EE was clearing a jam from a conveyor #1 head box chute. As the jam was cleared the EE's left finger was wedged between the pry bar and a cross brace. The result was a fracture to the left ring finger.

The injured employee was helping replace a conveyor pulley. The pulley was moved into place with a crane, and was then suspended by a chain. As he attempted to remove the lifting slings, the pulley shifted and his arm was caught between the chain and the pulley.

The employee was cutting a bolt out of a bearing housing using a 10 foot long burning bar. As he withdrew the bar, flame blew out of the hole and burned his arms and face.

Employee claims he stepped on a rock and twisted his knee while walking from the primary to a conveyor walkway. The result was torn cartilage in his knee.

Employee was closing railcar door, he lost his footing causing him to break his right ankle.

DURING PLANT OPERATIONS, BOLTS WORKED OUT OF THE FEEDER BOX UNDERNEATH THE BIN, CAUSING THE FEEDER BOX TO DROP INTO THE FEEDER. THREE EMPLOYEES ATTEMPTED TO PUT THE FEED BOX BACK INTO PLACE WHEN THE BOX DROPPED DOWN INTO THE FEEDER, PINCHING THE EMPLOYEE'S RIGHT THUMB, CAUSING HIM TO GET STITCHES IN HIS THUMB.

Water hose blew off the hose connection bushing. The employee put it back on and tightened it with a 6" crescent wrench. He turned the water back on and walked away. The hose blew off again and the water hit him in the back, knocking him into the feed box frame. He struck his left eyebrow on the frame and cut it.

INJ EE & ANOTHER EE WERE ATTEMPTING TO RETRACT THE LANDING GEAR OF FUEL TANK WHEN THE LEG GAVE LOOSE QUICKLY WITH NO TIME TO REACT. THESE LANDINGS ARE HEAVY LEGS THAT HAVE TO BE LIFTED MANUALLY AND FROM UNDER TANK. EE'S THIRD FINGER ON RIGHT HAND WAS INJURED RESULING IN LACERATION & FRACTURE

Employee was re-fueling off road truck when he slipped off the berm twisting his left ankle.

CUTTING CATWALK FROM CONVEYOR, METAL STRUCK EMPLOYEE ON NOSE.

BACKHOE TOOTH CAUGHT REBAR CAUSING IT TO STRIKE EE ON THE RIGHT SIDE OF HIS JAW CAUSING A PUNCTURE WOUND BELOW HIS RIGHT EAR AND FRACTURE TO RIGHT JAW BONE AND A BROKEN LEFT JAW.

EE WAS GETTING IN LINE TO BE LOADED, AS HE TURNED, HE LEFT OFF OF THE GUS AND SLUMPED OVER THE STEERING WHEEL AND COASTED INTO ANOTHER TRUCK BEING LANDED VERY SLIGHT IMPACT-LITTLE OR NO DAMAGE TO TRUCK.

HE WAS CARRYING A 5 HP MOTOR WHEN HE TWISTED HIS KNEE CAUSING A STRAIN. DR. SENT HIM BACK TO WORK WITH NO RESTRICTIONS ON 1/16. WEEK OF MAR 18TH DR. SCHEDULED ORTHOSCOPIC SURGERY FOR MAR 22 & RELEASED HIM TO RETURN TO WORK ON 3/25/02.

OPERATOR SLIPPED OFF STEP OF DOZER WHEN EXITING TWISTING LEG (RIGHT).

EE HOLDING WRENCH IMPROPERLY TIGHTENING BOLTS, WHEN BOLT WAS TURNED FROM OPPOSITE SIDE WITH AIR WRENCH, FINGR WAS CAUGHT BETWEEN WRENCH AND STEEL PLATE CAUSING CUT TO MIDDLE FINGER ON RIGHT HA ND, REQUIRING SEVEN STITCHES. EE WAS WEARING GLOVES AT TIME OF ACCIDENT.
