EE on 12/16/2025 reporting back pain, due to an injury on 12/13/2025. They started changing a return roller and felt pain and reported to supervisor, but thought was going to be alright. On 12/16/2025 employee is requesting medical care and taken to local ER.

This report is being filed due to hearing loss detected during annual audiometric testing. Loss was found in employees Left ear.

On 9/2/2025 two contractors were unable to lower the JLG manlift basket they had been working from due to a blown circuit power fuse. Another manlift was on site and it was used to transport the two safely to ground level.

On 9/2/2025 two employees were unable to lower the rental JLG manlift basket they were working from due to a blown circuit power fuse. Another manlift on site was used to get the two employees safely to the ground.

EE was traveling between number 2 and number 3 kiln at ground level when they slipped and fell. This fall caused their left leg to bend back under them and caused pain to left knee.

EE states while moving a manlift from the operator's basket EE sustained an injury to EE's mouth. While the machine was in motion, EE hit a hole in the roadway resulting in a bounce causing EE's mouth to contact the man basket.

At approximately 10:30PM Friday June 27, 2025, a roof fall was found in 13w/95. Due to being above the anchor point MSHA was notified by phone. MSHA representatives where on site and issued a K-order on 6/30/2025.

An underground employee was injured during the unsafe operation of a Kawaski ATV Mule. The employee was traveling as a passenger on the ATV which was being driven in reverse by another employee. This incident was not reported to management until 3 days after the incident. The employee received medical treatment the day of notification, and after receiving suspension.

An underground employee was injured on 5/30/2025, during the unsafe operation of a Kawasaki ATV Mule. The employee was traveling in reverse and rolled the ATV with another employee on board. This incident was not reported to management until 3 days after the incident. The employee states they received medical treatment on the day of injury.

Employee states rust entered EE's right eye while using a chain hoist at the surface wash plant. A piece of rust dislodged from the steel beam the hoist was attached to. The incident happened on 3/30/2025 but was not reported to management till 3/31/2025.

Management was made aware at approximately 9:40 pm on 3/10/2025 that material was on the primary escapeway track. MSHA was notified and miners were evaluated using the secondary escapeway. It was determined material from the adjacent conveyor had spilled the material. Primary escapeway was put back in service at 12:25 am on 3/11/2025 after cleaning.

EE states wind blew dust in EE's right eye on 12/23/2024 at approximately 9:00 am in the plant area. EE tried washing EE's eye and had another employee try again just before the end of EE's shift. At the start of EE shift on 12/24/2024 EE's supervisor noticed irritation to EE's right eye. The employee was taken to ER and found to have a right cornea abrasion.

EE reported pain in right shoulder after attempting to move a water line connected to a relief valve. As EE was moving the hose it became charged with water causing EE's right arm to be thrown up and back.

While field tech was getting down from the back of their work truck, they noticed a pop in their knee. Anytime now they extend their leg down they notice a pop and feel pain.

4/8/2025 management was notified that employee was reporting an incident/injury that they state occurred on 11/13/2024. Employee states injury occurred when pulling a fire hose over a hand railing located at the lower deck of the fines dock. Employee states injury is herniated disks in back.

EE was assisting in moving an electric motor when EE felt pain in EE's back. Three mechanics were dragging an electric 30 hp motor over a steel graded floor when one of the employees complained of back pain.

The hydrate operator reported feeling a pop in EE's right shoulder and numbness in EE's right arm after using a sledgehammer. EE was striking the Pre-Mixer due to material build up when this incident occurred.

On 5/12/2024 smoke and a smell were reported at the top of the underground slope. MSHA was notified and the mine rescue team started underground exploration. The team found a mechanics' standing toolbox that had been on fire. The fire was out when the team arrived on scene.

On 4/10/2024 an employee reported they had slipped on steps going to the barge loadout. They continued to work regular until 4/16/2024 when they requested to be seen by a doctor due to pain in right knee. Employee was put on restricted duties at this time.

On 3/26/2024 EE reported that EE had an accident on 2/13/2024. EE states EE was struck by a piece of chain in the front of EE's mouth causing tooth damage.

The EE was installing a drive line on a Caterpillar 980M front end loader in the shop area when left middle finger became pinched. Seven stitches were required to close the open wound.

An employee received an eye injury after getting lime in left eye. After using the vac system to remove material from the screen floor employee started cutting corrugated 4-inch vac hose into manageable sections to be removed from the job site. As employee cut a section of pipe it flipped lime in employee's left eye.

A mechanic received lime burns on both upper extremities while working in the product bins. The burns were located on the anterior region in the crease where the arm and forearm join. Medication was prescribed for this injury on 8/26/2023.

EE was unloading tools from work truck and while carrying a bucket tripped. This injury was the result of a trip over a piece of scrap conveyor belt with EE's left foot. EE states EE twisted and buckled right knee. EE also scraped right forearm on truck bumper during the fall.

