Raw Mill extension chute broke and fell, landing near to and injuring an employee. Employee was taken to hospital for evaluation.

Employee was reinstalling a port cover to an access port on the 565-SC1 screw conveyor (which was locked out), and the employee reported that the cover sucked in quickly, pinching EE's finger between the port cover and the port opening.

Fire occurred on 8/30/2024 at approx 3:30am but was contained quickly(approx 25 minutes). Our local MSHA Office recommended we report the incident via 7000-1. Buildup of hot material in the Clinker Cooler resulted in the hydraulic seal to the equipment melting and hot material caused a hydraulic oil fire in a small piston compartment on the South side.

Employee was trying to remove a dust collector bag/cage from a locked out screw conveyor and they leaned their torso against the metal frame of the equipment, when they felt a pop in their ribs. Employee didn't feel any pain but finally reported the incident at 7:00 am on 7/1/2024

Injured employee was working in a small group, removing bags/cages from a dust collector compartment for 3 days. The employee felt sore in EE's left shoulder at the end of day 1 but was feeling increasing pain and discomfort in EE's left shoulder. The employee decided to seek doctor evaluation on 5/16 and did not report to EE's Supervisor that EE was hurt until 7am on 5/16.

Conveyor belt fire that exceeded 30 mins.

Was using a hammer and accidently struck team members own left hand. Originally report was just a first aid, however due to increased swelling, team member went to Dr. on yesterday.

Employee was putting back on a metal side panel guard, when left ring finger was smashed between two guard pieces resulting in an laceration and fracture to the finger tip.

EE was struck by raw mill shackle on right ankle. The impact caused the ankle to dislocate and a fracture of the medial malleolus/distal fibula.

Team member was walking from the raw mill to the PSB and allegedly walked through the CKD mud that was located off of the designated pathway.

Employee was changing out a liner in one of the pigment loading bins in the Rainbow tower. Employee was pulling the old liner out of the hopper and bumped the lid with the liner. When the liner bumped the lid, the lid fell on the employee's left pinky finger. Resulted in finger tip fracture and 14 stitches.

EE reports EE was trying to pull the guard off of a counter weight due to material build up. Once the weight came loose EE reports EE fell backward and stuck out EE's left hand to brace self. EE is right hand dominant and has no open wounds from EE's fall. Resulted in closed fracture.

3rd party owner operator (3PO) failed to appropriately raise the safety platform. 3PO failed to depress the safety pedal to disengage the lock while pulling on the raise rope. Rope popped, causing 3PO to fall onto the platform. 3PO complained of right knee pain. 3PO desired evaluation and transport via EMS.

Lab technician was rolling a sample mill out of the grinder in the lab when they pulled a muscle in their back.

Employee was handling, carrying and using cardox tubes throughout the shift to clean the tower. Began to feel pain in EE's upper back.

EE was assisting with the removal of the burner pipe from the lance. Employee slipped and fell while balancing the pipe.

Employee was preparing rigging on a 240 pound flywheel when it rotated slightly, pinching right thumb between the flywheel and the structure it was resting on.

Employee was walking behind mill building when they slipped in a water puddle with algae growing in it. Employee fell backwards and hit their hardhat/head on the concrete.

Employee attempted to open rear door on loader to fuel it. Rear door was hard to open. Employee strained their shoulder.

Employee was crawling under a D11 dozer to help replace a hydraulic line. At the end of shift employee reported discomfort in the knee. Doctor recommended an MRI. MRI results today revealed a torn MCL. Normal work without restrictions or time off at this time.

The employee was engaged in vacuuming cement dust when they felt a sharp pain in right eye, followed by a burning sensation. The employee's eye was flushed and they returned to work. That evening, the employee went to a medical facility. The employee was diagnosed with a corneal abrasion, treated and released with a prescription for eye drops.

EE was adjusting a limit switch on stacker. As EE adjusted, EE pushed on lever and opposite side of lever impacted EE's right hand. Resulted in a cut and bruising of the middle finger of the right hand. Employee was diagnosed with fracture and cut put together with glue, meds given.

Employee had finished cleaning the hot gas generator fan 353-FN1. While closing the door to the fan, they felt a discomfort in their back, due possibly to "moving the wrong way". EE reported to mgmt on 9/19/19 and nurse prescribed home care, EE went to doc on own on 9/20 and given injection.

