Employee was opening a hinged guard to add oil to a machine that was down. Employee finger pinched in hinged guard causing fracture and laceration.

Employee was putting away EE's tools for the day and got cement dust in EE's eye causing it to become irritated.

Employee was unfolding a forklift attachment to move barrels, as EE unfolded the attachment EE felt a pain in EE's left arm. Employee reported to supervisor and attempted to treat with OTC meds until EE requested to seek a doctor on 8/7/2023 at which time it became reportable.

EE was using a chipping bar to clean a chute on the pug mill. EE hit the side of the door with pinky finger causing laceration and fracture.

EE was performing dye test on dust collector when door to compartment slammed shut on right hand. EE received several lacerations to hand.

Two employees reported that they were stuck in the pre-heat tower elevator between the 4th and 5th floor. RICO elevator tech released both employees on the ground level at 2151pm. No injuries or illness were reported.

Employee was ridding elevator to top of silo to take measurements. The elevator braking system locked up causing the elevator to become stuck. RICO elevator arrived and released the elevator allowing the employee to exit the elevator at 0159am with no injuries.

Employee was using a bobcat to lift an old pipe guard a foot off the ground and move it. Employee noticed that pipe guard started to slip so they went into reverse and the pipe fell out of the bucket and the bottom left leg of the pipe guard went into the cab of the bobcat and struck them in the left ring finger causing a laceration.

Plant was running on overflow due to upset in kiln. Pan conveyor 800 was not shut down and carried hot material to C-41 causing skirting and belt to catch fire.

Employee was using a shuttle car to move two rail cars, due to wet slippery conditions shuttle car unable to stop. Shuttle car and two rail cars went to derail and ran off track. Employee worked for several days and did not request medical until 4/19/22 no restrictions given released full duty. Second Medical 4/22/2022 is when restrictions issued.

Miner parked loader, descended ladder and tripped over a wheel chock on the ground landing on left side /arm. After subsequent evaluation the next day miner was placed on restricted work.

Operator entered new silo elevator to access top of silos, elevator stopped after about 4 ft. of travel. Elevator Technician arrived on scene and found that the coupling had disconnected from the gear box. Connection was re-established, elevator tested and placed back into service. No injuries.

During outage contractor riding in elevator tried to open inside elevator door before reaching landing. This caused the elevator to stop and the rope gripper to engage. Elevator technician arrived on scene released contractor. No injuries reported.

Employee was removing chunks from silo during maintenance, when a barrel valve was activated. The right wrist was grazed by the valve causing a minor laceration.

Employee was riding elevator down to ground level, when the coupling set screw backed out due to vibration causing the motor to spin but the coupling was not engaged. Elevator technician tightened screw. Elevator was lowered, and tested with no further issues. No injuries or illness is reported

Employee was climbing down a ladder, believed they were at ground level, stepped back and fell aprox. 3-4 ft to the ground.

Rig welder was feeling ill. Plant staff was notified and escorted subject to a job trailer. Personnel noticed signs of dehydration and called for an ambulance. Ambulance arrived at job trailer and administered an IV with fluids. Welder recovered and was not transported. This was around the end of EE's shift so EE went to hotel for rest. Welder reported back to work the next day.

Employee was backing a WA600 Front-end loader when EE struck a pot hole causing a sudden jerk.

Employee was climbing down the ladder of a 980K loader when the bottom rubber step gave way while performing routine maintenance. Employee fell approximately two feet to the ground striking back and left wrist.

Employee was cutting the skirting off a conveyor belt during a belt change. Using a utility box cutter ee slipped and received a laceration to left forearm.

Employee was replacing an inspection door after performing work when EE pinched/crushed right index finger between the approx. 10 lb. door and the housing.

The vent cover of the in cab overhead AC unit (air mover)fell off. The operator attempted to replace the vent cover, as they pushed up on the vent cover to place in back, in the air mover, it slipped causing their fingers to enter the unit. The fan blade struck the operators fingers causing a laceration on the middle and index finger of the right hand.

Lab tech transferring cement powder into a bucket after testing received cement dust in eye.

Employee (Maintenance Lubricator), was walking in plant slip/trip/fall spraining right thumb.

2/6/2018, employee was walking on ground level walk way, a hose from a super sucker vacuum truck was lying across walkway. As employee attempted to step over hose EE tripped and fell. Employee injured right knee. Employee was able to continue to work regular work and shift at full time status. 7/31/2018 employee diagnosed with meniscus tear and received surgery.

On 1/11 employee slipped and strained knee. Worked two scheduled days was then off two scheduled days. When employee returned to work on 1/16 requested medical was seen and prescribed physical therapy with restrictions.

While lowering a VFD (variable frequency drive) the injured and an other employee were going to bring down this 120 Lbs. unit from shoulder height. Injured employee had the heavy end on the unit and as it got to waist height the weight shifted to the side as they bent over. causing a pain to the right inguinal area.

At approx. 1:15 pm our Old silo elevator stopped with one miner inside. This miner was later rescued unharmed.

