Hernshaw Mine Coal

Employee was diagnosed on 5-13-20 by a B reader that EE has pneumoconiosis, last day worked was 3-11-20. We received this notification on 12-3-2020 that EE has pneumoconiosis

EE was walking between #2 and #3 entry, EE's head encountered/bumped a pizza pan that was slightly lower than the mine roof. EE was diagnosed with a neck injury, cervical disk disorder. EE was told to follow up with EE's prior treating Dr for EE's neck as that is the same diagnosis as last time that occurred in 2018.

employee was taking coal height measurements in #8 heading when a piece of drawrock fell striking employee in the lower back, employee was down on knees getting the measurements when the rock hit them, height in the area was 77". Employee was treated and released to return back to work and worked until 7/13/20 when employee was taking off work

Employee was tramming a shuttle car from the miner to the feeder. At spad #13980 there were three hangers stranded together spanning from one bolt to another. One end of the hanger became dislodged swinging down catching the left shoulder of jacket and pulled them from the shuttle car causing injury to left shoulder and right foot.

Employee was crossing the #1 overland belt at the tail crossover. When EE stepped onto the ground EE's right ankle rolled causing a sprain. The employee was scheduled for physical therapy for this injury on 2/19/2020.

Employer was notified on 11-25-19 that employee was diagnosed with occupational pneumoconiosis on 9/13/19 .

Employee felt pop in pec muscle when lifting themselves out of forklift. Was given off work 2 mths and prescribed PT, we ask this be charged as a OCC Illness due to no event, this would be no different than getting out of a chair, on 11-12-19 EE was taken to hospital the prior shift for chest pains while standing in track entry two similar occurrences in 2 days?

Roof Fall was a continuation of an existing fall at #26 break on the #2 beltline in the #6 entry extending to the #27 break in the same entry.

The employee dropped a bottom roller on the offside of the belt and while coming back across the belt they grabbed a belt chain and put their left foot on the belt rail to step down to the mine floor when their foot slipped and their leg went between the belt and the belt rail hyperextending their knee.

The employee was nailing curtain to fly board using a miner bit with right hand when ee missed the nail striking right pinky finger between the miner bit and fly board.

An employee was struck by 4" waterline that came out of scoop bucket causing a fracture to right lower leg. Surgery was required to repair the fracture.

Employee was reaching for drill steel in the tray of the roof bolter when upper back & Rt arm started to hurt. EE was taken to hospital & diagnosed with cervical disk disorder with radiculopathy. EE continued to work until 6/8/18 EE was taken off by a specialist & told EE needed an MRI. Attached are medical documents showing no mechanism of injury & diagnosis.

There was an unplanned roof fall that didn't fall above anchorage but did have the secondary escapeway blocked.

A non-injury roof fall occurred in the #6 intake entry between crosscuts 37 and 38. The fall measured approximately 40 feet long, 19 feet wide and 4 feet thick.

Employee was removing a set of run-through ventilation curtains from the board they had been nailed to and when employee jerked the second section employee felt a pain in employee's right shoulder.

Employee was dragging a roof bolter cable on right shoulder as EE was going through a set of fly pads. While using left arm to raise the fly pad EE struck a roof bolt plate with left elbow, causing a puncture and small laceration.

While employee was getting two buckets of bits off the continuous miner, the handle of one of the buckets containing the bits broke, causing the bucket to fall and strike the right foot of the employee. Employee was wearing metatarsal boots.

Employee felt a pop in right bicep muscle while holding to a conveyor top-cover post on the conveyor belt to assist self in getting off conveyor belt structure. Employee continued to work until 10/13/2017 when surgery was performed to repair a torn bicep muscle.

Claim filed for pneumoconiosis. X-ray read date of 09/12/2017.

A roof fall was found in the primary escapeway at X-Cut #69 at spad #3491 the fall measured 4ft X 20ft X 30ft no equipment was involved and no injuries reported. all the men were evacuated from the mine

Employee was exiting the deck of the underground forklift and EE's foot got caught on the back of the seat, causing EE's left knee to twist. Employee continued to work until 06/05/2017.

Employee was retrieving a grease gun off the 4-wheeler and as ee was leaning over foot slipped. EE sought medical attention two shifts later and was diagnosed with moderate degenerative changes. Because experience in low coal seams and the repeated trauma to knees we are reporting this as an occupational illness.