EE was building guard using grinder. Guard was clamped to table but EE was using left index finger to help stabilize. Grinder hit protrusion, caught EE's glove between disc and grinder guard. This caused the wheel to cut the employee's glove, pull EE's index finger towards the wheel and cut EE's finger. Reqd 6 stitches to close.

EE was in the discharge of cement mill #4 removing buildup from discharge screen. While exiting the mill, EE's right foot was held in place at the door opening while EE stepped out onto the ladder with left leg. EE was ok at the time, reported just in case. Still had discomfort and on 9/13/2019 was issued physical therapy and restricted duty for sprain.

A scaffold was being dismantled in the clinker cooler at the kiln discharge (riser duct). A piece of buildup from the kiln nose fell and struck EE in the back of EE's hard hat. EE climbed down the scaffold and laid on the ground beside the scaffold refusing treatment until the EMT's arrived. EMS took EE to the hospital, and diagnosed with a slight concussion and released.

Employee was cleaning under the packaging rainbow line and felt discomfort in EE's lower back after picking up a partially filled cement bag. Same day self-care measures & OTC medication used; however, discomfort continued so EE was seen by occupational clinic on April 25 and placed on restricted duty.

Employee sliding a dam ring out the de-energized Coal Mill. EE twisted, bent over, and manually slid the heavy metal part resulting in a back strain. Same employee had non-work-related back surgery in Nov. 2018. Occupational doctor placed employee off work due to pain, limited mobility, and complexity of issue. Employee to see neurologist and complete diagnostic testing.

A small piece of scale/debris fell inside the bottom stage cyclone and struck the employee on the backside of EE's hardhat resulting in a laceration on the back of EE's head. Nine staples were required to close the wound.

Employee entered the Packaging Rainbow doorway and felt a pop in the right knee. EE stated they did not feel any pain at the time. They continued walking about 50-feet and stopped at a desk to get something to drink. When leaving the desk they turned and felt a pop again and experienced pain in the right knee.

Employee found collapsed and having health trouble (possible seizure) in the Main Office downstairs hallway. This incident is a result of a pre-existing health condition that is not work-related, and non-compliance with prescription medication.

Two employees re-installing a gearbox to a roller crusher. One employee was underneath the gearbox tightening the last two nuts overhead. The wrench slipped from the nut and fell about 18 inches striking their two upper front teeth resulting in both teeth being partially fractured. Both teeth were filled at a Dentist, and the employee returned to work without further issue.

**Non-Miner** Contract over-the-road flat-bed trailer truck driver tarping cement bag load at designated safety platform. Driver did not align the trailer properly inside the safety platform. While tarping, the driver struck the platform handrailing resulting in a cut on right forehead needing stitches.

A Material Handler had finished tightening the man way bolts on top of a rail car and then dropped a 5 pound socket on the top of right foot near the ankle. Resulted in a contusion that required 2 days of restricted work duty. The employee has been fully released.

Employee reset master fuel trip on kiln burner floor. While walking away at South side kiln hood hot dust & gas blew out of a view port & struck employee on left side. Employee received burns to left ear, neck & cheek. Employee transported to ER & was released. On March 3, employee treated for burns at burn clinic & issued work restrictions to allow time to heal.

Contract employees were removing liner plates from Finish Mill #5. A jackhammer being used outside the mill forced a bolt inside the mill and struck a contract employee's right forearm resulting in a laceration that required 8 sutures.

While backing the fork truck and looking over right shoulder, the employee felt a "snap" or "pop" in right elbow, including numbness in the middle, ring, and little right hand fingers. Employee did not hit or make contact with anything. Same day medical treatment included use of sling and restrictions. Reexamined by medical professional and fully released on July 13.

Inside clinker cooler, backing and turning walk-behind skid-steer machine and machine turned faster than normal due to metal grates and clinker material. This caused skid-steer to catch employee's right knee between clinker cooler inner wall and machine resulting in swelling and restricted duty (soft tissue injury).

On May 18 employee claims while cleaning under raw mill rotary feeder with air lance experienced pain in right elbow. Employee did not report. On June 20 employee reported on-going elbow pain to supervisor. On June 22 employee was evaluated by medical professional, diagnosed with tennis elbow, and issued temporary work restrictions and to wear elbow band-brace.

Driver was attempting to dump a load of BASF material. The load would not come out so the driver was lowering bed when the load shifted and the truck and trailer turned over.

2:55am employee was short of breath with chest discomfort. 9ll was called and 3:30am ambulance arrived onsite. 3:45am we received a call that another EMS responder was on the way. At this point we knew the situation was life threatening and MSHA was notified at 3:56am. By 6:30am we were informed of the employee's death.