At approx. 11:20 am the pre-heat elevator stopped with one miner inside. That miner was rescued unharmed.

An alarm was sounded and confirmed that we had a fire inside our Coal Mill building. The fire department was called and found that C-720, WF-720 and WF-721 were on fire. They put the fire out, but not within 30 minutes.

While walking, ee slipped on clinker landing on left forearm & wrist.

While working on a water pump, ee alleges something stung ee on the right elbow. 6/4/2017 the elbow was swollen with pain. 6/5/2017 Dr. said it appears to be infected or fractured.

At approximately 11:20 pm the pre-heat tower elevator with four miners inside stopped between floors 3 & 2. Miners were in constant communication with outside personnel. At approximately 2:16 am the elevator repair person arrived on site and the miners were removed at 3:17 am.

Carried a bundle of conduit, as employee twisted to place it down on the floor. Employee's lower back popped.

While exiting a man-lift basket, EE claims EE stepped & slipped on a rock on the ground. EE had no pain to right knee, until mid day same day.

While kneeling on right knee, went to stand-up. Left knee popped causing pain to outer knee area.

At 07:30 pm a supervisor noticed coal smoldering on the southeast corner of the east side pile in the coal storage building. Supervisor ordered a loader to start to open the pile up. At 11:20 pm the smolder was smothered.

Walking down stairs in FM-5 from second step to ground floor. Twisted right ankle, felt a crunching.

While driving a golf cart in the direction of the sun, he was blinded by the sun. He drove into a concrete column on the roadway.

Found a small smolder of coal on the east side of the ramp in the coal storage building, it was considered totally out at 10:52 pm.

Hot clinker was feed onto C-41 belt, causing the belt to stop and start to burn. This belt took longer than 30 minutes to stop burning.

While walking on top of silos, he slipped & fell on wet flooring to same level. As he tried to break his fall by using his right arm, it took the brunt of the fall.

Miner reported he was dizzy, he was taken by Supervisor to Hospital where he was x-rayed and evaluated and diagnosed with Heat related illness

At 9:33 pm a supervisor noticed hot spot on the coke pile, he investigated farther and discovered that infect it was smoldering. he ordered the loader operator to start to open the pile up. at 11:40 am the smolder was smothered completely out.

While working in coal mill, coal dust ignited causing burns to three workers.

While securing dome lids on customer trucks, he went to close the lid on a truck. As he did the driver thought that it was ok for him to proceed. As the truck drove off from that station, the employee securing the lids fell, but was wearing fall protection.

While working on bin 24 breaking-up some build-up material, using a chipping bar, his right hand was placed at the end of the bar. a large chunk of material hit the tip of the bar, causing the end with his hand to go up striking an I-beam.

On 3:15 pm EE called by phone to the control room, a co-worker answered the call, EE stated that he was inside the PE-1 elevator and that the elevator was not moving. Co-worker asked if he was hurt in any form or fashion. EE informed him that he was fine. He was advised that the elevator techs were being informed and they would get here as-soon-as possible.

At 1:45 pm shift foreman noticed some smoldering/smoke coming out of the upper west corner of outside coal stockpile. It was inside the pile about 15-20 foot. Loaders were called to dig-it-out within 30 minutes, but they were un-successful. At 4:00 they finally hit the hot spot and were able to extinguish the smoldering coal out by smothering it on the ground.

While walking-up the stairs, he felt a pop and pain to his left knee.

While removing a 70 Lbs chain fall from I beam located 10feet on roof, he went to lift the unit and sustained a hernia on left side of lower abdominal area

Employee was walking out of silo #12 due south next to a parked truck getting loaded. The truck pulled out of the loading area, striking him on the left thigh.

Was using a Bobcat, cleaning the drainage area near the elbow dump area. When mud fall from his load hit the ground, some hot dust/steam came-up to his face and ears.

While removing ball drop doors. while using a hammer he hit the fleshy area of his left hand between the thumb and the index finger.

Was stacking bags, when a bag hit his right thumb, bending it back.

While lifting a wheel barrel with product, he felt pain to his lower back.

While using a cutting torch, above ground. the slack hose caused the torch to slip off his left hand and burn the forearm on his right hand.

Dust blew into left eye, when he turned on the AC on the loader.

The scaffold upright (vertical)supports have receiver cups that are welded by the manufacturer onto the upright poles at predetermined intervals. Thus it was slightly higher than the preferred level. Due to the construction, it would have been either be too low, causing employees to extend their reach too far, or too high, causing employees to be working very slightly bowed.

Employee was elevator attendant for the day. When the elevator just stopped for no reason. he attempted to re-engage, but it just would not engage. So he picked-up the phone and called the control room. Did not inform them that he was in side the elevator. The control room operator called for service. So the service personnel did not place the call on priority mood.

Was stacking 50 - 60 lbs bags onto pallets, he felt a sting to the lower back. It got worse as the day went by.