A non-injury roof fall occurred at approximately 05:30 am in the number 6 entry (primary escapeway), break # 55 along the #2 belt flight at or near SS spad # 1179. The area has been timbered and dangered off and the life line has been rerouted. MSHA and the State agency have completed their investigations, this fall will not be cleaned up.

Employee was cutting the bands on a pallet of roof bolts when EE cut the 2nd of 3 bands the bolts shifted, hitting the arm that was holding the knife, knocking it into left arm causing a puncture wound that required 2 stitches.

Employee stated that after dumping load of coal on the feeder ee started back toward the miner. About 15 feet from the intersection inby the feeder the shuttle car breaker knocked causing the park brake to engage. The employee was thrown forward and in an attempt to brace self stiffened causing pain in lower back and down legs.

Employee was walking from the intersection of #6 to #7 entry when ee slipped and tried to catch self causing ee to twist knee. The employee worked until 7-5-2016 at this time ee was taking off from work.

employee was using a sledge hammer to remove a pin from the ATRS of the #2 Roof bolter. while swinging the hammer, the handle contacted EE's side resulting in the hammer missing the pin and causing three of their fingers to mashed between the frame of the ATRS and the hammer.

Employee's last day of employment was 11/10/2015. This mine was notified on 04/26/2016 that the employees has been diagnosed with an occupational illness of pneumoconiosis.

A non-injury roof fall occurred on the #1 belt line at break #5 along the 1st northwest mains at spad #19. Passage of persons was not affected. Ventilation was not affected. Equipment involved was the belt structure.

The employee has filed for Occupational Pneumoconiosis.

We was notified of his diagnosis with coal workers pneumoconiosis on 9/9/2014.

Employee was in the process of loading the scoop duster with the 4th super bag of rock dust when he lost his balance and fell backwards allowing the knife to strike his left forearm which left a puncture which required 9 sutures to close the wound.

No event. Employee was crossing the belt at break # 43 on the # 2 belt when he felt a pain in his right knee. Employee sought medical attention on 6/21 and again on 6/24. Each time the employee was released to return to work. On a follow up visit on 7/8 the employee was taken off work due to continued pain in knee.

An unintentional roof fall occurred along the #2 belt line in the #7 Primary escapeway entry. The fall was approximately 14' wide by 60' long and 24" thick. The fall was 6900' feet from the active face.

Employee was walking down the # 5 entry between the shuttle car and rib where the trailing cable was 16" of the rib. Tripped on miner cable and twisted left knee. EE was taken to hospital and released to return to work, continued to until work until 6-25-14 at which time the DR told them they would need surgery to repair the ACL and was taken off work.

Employee was helping the left C/M operator move cable slack. As the C/M operator pulled the slack, it looped around the employee's foot, causing employee to fall backward and landing on the mine floor.

Employee was cutting sandstone roof to achieve more height in the #9 entry when a piece of the continuous miner bit broke off and striking employee on the chin.

Employee was bolting the #9 entry when a piece of draw rock fell from between the inside and outside bolt striking employee on the left foot.

Employee was gathering black and wedges to building a stopping. As he turned around, he caught his left ear on a nail that was sticking out of a fly board causing a puncture/tear.

An unplanned roof fall has occurred in the first northeast mains in the #2 entry at spad #6349. Approx. 5000' from the working face.

Employee making fireboss run, on the way back outside got off mantrip to clear rock off track and felt pain in lower back running into the groin area. According to the incident description there wasn't an event, we request this be an occupational illness. MRI report shows degenerative disc disease. This report shows they had an MRI in 2008 also.

Employee was stripping hi-line to add in when his knife slipped causing a laceration on his left pinky finger.

An unplanned roof fall occurred along the #1 belt at the #6 crosscut.

As the shuttle car was going to the feeder, employee was down on his knees hanging another S/C cable when the moving S/C struck employee's legs and feet.

Employee was coming back from the supply hole. As he was turning into a break, he ran over a bucket of degreaser. When the bucket busted, the degreaser went on employee's right arm, shoulder and neck.

Employee was operating a four wheel rubber tire ride when he ran over a piece of rock causing the steering wheel to jerk out of his hand. When the steering wheel jerked, the steering knob struck employee on the top of the right hand.

Employee was taking fly boards from the mine floor and loading them onto the scoop. As he was pulling one of the boards from the mine floor, the board struck him in the mouth, chipping his front tooth.

