Brooksville Rock Plant Metal/Non-Metal

Employee was washing rock from elevated catwalk with a high pressure water hose. The trailing hose slipped off the catwalk, pulling the hose from employees hands. The hose whipped around and the nozzle stuck him in the lower back on the right side. Injury was reported the day of occurrance, did not seek medical care until 10/5/07

A plant maintenance employee was replacing a screen bearing with the assistance of a co-worker. The two employees were moving the bearing housing when the co-worker lost his hold leaving the injured employee supporting the bulk of the weight causing a strained left shoulder. Employee reported incident to supervisor but did not seek medical attention at the time.

Employee was conducting clean up at the primary plant when a large rock was ejected from the grizzly screen located on the second level. The rock rolled down stairs and struck employee on the calf of left leg. Incident was reported and the employee continued to work. Employee began loosing time on July 27, 2006 after the injury worsened.

Employee went to pull hose out from underneath loadout screens. Slipped on wet surface. employee attempted to regain balance, twisting his body as he fell.

Employee was mounting D10 dozer at beginning of shift. His foot slipped off of broken step causing him to fall and stike his right wrist on the ground.

Employee was preparing to switch product gates at the final wash screen in preparation of running a different product. Employee pulled on handle of product gate and found it difficult to move as material had lodged. The employee used force on handle causing strain to lower back.

Employee was operating drill when hydraulic hose became tangled in the drill mast.EE climbed onto roof of drill to untangle hydraulic hose and slipped, striking left shin on hand grip on top of cab roof causing laceration and contusion to left shin. Incident was reported to supervisor but employee did not seek med attention until Feb10th when laceration became infected.

Employee had just completed pre-shift inspection on assigned haul truck. A fellow employee, who was ending his shift, proceeded to back his haul truck into the truck line. This individual misjudged where others were parked and backed into left front of haul truck that injured employee was standing on. The employees left foot was pinned between handrail and front of truck.

EE WAS USING A PRY BAR TO MANAGE A BREAST PLATE UNDER TENSION ON SECONDARY CRUSHER. WHEN EE RELEASED HOLD ON PRY BAR SO THAT THE BREAST PLATE COULD BE REPOSITIONED, THE PRY BAR SPRANG LOOSE STRIKING EE AND KNOCKING HIM OFF PLATFORM LADDER

EMPLOYEE WAS FUELING THE PIT LOADER DURING THE PRODUCTION SHIFT WHEN HE NOTICED FLUID LEAKING FROM THE RADIATOR CAP OF THE UNIT. AS HE ATTEMPTED TO TIGHTEN CAP, THE CAP DISLODGED FROM THE PRES SURE IN RADIATOR AND SPEWED HOT FLUID AND STEAM ONTO EMPLOYEE.

EE WAS REPLACING FLOOR LINER ON PRIMARY CRUSHER. EE WAS STANDING ON MOTOR COVER REACHING OVERHEAD WITH TORCY TO CUT BOLTS. HIS FOOT SLIPPED AND HE TWISTED HIS LEFT KNEE AS HE CAUGHT HIMSELF FR OM FALLING. DUE TO RECENT MEDICAL PROCEDURE EE BEGAN LOSING TIME 7-8-02.

THIS IS A RE-INJURY OF AN ACCIDENT THAT WAS MEDICAL ONLY. EE HAS SINCE HAD OUT-PATIENT SURGERY TO REPAIR KNEE. EE WAS INSTALLING BREAST PLATE IN SECONDARY CRUSHER, STANDING ON V-BELT GUARD AND STEPPED DOWN ONTO COUPLING GUARD. THE GUARD SHIFTED CAUSING EE TO LOSE BALANCE AND FALL.

EMPLOYEE WAS USING A WATER HOSE TO WASH CATWALK, WATER HOSE FELL OFF CATWALK AND BECAME ENTANGLED IN TROMMEL. TROMMEL PULLED HOSE WHICH BECAME ENTANGLED AROUND LEFT FOOT. HOSE GAVE WAY RESULTI NG IN 3 FRACTURED TOES.

EMPLOYEE WAS CHANGING OUT A BEARING ON THE NORTH LOADOUT SCREEN. HE WAS PULLING ON THE BEARING HOUSING TO REMOVE IT FROM THE SCREEN SHAFT. FINGER WAS PINCHED BETWEEN THE HOUSING AND THE SHAFT.

WHILE USING PRY BAR TO REMOVE COUNTER-WEIGHT FROM #3 SCREEN, PRY BAR SLIPPED OFF OF COUNTER-WEIGHT AND STRUCK EE IN MOUTH CAUSING LACERATION INSIDE MOUTH.

IN PROCESS OF DRIVING COUPLER ONTO INPUT SHAFT OF GEARBOX, THE PIN BEING USED TO DRIVE COUPLING ONTO THE SHAFT FRACTURED SENDING A PIECE OF METAL IN DIRECTION OF INDIVIDUAL STRICKING HIM IN TH E WRIST AREA OF RIGHT ARM.
