EE was clearing a jam from a conveyor #1 head box chute. As the jam was cleared the EE's left finger was wedged between the pry bar and a cross brace. The result was a fracture to the left ring finger.

The injured employee was helping replace a conveyor pulley. The pulley was moved into place with a crane, and was then suspended by a chain. As he attempted to remove the lifting slings, the pulley shifted and his arm was caught between the chain and the pulley.

THREE EMPLOYEES ATTEMPTED TO PUT THE FEED BOX BACK INTO PLACE WHEN THE BOX DROPPED DOWN INTO THE FEEDER, PINCHING THE EMPLOYEE'S RIGHT THUMB, CAUSING HIM TO GET STITCHES IN HIS THUMB.

Water hose blew off the hose connection bushing. The employee put it back on and tightened it with a 6" crescent wrench. He turned the water back on and walked away. The hose blew off again and the water hit him in the back, knocking him into the feed box frame. He struck his left eyebrow on the frame and cut it.

WHILE UNLOADING THE SCREEN CLOTH EE DROPPED HIS END OF THE SCREEN W HILE EE WAS STILL HOLDING HIS END. THIS CAUSED ONE OF THE HOOKS ON THE SCREEN TO PUNCTURE HIS LEFT HAND JUST ABOVE THE THUMB.