Alleges, that on Friday while he was welding he felt something hot. Boot is not burnt on the outside, No indication of burn. (Note: employee is diabetic and is not seeking Dr. treatment as of today).

While working a steel shaft on a work bench, was moving the shaft into position. The work bench had holes on the table, the bolt nearest the hole fall-in while his left hand was under the shaft base which smashed his left middle finger at the first joint.

While walking down the stairs, two steps from the bottom/or landing his left ankle twisted causing him to fall and while landing on the floor, a hammer that he had on his right hand hit his right leg on the thigh.

He was reaching out to grip the door handle, when someone else opened the door from the inside. The door handle hit his left hand.

Employee was trying to loosen a bolt, when the bolt broke loose he hit his left knuckle middle finger on a stud.

Employee stepped on a wooden pallet, the pallet board broke causing him to roll his right ankle.

While walking on uneven ground, he rolled his right ankle.

While pushing/pulling on elevator door, may have pulled muscle on neck.

While getting off the loader inside the clinker building, he slipped on the clinker on the ground, falling to the ground and landing on his buttocks. not reported in timely fashion: not reported till 11/14/2011

Working on dust collector, he got dusty and alleges he developed a rash between his legs.

While walking on muddy floor, slipped and on the process of catching himself, twisted his left lower back.

While closing the door on the tertiary air duct, the bar used slipped from the door placement, hitting EE on the right side jaw and neck.

While pulling on 60' of 4" hose used on the super sucker truck to clean/vaccum dust off conveyor belt floor, he was standing on an incline walkway, pulling the hose with it under his arm pit.

He alleges that on or about 10/20/2010 he was stacking type I cement and as he went to flip the bag to place into position, he felt a sting to the inside of his right elbow.

While removing a slide gate using a come-a-long on Bn-305, he went to move the cable to one side, when he did the cable snapped pinching his index finger on the frame.

EE was cutting bolts holding up plate liner and it fell on top of left foot causing a fracture.

While stepping forward on uneven grading, his ankle rolled off of grading.

Using a lot port-a-power, kicked back, struck employee in mouth

Slipped off 6" concrete step, rolled ankle.

While assisting, removed a star feeder from its flang, he pushed the unit out of its position. It fell, striking his left wrist on bottom flang.

While working on ID-360 fan, went to stop the fan from turning, using a pipe wrench. When the wrench got the shaft the wrench also went down pinning the right index finger against the frame of the shaft housing.

Employee was pulling on a wrench to loosen a bolt that was too tight and continued to pull, he felt a strain to right shoulder.

While cleaning a probe, he started to climb down a short ladder to the lower level. While going down, his right shoulder locked and popped. When he got to the bottom it hurt with movement and pops.

While positioning a large motor back into its place, he placed his left hand near a piece of tin. The motor moved up some, causing the motor to pin his left hand against the motor.

While placing a seal on the bottom side of a rail car, he stepped on the track twisting his right ankle, and fell forward landing on his left knee and arm.

Employee was pulling out worn liners from RM-310A roller mill.

Jammed neck while walking under stairway area, walked into bottom of stairs.

While working on RM-310, the table was hot. He received a burn to his shin on right leg, it was not severe, but he got it infected.

While lifting a pneumatic Device, used to open railcar doors. he and three other employee's were lifting the device onto the bed of a pick-up truck, to take to repair. he felt some pain to the left side of his abdominal area, below the naval.

While working on FM-1 bullgear, which is a very dusty area, he got cement dust all over himself. He did not notice any problems that day, he claims that he noticed a rash three days later, but did not report it to his supervisor until 11-17-2008.

While trying to close the door on a coal railcar, by pushing the door with his foot/leg, he felt a small pain on his lower back.

Reported on 8/19/2008: Employee was cutting overhead bolts and hot slag fell on to both arms causing small burns. This job ended on 8/14/2008.

EE was walking from shop building to his pick-up truck, when a gust of wind blew something into his right eye.

While performing welding operations, a piece of hot slag got into his shirt and worked its way down to his waist line. This is a forth attempt to submit, the first attempt was made on 5/26/08, but did not receive the submission verification.

Employee allegies that on the 3rd of May he was stacking cement bags onto pallets on bagging process, at which time he claims that he strained his back. He did not have any pain untill May 23 at which time he reported to his supervisor. Late reporting.

While stepping to ground level from stairs, he stepped with left foot, causing the foot to roll.

While retrieving a stray cat from an air duct, the cat bit him on the right finger.

While extracting broken bolts from under D-9N tracks, metal and dirt that was on his face went into his right eye.

While getting off of WF-601, the guard to the feeder came off striking him on the left leg.

While opening the elevator door with forearm, he felt pain to neck, L/shoulder and L/arm. Possible strain to neck.

While walking to lunch room from parking lot, he slipped and fell on Icy walkway.

While walking in plant area he slipped and fell to ground, hurting his left wrist.

While attempting to lift an all rubber bob-cat tire, He strained his lower back.

While operating a cement bagging machine, the cement bag broke, blowing cement up to operator. The cement flew up to the face of the operator and between the safety glasses.

