Haynes Materials Company · (Not Elsewhere Classified)

Subcontractor employee was administering maintenance, took a break, went to employee's pickup truck. Was found in the vehicle unresponsive. Cardiac arrest. Employees notified 911 and administered CPR until paramedics arrived. Employee was then transported to local hospital, where employee was pronounced deceased.

Employee was cleaning the chute on the wash plant, trying to remove the angle iron on the bottom of the chute. As employee drove a steel pry bar in between the angle iron and the chute, the bar pinched employee's left middle finger against the chute.

WAS WALKING DOWN THE CONVEYER AFTER GREASING THE HEAD PULLEY IT WAS REALLY COLD OUTSIDE SO THEIRWAS FROST ON EVERYTHING AND HE SLIPPED AND FELL OFF THE CONVEYER WHERE THE LADDER WAS SET UP ON THE BOTTOM OF THE CATWALK, FALL WAS ABOUT 5-6 FEET FROM GROUND, BRUISED AND SWOLLEN MUSCLES IN HIS BACK.