An unplanned roof fall occurred along the #2 belt in the #6 entry at break #47 spad #1151 7,600 feet from the active face.

Member was bolting in the #5 entry, when the drill steel he was using got stuck in the top. Member went to pull the drill out of the top, when he slid his hand down the drill steel a piece of the steel cut his hand.

Member was cutting the insulation off of a 6/3 cable to install a cathead, when the knife he was using slipped and cut his left thumb.

Member was installing jacks and beams at the #2 head tail piece. Member was putting a crib over the beam to tighten it against the roof, when the crib slipped out and hit members thumb against the beam causing a small laceration.

An unplanned roof fall occurred on the #2 section #3 entry in the return at spad #5660 5,500' from the face.

Member was installing a keeper pin on the ripper head of the left miner when a piece of metal broke off and struck him in the forehead.

An unintentional roof fall was discovered by foreman in old works. Fall measured 19' wide x 100' long x 4' high. Fall located at cross cut #21 in #3 entry next to the #3 belt entry. Did not fall on belt conveyor.

Member had trammed the roof bolter over to the 6 right break. After setting the bolter up to bolt 6 right, member turned to check his cable behind him. When he turned back around he struck his head on a fly board.

A coal rider seam 1" thick that was 2' above test hole length.

Member was rolling up fly pad, when roof bolter trammed thru fly pad and pinned EE between the roof bolter and continuous miner boom.

Member was in process of putting bolts back into panel, when his thumb slipped, hurting his thumb.

Member was cutting belt line on overhead belts when knife slipped cutting leg.

Member was tightening bolts when wrench slipped causing member to hurt ankle.

A FALL OCCURRED IN THE #4 ENTRY, 1 BREAK INBY SPAD #1125 IN THE SECONDARY ESCAPEWAY. THE FALL WAS APPROX. 30' LONG X 18' WIDE X 30" HIGH. AREA OF FALL WAS BREAKERED OFF.

EE was walking to section near power center and hurt knee on uneven bottom. EE last day of work was 7/13/2006 and is still off at this time.

EMPLOYEE WAS HIT IN THE EYE WITH A PIECE OF AQUA LIME. EE MISSED 3 DAYS AWAY FROM WORK THEN CAME BACK & WAS PUT ON RESTRICTED DUTY UNTIL 2/23/06. THE SAFETY DEPT. WAS NOT NOTIFIED OF THIS ACCIDENT UNTIL MARCH 6TH, 2006.

Member was operting mantrip, the mantrip jerked. Member hit top of canopy of mantrip hurting his neck.

There was a roof fall measuring 40' long x 20' wide x 5' high.

Employee was packing tree lead cable, unloading supply car when the cable caused him to twist his left knee and fall to the ground, causing pain.

THERE WAS A ROOF FALL MEASURING 20' L X 20' W X 6' H.

EE WAS PRYING ON 8" WATER LINE WITH A SLATE BAR WHEN THE SLATE BAR SLIPPED AND HE STRUCK HIS RIGHT THUMB AGAINST A BELT RAIL.

EMPLOYEE WAS DISMOUNTING THE FORKLIFT WHEN HE MISSED THE BOTTOM STEP. THIS CAUSED HIM TO FALL & STRIKE HIS BACK ON THE CORNER OF THE GUARD AROUND THE ROCK DUSTER.

Member was taking a scoop off charge when he struck his left knee on the side of the scoop.

A ROOF FALL OCCURRED ON THE INTAKE ENTRY 1 BK OUTBY SPAD #1141. IT MEASURED 70' LONG 15' WIDE AND 4' HIGH. THIS MINE IS IN THE PROCESS OF BEING REOPENED.

EE WAS ASSISTING THE LEFT SIDE CM OPERATOR WITH HIS CABLE & WATER LINE WHEN THE EDGE OF A ROCK RIB MEASURING APPROX 7'X 8' X 2 1/2' FELL STRIKING HIS RIGHT FOOT. EE WAS TRANSPORTED TO LOGAN RE GIONAL MEDICAL CENTER WHERE X-RAYS REVEALED FRACTURES TO THE THREE MIDDLE TOES. RECOVERY TIME UNKNOWN.