Employee was in the basket of a manlift shutting the machine down. When he reached to push the off switch, another contractor's crew dropped a 4# hammer from approx. 25' above and it landed on employee's (R) index finger. Finger tip was lacerated and fractured.

While removing a pipe at gound level, he claims that while kneeling to remove four bolts on the flung, he went to get up and felt pain to the top of left knee.

WHILE CLIMBING A PERMANENT 8 FOOT LADDER TO A WORKPLATFORM, HE CLAIMS HE HIT HIS HEAD ON AN OVERHEAD WATERPIPE LOCATED APROX 24" BACK FROM THE LADDER.

OPERATOR 0F 631E, EE COLLIDED WITH ANOTHER 631E DRIVER BY(EE'S NAME) INJURED EE WAS ONLY PERSON INJURED.

EE WAS USING A PIECE OF ANGLE IRON INSTEAD OF A HAMMER TO SECURE THE LID OF A 55 GALLON DRUM. HE STRUCK HIS RT. SMALL FINGER WITH THE ANGLE IRON. THIS RESULTED IN A FRACTURE TO THE TIP OF THE FINGER AND DAMAGE TO THE NAIL.

WHILE ON TOP OF TANKER TRAILER, LUBRICATING MIDDLE DOME LATCHES, THE DRIVER RAISED UP THE PROTECTIVE GUARDRAIL ON THE SAFETY ACCESS PLATFORM AND WALKED ON TOP OF THE TRAILER. HE LOST HIS BALAN CE, FALLING TO THE GROUND.

WHILE DESCENDING FROM AN UPRIGHT LADDER, EMPLOYEE'S FOOTING SLIPPED, CAUSING EMPLOYEE TO FALL APPROXIMATELY 3 FT TO GROUND, TURNING HIS RIGHT FOOT.

WHILE ASSISTING OTHERS TO CLOSE THE DOOR ON THE CRUSHER, THE DOOR OPENED OUT ONTO THE EE.

WHILE OPENING THE DOOR ON IC-2 CRUSHER TO INSPECT FOR WEAR, THE DOOR CLOSED BACK ONTO THE EMPLOYEE'S FEET.

WHILE STRETCHING HIGH TO REMOVE BUILDUP IN MARL BIN, HE FELT A PULL AND PAIN TO THE STOMACH AREA.

WHILE LOWERING A THREE STAGE HYDROLIC CYLINDER FROM ITS NORMAL POSITION TO THE GROUND. HE PLACED HIS FINGERS BETWEEN THE TWO UPPER STAGES, AND THE UNIT DID NOT HAVE PRESSURE WHICH ALLOWED IT T O MOVE AND PINCH THE FINGERS ONTO THE STAGE SLEEVE.

WHILE DESCENDING OFF OF SCAFFOLD, HE (EE) SLIPPED AND FELL, HITTING LEFT SIDE OF HIP.

WHILE LIFTING A 15' PIECE OF FLEXIBLE PIPE UP TO A WORK PLATFORM, APPROX. 30' UP AND OVER THE HANDRAIL, HE FELT PAIN TO NAVAL AREA. THE NEXT DAY HE HAD A BUMP.

WHIEL CUTTING W/TORCH THE PIPE HE WAS CUTTING ROLLED OFF THE TABLE AND LANDED ON OXYGEN & ACETYLENE HOSE CUTTING THEN IN TWO, HE TRIED TO CRIMP THE HOSE USING HIS HANDS, BUT HIS L HAND SLIPPED ONTO THE HOSE.

WHILE REMOVING MATERIAL FROM FLOW GATE, WITH LEFT HAND, HE REMOVED MATERIAL THAT WAS HOLDING GATE UP. THE GATE CLOSED ONTO LEFT THUMB; PARTIAL AMPUTATION.

EMPLOYEE WAS WALKING OVER TO HIS WATER WAGON. THE ROAD HE WAS WALKING ON HAD BEEN WET DOWN FORDUST CONTROL. HE SLIPPED AND FELL, BREAKING HIS LEFT WRIST. THE SHIFT STARTED AT 6:00AM. NORMAL PROCEDURE IS TO MEET AT THE BREAK ROOM AND BE TRANSPORTED TO EQUIPMENT IN THE TRUCK.

EMPLOYEE WAS UNLOADING EQUIPMENT, GRABBED PICKUP RAIL CATCHING LOOSE NAIL ON LITTLE FINGER, HAD PULLED NAIL OFF. EMPLOYEE HAD PREVIOUSLY SMASHEDFINGER APPROX 2-1-00.

EE WAS WASHING DOWN CONCRETE WITH HIGH PRESSURE HOSE. WORK CART RAN OVER HOSE BEHIND EE, JERKING NOZZLE INTO LEFT LOWER ABDOMINAL REGION. AREA BECAME SORE AND WHEN SORENESS DID NOT SUBSIDE, EE TAKEN TO DR. EXAM REVEALED LEFT INGUINAL HERNIA. POSSIBLY PRE-EXISTING BUT IMPOSSIBLE TO PROVE.

