When employee returned to site at 1 a.m., the supervisor took EE to ER for further evaluation.

No time was lost and able to perform their normal job.

The Grader operator was pushing material to the right rib at roughly 3mph when a rock measuring 1'X 1.5'X 1.5' rolled over top of the blade and then under right rear tires of the Grader. When this occurred the seat-belted operator's right elbow hit the armrest forcefully, causing a sprain to the employee's right shoulder. The full extent of the injury is currently unknown.

Fractured rock material then fell from the area and came to rest against the right side of the bolter.

Prior to turning on the water line the employee looked toward the Jumbo and then opened a valve on an adjacent air header that was connected to an air hose, which resulted in a laceration and fracture to the nose.

State elevator inspector wanted the pit door switch moved to the striking side. Once the switch was moved to the striking side, it took on water causing it to fail. During maintenance the alimak car traveled down the shaft 50ft without anyone in it and tripped a circuit breaker.

Upon inspection it was discovered the breaker which feeds the Alimak was tripped. After resetting was unsuccessful, testing was performed on the feeders to the car. It was discovered that the cable from surface to the car was shorted out.

Two miners were working together to install utilities when they loosened the bolts on the Victaulic clamp on the air header prior to relieving the pressure. One miner received a fracture to right forearm, the second miner was treated for eye injuries when glasses were blown off, also received soft tissue injuries to the left forearm.

While completing a thorough inspection of the face (face prep) the miner was hit on the top of the hard hat by loose material. The material came off of the face above the location of the miner. The injured miner required staples to close a small laceration located on the top of EE's head. Post treatment, the injured received a full duty release.

During a post blast inspection of the 215 level, a fall of ground was identified in the 1485 stope. The ground failed to an unidentifiable contact in excess of 20 feet above the back of the stope. The area was supported in accordance to the ground control management plan.

A mechanic/electrican apprentice was using a Milwaukee utility knife to strip the outer jacket off a 410 cable; he was pulling the outer jacket with his left hand and cutting around the jacket with his right hand; the blade slipped and cut his left palm. Employee was wearing cut resistant gloves at the time. Employee received 5 stitches on his left palm.

While patrolling the south fence the security guard was making a loop to turn back onto the trail. There is a slight lip where the trail is lower than the snow the guard came off. The snowmobile was going very slow at the time. IT tipped on its left side and the guard rolled off as it tipped over on its side. No damage to trail/snowmobile. Guard reminded to use extra caution.
