The employee picked up a roller when EE lost EE's grip. It slipped out of EE's hands, and the shaft's keyway cut the ring finger on EE's right hand. The worker received four stitches to close the wound.

Employee tripped getting into cab and fell with hands forward into cab. Employees right hand middle finger contacted cab floor first and dislocated to the right.

Employee was working to remove the well at the re-crush plant. The well got hung up on a collar. Employee used a hammer and wedge to remove collar. Once collar got knocked free the well casing slid down and pinched employee's left hand ring finger tip.

Employee was walking to car, slipped and fell. Tweaked ankle/foot/knee & hurt right hand/elbow. *This is not a reportable injury. The employee has only received first aid. I am reporting to avoid citation if the employee does receive more than first aid outside of the 10 day reporting requirement (as advised by an inspector over the phone).

Employee was exiting M3 Chute after cleaning an overflow. Pant leg got caught on the chute lip and caused employee to lose balance. In regaining balance, employee tweaked back.

Employee was shoveling material into wheelbarrow. When employee went to empty wheelbarrow, felt a tweak in right shoulder. Full rotator cuff tear.

Manually pulling on V belts for crusher installation, fingers of employee got pinched between the belt and the sheave.

Employee was changing a M1 gear. It was raining and employee's safety glasses fogged up multiple times. Employee hung glasses on shirt and tapped the gear in place and felt something in eye.

Employee was checking the main conveyor over the portable screen and as EE was climbing down, EE hyper-extended right shoulder backwards and felt a sharp pain. First aid was administered. Employee went to doctor on 10/13/16 and received two prescriptions.

Employee was asked to cut down a pine tree near the primary. Employee cut a wedge on the back side of the tree. As employee was cutting through the tree, the chainsaw kicked back and lacerated left knee.

While an employee was lifting safety grating his foot slipped striking his shin on one of the divider plates within the chute.

While swinging a sledge hammer to clean process chute, employee's finger got caught between the sledgehammer handle and the angle-iron of the chute. Employee reported the incident to his supervisor at lunchtime. Employee went to the hospital on 6/5/15, where it was determined that the employee has dislocated and fractured his finger.

Employee stepped down off loader ladder and felt pain in lower back. Employee examined at medical facility on Monday, May 13, administered a prescription and released to light duty work.

Employee was shoveling under conveyor near tail-pulley when he felt his right knee pop.

While tapping a gear off a shaft with a hammer, a piece chipped off the gear hitting him in the stomach. Treated at ER for stomach puncture.

While working in a transformer vault and installing a 3-inch high voltage electrical cable, he started to adjust the cable supports. The cable slid back into the 5" conduit pulling his hand into it. Finger was sore and ice applied. An hour later, he went to squeeze a hook and felt a pop in his finger.

While observing cutting and grinding being done in screen house, it was very windy and particles of debris flew into his right eye. Was wearing safety glasses. Washed eye thoroughly. But 3 days later felt pain in eye and blurred vision. Sought medical care. Infection from small rusted particle.

Employee was descending the ladder of his front end loader when his left boot heel got caught on the step "tweaking" his knee. This led to pain/tightening of his inside back of the knee and tenderness to the kneecap.

Employee had just decended a fixed ladder from the rock grab next to the primary. When he reached the floor level, he took one step away from the ladder, when his left foot / leg went through the floor. It gave way beneath him, resulting in him striking against the floor, causing pain in his left hip and left shoulder.

Claimant was on a vehicle when he twisted and hurt his back.

Employee states he twisted right knee sometime during shift, nothing to pinpoint the accident, just that he had pain and it worsened towards end of shift.

The employee had stopped the fuel truck that he was driving on a flat, clear, dry area of the quarry floor. He dismounted the cab of the truck and when his right foot hit the ground, as he exited the vehicle, he experienced pain in his lower back.

Employee was attempting to drive shaft out of sprocket/shaft assembly. To do this it was required that he lift this assembly onto two railroad ties to allow room for shaft to be driven out of sproket when hit from above. When employee set sprocket onto tie #2 he mis-judged the location of the tie and caught thumb between sprocket and railroad tie.

Was assisting another employee who was welding and while holding steel, glove caught fire causing burn to pinky finger.

There are no conditions that lead to this accident, employee has history of ankle problems and thinks it happened at work because of walking on uneven surfaces.

While reaching to uncoil cable felt a pop in the back

Employee complaining of pain in his right low back radiating down his leg with numbness. There is no known cause.

Hyper-extended his arm while climbing a ladder inside the bin.

cleaning the dust pipe, and while placing back into position felt a pinch in right hip.