WHILE REPAIRING AN AIR VALVE, IT FAULTED TO WORKTHEY REMOVED IT TO TEST IT THEY DID NOT REMOVE THE AIR LINES. THE VALVE STARTED TO CLOSE WHILE HIS FINGER WAS STILL IN THE VALVE AREA

INJURED OVERLOADED WEIGHT GUIDE W/WIRE WEIGHTS CAUSING GUIDE SUSPENSION TO FAIL.

EE STRAINED BACK WHILE HELPING TO RAISE 1OGA CHANNEL INTO POSITION

EE STATES THAT HE WAS HOLDING A WIND BRACE TO STABILIZE IT. THE BRACE ROTATED 1/2 TURN, PINCHING HIS RT INDEX FINGER BETWEEN THE WINDBRACE AND THE STEEL FLOOR, CAUSING THE LACERATION.

EE STATES THAT HE HUNG A 1 1/2 TON COME-A-LONG ON A PAD EYE. HE THEN PUT THE COME-ALONG IN THE FREE WHEEL POSITION. THEN HE STARTED PULLING THE SLACK OUT OF THE CHAIN. THE FREE WHEEL DISENGAGE D CAUSING THE HANDLE TO STRIKE HIM IN THE MOUTH.

EE CLIMBED DOWN A FIXED LADDER TO THE 5 1/2 FLOOR PLATFORM. HE THEN STEPPED BACK INTO A FLOOR OPENING, FELL 16'3" TO THE 5TH FLOOR. THE EE SUSTAINED 1 LACERATION TO THE LEFT SIDE OF HIS SCALP AND LEFT SIDE OF HIS FACE.

EE STATES THAT HE WAS HELPING THE IRON WORKERS OPERATE A CHAIN FALL AND A SHAKEL FELL APPROX 10', STRIKING HIM IN THE MOUTH, CAUSING CONTUSION OF THE MOUTH AND 2 FRONT TEETH.

EMPLOYEE STATES THAT WHILE OPERATING A 50 TON CRANE A GUST OF WIND BLEW DUST INTO HIS EYES CAUSING THE EYE IRRITATION.

EMPLOYEE STATES THAT WHILE MOVING MATERIAL ON THE 6TH FLOOR HE STEPPED ON A GRATING REMOVAL EYE WITH HIS LEFT FOOT, HIS ANKLE TURNED OUTWARD CAUSING THE SPRAIN.

EE WAS PUTTING 4" NYLON STRAPS AND 3/4" CHOKERS ON 65 TON LINK BELT CRAN. THE STRAPS AND CHOKERS WERE APPROX. 12', WEIGHT AROUND 10# EACH. EE SAID HE MUST HAVE TURNED WRONG OR SOMETHING.

WHILE PRYING TO MOVE A SECITON OF DEMO'D VESSEL THE LOAD SHIFTED AND CAUSED TH ESLEEVER BAR TO SHIFT RAPIDLY CUTTING EMPLOYEES RIGHT AR JUST BELOW THE ELBOW (INSIDE)-LACERATION 1 TO 1/2 INCHES .

EE STATED THAT PICKING UP, TURNING, TWISTING AND REMOVING BRICK FROM KILN, SHE STARTED TO FEEL STIFNESS TO MIDDLE AND SIDE OF BACK.

EE WORKED THROUGH THE SHIFT AND DID NOT FEEL SOMETHING IN HIS EYE UNTIL THE NEXT DAY.

EE WAS AAMERING REBAR TOOK OFF GLOVES AND CONTINUED WORK STRUCK REBAR AFTER MISSING WITH HMR.

EE WORKED THROUGHOUT THE SHIFT AROUND GRINDING CUTTING AND WELDING OPERATIONS. HE WENT TO FIRST AID BELIEVING HE HAD DUST IN HIS EE.

EE STATES: WHILE PICKING UP A GEAR HOUSING, HE FELT A PINCH TO THE LOWER BACK.

WHILE OPERATING A PAINT SHAKER, HE WAS GOING TO TIGHTEN THE CAN, HE THEN GRIPPED THE UNIT, AS HE DID IT ROOLED CAUSING PAIN TO THE LEFT SHOULDER.

WHILE SQUATTING DOWN TO PICK UP SOMETHING, THE PATELLA DISLOCATED, THEN IT POPPED BACK IN AS HE STOOD UP FURTHER.

EE WAS CUTTING METAL BEAMS. WHEN THE BEAMS WERE CUT THE TWO ENDS CAME TOGETHER & PINCHED THE LITTLE FINGER OF HIS LEFT HAND.

WHILE CLIMBING OFF A TRUCK ONTO A LADDER HIS FOOT SLIPPED. HE CAUGHT HIMSELF WITH HIS RIGHT HAND TO CUSHION HIMSELF FROM FALLING. CUSHIONING HIS FALL RESULTED IN PAIN TO HIS RIGHT SHOULDER.