Ee attempted to place hitch into vehicle receiver, hitch slipped out of receiver, pinned left hand little finger in the hinge mechanism (Pintle type), and hitch's weight (20.4#) dropped resulting in fracture of distal phalanx. Ee went to ER same shift. Ee examined by Orthopedist and fully released on Aug. 31.

Lime silo 4-inch port cap blew off chute between lime silo and feeder causing lime to flow out which got into employee's eyes underneath safety glasses. Employee treated at ER resulting in Rx eye drops and restricted work duty.

Maintenance man injured right thumb attaching trailer for welder to hitch of pickup truck. Miner taken to ER for initial treatment and returned to work Dec. 11. On Dec. 14, for first available evaluation, orthopedist issued restrictions and confirmed distal tip fracture. Restrictions are being accommodated, no lost time expected, and full recovery expected in 8-weeks.

EE had Synfuel splash in his eyes that got beyond his safety glasses while unhooking a hose with pressure on it from a tanker truck.

Fire was first spotted by an ee, and then water was used to extinguish the fire. Inspection of the bin revealed a hole approximately 1 ½ in diameter. Hole would have allowed outside damp air to come into contact with the powdered coal which resulted in oxidation followed by the fire. This hole was repaired prior to putting the system back into service.

*3rd-Party Consultant* held flange with left hand to stabilize. Mine Operator Plant Electrician used reciprocating saw to cut rusted, seized bolts from flange. Saw jumped and its blade cut into the glove of the left thumb of the *3rd-Party Consultant*. On Sep. 11, *3rd-Party Consultant* was examined by orthopedist and told surgery is needed to repair left thumb's extensor tendon.

Third-party truck driver alleges he strained his lower back after he dismounted from the cab of his truck. He alleges his left foot stepped into a hole on the ground and he twisted his left ankle resulting in him almost falling. The treating physician issued work restrictions and pain medication. The truck driver was unable to return to work and lost four scheduled work days.

Employee pickup debris from work area. He picked up a piece of expanded metal and it cut/punctured his middle finger on right hand through his gloves. Until 4/15/15 this injury was first aid only, no lost time until rechecked by doctor and it was determined it needed additional treatment.

Injured employee was cleaning at Main Office downstairs women's bathroom and backed her left elbow into a sharp, broken door latch. The latch lacerated her left elbow. She received medical attention requiring 3 stitches and returned to work resuming her normal work duties.

While jackhammering in discharge end of Finish Mill #5 employee bumped his left elbow on the water spray pipe on Jan. 7. Reported to management on Jan. 12. Received medical attention on Jan. 13, 16, & 23 with no restrictions and fully released on Jan. 23. Left elbow issue continued. Received orthopedic exam on Feb. 9 resulting in work restrictions, which are being accommodated.

Employee was using a 6-ton come-along trying to move an I-beam into position as he torqued the come-along the pad eye came loose & flew back striking him on the left leg shin gashing his leg open resulting in 6 stitches to repair.

Contract cleaning employee climbing stairs while carrying items in both hands lost balance and fell; however, caught herself with left hand. She received medical treatment the night of June 3 at emergency room. Medical treatment included sling for left arm & shoulder and out-of-work excuse for 1-day June 4. Injured contract employee reported injury to mine operator on June 5.

Employee was coming down off a ladder when he stepped back tripping over an air hose he did not see behind him. He put his hand out to break the fall fracturing the 3rd finger of his right hand.

Employee at Coal Mill using a hammer to fill the distance between the tie rod eye he was pushing out with a hydraulic jack. Surface of the hammer not being flat caused the hammer to slip out and the hydraulic jack struck the employee on the mouth. Injury resulted in one tooth being broken and another fractured.

Employee operating hand grinder with right hand while grinding a chain conveyor connector pin being held with left hand. Connector pin believed to have caught grinder's flap disc, then ricocheted around the grinder's guard, and struck his neck causing a puncture requiring stitches. Employee's body position allowed the connector pin to pass under the face shield.

Walking near pond behind main office when wind-blown material entered right eye around safety glasses. First aid eyewash solution was used. Overnight eye became red and irritated. Medical treatment was administered next morning, including removal of sand-like material and prescription eye drops. Employee returned to work without restrictions and no lost time.