EE reported pain in upper back after lifting a chain hoist. The hoist had been placed in a five-gallon bucket for transport and weight approximately 45 lb. As the EE lifted the bucket, EE states the pain in their back started.

After an employee exited a Kawasaki side by side, EE rotated on EE's left foot to reach EE's lunch box in the back. When EE turned back, EE felt a pop in EE's left calf muscle.

An employee was injured while attempting to make repairs to an underground slurry line. As the last of four bolts was being removed stored pressure sprayed lime slurry on the EE injuring right eye.

Management was made aware at approximately 9:10 a.m. on 4/12/2023 that there was coal on fire in # 3 storage silo. MSHA was notified at 9:39 a.m. The fire was determined to be out on 4/13/2023 and the K Order was modified by the MSHA inspector. An update was given to the 800 number on 4/13/2023 @ 1:45 p.m.

As the employee removed the last bolt in the flange coupling the unsecured pipe riser started to fall. In an attempt to keep the approximate 100 lb. pipe valve assembly from striking the concrete floor EE tried to catch it. Due to the fall and weight EE's two middle fingers were fractured between a guard and the assembly.

Management was made aware at 11:30 am that the telehandler been used to clean in the slope would not start and was blocking the slope car. It was located about 600 feet from the slope bottom. MSHA was notified at 11:40 a.m. There was room to walk around the telehandler to ride the slope car to surface. At 1:16 p.m. slope was cleared and MSHA was notified.

At 2:50PM on 6/8/2022 MSHA Arlington Headquarters was notified that we were bringing everyone out of the mine due to the Slope Hoist Rope meeting retirement criteria. MSHA was also made aware that we had an Emergency Hoist and that we could exit the slope by walking. No miners were injured due to these findings.

During the task of lifting a drill steel, EE felt pain in their back. With help from another employee the EE was lifting a 10-foot drill steel from the storage rack. As EE lifted their end of the steel, EE felt pain in their back and tingling down the left leg.

Client did not inform of dangers from poor housekeeping, improper material storage and lack of notification/signage/barricade/training, etc. As a result worker received burns on leg and went to urgent care.

An employee was observing the shooting of an ash ring in # 1 kiln when EE felt a piece of material enter left eye. After attempts to wash the eye on site EE was taken to taken to a physician for treatment. EE states EE was wearing prescription safety glasses with side shields and a face shield.

A roof fall above the anchorage points was discovered on May 4,2022 at approximately 9:00 a.m. in the I 17/104 area. MSHA was notified by phone at approximately 1:00 p.m. on 5/4/2022 of the incident. No miner was injured nor was there any property damaged. An MSHA Inspector has been on site since this incident was reported.

EE states on 3/28/2022 while exiting the underground powder truck they slipped and fell. This fall resulted in pain to left thoracic side. The incident was not reported to management until 3/29/2022. At that time management was informed they would be seeking medical attention.

EE slipped and fell after stepping on a plate that had been removed from a roof bolter. The employee had removed a plate from a roof bolter and leaned it against a UTV Gator. Then needing to use the Gator for transportation the plate was laid on the mine floor. After returning and exiting the Gator EE slipped on the same plate listed above.

Installing boom attachment on equipment, missed handhold on machine and lost balance falling off machine landing hard on feet causing pain in back and right knee.

An employee was changing a valve in a pump. EE was leaning over to pull or pulling up/over the lines and felt a catch in their back. EE went back to work and completed the job and finished the day out. EE called in the next day to report the injury and said was not coming in and going to see the doctor.

The employee was doing routine maintenance on a roof bolter and when EE stepped over a grease hose felt pain in legs, lower back, and knees. EE was working at ground level and states: raised left foot about four to six inches to make a step over a grease hose. When EE put left foot on the mine floor after making the step is when the pain started.

An employee was injured while working in #1 kiln. EE was hand scaling down coating in the kiln shell when a piece fell and struck EE's left foot. This contact resulted in two fractures to EE's left foot.

Employee reported that EE injured neck when EE was jostled inside the haul truck while being loaded.

An employee was injured while operating a Toro Dingo. Operator entered 200 Tunnel in a forward direction and proceeded down the incline. EE filled loader bucket with muck, raising it approximately 3 feet and started to back up the incline The rear of the Dingo started to raise where the operator was standing. EE was thrown off the machine landing on the tunnel floor.

Employee was stepping off man car and when left foot touched the ground with employee's weight, employee felt a sharp burn in the center of lower back area.

Power was lost to the slope hoist & 10 employees walked approx. 1400' up the slope to the surface. EE contacted MSHA once reaching the surface at 4:45 p.m. to report the power outage. Five employees exited the mine via the Escape Hoist. All personnel were evacuated by 5:30 p.m. Power to slope hoist, communication and ventilation were restored by 3:00 a.m.

Employee felt something enter left eye after exiting # 1 kiln's baghouse. EE had earlier been working putting bags in # 3 compartment and went outside to stand for a moment. After leaving the compartment, EE removed dust mask and glasses for cleaning. This is when EE felt something in left eye.