PICKING UP METAL TO REPAIR A LOADER JUST RECEIVED REPORT ON 7-8-03

EE AND COWORKERS WERE MOVING A RR II THAT HAD A BROKEN OPERATOR FRONT SIDE TRAM CHAIN 1 BREAK OUT BY THE LOB. THEY HOOKED A SCOOP UP TO THE BACK OF THE RR II CHAIN TO MOVE. EE WAS STANDING ON THE INSIDE OF THE RIB WATCHING CABLE WHEN THE ATRS STRUCK HIS RIGHT KNEE. HE STATED HE FLAGGED THE OPERATORS, BUT NEITHER SEEN HIM.

EE WAS AWARDED 5% OCCUPATIONAL PNEUMOCONIOSIS BY WEST VIRGINIA WORKERS COMPENSATION. EE DID NOT HAVE ANY TIME AWAYFROM WORK. THISIS CLASSIFIED AS OCCUPATION ILLNESS.

EMPLOYEE WAS OPERATING A 10 SC SHUTTLE CAR WHEN HIS ARM WAS CAUGHT BETWEEN THE COAL RIB AND THE SHUTTLE CAR. HE WAS TREATED BY EMT AND TRANSPORTED TO LOGAN GENERAL. HE WAS DIAGNOSED WITH A DIS LOCATED LEFT ELBOW AND TISSUE DAMAGE. RECOVERY TIME IS UNKNOWN.

EE WAS BLOCKING UP THE PAN OF A MINER WHILE LIFTING IT WITH A SCOOP. THE PAN SLIPPED OFF THE SCOOP BLADE AND LANDED ON THE BLOCKING. A HALF-HEADER FLEW UP AND HIT EE IN THE MOUTH ON IMPACT. EE FINISHED HIS SHIFT & DROVE HIMSELF TO THE HOSPITAL & RECEIVED 8 STITCHES IN HIS UPPER LIP.

EE WAS IN THE LOADER & HE LET THE BUCKET DOWN TOO FAST & HARD. WHEN IT HIT THE GROUND IT JOLTED HIS BACK.

EE HAD JUST ARRIVED ON THE #1 SECTION & WAS STANDING IN THE SPAD #2656 MIDDLE OF THE #4 RT. X-CUT ONE BREAK INBY THE FEEDER. WHILE TALKING TO THE SCOOP OPERATOR A PIECE OF DRAW ROCK APPROX. 40 "X 33"X 1-1 1/2" FELL FROM BETWEEN THE MIDDLE BOLTS DRIVING HIM TO THE MINE FLOOR. HE WAS TRANSPORTED TO HOSP. WHERE X-RAYS WERE NEGATIVE. SCHEDULED FOR PHYSICAL THERAPY FOR SORE BACK.

EMPLOYEE WAS HELPING MAKE A BELT MOVE. HE GOT HIS FINGERNAIL TORN OFF BETWEEN SHIEVE ROPE AND CHAIN AS IT WAS BEING TIGHTEN BY SCOOP, INJURY REQUIRED FOUR STITCHES.

EE & CO-WORKER WERE RECOVERING 60 LB RAIL 1 BREAK INBY OLD #5 HEAD, WHEN THE ACCIDENT OCCURRED. CO-WORKER WAS USING A FORKLIFT TO PULL A 60 LB CURVED RAIL INTO A CROSSCUT. THE RAIL STRUCK THE RIB CAUSING IT TO SWING & STRIKE EE IN RT FOOT.

EE WAS WALKING ACROSS THE SECTION #3 ENTRY WHEN A HANGING ROPE HANGER HOOK CAUGHT HIS NOSE. EE RECEIVED 6 STITCHES IN HIS NOSTRIL.

AN UNPLANNED ROOF FALL OCCURRED IN THE 34 ENTRY @ BREAK #4, SPAD #15, MEASURING APPROX. 90' X 18' X 6'. THE FALL WAS APPROX. 6900' FROM THE ACTIVE SECTION, NO EQUIPMEN INVOLVED. AREA WILL BE R EHABILITATED.

EE WAS WALKING AROUND TRUCK & KNEE GAVE OUT, BUCKLED & TWISTED. STATEMENT BY MR. HIPSHIRE

INJURED & 2 CO-WORKERS WERE HANGING MINER CABLE (14CM15) & WATERLINE (1 1/2") WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. HE WENT TO LOGAN GENERAL WHERE HE SAW A SPECIALIST IN JULY 2, 2002 & WAS DIAGNOSED WITH A STRAINED BACK. THIS ACCIDENT DID NOT BECOME LOST TIME UNTIL 7/8/02 BECAUSE EE HAD VACATION SCHEDULED FROM 6/28/02 TO 7/7/02.