Injured employee had finished a task at the Coal Mill then stepped backwards, turned around, and lost his balance/footing resulting in a fall to the ground. Injured employee caught his fall with his hands and suffered a closed fracture of the carpal bone to his right hand's little finger. Injured employee's medical treatment includes a splint and restricted work (no lost time).

Employee was, bent over, throwing clinker down a chute at Clinker Cooler when he twisted and felt something in his lower back pop. It has been determined employee was using an improper lifting technique and body position.

EE was opening a ball valve located on the tank farm and in the process of doing so sprained his left shoulder

4 EE's were removing coal pipe from inside the Coal Pfister Room. The ee unsecured the flange as 2 others began to lift the pipe with a come-along. When it was being removed it began to swing and swung into the hand of the ee which was resting on a nearby pipe unrelated to the task.

EE was closing a valve on a tanker truck and in the process of doing so the valve sprung back on his right thumb

EE was clearing a cardox port with a pipe and a hammer. As the ee struck the end of the pipe with his left hand, his right hand was caught in a pinch point between the pipe and the cardox collar. As a result, his right index finger suffered a laceration which required 8 stitches.

EE had worked on the pre-heat tower for a full shift during process upset conditions. As a result, a majority of the day was spent in aluminized heat resistant suit and the ee was working near elevated temperatures. EE felt ill after work and visited a local clinic and was diagnosed as being dehydrated. The physician placed the ee on heat avoidance restrictions for two days.

Lightning struck near a rail car of hazardous waste that was being unloaded. A fire occurred at the dome lid and was extinguished within 15 minutes. There were no injuries; a hoist and a couple of roof panels needed to be replaced.

ee was attempting to start a metal lathe. Once the lathe was started the ee realized that it was operating at too high of speed and was reaching down to turn of the lathe. As he reached down he was struck by a piece of round stock in the metal lathe on the hard hat. This caused the headgear from his hardhat to cut his right ear.

Hot work activity was conducted at the Solid Fuel AFR system which likely caused a ignition point which caused a fire on the V93-BC2 transfer belt. Fire caused approximately $30,000 in damages to belt transfer systems.

EE was attempting to lift a small steel plate (12" X 10") and dropped it on his left shin. Treated with 3 stitches.

EE was attempting to lift a small steel plate (12" X 10") and dropped it on his middle fingers. The tip of the middle finger was fractured.

EE was raising the boom of a portable light plant. While raising the boom, the cable pulled free from the boom causing it to fall. As the ee attempted to prevent the mast from falling, it struck his left wrist.

EE was removing the pfister feeder housing bolts. He was pushing an adjustable wrench when it slipped from the nut. The wrench hit a side brace of the pfister feeder frame, pinching the tip of his left ring finger.

While working on the coal mill, ee stepped on loose piece of coal, which caused him to twist his right knee. His knee continued to bother him and he was scheduled for a MRI. On 10/22/2010 he completed an MRI, which indicated a torn meniscus in his right knee.

EE was in the process of lowering 60# liner plates through the mill door. While stacking the liners, one of the liners slid into another catching the ee's left pinky finger. This resulted in the employee receiving a laceration which required 6 stitches.

EE was shoveling material from the tertiary air duct when he noticed discomfort in his side. After first thinking it was a pulled muscle, the ee went to the hospital after leaving work and was diagnosed with pneumo thorax. The treating Dr. was not able to determine whether the injury was related to work activity or not and the company will continue to investigate the applicability.

A brush fire on the company property was discovered on an inactive portion of the site in the middle of a marsh. It did not effect active mining areas and burned itself out approximately 1.5 hours after being discovered. There was no active work in the area over the last several years.

EE was attempting to slide a guide pin onto a pallet to evenly distribute the load. As he pushed against the guide pin a metal burr cut through his glove and caused a laceration to his left hand.

EE was putting drive motor on the cooler in a restricted area. He twisted awkwardly injuring his knee. He was seen on 12/22/09 and continued working while the treating physician wanted to await progress. A follow up 2010 MRI revealed a torn meniscus which was surgically corrected on March 2, 2010. Since then, the ee has been placed on restrictions awaiting complete recovery.

EE was attempting to unplug a pressure tap on the Kiln Pre-Heat cyclone when hot material blew out and struck the EE. The EE removed his PPE, he found that he had suffered burns to his forearms.

ee was carrying an input shaft assembly for a gearbox down a flight of stairs. The ee reported that his back was initially sore, however, it stiffened up overnight and became worse.

A belt fire was identified approximately 12 hours after contractors had conducted hotwork activities in the area. It is believed that accumulated combustibles between the head drum and belt smoldered for a period of time prior to the fire starting.