WHILE REMOVING A CRACKED LAMP ON A LIGHT FIXTURE, THE LAMP SHATTERED, CAUSING A LACERATION TO FINGER.

EMP WAS DESCENDING THE BOBCAT LOADER. STEPPED DOWN ON MUDDY SURFACE, AND FOOT SLIPPED, CAUGHT HIS FALL WITH RIGHT HAND.

EMPLOYEE WAS PULLING WIRE, WORKING NEAR A KILN AND WAS OVERCOME BY HEAT.

EE WAS TURNING STEEL BEAM OVER AND WHILE TRYING TO SET BEAM DOWN, THE BEAM WAS SET ON FINGER AREA OF RIGHT HAND. CAUSING A FINGER SPRAIN.

CLEANING UP PLANT AREA. EMPLOYEE SLIPPED ON WET FLOOR CAUSING HIM TO FALL ON RIGHT ELBOW AND HURTING HIS RIGHT SHOULDER.

DRILLING HOLES IN METAL PLATE FOR FAN BASE. DRILL BIT HUNG AND DRILL MOTOR JAMMED THUMB NAIL.

NOT PAYING ATTENTION TO SURROUNDINGS

EMPLOYEE WAS ASCENDING A LADDER TO MEASURE A MATERIAL BIN. HE WAS NOT FAMILIAR WITH AREA AND DID NTO NOTICE A CROSS BEAM AT TOP OF TANK. HE STRUCK HIS HARD HAT ON BEAM ASCENDING FROM LADDER TO TOP OF BIN.

CAUGHT THUMB IN MANLIFT DOOR.

EMPLOYEE HEARD A PIECE OF METAL GOING THRU THE SECONDARY CRUSHER. HE WENT UPSTAIRS TO CHECK THE CONVEYOR FOR THE PIECE OF METAL, SAW IT, AND GRABBED FOR IT AND THE SHARP JAGGERED EDGE CUT HIS FINGER.

EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE PULLING A PUMP OUT OF A SUMP, WITH THE ASSISTANCE OF A ROPE. THE ROPE BROKE AND THE EMPLOYEE SLIPPED ON THE WET FLOOR AND SEVERELY TWISTED HIS KNEE.

EMPLOYEE WAS INSIDE CEMENT COOLER HELPING TO CLEAN IT. HE BECAME OVERHEATED & WAS AFRAID HE WOULD FAINT. IN PANIC HE TWISTED HIS BODY COMING OUT OF A RESTRICTED OPENING. WHEN HE TWISTED HIS BO DY HE TORE LOOSE SOME SUTURES FROM A PREVIOUS HERNIA OPERATION. THIS WAS NOT DISCOVERED UNTIL SOME TIME LATER.

EMPLOYEE FELL OFF CRANE & LANDED ON HIS FEET & FRACTURED HIS RT HEEL

EMPLOYEE WAS OPERATING A MANLIFT AND CAUGHT FINGER BETWEEN MAN LIFT AND BUILDING.

MOVING STEEL HANDRAILS LIFTED ONE HANDRAIL & THE OTHER HAND RAIL FELL & CRUSHED EMPLOYEE FINGER BETWEEN BOTH HANDRAILS

EE WAS ATTEMPTING TO SECURE PIPE TO PIPE STAND, PIPE SLIPPED AND STRUCKED EE BELOW THE LEFT SHOULDER EE DISLOCATED HIS LEFT SHOULDER.

EE WAS ASSISTING IN THE REPLACEMENT OF TRUNION ON THE WET KILN. A LOT OF HEAVY LIFTING AND EXCERTION CAUSED THE AGGREVATION OF AN EXISTING LUMBAR PROBLEM.

LIFTIN METAL PLATE LOST GRIP AND FINGERS WAS CAUGHT BETWEEN THE METAL PLATE AND ANOTHE METAL PLATE. FRACTURE RIGHT INDEX AND MIDDLE FINGERS, MIDDLE FINGER WAS ALSO LACERATED.

EMPLOYEE WAS OPERATING BAGGING MACHINE. HE PRESSED AN OPERATING BUTTON WITH HIS INDEX FINGER AND HIT HIS MIDDLE KNUCKLE ON SHROUD.

EMPLOYEE WAS COMING OUT OF THE FINISH MILL CRAWLING OVE RHAND-RAIL.. JUMPED FROM HAND-RAIL ONTO CATWALK AND HIS RIGHT FOOT LANDED ON A AIR HOSE, TWISTING IT.

EMPLOYEE WAS INSIDE OF BALL MILL CLEANING THE DISCHARGE SLOTS WHILE ATTEMPTIG TO COME OUT OF MILL HE PANICKED & FORCED HIMSELF THROUGH SMALL OPENING

EMPLOYEE WAS ATTEMPTING TO ENGAGE THE POWER TAKE-OFF FROM UNDER A DUMP TRUCK.EMPLOYEE BY MISTAKE PULLED THE WRONG LEVER AND BED CAME DOWN ON HIS HAND.