While an employee was descending stairs at scalping screen, they tripped/twisted right ankle.

During the lowering of the loading spout at the Lime Truck Load out an employee had a piece of lime enter right eye. Employee was standing at ground level and waiting to inspect the spout when this accident happened.

An employee was seen at a medical facility on 12/11/19 for hearing loss & I received the report on 1/10/20. The findings from this visit are that within a reasonable degree of medical probability that this employee suffers from an occupational-related noise induced sensorineural hearing loss.

When exiting fixed ladder employee stepped to avoid vac hose which in turn put EE's body in a awkward position. This caused EE to step wrong on landing hurting lower back.

Employee states on 9/18/2019 employee felt a little stomach pain while loading 50 lb. boxes of explosives on underground powder rig.

Employee was walking at ground level around area at # 2 kiln, and tripped resulting in a fall. Employee fell into a rubber tire on a parked man lift. This fall resulted in a fractured nose.

During a pre-shift inspection on the portable Power Screen an employee raised an inspection door to check fluid levels. When the door latch was released the door raised quickly striking them in the area below the bottom lip. This contact resulted in a laceration and a fractured tooth.

At 8:19 a.m. the hoist slope car derailed and MSHA was notified at 8:39 a.m. No miners were on the car when this event took place. The car was put back on track and the rails were inspected with no findings. After several complete runs of the hoist slope car it was put back in service at 11:40 a.m. MSHA was notified at this time.

At 6:10 a.m. it was reported the hoist was not working. MSHA was notified by phone at 6:15 a.m. After trouble shooing it was determined that the DHSS (Electronic Lilly switch) was not in the run mode, and a magnetic switch had moved causing the error. After corrections were made and hoist tested several times hoist was put back in service at app. 9:30 a.m.

Power was lost to the main hoist last night (5/22) at approximately 10:15PM. Employee called the hotline to report the hoist being down. Power was restored at approximately 1:20AM (5/23) and employee contacted the hotline again at that time. Power loss was due to a line fuse blown on KU's end.

At approximately 4:20pm the main hoist would not reset, after a power failure and a blower fan motor failure. A call was made to MSHA at approximately 4:40pm. The hoist was back in service at 10:00 p.m. after repairs were made.

While testing the running program on the slope hoist, it was realized at app. 1:15 p.m. that the hoist would need to be taken out of service. After trouble shooting the software, problem was corrected. The hoist was tested several times and put back in service at app. 3:30 p.m.

Employee was walking from the underground restroom back to the crusher cab when knee gave out. When ee fell ee caught self with left hand causing some swelling on this hand. After seeing a Dr.it was determined employee had suffered a fractured wrist.

Employee was cleaning slurry tank. After striking a piece of hardened material dust became airborne entering eyes.

The Main Slope Hoist was kicking out due to an incorrect position on indicator. Adjustments were made and put back in normal operations at 10:30 p.m.

Injured person was moving a piece of steel from the back of a pickup truck with help from co-worker. Employees finger was cut by a piece of metal sticking downward. The employee received stitches on the left second finger.

Employee was changing hydraulic hose on scaler using a pry bar when the bar slipped and struck employee on lip causing a laceration.

Main Slope Hoist lost power due to a faulty e-stop switch. Power was restored at approximatley 1:45pm.

Employee was climbing up the steps of a 771 D truck to get back in the cab and EE's foot slipped on the bumper step. When EE's foot slipped EE's right inner thigh was cut by the hanger bracket for the steps.

Employee was loading roof bolts onto roof bolter and strained right shoulder. Missing work due to prescribed narcotic medication.

Slope hoist became inoperable at 1:30pm due to a failed solenoid on the hydraulic system. Repairs were made and operating at 4:00pm.

While bringing out a man lift from the underground mine up the inclined slope the functions of the lift failed and could not be moved blocking the travel way. Foot traffic around the stalled machine was still available.

The main hoist went down at 6:23 am on May 12, 2016. The hoist was down about 15 minutes. At 6:35 am it was shut down to check the brushes. At 7:18 am it was determined the motor had to be replaced. The hoist service was restored at 4:55 pm.

Employees were repairing hydraulic hose and while using grinder to cut hose the grinder kicked back causing a laceration on other employees hand that required stitches..

A fuse on the main line coming from Kentucky Utilities blew taking power from the mine main hoist. Fuse was replaced and hoist returned to normal operations.

Roof fall occurred in 13,14,15-J West panel around 12 deep by 100 long and 40 wide. No injuries.

Roof fall in 27wj at 84n was discovered at approximately 11:20pm. The area around the fall was immediately bermed off.

Changed electrical control switch in hoist house. Turned power on hoist would not reset. Replaced control switch and computer restoring service at 11:45pm.

The power to the slope hoist went down about 2:25pm due to storm. MSHA was notified about 2:50pm. The electric company had to replace a pole fuse to repair the issue. The power came up at about 6:30pm.

Employee stepped in slurry. Sought medical attention on 1/12/2015. Restricted duties assigned.

