOWER BACK. WAS PLACED ON RESTRICTAED DUTY UNTIL 3/3/98 FOLLOW UP. CALLED SUPERVISIOR AT HOME TO INFORM HIM OF INJURY. OCCURRED IN GARAGE AREA OF QUARRY.

DIAGNOSIS WAS FOREIGN OBJECT IMBEDDED IN CORNEA- REMOVED BY DOCTOR.
