Hoban Pit And Plant Metal/Non-Metal

An employee was standing on VIS Crusher, trying to unclog/clean unit. The employee was holding a 1-1/2" water line, awaiting it to be turned on so that they could wash the clogged VSI. When the water was turned on, the employee lost their balance and fell onto the deck below EE, approximately 4.5 feet below where EE was working.

Employee notified supervisor at 6:00 am on 3/25/2024 that they were not going into work due to debris in the employee's in right eye. Employee was unsure of where and when the debris entered eye. Last time operating a grinder was on 3/21/2024.

Employee was getting welding rods out of the front right side toolbox on the welding truck bed. The employee was on top, kneeling on the toolbox while unlatching the door. The door started to swing downward away from the employee, and employee attempted to catch the falling door with right hand. The door smashed the employee's right ring finger tip.

EE was trouble shooting an issue with the right rear axle. As EE loosened the plug/ bolt pressurized hot oil shot out, hitting EE in face, neck, chest, arm, and stomach. EE suffered 1st and 2nd degree burns.

While reaching under a cone liner on the ground to connect a lifting chain the block on the ground collapsed and the cone liner fell on the miners right hand fracturing four fingers.

On 6/13 a Mechanic dismounting CAT 980K Loader missed a step on ladder and while catching themselves, strained right shoulder. Employee managed to continue to perform all work duties until 7/31 when seen by a doctor and was prescribed Physical Therapy and restrictions.

Employee was cleaning under a scalper screen when stepped wrong and sprained ankle.

Maintenance helper holding a plate on a bearing being installed on a screen was struck in the left thumb with hammer by other worker resulting in 6 stitches

Contracted Temp worker was assisting with performing crusher maintenance on Spokane crusher and strained stomach while removing 3/4" impact wrench.

Employee was adjusting the position of a tire/wheel assembly with boom hands during tire installation. The tire/wheel assembly was pressed against the rock bar on unit. As EE reached in to move the rock bar out of the way, the tire wheel assembly moved forward "pinching" his right middle finger between the boom hand and rock bar.

While cutting thick rubber conveyor material at the hot mix plant with a utility knife, the knife slipped and cut the employee above his left knee. The employee went to the clinic where his cut was sutured and returned to work the same day without restrictions.

Bent over to pick-up a 2 lbs roller of wire, and when went to get back-up, he felt pain to his lower back.

IP reports that he injured his left knee while standing on his right leg and twisting his left leg behind his right leg. IP was assisting two other employees on the feeder belt. IP placed on restricted duty on 8-18: walk on even terrain only. This is 2nd injury IP reports to his left knee in 5 months. The mechanism of injury does not match the claimed injury.

IP was in the plant area working on the VSI crushers. IP went to retrieve a tool from a welding truck. IP reports that he stepped on a rock of unknown size. IP did not see the rock. IP reports that his knee popped. IP was diagnosed with a strained left knee and placed on light duty.

Employee had been shoveling; states he first experienced pain at work but is uncertain of the exact time or date. Employee cannot remember an event with the potential to cause an injury. The employee's first report of this to the employer was on 9/11/07.

Employee was repairing a 30 inch water line in a 15 foot deep excavation. Wall caved in on employee burying him up to his waist. He was immediately dug out using shovels. Injuries suffered included brusing to legs and hips and pain in back. Employee was taken to the doctor where he was prescribed muscle relaxer and pain medication. Employee was placed on light duty.

Employee was assisting a customer into a defined area in order to dump some material. Employee was closing the tail-gate to the truck and it closed on his hand. Employee refused to go to the doctor. A couple of days later the injury had not improved. The employee then asked supervisor to take him to the doctor on 07/12/2005.

Employee did not know proper usage of air seat. Caused some soreness on tailbone,

EE WAS ASCENDING THE LADDER TO HIS LOADER WHEN HE FELT A PAIN IN HIS LEFT SHOULDER.

EE WAS COMING DOWN PRIMARY PLANT STAIRS WHEN HE LOST HIS FOOTING ON THE SECOND TO LAST STEP, HURTING HIS LEFT KNEE.

THE SHIFTER ON THE FRONT END LOADER BECAME STUCKEE BEGAN TO PUSH AND PULL THE SHIFTER PULLING A MUSCLE IN HIS LT. ARM.

EE WAS WELDING ON GRIZZLY IN THE RAIN, WELDING LEADS WERE SHOCKING WELDER.

EE WAS TIGHTENING SOME BOLTS ON THE BEARING HOUSING OF THE SHAKER WITH A RACHET AND CHEATER PIPE. HE SLIPPED AND FELL HITTING THE EDGE OF THE SHAKER WALL RESULTING IN AN INJURY TO HIS RIGHT SI DE RIB CAGE AREA.

EE WAS CLEANING A CHUTE WITH A PICK. AS HE WAS CLEANING A CHUNK OF COMPACT DIRT AND CLAY BROKE FREE AND LANDED ON HIS RIGHT LEG AND FOOT. \

EMPLOYEE WAS JERKED BY ANOTHER EMPLOYEE AND SPRAINED HIS BACK.