While descending from the water truck, the employee slipped off the bottom step and fell to the ground and sprained his knee.

While dismounting tractor employee stepped on loose rock spraining ankle.

The alarm system on the slope hoist indicated a brake issue. Hoist was shut down and repairs made. The brake adjustment relay out of adjustment. Hoist put back into operation at 1:42pm.

Main slope hoist lost power at 4:53pm and was restored at 8:20pm. Fuse had blown on the transformer providing the electric service to hoist.

Loader bucket struck the bed of the haul truck employee was operating while being loaded causing a neck strain.

Employee was operating manlift and caught 2 fingers of his right hand between the guard covering the lift controls and the back. Injury resulted in laceration requiring stitches.

Main hoist went down due to a fault in the brake system. A contact was stuck on a brake solenoid and was repaired at 2pm.

The fan was rubbing the guard on the hoist motor. They took the guard off and put a set screw on the fan which twisted off. Hoist was operational at 3:10 pm.

At 12:40pm the electricity providing power to the main slope hoist was disrupted. Fuse on incoming line was found in disrepair and replaced. Hoist was operational 1:20pm.

While installing brakes pads on CAT 772 haul truck the pad slipped out of #1 mechanic hand and struck #2 mechanic above left eyebrow requiring stitches.

At 10:45pm the electricity providing power to the main slope hoist was disrupted. Fuse holder on incoming line was found in disrepair and replaced. Hoist was in operation at 1:15 am 10/8/2013.

Main hoist found inoperable at 7:00am. No employees underground. Reported 9:04am to MSHA. Employees entered mine approximately 10:00am maintaining 2 escapeways. Timer relay on hoist PLC was replaced and put back in operation at 8:50pm.

Fall of ground above the anchorage zone in 24 West J Panel at 79 North crosscut.

Roof fall occurred at or above the anchorage point in 17 West J X 80 South.

Employee was exiting the cab of a mechanical scaler and slipped on step causing a laceration to his left hand requiring 3 stitches. No gloves were worn at the time of the incident.

As the employee was dismounting a transportation carrier he fell to the ground causing a fracture to his right wrist.

Slope hoist's DC motor had a bad brush and was replaced. Operator was made aware of the problem at 9:04AM and called in at 9:15AM to cover the 15 minute rule. Hoist was back into operation at 9:25AM which falls below the 30 minute time frame.

Employee's audiogram showed a reportable shift.

Employee reported minor discomfort in back after installing skirtboards. Stated he had prior visits to chiropractor for previous back issues. Visit to the chiropractor on 2/6 resulted in back spasms which resulted in missed work days. Unsure if incident resulted in the injury.

EE was tied off with lanyard as he was on belt preparing to rig the pingate. Client's didn't see lockout causing belt to release and EE to lose footing and fall into pingate.

Employee was underground in the mine at Carmeuse. He was laying on his back underneath the undercarriage of the 320 using a four inch grinder to grind on the undercarriage. Once he was done grinding, he sat up and the dust on the face shield(inside it)fell down and got under his safety glasses and a piece of debris entered his right eye.

While load explosives employee removed his gloves to tie the primacord together. While was pulling the cord, a small rock fell from above and struck his left hand between the thumb and index finger causing a laceration to his hand.

Employee was training on powder loading of face holes in heading when a rock fell from the back striking right leg. Laceration to lower part of leg and hairline fracture of knee cap.

Man Car lost communications with hoist operator. Replaced microphone on radio in hoist house restoring communications at 12:30pm.

Employee alleges back injury while operating loader over uneven ground.

While loading a face, a baseball size rock fell from the face and struck the employee on the right upper side of the back.

Fuses blew on the transformer that powers the slope hoist. This caused the slope car to be inoperable from 9:30 PM to 2:00 AM.

Employee reported Carpal Tunnel Syndrome case on 9/21/10. Claim was filed with Workers Compensation Carrier and after investigating it was determined work related on 2/10/2011.

Main Hoist went down at approx. 11:45pm due to a breaker failure. Repairs were made and back in operation at approx. 4:55pm July 6, 2010.

Employee was in process of removing shims from a jaw crusher. A hydraulic cylinder is used to push the jaw forward to allow clearance for removal. As pressure was being applied to the cylinder, the backing plate broke.

Solenoid sticking on one of the brakes of main hoist. When the computer senses too little or too much brake pressure, or an abnormality it sets all the brakes, until a system check is complete in the logic. Call made to MSHA 10:19am. Hoist returned to service approximately 10:40am. Parts odered for repairs.

Operator of the mechanical high scaler suffers from Carpal Tunnel Syndrome in his wrist which requires surgery.

Lost power to main hoist resulting in the reprograming of computer system. Power restored, computer reprogramed hoist resumed normal operation 2:20pm

Employee was tramming bench drill to top of bench area, when the drill started sliding on an incline. The drill struck a rib, causing the operator to strike his head inside the cab, which caused a scalp laceration requiring staples.