EE RECEIVED SHOCK AFTER UNHOOKING A THUMPER FROM FINDING A GROUND IN MINER CABLE. HE SAID HE FLIPPED TOGGLE SWITCH OFF, DEPRESSED ACTIVATOR WHEN HE FELT A SHOCK. HE WAS TAKEN TO HOSPITAL WHERE SEVERAL TEST WERE PERFORMED. UPON INVESTIGATION THE THUMPER KNOB WAS FOUND ON HIGH. THIS REPORT IS FILED DUE TO AN MSHA VIOLATION. MANAGEMENT BELIEVES A MEDICAL CONDITION KEPT EE FROM WORK.

EE CO WORKER WERE HANGING MINER CABLE AND WATERLINE INT HE #5 ENTRY WHEN HE FELT A SHARP PAIN IN HIS BACK. HE WAS TREATED BY SECTION EMTS AND THEN TRANSPORTED TO LOGAN GENERAL HOSPITAL WHERE H E WAS DIAGNOSED WITH A SEVERE BACK SPRAIN. RECOVERY TIME IS UNKNOWN. NOTE ACCIDENT OCC ON 5-9 BUT DID NOT COME REPORTABLE UNTIL 5-13-02. DUE TO UNSCHEDULED WORK DAYS AND WAITING ON RESULTS.

EMPLOYEE WAS AWARDED 5% FOR OCCUPATIONAL PNEUMOCONIOSIS BY THE WV WORKER'S COMP. DIV., OFFICE OF OCCUPATIONAL PNEUMONCONIOSIS.

EE WAS SHOVELING THE OFFSIDE OF THE #2 BELTHEAD WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. AFTER SEEING A SPECIALIST HE WAS DIAGNOSED WITH A SEVERE SPRAIN TO HIS LOWER BACK.

EE WAS BENIDNG A BOLT TO PUSH IT INTO THE TOP AS HE APPLIED PRESSURE ON THE BOLT, IT SLIPPED OUT OF HIS HANDS AND STRUCK HIM CAUSING THE INJURY.

EE SECTION FORMAN WAS HANGING MINER CABLE SLACK INT HE #3 ENTRY WHEN HE FELT A PAIN IN HIS RIGHT HAND. HE WAS TRANSPINTED TO LOGAN GENERAL WHERE X RAYS WERE NEGATIVE. HE FOLLOWED UP WITH A SPC ILIST WAS DIAGNOSED W/A STRETCHED LIGAMENT BETWEEN THUMB INDEX.

EE & CO-WORKER WERE MOVING A NEW CLA TO INSTALL WHEN HE LOST HIS FOOTING & FELL. THE NEW CLA HE WAS ROLLING CAUGHT HIS LEFT LEG BETWEEN THE OLD CLA & THE NEW ONE. HE WAS TRANSPORTED TO LOGAN G ENERAL HOSPITAL WHERE X-RAYS WERE NEGATIVE. EXPECTED RECOVERY TIME IS THREE WEEKS. HE WAS DIAGNOSED WITH A DEEP CONTUSION TO THE LEFT LEG & RELEASED.

EMPLOYEE AND WORKERS WERE IN THE PROCESS OF BUILDING A ROCK BOX AT NEW #6 HEAD. EMPLOYEE AND CO-WORKER LIFTED A 4'X 8' PIECE OF 1/4" SHEET METALAND HE FELT A BURNING SENSATION IN HIS LOWER BACK. HE FINISHED HIS SHIFT AND FILLED OUT AN ACCIDENT REPORT AT END OF SHIFT. NEXT DAY HE WENT TO HOSPITAL AND WAS DIAGNOSED WITH A MUSCLE STRAIN TO HIS LOWER BACK.

EE WAS ON HIS KNEES SHOVELING BELT, WHEN A PIECE OF ROCK FELL FROM THE RIB HITTING HIM ON THE HEAD & BACK FORCING HIM TO THE BOTTOM CAUSING THE INJURY. (I WASN'T INFORMED ABOUT THIS ACCIDENT W UNTIL 9/17/01. THAT IS WHY THIS REPORT IS LATE.)