Employee was walking across the gravel parking lot towards her loader when she stepped on a rock and twisted her ankle.

During shutdown of coal mill, smoke was seen in duct to the baghouse. The Control Room intiated corrective actions. Flooding the system with pressurized inerting gas which resulted in an explosion door being blown open in the baghouse. The investigation shows that sudden pressure within system was likely a result of the inerting system versus an explosion of coal within the system.

Driver had an asthma attack while unloading. Medical reports and investigation are inconclusive as to source of irritant.

While setting wear plates inside of Raw Mill the employee was holding the plate on the table with his left hand; he looked away and the plate moved. He turned quickly back around grabbing the plate with his right hand and pinched his 5 digit (Pinky finger) of his right hand against the plate and the roller pair resulting in a cut that required 3 stitches.

The employee went to the pan conveyor and proceeded down a flight of stairs. As the employee entered the area, he stepped into 5 to 8 inches of hot material with his right leg. The employee received 2nd degree burns from his shin downward to his right ankle.

Employee, while driving work cart to Raw Mill, broke his left leg. A preshift report came out of side consoul and he leaned to his right to grab it, the cart swayed to the right and he over-corrected his steering, tipping the cart over, resulting in a fractured left leg.

Employee, while working with anson rail pry bar to close a rail car bottom chute, lunged forward and bit into his lower left side lip. The bite resulted in 4 sutures.

Moving a piece of equipment (large shakle in raw mill). The equipment moved suddenly and pinned EE's knee between the equipment and wall.

Employee was welding. Hose broke loose off torch and started a fire to clothing, burning his stomach.

Employee, while working on automated bagging machine, complained of sore lower back. On Dr visit 21 Nov 06, he was diagnosed with sprained lower lumbar muscle(s) and placed him on modified duty.

While the EE was lifting a hydraulic cylinder, his right index and middle fingers were "pinched" in the object. As a result, the EE was diagnosed with a right index finger fracture.

Employee was cutting off a tie wrap on a bag with a knifte. He cut his left thumb.

While EE was lifting bags of sugar in the finish mill, the EE twisted his back.

While transferring materials, employee came into contact with residual fluids (acetic acid/toluene) left in transfer line. Employee showered and was taken to doctor for evaluation. The employee was treated for 2nd degree burns to his left forearm, stomach and left leg. The employee was released to return to work the next shift.

Demolition of Ball Mill by means of torch cutting. Torch cutter burns left leg when he ignites pant leg with burning slag that was inadvertently directed towards his leg due to awkward cutting angle and rebounding slag. Second degree burn from bottom of knee to just above ankle approxiamtely 3-4 inches in width. Burn was superficial (no deep tissue damamge) but very painful.

EE was driving a shaft through a platico value with a hammer, when he struck his middle finger with the hammer.

EE was climbing stairs while lifting a motor and gear rducer when he twisted his rt knee.

EE was installing a head (door) located on the east end of the mill. EE bent over to lift the head and felt a sharp pain in his lower back. EE sustained unknown injuries to his lower back. Started losing time as of 10/7/05.

The EE was cleaning a feed shelf when dust fell into his eyes.

The EE was attempting to connect a pump off hose. As he struck the hose with a metal pipe, the pipe struck his finger. The EE ststained a contusion to his left fifth finger.

On June 22, employees reported hearing a loud thunder-like noise. An immediate investigation was initiated to determine the source of the unexplained noise. Personnel discovered that 3 gaskets were blown out on the oxygen enrichment channel. Deformation on the pipe and flanges was observed. There were no employees working in the area, no injuries sustained.

EMPLOYEE WAS CARRYING BOX UNDERNEATH SCAFFOLDING.

EE was at raw mill location on 2nd shift maintenance pulling and adjusting an air nozzle, when he felt a pull in his lower back, resulting in a strain.

EE was assisting another EE move a pallet, using a prybar. The prybar slipped off the pallet, smashing his right thumb between the prybar and a beam. Specific injury is unknown.

While EE was lifting cement bags and placing them on a pallet, he injured his back. This resulted in an unknown injury to his back.

The EE was reaching his left hand into an air slide to clean it when he came in contact with a lump crusher. EE sustained several puncture wounds to his lift hand and some tissue damage.