WHILE SERVICING A 882 LOADER, SLIPPED ON STEP AND BRUISED LEFT KNEE.

EMPLOYEE WAS TRYING TO CLIMB OVER METAL STRUCTURE AND PULLED HIS GROIN MUSCLE.

EMPLOYEE WAS ON TOP OF RAW MATERIAL BIN USING A HEAVY STEEL BAR TO BREAK LOOSE CAKED MATERIAL. HE FELT A SHARP BURNING PAIN IN LEFT INGUINAL AREA.

EE WAS STADING NEXT TO CONVEYOR BELT HELPING TO REPLAE THE TYROUGHING ROLLERS. HE WAS HOLDING ON TO THE CONVEYOR FRAME WITH ONE HAND AND REACHING AROUND TO GRAB THE ROLLERS WITH THE OTHER HAND . IN SO DOING HE TWISTED AND STRAINED HIS BACK.

EMPLOYEE WAS IN MATERIAL TUNNEL CLEANING OUT A FEEDER WIOTH A CLEANING BAR USING IT TO BREAK UP MATERIAL STUCK TO INSIDE OF FEEDER HE STRUCK THE MATERIAL WITH THE BAR THE BAR WITH HIS HAND ;ST RUCK THE CORNER OF THE FEEDER

EMPLOYEE WAS WALKING ACROSS A WET CONCRETE SLAB TO GO TO ANOTHER PART OF PLANT HE STOPPED TO TURN & IN SO DOING HIS FEET SLIPPED OUT FROM UNDER HIM & AS HIS BODY WAS COMING DOWN HE ATTEMPTED T O STOP HIS FALL WITH HIS LEFT HAND & ARM

EMPLOYEE WAS DESCENDING A LADDER AT THE PRIMARY CRUSHER. AS HE WAS COMING DOWN HIS FOOT SLIPPED AND AS HE WAS TRYING TO CATCH HIMSELF HE TWISTED HIS BACK.

EMPL HEARD SOMETHING RATTLING INSIDE OF CONE CRUSHER. HE CLIMBED UP ON CRUSHER TO SEE WHAT WAS MAKING RATTLING SOUND. AS HE WAS REACHING TOP OF CRUSHER HE HAD HAND ON TOP PART OF CRUSHER & AT SAME TIME HAMMER THAT HADD BEEN DROPPED ON BELT FEEDING CRUSHER MAKING RATTLING SOUND CAME FLYING OUT & STRUCK EMPL FINGER AGAINST SIDE OF CRUSHER.

EMPLOYEE, ALONG WITH TWO OTHER EMPLOYEES WAS CARRYING AN 8' X 20' SCREEN UP THE STAIRS TO REPLACE THE SCREEN IN THE SHAKER. HE WAS RESTING THE SCREEN ON HIS RIGHT HIP AS HE WAS CARRYING IT. W HEN THEY FINISHED CARRYING IT TO THE TOP, HE NOTICED A PAIN IN THE HIP AREA. NOT HURTING ENOUGH TO GO TO THE DOCTOR AT THE TIME, BUT AS TIME WENT ON IT PROGRESSIVELY GOT WORSE. NOTE: DID NOT

EMPLOYEE WAS PICKING UP A PIECE OF SCREEN TO MOVE IT TO POSTION. HE DID NOT BEND HIS KNEES TO LIFT AND JERKED TO LEFT. IN SO DOING HE NOTICED A BURNING SENSATION AND SUDDEN PAIN IN HIS LEFT LOWER FRONT SIDE. PAIN WENT AWAY HE THOGUHG IT O.K.

EMPLOYEE WAS DRIVING A BELLY DUMP TRUCK, DUMPING STRIPPED MATERIAL ON A FILL PILE. EMPLOYEE DROVE TO THE EDGE OF THE FILL PILE AND TRUCK SLID OFF OF EDGE AND TURNED OVER. EMPLOYEE HAD HIS SEAT BELT ON AND THE BOLT HOLDING THE SEAT BELT TO THE TRUCK WAS BOLTED UPSIDE DOWN. HE HIT HIS LEFT THIGH ON THE BOLT, CAUSING ABRASIONS AND BRUISING.

EMPL WORKING IN LAB DETERMINING SCREEN ANALYSIS ON AGGREGATES.BENT OVER,PICKED UPAN OF AGGREGATES WITH HIS BACK INSTEAD OF HIS KNEES.BACK BEGAN HURTING LATER THAT DAY.

CONTRACT LABOR WAS CLEANING A GRIZZLE CHUTE WITH A BAR WHEN A LARGE ROCK DISLODGED STRIKING HIS RIGHT HAND.

EMPLOYEE WAS CL2SING A GATE ON A ROCK HOPPER, HE CAUGHT HIS FINGER BETWEEN THE GATE HANDLE AND THE STOP.

EMPLOYEE WAS OPERATING 980 B LOADER HE WAS DISMOUNTING OFF LOADER WHEN HE SLIPPED AND TWISTED LEFT ANKLE.