During normal operation of the hoist. The hoist stopped running and would not reset. The code given was another brake solenoid in fault. Repairs were made changing one of the main brake accumulators, and solenoid reset. Hoist has resumed normal operation.

During normal operation the Main Hoist stopped operating giving a halt signal message on the computer for solenoid brake switch in halt. The problem was identified and solenoid relay reset. Hoist has been operating as normal.

During normal operation the Main Hoist shutdown and would not run from the control switches in the Hoist House. A relay in the Wonder Ware was receiving a fault alarm in the Slack Rope circuit. Electricians found the tripped relay and the hoist was back in service in approximately 28 minutes, however the call to MSHA was made as a precaution.

Employee was working a long and elevated conveyor in the mine. When he descended the ladder back to the lower level, he stepped on a rock below the water level under the conveyor and sprained his left ankle. Given restricted duty.

Mine Maintenance employee was attempting to dismount a piece of equipment underground, when his foot slipped causing him to contact the tracks on the excavator with his right shin causing a substantial laceration. The employee received multiple stitches to close the cut. No restricted or light duty given.

A fall of ground happend on the main slope of the mine at Black River. This fall of ground blocked access of the main hoist. The fall of ground is approx. 250 feet down the sloped incline into the mine. MSHA was immediately notifed on their 1-800 number, and the Lexington Field Office.

Technician was working in a space with large amount of limestone & dust. A large piece of limestone entered the top of his boot and worked down to the ankle. The piece of limestone created a burn on his ankle and infection to the lower leg. Technician was prescribed antibiotics and infection cleared within six days.

At or about 3:10 pm power was lost to the mine amd some of the surface facilities due to power loss from the incoming service line. The power lost effected the Main Hoist and stop operation until line was repaired for approx. 2 hrs.

While conducting the bi-weekly inspection and operation the hosit was unable to decend down the shaft due to a build up of ice in the shaft. The build up of ice covered approx. two-thirds the diameter of the shaft. The mine was immediately evacuated and recovery plans made to return the mine to normal operation.

Employee was lowering a steel Hammer plate (side wall of a hammer tool for scaler boom) to the ground off of lift forks and was trying to stand it up at the same time. The steel fell over on upper part his right foot, breaking his big toe's metatarsal bone (up toward the ankle).

During normal operations, main hoist stoped operating, due to fault code in the direct drive system. Reset direct drive system and hoist is operating fine.

Lost power to whole mine and main hoist for 45 mins. Evacated employees to Main Shop per SOP. Troubleshooted the problem. Found bad 10 amp fuse for the main switch gear circuit board control. Called and left voice mail for Don Ratliff, MSHA Field Supervisor at 3:10 am and called back later when hoist was fixed at 3:30am.

Around 11 am, the Main Hoist lost power and would not reset. Electrican immediately called in to troubleshoot and fix problem. The microprocesser went bad and was replaced.

Around 2:00 am, main hoist began periodically running in the halt position. Then after numerous times of resetting power, the hoist finally stopped operating. Discovered that the microprocessor that runs the PC was defective and was replaced along with several data entry cards.

Employee has experienced a STS in his right ear hearing levels. Employee wears hearing aids and has for several years. Some levels of hearing have been very high or non responsive.

Mine fire occurred on P19 belt, between the new crusher and the sizer/transfer area. It started sometime between 4 am, 4-9-06 and 6 am, 4-10-06. The mine was empty at that time. Our investigation began @ 7:30 am. MSHA was notified by 9:30 am as a precaustion. A fire wasn't discovered until later after lunch time.

Mining operation was in process of changing the main hoist cable. Employees were using the escape hoist to enter & exit the mine. While 3 employees were on escape hoist, the hoist malfunctioned, suspending the 3 for approx. 15 mins. Temp. repairs made to extract miners. Mine shut down until permanent repairs made. MSHA notified at 0500 hours & left voice message.

EE was walking down a slightly rocky slope and caught right foot on a rock. He tripped and fell forward and landed on his hands. He clipped one of the bones in the top part of his foot midway between the toes and ankle.

Allegedly felt slight pain in back while putting in a idler roller for an UG conveyor. Employee waited several days before reporting injury. The injury occurred on his last day on shift and he decided not to come in due to a dr appt. for one day. we are denying claim with the insurance company.

Power outage from 0115 to 0315 7-1-05 Friday morning in Mine due to heavy storm lost power from KY Utility. Evacuated to the Shop. Called in to Roger Rowe at 7-6-05 at 10:20 am and reported. Mining staff at time was unaware of immediate reporting requirement.

Roof fall in K west panel 90 West/206 South. Dimensions of Roof fall: @ 40 ft long by 20 wide and @ 6 feet thick. This area and depth of rock is above the 4 foot anchorage point. This was an extention of a previous roof fall. Unknown time of fall. It was found yesterday on 4-14-05 and notified Lexington MSHA at @ 5pm 4-14-05.

