G5 industrial Services was preparing to install a discharge pipe under the log-washers. The pipe had a ninety-degree elbow on one end that was facing upward while in the air. When the pipe was placed on the ground it rolled to the side, EE misjudged where the ninety-degree elbow would lay down to rest. The pipe landed on the top of EE's foot causing injury.

An employee was standing on VIS Crusher, trying to unclog/clean unit. The employee was holding a 1-1/2" water line, awaiting it to be turned on so that they could wash the clogged VSI. When the water was turned on, the employee lost their balance and fell onto the deck below EE, approximately 4.5 feet below where EE was working.

Employee notified supervisor at 6:00 am on 3/25/2024 that they were not going into work due to debris in the employee's in right eye. Employee was unsure of where and when the debris entered eye. Last time operating a grinder was on 3/21/2024.

Employee was getting welding rods out of the front right side toolbox on the welding truck bed. The employee was on top, kneeling on the toolbox while unlatching the door. The door started to swing downward away from the employee, and employee attempted to catch the falling door with right hand. The door smashed the employee's right ring finger tip.
EE was trouble shooting an issue with the right rear axle. As EE loosened the plug/ bolt pressurized hot oil shot out, hitting EE in face, neck, chest, arm, and stomach. EE suffered 1st and 2nd degree burns.

While reaching under a cone liner on the ground to connect a lifting chain the block on the ground collapsed and the cone liner fell on the miners right hand fracturing four fingers.

On 6/13 a Mechanic dismounting CAT 980K Loader missed a step on ladder and while catching themselves, strained right shoulder. Employee managed to continue to perform all work duties until 7/31 when seen by a doctor and was prescribed Physical Therapy and restrictions.

Contracted Temp worker was assisting with performing crusher maintenance on Spokane crusher and strained stomach while removing 3/4" impact wrench.
Employee was adjusting the position of a tire/wheel assembly with boom hands during tire installation. The tire/wheel assembly was pressed against the rock bar on unit. As EE reached in to move the rock bar out of the way, the tire wheel assembly moved forward "pinching" his right middle finger between the boom hand and rock bar.

While cutting thick rubber conveyor material at the hot mix plant with a utility knife, the knife slipped and cut the employee above his left knee. The employee went to the clinic where his cut was sutured and returned to work the same day without restrictions.
Bent over to pick-up a 2 lbs roller of wire, and when went to get back-up, he felt pain to his lower back.

IP reports that he injured his left knee while standing on his right leg and twisting his left leg behind his right leg. IP was assisting two other employees on the feeder belt. IP placed on restricted duty on 8-18: walk on even terrain only. This is 2nd injury IP reports to his left knee in 5 months. The mechanism of injury does not match the claimed injury.
IP was in the plant area working on the VSI crushers. IP went to retrieve a tool from a welding truck. IP reports that he stepped on a rock of unknown size. IP did not see the rock. IP reports that his knee popped. IP was diagnosed with a strained left knee and placed on light duty.

Employee had been shoveling; states he first experienced pain at work but is uncertain of the exact time or date. Employee cannot remember an event with the potential to cause an injury. The employee's first report of this to the employer was on 9/11/07.
Employee was repairing a 30 inch water line in a 15 foot deep excavation. Wall caved in on employee burying him up to his waist. He was immediately dug out using shovels. Injuries suffered included brusing to legs and hips and pain in back. Employee was taken to the doctor where he was prescribed muscle relaxer and pain medication. Employee was placed on light duty.
