EE was standing on top of the miner hanging the miner cable he was holding against the top, he strain his back. EE tried to work he called in on 1-27-12 and said he hurt his back. I didn't get to talk to him til 2-7-12. I tried numerous time to get in touch with him. EE didn't say anything to the men on the section or management at the mine.

Roof bolter operator was drilling hole for 10' rope bolt when steel clogged with mud. Operator removed 3' pusher from hole leaving 7' of steel in top. While beating 3' pusher on pot to remove mud from steel the remaining 7' of steel fell from hole hitting operator in knee.