EE WAS HAMMERING CONCRETE OFF STEEL BEAM WITH SMALL WELDING HAMMER. EE WAS STANDING, STRADDLING METAL PIECE, & SWINGING HAMMER WITH RT. HAND. HAMMER STRUCK EE'S LOWER INNER L. ANKLE. TAKEN TO DR. WHO FOUND NO BROKEN BONES. IMPACTED NERVE WITH SWELLING & BRUISING. NO LOST TIME BUT ON RESTR. DUTY UNTIL 3/7/05 RE-EVALUATION. 10/11/05: NO RESTR. DUTIES & RX - PAIN MED. ONLY.

While the EEwas entering the sweeper truck, she felt a pain in her knee. As a result EE sustained a sprain to her rt knee

EMPLOYEE HAS HAD TOLERABLE BACK PAIN OVER THE YEARS FROM AN INJURY HE RECEIVED BACK IN 1998(?) ON ANOTHER WORK SITE (NOT MSHA). HIS BACK BEGUN TO INCREASE IN PAIN DURING THE COURSE OF WORK IN THE LAST FEW WEEKS. JUST THE WEAR AND TEAR OVER THE YEARS BROUGHT THE PAIN TO WHERE HE IS NOW SEEING A BACK SPECIALIST.

EE was walking downstairs when he felt a sharp pain in his rt knee. He sustained a sprain to his right knee.

While EE was working on the rainbow bagging machine at the rainbow area, he turned off the machine to check the broken bolt. The upper portion of the bolt snapped down by air pressure and struck the EE's right index finger.

EE FAILED TO CLOSE PROPER VALVE TO SHUT OFF FLOW OF MATERIAL PRIOR TO DISCONNECTING A HOSE USED FOR UNLOADING TANKER TRUCKS

Employee was moving bags of product from south doorway of Rainbow Facility. When he bent down to lift bags, on way back up felt pain in lower back.

EE. was making rounds, when he slipped and fell on the wet dust which appeared to be dry to him. He sustained an injury pain to his rt hip and elbow.

While walking down steps of Bucket Wheel Scooter, Employee stepped on marl and twisted left ankle.

Employee was manually manuvering metal rails already loaded on the forks of a forklift, the rails shifted which clipped the end of his middle finger on left hand. This caused a smashed finger tip.

Material handling while working on concrete silo roof. Heat index for the day was 105 degrees.

Employee stated he raised his face shield and then small pieces of ground up material(from grinding with a hand grinder), slipped down behind the upper part of the face shield and got into his eye.

Employee was checking the cooler fan and something stung his Right Hand

Employee manual material handling a few metal drums; lid on one drum was damaged, drum shifted causing a small laceration to back of right hand.

EE STATED THAT WHILE WORKING ON A MOTOR/BRAKER, DEBRIS FLEW INTO HIS LEFT EYE.

EMPLOYEE WAS SHORING I BEAM WHEN A PIECE OF SHORING SLIPPED KNOCKING EE TO GROUND. THE SHORING LANDED ON EE'S HIP CAUSING FRACTURE OF PELVIS.

WHILE EE WAS TIGHTENING A BOLT BY PULLING THE WRENCH TOWARD HIMSELF, THE WRENCH SLIPPED AND STRUCK HIM IN THE FOREHEAD. THIS RESULTED IN A LACERATION IN HIS FOREHEAD.

EMPLOYEE WAS PERFORMING MAINTENANCE REPAIR WORK ON A LOWER DAMPER OF THE RAW MILL BY-PASS DUCT (EQUIP #423LD3). EMPLOYEE ACCIDENTALY STEPPED INTO A PILE OF HOT DUST MATERIAL, CAUSING BURN TO T OP OF LEFT FOOT. EMPLOYEE DID NOT THINK THIS MATERIAL WOULD BE HOT ENOUGH TO CAUSE INJURY BECAUSE THIS SYSTEM WAS SHUT DOWN SEVERAL DAYS BEFORE.

EE WAS CLEANING OUT DISCHARGE HOPPER. MATEIRAL BROKE FREE AND PINNED HAND AGAINST HOPPER WALL EE SUSTAINED FRACTURE TO RIGHT PINKY FINGER.

A PALLET WAS ON THE FLOOR IN FRONT OF EMPTY PALLET LIFT. EE LEANED OVER TO SLIDE PALLET BACK AS THE PALLET FORKS CAUGHT A PALLET ON THE BOTTOM OF THE STACK THAT HAD A BROKEN BOARD. THIS SHOOK THE STACK CAUSING THE TOP PALLET TO SLIDE OFF CATCHING HIM ON THE LEFT EAR.