Was washing drain pipe with hose, it became muddy and employee slipped on it hitting elbow.

Employee was climbing out of crusher door and " something popped in right knee"

Was moving an Iron beam and felt a pop in arm

WAS TIGHTENING VISE FELT PULL IN BACK.

WHILE SHOVELING SNOW AND HAVING TO THROW IT A GOOD DISTANCE EE EXPERIENCED PAIN IN THE GROIN AREA THAT EVENING AT HOME. THE INJURY WAS REPORTED THE NEXT DAY.

EE WAS PREPARING TO USE THE BANDSAW WHEN HE BECAME DISTRACTED. WHILE ATTEMPTING TO ADJUST THE WATER PRESSURE FOR COOLING AND CLEANING THE BLADE THE EE BECAME DISTRACTED FOR A MOMENT. EE S RIGH T THUMB THEN MADE CONTACT WITH THE BLADE.

EE WAS IN THE PROCESS OF CORRECTING THE TRACKING ON HTE SCR DRIVE-A BELT. EE WAS ADJUSTING TWO BOLTS TOWARDS THE REAR OF THE TAILPULLEY ASSEMBLY TO CORRECT THE TRACKING. STONE FELL ONTO THE BE LT. EE CLIMBED UP AND ATTEMPTED TO REMOVE THE STONE. CLOTHING WAS CAUGHT AND EE SUFFERED BONE FRACTURE AND TENDON INJURY. Y

WHILE DRIVING A FORKLIFT ON THE UPPER HAUL ROAD, WIND BLEW AN OBJECT INTO EE'S EYE. EE REPORTED INCIDENT TO HIS SUPERVISOR ON 9-17-01. DIDN'T KNOW IT WAS REPORTABLE UNTIL WE RECEIVED COPIES OF PRESCRIPTIONS FROM EE ON 9-27-01.

A CO-WORKER WAS STRIKING THE WRENCH THAT THE EMPLOYEE WAS HOLDING (THEY WERE TIGHTENING BOLTS). THE WRENCH STRUCK THE EMPLOYEE'S THUMB, FRACTURING IT.

WHILE SHOVELING, MUSCLE IN CHEST WAS STRAINED. EE GIUVEN IBUPROFEN & PLACED ON LIMITED DUTY FOR ONE WEEK.

WHILE DESCENDING LADDER OF LOCOMOTIVE WITH ARMS SLIGHTLY ABOVE HEAD LEVEL, EE TURNED HIS HEAD SLIGHTLY AT THE SAME TIME & FELT A POP INHIS NECK. EE DIAGNOSED WITH A CERVICAL STRAIN & PLACED ON LIMITED DUTY.

WHILE HEATING BUCKET FOR WELDER EE RECEIVED FLASH BURN TO BOTH EYES OFF OFF REFLECTIONS OF EQUIPMENT, WALLS. UNKNOWN THAT EE SOUGHT MEDICAL ATTENTION & IF REPORTABLE UNTIL 9/4/01.

AS EMPLOYEE WAS DRIVING DRILL INTO TRAILER, INJURED EMPLOYEE ACTING AS A SPOTTER. HE WENT TO UNHOOK THE RAMP AND IT FELL AND STRUCK HIS FOOT.

WHILE STEPPING OFF OF A CONCRETE BLOCK, EMPLOYEE LOST HIS FOOTING ON LOOSE STONE AND TWISTED HIS KNEE.

EE WAS PUTTING A PLATE IN CHUTE, IT BECAME STUCK & EE PULLED ON IT TO FREE IT STRAINING HIS SHOULDER

EE WALKING WITH ONE OF THIS SUPERVISOR AND TURNED TO WALK INTO THE RECRUSH TRAILER. HE FELT HIS KNEE GIVE OUT AND HE FELL TO THE GROUND. WE ENTRY ON SECONTARY DUTY ANTICIATE LOST TIME.

EE WAS REMOVING/REPAIRING AN M-3 SCREEN DECK. A PRYBAR WAS BEING USED BY SOMEONE ELSE ON AN UPPER LEVEL. PRYBAR WAS DROPPED, SLID DOWN SCREEN THROUGH THE CRUSHER AND EVENTUALLY STRUCK EE IN TH E NECK.

WHILE BLOWING WATER ICE OUT OF LINES, EE AND ANOTHER WERE HEATING THE LINES TO CLEAR THEM. EE WAS STANDING AT THE FRONT OF THE PIPE WHEN A PIECE OF ICE FLEW OUT AND STRUCK HIM IN THE THIGHS.