EE REMOVED THE RIGHT RIB CATHEAD FROM THE SECTION POWER CENTER LOADING TO FIND A GROUND ON THE CABLE HE WENT TO RIB WHERE REFUND THE GROUND. AS HE WAS LEAVING HE SLIPPED AND FELL CATCHING HIMS ELF WITH LEFT HAND AGAINST RIB RECEIVING A SHOCKFROM THE TESTED TRANSPORTED TO LOGAR GROUND FOR OBSERVATION EKG NO ABNORMALITIES.

EE EXITED SCOPP TO UNLOAD OIL. HE REALIZED HE WAS STANDING ON MOVE BELT LYING ALONG RIB. HE BRACED HIMSELF WITH LEFT HAND ON SCOOP CANOPY. HE TOOK A COUPLE OF STEPS THEN SLIPPED; FELL ON RIGHT SIDE LANDING ON SCSK. HE FINISHED THE SHIFT CONT'D TO WORK UNTIL 8/12/01 RECOVERY UNKNOWN

EMPLOYEE WAS BACKING THE CM WHEN A PIECE OF DRAW ROCK APPROX 48" BY 45" BY 1-6" FELL ONTO THE CM AND SLID ONTO HIS RIGHT FOOT. EE WAS DIAGNOSED WITH A CONTUSION TO THE RIGHT FOOT. RECOVERY TIM E 2-3 WEEKS.

INJURED WAS CARRYING A FAN COVER FOR A BELT MOTOR WHEN HIS RIGHT HAND WAS CAUGHT BETWEEN THE FAN COVER AND HANDRAIL FOR STEPS TO THE OFFICE. INJURED RECEIVED SUTURES TO THE WOUND AT LOGAN GENE RAL HOSPITAL.

INJURED HAD COMPLETED BOLTING FIRST ROW OF BOLTS IN THE #3 HEADING. HE NOTICED A LOOSE PIECE OF DRAW ROCK APPROX. 33"X23"X3" FELL BEHIND HIM STRIKING HIS LEFT SHOULDER. HE WAS TAKEN TO LOGAN G ENERAL WHERE X RAYS REVEALED A HAIRLINE FRACTURETO THE LEFT SHOULDER.

EMPLOYEE WAS USING A SLATE BAR TO PUSH A RAIL OVER IN ORDER TO LUG IT INTO A TIE, WHILE DOING SO HE FELT PAIN IN HIS LOWER BACK, HE CONTINUED WORKING BUT AFTER A COUPLE OF HOURS THE PAIN GOT WORSE. HE THEN WENT TO THE HOSPITAL AND HAS BEEN OFF WORK SINCE THEN.

EE WAS UNLOADING SUPPLY'S FROM A FLAT CAR WHEN A ROCK 3 FT X 4 FT BY 10 IN. FELL FROM BETWEEN THE BOLTS STRIKING HIM ON THE FOOT CAUSING THE INJRY.

EMPLOYEE WAS PUTTING A FORK LIFT ON THE BAR OF THE FORKLIFT AT THE TIME OF THE ACCIDENT. THE FORK DIDN'T CATCH WHEN HE DROPPED IT ON THE BAR AND HIS LEFT HAND WAS SUPPORTING THE FORK. HIS HAND GOT CAUGHT BETWEEN THAT FORK AND THE OTHER FORK CAUSING THE INJURY TO HIS HAND.

EMPLOYEE WAS HELPING UNLOAD BELT FROM A FLAT CARBY USING PLIERS TO HOLD ONTO THE BELT IN ORDER TO GRIP THE BELT. THE PLIERS SLIPPED WHICH LET HIM FALL CAUSING THE INJURY.

EMPLOYEE WAS IN THE PROCESS OF TRAMMING R/B FROM THE OPERATORS COMPARTMENT WHEN THE ACCIDENT HAPPENED. HE WAS BACKING OUT OF THE #4 HEADING AND BEGAN TO TURN INTO X-CUT BETWEEN #4, #3 WHEN HIS LEFT ELBOW HIT THE RIB. HE WAS TAKEN TO GENERALHOSPITAL WHERE X-RAYS WERE NEGATIVE. HE SAW A SPECIALIST AND A HAIRLINE FRACTURE WAS FOUND.

EE WAS ATTEMPTING TO UNLOAD MINE RAILS FROM A FLAT CAR USING A SLATE BAR. HE HAD PLACED THE BAR UNDER THE RAILS TO PINCH THEM OFF THE CAR. THE BAR SLIPPED CAUSING HIM TO FALL DOWN AGAINST THE RAILS AND THEN DOWN ON THE FLAT CAR.