Loss Electrical power to underground mine due to ice/snow storm. Power came on and off several times until around Friday morning, 12-24-04, it finally remained on. The mine was evacuated during that time until electrical power was restored. No one was hurt or injuried.

Employee climbed out on unprotected area of haul truck catwalk to retrieve misplaced lunchbox, lost footing and fell @ 6-7 ft on lower back/tailbone area.

EE states that she strained her neck while monitoring the crusher hammer in operation. She had her head turned to right and down some while sitting and operating crusher/crusher hammer.

WHILE LOADING HAUL TRUCKS AT THE BENCH ON ROUGH GROUND THE BOUNCING OF THE LOADER CAUSED A NECK MUSCLE TO BECOME STRAINED.

WHILE WALKING TO WORK SITE EMPLOYEE LOST FOOTING WHILE WALKING THROUGH MUD AND WATER.

EE WAS DISMOUNTING FROM THE DRILL, LOST HIS FOOTING FROM STEP FELL AND STRUCK LEFT ELBOW. IT DID NOT APPEAR TO EE AND HIS SUPERVISOR HIS LEFT ELBOW WAS SERIOUSLY INJURED, MERELY BRUISED. ON 2- 20-04 THE EE COMPLAINED HIS ELBOW CONTINUED TO BOTHER HIM AND HE WAS SENT TO DR FOR EXAMINATION AND RETURN TO WORK WITHOUT RESTRICTION AND REFERRED TO ORPHTOPEDIC FOR FURTHUR EXAMINATION.

TWO EMPLOYEES (THE INJURED EMPLOYEE AND ANOTHER) WERE ATTEMPTING TO MOUNT A HAMMER ON THE HAMMER ASSEMBLY. WHILE THE EMPLOYEES WERE LIFTING A PLATE THE INJURED EMPLOYEE FELT PAIN IN HIS LEFT S HOULDER.

OVER A PERIOD OF SEVERAL DAYS EMPLOYEE WAS USING IMPACT TOOLS. INITIALLY THIS WAS THOUGHT TO BE A NON-WORK RELATED INJURY. SUBSEQUENT TO A STRAIN TO THE EMPLOYEES LEFT SHOULDER & FURTHER INVESTIGATION BY INSURANCE CARRIER IT WAS DETERMINED THAT THIS SHOULD BE WORK RELATED AND TREATED SEPARATELY FROM 11/6/03 INJURY. tHIS IS WHY IT IS BEING FILED AFTER 10/14/2003.

EE WAS ASSIGNED TO REINSTALL A TIRE ON A POWDER TRUCK WHICH WAS INSTALLED BACKWARD BY HIM ON 9-2-03. THE EE REMOVED ALL THE LUGS BUT LAST LUGBROKE AND TIRE SPRANG FORWARD ABOUT 2 FEET, EE ATTEMPTED TO AVOID TIRE. THE TIRE STRUCK THE EE ON THE RIGHT LEG JUST ABOVE THE ANKLE RESULTING IN A FRACTURE OF THE DOTOL FIBULA.

THE MAIN HOIST WHICH IS FOR THE TRANSPORTATION OF MINERS AND ALSO EQUIPMENT WAS TRIPPING OUT ON THE OVERLOADS. IT WAS RESET SEVERAL TIMES DURING THE SHIFT. THE HOIST WAS SHUT DOWN AT 4:00 P.M. TO CHANGE THE COMPLETE ELECTRIC BOX. THE JOB WAS COMPLETED AT 7:00 P.M. AT THAT TIME THE HOIST WAS MONITORED TO INSURE PROPER OPERATION, AFTER WHICH IT WAS RETURNED TO NORMAL OPERATION.

THREE MECHANICS WERE CHANGING CONVEYOR BELT ON INCLINED SLOPE BELT 14-100. INJURED EE WAS GUIDEING THE BELT ACROSS THE TOP OF RETURN ROLLERS ONBOTTOM SIDE OF BELT. RETURN ROLLER CAME OUT OF HANGER WHICH IN TURN STRUCK EE ON RIGHT SIDE OF LOWER BACK RESULTING IN CONTUSION AND ALSO STRAIN OF LOWER BACK. EE PLACED ON RESTRICTED DUTY DUE TO LIFTING .

EE WAS OPERATING A HAUL TRUCK WHEN EE POSSIBLY BLACKED OUT. THE HAUL TRUCK STRUCK ANOTHER TRUCK RESULTING IN DRIVER BEING TRAPPED IN TRUCK FOR 45 MINUTES. ALSO EE WAS UNCONSCIOUS FOR A SHORT PERIOD OF TIME. EE HAS PRIOR HISTORY OF HEART PROBLEMS. MEDICAL EVALUATION NOT COMPLETE AT THIS TIME.

EE WAS PREPARING TO CLIMB UP ACCESS LADDER ON 988 LOADER. HE PLACED RIGHT FOOT ON LADDER AND PULLED HIMSELF UP. HE FELT A SHARP PAIN IN RIGHT LEG. DIAGNOSIS WAS A RIGHT GROIN STRAIN.