EMPL REMOVING CLINKER COOLER GRATES OFF OF BACK OF PICKUP ONTO A PALLET ON THE GROUND.PLACING A GRATE DOWN,FELT PAIN IN LOWER BACK.

EE WAS STACKING CEMENT BAGS ON TO A PALLET COMING OFF OF A CONVEYOR BELT, WHEN ONE OF THE BAGS BROKE AND THE CEMENT BLEW UP INTO HIS EYES.

EMPLOYEE WAS ATTEMPTING TO MOVE A 55 GALLON DRUM OF OIL BY PUSHING IT.THE FLOOR WAS WET AND HIS FOOT SLIPPED,CAUSING HIM TO STRETCH HIS LOWER AREA.

EE AND ANOTHER OPERATOR WERE REPLACING A DOOR ON FINISH MILL 1 USING A BUILDING CABLE HOIST. HE WAS USING THE HOIST BLOCK AS A HAND HOLD TO STEADY HIMSELF. HE WAS WEARING LOBES AND AS THE CABL E WAS RETRACTING INTO BLOCK HIS GLOVE CAUGHT ON CABLE VURRS AND PULLED HIS FINGER INTO PULLEY.

EMPLOYEE WAS WORKING IN AND OUT OF A BALL MILL. HE HAD TO CRAWL IN AND OUT THRU A SMALL OPENING (DOOR) HE HAD TO CRAWL IN ON HIS HANDS AND KNEES. ALSO HAD TO TWIST HIS KNEES TO GET IN AND OUT. THIS CAUSED AN AGGREVATION OF AN OLD INJURY TO LEFT KNEE.

EMPLOYEE WAS USING A ACETYLENE/OXYGEN TORCH CUTTING SOME METAL. A PIECE OF THE METAL SLAG STUCK TO THE TORCH TIP CAUSING THE FLAME TO BE WITHDRAWN INTO THE TORCH. EMPLOYEE WIPED THE SLAG OFF O F THE TIP WITH HIS LEFT THUMB/HAND. IN SO DOING THE TORCH FILLED WITH GAS AND SUDDENLY EXPELLED FILLING HIS GLOVE WITH GAS AND IGNITED.

EMPLOYEE NOTICED THAT SOME OF THE SACKS OF CEMENT ON A PALLET WERE NOT IN LINE AND PROTRUDING OUT. INSTEAD OF USING THE FORKLIFT OR ANOTHER PALLET, THE EMPLOYEE USED HIS HIP TO KNOCK THE SACKS INTO PLACE. EMPLOYEE FELT SOME PAIN ON 11-24 AND THEN ON 12-5 PAIN WENT TO RIGHT FOOT. THEN WENT TO DOCTOR.

EE WAS DESCENDING THE BOB CAT BEFORE DESCENDING HE MOVED THE SEAT SAFETY BAR UP AND OUT OF WAY TO ALLOW HIM TO DECEND. APPARENTLY DID NOT MOVE SEAT SAFETY BAR UP AND IN THE LOCKING POSITION OR HE GRABBED THE BAR WHILE DESCENDING. THE BAR FELL DOWN AND CAUGHT HIS THUMB IN THE HINGED PORTION.

EMPLOYEE WAS USING CHIPPING GUN WITH A 6FT. BAR ATTACHED, TO BREAK LOOSE BUILT-UP MATERIAL IN ONE OF THE RAW MATERIAL BINS. THE BUILT-UP MATERIAL BROKE LOOSE FROM ABOVE IN TANK AND CAME DOWN A ND LANDED ON THE BAR ATTACHED TO THE CHIPPING GUN WHICH EMPLOYEE WAS HOLDING. THIS CAUSED THE CHIPPING GUN WITH BAR TO SUDDENLY MOVE UPWARD PULLING EMPLOYEE'S RIGHT ARM AND SHOULDER.

LIFTED MILL DOOR UP TO PLACE IT IN MILL DOOR OPERNING WORKING AREA IS RESTRICTED MILL DOOR IS TOO HEAVY FORM ONE MAN TO LIFT. FELT POP IN LOWER BACK

EE WAS USING A CHIPPING AIR HAMMER WITH A SIX FOOT BAR TO LOOSEN BUILT UP MATERIAL IN THE BOTTOM OF BIN NO. 1 THE BUILT UP MATERIAL FROM ABOVE BROKE LOOSE AND FELL ON THE BAR CAUSING THE BAR A ND CHIPPING HAMMER TO MOVE UPWARD RAPIDLY AND STRIKING EE ARM AND WRIST BETWEEN HAMMER AND BIN.

EMPL UTILITY LABORER WHICH INVOLVED HIM IN GENERAL CLEANUP OF PLANT.HELP WAS NEEDED IN SHIPPING DEPT AND EMPL VOLUNTEERED...EMPL DID SOME SACKING AND STACKING OF CEMENT SACKS REQUIRING HIM TO TWIST AND LIFT.

