EE was dumping metal 55 gallon trash can into dumpster. Dumpster height is 32 inches. EE described lifting the trash can up and feeling a pop in right bicep.

During screen cloth change the employee was using a pry bar to maneuver the screen cloth. The pry bar became stuck in the screen cloth opening. The employee used a hammer to hit the pry bar, where the bar goes into the handle. They struck upward and away from themselves. The pry bar flipped up striking the employee on the left eyebrow causing a laceration.

Employee was a passenger in a service truck driven by another employee that struck a concrete footer that supports the primary discharge conveyor on 3/10/2015. At the time employee felt a pain in his neck. He requested medical treatment on 5/18/2015.

While repairing a leak on a fuel nozzle, the wrench slipped causing the employees hand to strike a piece of metal causing a laceration to his left hand.

While rolling a rock out of the roadway the employee strained his right elbow.

Employee was cutting wear metal loose from a loader bucket and it made an unexpected movement striking his foot causing a laceration.

While in the process of cutting a piece of metal, the wind blew dust around the side of the employee's safety glasses and into the employee's eye.

While attempting to remove the bucket from a skid steer, the bucket came loose and struck IW in the shin.

Employee was tightening come-along when it slipped and hit him in the chin causing a laceration.

Two workers were setting up the crane to make lifts on a crusher. The operators cab was raised to let snow & ice slide off the top glass. The cab stuck in the up position. EE kneeled down & reached under the cab & loosened a hydraulic hose to lower the cab. The cab gently lowered onto his back and held him for a few minutes until co-worker was able to lift the cab to let him out.

Employee fell from the front access ladder on a haul truck to the ground.

The employee was using a Dye Grinder to cut a piece of metal from a pressure washer. Possibly, the grinder bolt caught the parabolic metal putting the wheel in a bind. The metal became a projectile and hit the employee in the arm and face causing open wounds needing medical care.

While attached to a rope & harness and scaling rock from the rock face, the rock came loose from the berm above and struck the scaler in the chin and caused a laceration.

Employee's left index finger was lacerated while replacing Cat 990 loader bucket segments using his left hand to hold the bolt from spinning instead of using a bar. The bolt cut through the Kevlar glove, receiving seven stitches.

Received a UPS shipment, ee was opening the box with a box cutter knife to insure that all parts ordered were received. Knife slipped while slicing box open and struck ee's left forearm. Took ee to Medical Center, ee received six stitches to left forearm.

EE had just finished tightening the track pad bolts on a Komatsu PC-300 hydraulic rockbreaker. He bent over to inspect his work, stood back up and strained a muscle in his lower back.

EMPLOYEE WAS SHOVELING MATERIAL IN TUNNEL. BACK BECAME SORE AFTERWARDS. EMPLOYEE WENT TO DOCTOR ON MAY 02, 2001 AND WAS PLACED ON LIGHT DUTY.

EE REPORTED A HEARING LOSS IN BOTH EARS TO THE TARMAC CORPORATE RISK MANAGER. OUR WORKERS COMPENSATION CARRIER DETERMINED THE CLAIM WAS VALID.