EMPLOYEE WAS OPERATING HAUL TRUCK. COMPLAINED OF PAIN IN LOWER BACK. THE MEDICAL EXAM SHOWED A BRUISED TAIL BONE. THE SEAT WAS IMPROPERLY ADJUSTED TO ALLOW FOR CUSHIONING OF RIDE.

EE WAS LOADING BLAST HOLES FROM BASKET WHEN A SLAB OF ROCK APPROX. 8" X 1/2" THICK FELL FROM BACK AND STRUCK EE ON RIGHT FOREARM. RECEIVED 3 STITCHES TO CLOSE WOUND.

EMPLOYEE WAS REMOVING JACK STAND FROM UNDER HAUL TRUCK. EMPLOYEE WAS SITTING DOWN JACK STAND AFTER REMOVING STAND HAD A METAL BURR ON THE SIDE OF STAND CAUSING LACERATION IN PALM OF RIGHT HAN D.

EE WAS OPERATING SCALING MACHINE WITH HAMMER, SCALING DOWN RIBS WHEN THE ROCK UNEXPECTEDLY CAME LOOSE, ALLOWING THE BOOM TO STRIKE THE FLOOR, JARRING THE OPERERATOR'S CAB.

EE WAS TIGHTENING BASE BOLTS ON MOTOR INSTALLATION. WAS UNDER DRIVE COUPLER GEAR BOX. EE ROSE UP AND STRUCK BACK ON COUPLER & GREASE FITTING.

EE WAS REPLACING GUARD FENCE AFTER CLEANING WAS DONE WITH LOADER TO ACCESS TAIL SECTION OF BELT. HE BENT OVER & FELT A SHARP PAIN IN LOWER (LUMBAR) REGION OF HIS BACK.

EE WAS REMOVING WELDING LEADS FROM CHUTE, CHUTE HAD BEEN CLEARED OF ROCK. SURFACE OF CHUTE WAS WET SLICK. EE SLIPPED ON WET SURFACE STRAINING BACK. EE HAS PRIOR HISTORY OF BACK TROUBLE.

EE STEPPED DOWN FROM WATER TRUCK AND STEPPED ON A ROCK, MAKING HIM LOSE HIS BALANCE WHICH STRAINED HIS BACK.

EE WAS INSTALLING ROOF BOLTS IN BACK, WHEN A ROCK CAME DOWN AND STRUCK THE EE'S LEFT HAND BETWEEN THE INDEX & WNE FINGER, FRACTURING LARGE BONE ON INDEX FINGER. ALSO REQUIRING 7 STITCHES TO CL OSE WOUND.

EE WAS PLACING NEW DRILL STEEL ON JUMBO DRILL. THE DRILL STEEL SLIPPED OUT OF HIS RT. HAND. THE STEEL MASHED THE INDEX FINGER ON HIS LT. HAND BETWEEN THE STEELS ON CARRIER. REQUIRED A SPLINT ON HIS INDEX FINGER.

MECHANIC WAS USING SLEDGEHAMMER TO REMOVE FINGERS FROM A VIBRATING FEEDER ON JAW CRUSHER.

EE WAS CLIMBING DOWN STEPS OF SERVICE RIG. WHEN HE TURNED AT THE BOTTOM TO STEP ON CABLE STEP AT BOTTOM, HIS (L) FOOT SLIPPED RESULTING IN (L) KNEE STRAIN.

EMPLOYEE HAD BEEN WORKING ON TAKE UP ON THE GETMAN MANLIFT ALL OF HIS SHIFT. EMPLOYEE FIRST FELT DISCOMFORT TO HIS LOWER BACK WHEN DESCENDING FROM MANLIFT.

EE WAS RIDING ON SLOPE HOIST CAR ENTERING MINE. SLOPE CAR HIT A ROCK WHICH WAS ON THE TRACK, CAUSING A SUDDEN JARRING OF THE EE. STRAIN TO BACK.

EE WAS IN A CLOSE AREA WAS USING A SLEDGE HAMMER& BAR IN AN AWKARD POSITION. RESULTING IN A LOSTTIME ACCIDENT. BACK STRAIN RESULTED IN LOST TIMEBEGINNING ON 6/4/01

EE WAS THROUGH SHOP WHEN HE TRIPPED OVER SCRAPING BLADE ON SHOP'S TRACTOR. HE FELL FORWARD, HE TRIED TO CATCH HIMSELF WITH HIS RIGHT HAND. HE FELT PAIN IN HIS RIGHT WRIST, SHOULDER AREA AND BA CK. HE WAS TAKEN OFF WORK BY PHYSICAL THERAPIST AND DOCTORE ON 5-30-02.

EE WAS LIFTING A SET OF PORTABLE TORCHES WEIGHING 20 LBS. WAS TWISTING TO PUT ON TRACTOR.

EMPLOYEE WAS CLIMBING ON TO SLOPE CAR WHEN SHE FELT PAIN SHOOT DOWN HER RIGHT LEG AND LOWER BACK.