EE WAS WALKING ACROSS AND UNDER RAW MATERIAL CONVEYORS WHERE WET MATERIALS HAD FALLEN ON SLAB ON SHIFT BEFORE. THE WET MATERIAL ON THE SLAB AREA MADE IT VERY SLIPPERY AND HAZARDOUS. EE WALKED ACROSS AREA AND HIS FEET SLIPPED OUT FROM UNDER HIM AND HE LANDED ON HIS BUTTOCKS AND LOWER BACK.

WAS REPAIRING A SHAKER SCREEN WHEN HE BENT OVER AND PICKED UP A PIECE OF 6" CHANNEL IRON, 4' LONG.

EMPLOYEE HAD A PRE-EXISTING EYE CONDITION. WORK ENVIRONMENT (CEMENT DUST) AGGRAVATED THE CONDITION AND CAUSED IRRITATION. EYE SPECIALIST RECOMMENDED REMOVAL OF AGGRAVATED GROWTHS. DOCTOR ALSO STATED THAT HIS WORK ENVIRONMENT CAUSED THE GROWTHS TO INCREASE IN SIZE AND AGGRAVATION.

EMPLOYEE WAS SACKING CEMENT ONE OF THE SACKS LODGED UNDER THE SEAT & WOULD NOT DISLODGE EMPLOYEE TRIED TO PULL ON SACK TO FREE IT FROM UNDER THE SEAT IN SO DOING HIS HAND SLIPPED OFF OF SACK & MOVED ACROSS THE SACK SET CUTTING HIS HAND ON WORN SHARP EDGE

EMPL WAS STACKING SACKS OF CEMENT FROM A MOVING CONVEYOR TO A PALLET ON THE FLOOR THE SACKER WAS NOT SPACING THE SACKS OF CEMENT PROPERLY TO ALLOW THE MEPL TO STACK THE SACKS IN A SAFE MANNER THIS CAUSED EMPL TO USE HIS HANDS AND WRIST IN A IMPROPER WAY TWISTING AND INJURING HIS WRIST

EMPLOYEE WAS ASKED TO GO INTO THE KILN ALONG WITH LEADMAN AND SOME OTHER WORKERS, TO TEAR OUT OLD BRICK. EMPLOYEE WAS OPERATING A JACK HAMMER FOR THE FIRST TIME AND HE HEADACHE RECK WAS NOT P LACED PROPRERLY. A BRICK CAME LOOSE AND CAME AROUND RACK AND STRUCK EMPLOYEE ON LEFT HAND WHILE USING JACK HAMMER.

EMPL LIFTING BUCKETS OF PORTLAND CEMENT-35 LBS FROM FLOOR TO COUNTER TOP.TWISTED HIS BODY WHILE KEEPING FEET PLANTED.THIS CAUSED RIGHT KNEE TO TWIST WHILE CARRYING FULL BUCKET.

EMPLOYEE WAS NOT WEARING GLOVES WHILE LANDING SHEET METAL AND CUT HIS FINGER.

EMPLOYEE WAS HELPING MAINTENANCE AND LABORERS REPLACING THE SUPPORT CABLES ON THE PRECIPITATOR PLATES. ONE OF THE SUPPORT CABLES CAME LOOSE AND THIS CAUSED EMPLOYEE TO LOOSE HIS BALANCE AND FA LL BACKWARD HITTING HIS HEAD/NECK ON PRECIPITATOR FRAME.

PICKED UP 6 VOLT BATTERY OFF FLOOR INSTALL IN SCOOTER AS PICKED IT UP BATTERY HE TWISTED BODY TO CHANGE DIRECTION TO SCOOTER MOTION TWISTED ANKLES AND TWISTED LEFT ANKLE.

EMPLOYEE WAS WORKING WITH A FRONT END LOADER MOVING CLINKER INT HE CLINKER BUILDING. WIND WAS LBLOWING AND OPERATOR HAD DOORS OPEN ON LOADER BECUSE AIR CONDITION WAS NOT WORKING. SOME CLINKE R DUST BLEW AROUND HIS SAFTEY GLASSES AND INTO HIS EYE.

EMPLOYEE WAS DOING SOME WELDING IN THE KILN. KILN HAD BEEN ROTATED JUST PREVIOUS TO EMPLOYEE ENTERING. SOME DUST AND PARTICLES OF KILN CASTABLE WERE IN THE AIR, AND ONE OF THE PARTICLES BLEW I NTO HIS EYE WHILE HIS WELDING HOOD WAS UP.

EMPLOYEE FINISHED DOING SOME CLEAN-UP AROUND CONVEYOR. HE RETURNED HIS TOOLS AND REMEMBERED LEAVING HIS FLASHLIGHT. WHILE REACHING FOR HIS FLASHLIGHT, EMPLOYEE'S FEET SLIPPED ON SOME CLINKER A ND WHILE TRYING TO CATCH HIS BALANCE HIS RIGHT HAND GOT INTO THE CONVEYOR/SKIRT AND HIS HAND AND ARM WERE PULLED IN.