EE LOST FOOTING WHILE DESCENDING LADDER ON WATER TRUCK, SLIPPED & CAUGHT HIS R ARM ON THE LADDER.

WHILE PREPARING HOLES TO BLAST, EE WAS BLOWING OUT HOLE WHILE STANDING ABOVE IT, OBJECT BLEW OUT OF HOLE & STRUCK EE.

EMP WAS CLEANING BUILD UP ON THE 3/8" CONVEYOR. THE SNUB PULLEY BAR KICKED UP AND STRUCK EMPLOYEES SHOULDER.

EMPLOYEE WAS EXITING WATER TRUCK AND STATED THATHE LOST HIS FOOTING AND FELL BACKWARDS APPROXIMATELY THREE FEET TWISTING HIS LEFT KNEE. WHILE FALLING, HE REACHED BACK WITH HIS RIGHT LEG STRAIN ING THAT KNEE. EMP WAS UNABLE TO PLACE ANY PRESSURE ON LEFT LEG. SWELLING WAS NOTICED AND ICE PACK WAS APPLIED AT THE SCENE.

EMPLOYEE WAS TIGHTENING WEDGES ON THE 4 1/4" CRUSHER AND LOST HIS FOOTING STRIKING HIS BACK ON THE CATWALK. THE EMPLOYEE WAS THEN STRUCK BY HIS FALLING SLEDGE HAMMER ON HIS LEFT HIP.

WHILE GRINDING THE BIN WALL, DIRT & METAL PARTICLE LODGED IN EE'S LEFT EYE. EYE PROTECTION WAS BEING WORN, BUT THE PARTICLE ENTERED FROM THE SIDES. EE FELT DISCOMFORT THE NEXT DAY & WENT TO HI S PERSONAL PHYSICIAN.

WHILE EE WAS WORKING ON THE CRUSHER, HE WENT ON THE CONVEYOR BELT LOCATED UNDER THE CRUSHER TO FINISH HIS TASK. GREASE HAD FALLEN ON THE BELT. THIS TOGETHER WITH THE INCLINE OF THE BELT CAUSED EE TO SLIP.

WHILE EE WAS EXITING THE FORKLIFT, HE STEPPED ON A PIECE OF STONE INJURING HIS ANKLE.

EMPLOYEE CLAIMS PAIN TO THE TOP OF HIS LEFT SHOULDER DUE TO CONSTANT TURNING OF STEERING WHEEL OVER THE PAST YEARS.

WHILE OPERATING THE LOADER ON AN UNEVEN GRADE AND DUMPING THE BUCKET AT THE SAME TIME THE LOADER SHIFTED. THE EMPLOYEE HIT HIS RIGHT EAR ON THE RADIO MOUNTED IN THE CAB OF THE LOADER.

WHILE CLEANING A ZINC POUR HOLE IN JAW MAIN FRAME WITH TORCH, FOREIGN OBJECT FLEW PAST PROTECTIVE SHIELD AND INTO EE S EYE.

WHILE CHANGING A SCREEN IN THE MILL, EE WAS HOLDING SCREEN SO IT COULD BE KNOCKED INTO PLACE AND ANOTHER EE SWUNG WITH HAMMER MISSING SCREEN AND HITTING EE'S THUMB.

WHILIE RELEASING HAND BRAKE CLIMBING ON LADDER, HE LOST FOOTING ON LADDER NAD HUNG ON WITH LEFT HAND CAUSING STRAIN TO SHOULDER.

WHEN REMOVING SAMPLE FROM THE LAB TRUCK EE FELT A SHARP PAIN IN HIS RIGHT SHOULDER. APPROX. WEIGHT OF SAMPLE 40 LBS.

EE WAS LEANING OVER THE BOX HELPING TO REMOVE A BRIDGE. AS HE WAS PULLING A STONE UP HIS FINGER WAS PINCHED BETWEEN THE STONE AND THE WEAR PLATE (MAG CLOTH) OF THE BOX. (EMPLOYEE WAS WEARING G LOVES).

WHILE ATTEMPTING TO MOVE M5-SCREEN DISCHARGE BOXINJURED REACHED DOWN TO REMOVE MATERIAL BUILD UPUNDER WHEEL. THE PLANT OPERATOR ASSUMED MR. VARGAS WAS PULLING BOX AND HE PULLED ALSO. THE BOX MOVED PINCHED INJURED'S FINGER BETWEEN WHEEL & TRACK.

WHEN CLIMBING DOWN METAL LADDER TO CHECK M-1 CONV. BELT FOR METAL INJURED SLIPPED STRIKING RIGHT KNEE ON ANOTHER RUNG.