EE WAS CLIMBING UP INTO JUMBO DRILL, SLIPPED ON BOTTOM STEP, REPORTED TWISTING BACK ON 1/8/02. DR. CARE ON 2-21-02. DR. PLACED EE OFF WORK TO COMPLETE THERAPY REHABILITATION.LOST TIME BEGAN ON 4-11-02.

EMPLOYEE WAS SERVICING EQUIPMENT, STEPPED DOWN FROM SERVICE PLATFORM ONTO UNLEVEL SURFACE TWISTING ANKLE.

HAUL TRUCK DUMPED LOAD OF ROCK INTO CRUSHER, BIGROCK SLID DOWN INTO (R) FRONT SIDE OF CRUSHER CAB STRIKING CORNER. IT KNOCKED EMPLOYEE AGAINSTWINDOW CAUSING PAIN IN RIGHT SHOULDER, UPPER AND LOWER BACK. LOST TIME BEGAN 11/05/01.

EE WAS TRYING TO POSITION TIRE FOR LINEUP ON STUDS WHEN STUDS SHIFTED CAUSING A TWISTING MOTION TO BACK, STRAINING IT.

EMPLOYEE WAS TALKING TO HIS SUPERVISOR WHILE ANOTHER EMPLOYEE WAS RUNNING POWDER RIG. TRACTOR WAS PARKED BEHIND POWDER RIG WHEN BACKED UP AND CAUGHT R HAND IN BETWEEN EQUIPMENT REQUIRING 4 STI TCHES R PALM OF HAND.

AT THE INTERSECTION OF 40N AND 6E IN KEASTON THE NORTHSIDE OF MINE A ROOF FALL OCCURRED. THE FALL WAS UNWITNESSED AND NO INJURIEDS RESULTED. THE FALL WAS ABOUT 50 FEET IN DIAMETER AND ABOUT 15 FEET HIGH. THIS WAS A ABOVE THE ANCHORAGE ZONE.IT WAS DISCOVERED BY EE AS HE WAS TRAVELING TO TEMCO PHONE.

THE HAND GRAB ON THE FRONT OF 06-018 HAUL TRUCK GAVE WAY CAUSING EE TO STRIKE GROUND WITH LOWER RIGHT ARM AND HAND.

STEPPING ACROSS FROM THE HOOD TO THE CATWALK BEHIND THE CAB, EE MISSED THE CATWALK AND ROLLED HIS LEFT ANKLE. ANKLE HAD SOME SWELLING.

WHILE DISMOUNTING FROM SCALER WHICH HE WAS REPAIRING, EMPLOYEE TWISTED HIS LEFT FOOT ON SMALL ROCK ON MINE FLOOR.

THE HEPBURN HOIST WILL BE SHUT DOWN 10:00 AM ON 3/9/01 & BE DOWN UNTIL 12:00 PM OM 3/11/01. WILL BE CHANGING THE HOIST GEAR BOX. THE ESCAPE HOIST WILL BE MANED AND IN OPERATION FOR PRIMARY ENT RANCE & SLOPE WILL BE SECOND ACCESS.

THE FRONT AXLE ON THE FRONT SLOPE CAR BROKE ON THE BELT SIDE. REPAIRS OF AXLE WILL TAKE APPROX 4 TO 6 HOURS. THE EMERGENCY HOIST IS FULLY OPERATIONAL AND THE SLOPE IS OPEN FOR EXIT PURPOSES DU RING THIS TIME.

EE WAS WORKING ON A HYDRAULIC PUMP LOCATED ON A 988 LOADER, WHEN THE RING THE PUMP BLEW CAUSING HOT HYDRAULIC FLUID TO SPRAY INTO EE FACE THE FORCE OF THE SPRAY KNOCKED HIS HAND HOT AND SAFETY GLASSES OFF. THE EE THEN SLID DOWM THE TIRE, PUSHED OUT AND LANDED ON THE GROUND WITH BOTH FEET, FELT HIS RIGHT KNEE GO OUT, CAUSING HIM TO FALL TO THE GROUND.

THE PLC WHICH IS THE COMPUTER PROGRAM THAT CONTROLS THE SLOPE HOIST MALFUNCTIONED AND WAS CORRECTED BY 9:00 PM. THE EMERGENCY HOIST WAS FULLY OPERATIONAL AND THE SLOPE WAS OPEN FOR EXIT PURPOS ES DURING THIS TIME.

PRODUCTION SUPERVISOR WAS PREPARING TO MARK A HEADING, AND WHEN HE STEPPED OFF THE TRACTOR, HE STEPPED ONTO A ROCK AND TWISTED HIS LEFT ANKLE.

A ROOF FALL OCCURRED AT THE INTERSECTION OF 123 AND 36N (J EAST PANEL); THE FALLEN PORTION APPEARED TO BE APPROX. 19' DEEP, 55' LONG, 45' WIDE AND DOME-LIKE.
