Employee stated they were assisting an electrician to place a rub rail back on the miner, they smashed their left index finger between the rub rail and the frame of the miner causing a laceration, needing two sutures.

EE has been diagnosed with Occupational Pneumoconiosis

Employee has been diagnosed with Occupational Pneumoconiosis.

Employee stated while running the miner, loading out cut rock, a piece of rock ricocheted back striking them on right hand. Employee was seen at Medical Center and received six sutures and a follow up with a physician for fracture to end of finger.

Employee stated they twisted their back and felt a sharp pain in lower back while loading glue bolts onto the left roof bolter.

Employee was using the side of EE's drill pot to push EE's roof bolt high enough for clearance to install into the drill chuck. EE dropped the pot down to far and pinned EE's left foot between the drill pot and the mine floor.

Employee was asleep while riding on #7 diesel mantrip, when the operator moved over to the right to allow clearance for a pick-up truck to pass. Employee's foot was caught between a jersey barrier and canopy post.

Employee stated EE was moving a bucket of plaster, when EE set the bucket down, the plaster splashed into both eyes. Prescription medication was used to treat both eyes.

Employee stated they were applying brakes, when the front end jerked causing them to run under the moving slope belt.

Electricians were trouble shooting panel on miner, before they knocked the breaker injured bent over to look inside to see where the smoke was coming from when an arc flash occurred. Employee was burnt on left side of face and right hand, removal of burnt skin was needed.

Employee was diagnosed with pneumoconiosis.

While drilling for cable bolt, reached with left hand grabbing cable bolt from tray and went to pull it off, cable bolt slipped through hand and momentum caused them to hit their hand on drill pot causing laceration.

Employee stated they were changing out the belts on the blower motor on a roof bolter, they had crawled to the edge of the machine, when the shield flipped up knocking them to the ground landing on their right arm/shoulder.

Employee stated EE was trying to pull a P-50 pump that was stuck in the mud, when EE felt some pain in EE's right groin. EE is awaiting surgery to repair the damage.

Employee was re-stacking a busted pallet of block onto a new pallet, when they picked up a block and twisted around, they felt a pain in their back. Employee began missing work on 07/28/2025 after following up with their family physician.

Employee was diagnosed with pneumoconiosis.

Employee stated they were pulling draw rock with a slate bar, when a piece of draw rock measuring 40"long X 27"wide X 3" thick caught a pie pan flipping the rock over onto their right foot.

Escape hoist malfunctioned for more than 30 minutes, no injuries, no mine emergency has occurred. Paul's Fan Repair, replaced the cable and repaired the hoist.

Employee was attempting to flip 60" belt over by hand, when EE felt a pain in EE's right arm.

While lapping belt EE hooked up a pallet puller to a 15' piece of 60" belt, when the scoop operator began to pull, the belt flipped up knocking EE to the ground, landing on left shoulder. "Employee continued working until 06/09/2025".

Employee was diagnosed with pneumoconiosis.

EE stated while riding the MAC-8 to the section, from the end of the track, a piece of draw rock fell striking them on top of the left hand resulting in a laceration.

EE stated they were putting up the outside bolt, EE was trying to straighten the bolt when they slipped and fell into the bolt and pie pan. EE went was seen at Hospital and received 5 sutures and made an appointment with a dentist.

Employee stated while backing employee's 4-wheeler over to 1-A belt, employee's left thumb was caught between the steering wheel and an elevated fire valve that was pointing downward.

Employee stated EE was installing a rib bolt when a piece of drawl rock measuring 32" long X 1" wide X 2" thick fell sticking EE in the face.

While working on the right miner conveyor chain EE was using a 2lbs hammer to strike one of the conveyor links when a piece of metal entered the right eye. EE was wearing safety glasses, however the glasses became repositioned on the face slightly exposing the eye. The metal was surgically removed from the right eye.

EE stated EE was reversing drill steel to remove from mine roof, as EE was lowering the drill pot the steel flew around and struck EE's right hand.

EE stated while installing rib bolt a piece of draw rock (7"W X 7" L X 4" T) fell from top sticking EE on the right ring finger causing a fracture to the end of the digit.

Employee stated EE smashed finger between 4" I beam and scoop bucket. EE finished EE's shift and sought medical treatment. X Ray revealed broke R index digit. EE did not miss any scheduled shifts.

While unloading prop setters from a scoop bucket, a coworker tossed a prop setter striking injured's middle finger resulting in a dislocation and a laceration requiring seven (7) stitches.

Miner was diagnosed with pneumoconiosis.

Employee was changing batteries on a scoop at Break 11 on 1B Belt when one of the battery lids fell striking the left hand resulting in a laceration requiring three (3) sutures.

Miner was diagnosed with pneumoconiosis.

Injured was stepping off of the DDR Roof Bolter, twisted ankle after stepping into a hole resulted in a fractured foot.

Employee was handling waterline for the continuous miner when a rib measuring approximately 5 feet long x 42 inches wide x 18 inches thick rolled off the corner of a pillar block striking the employee injuring EE's right lower back.

A non-injury roof fall was discovered at No. 9 Break adjacent to the Main Line No. 4 Conveyor Belt near spad 7660. Fall measured approximately 54 feet long X 20 feet wide X 6 feet high.

Employee was operating a DDR roof bolting machine. The tram was engaged to move the machine when a piece of drill steel lodged between the frame and the cats came out striking another employee working near the machine just above the left eye. Resulted in four (4) stitches.

A non-injury roof fall was discovered in a return airway measuring approximately 60 feet long x 20 fee wide x 10-12 feet high. Location; Old 2A Panel; 31 break; near spad 3937.

After setting up DDR Roof Bolter in the No. 6 Heading, employee stepped off machine and stumbled falling to the mine floor and landing on radio injuring lower back.

Employee attempted to carry two 50 pound bags of rock dust, slipped and fell landing flat on back.

While changing out waterline on the continuous miner, employee's forearm struck a burr on a shield causing a laceration requiring 10 stitches.

While monitoring MRS gauges, a piece of rib rock fell and struck employee on leg. Resulted in a fractured fibula, ankle and toe.

Employee was working on plastering a stopping when TC Spray splashed in their eye.

At approximately 08:35 pm a non injury ignition occurred on the No. 3 Section near the No. 3 Entry.

Miner was servicing planetary on the right side continuous miner on the No. 2 Section. While working, the inside cap of the planetary fell striking EE's left pinky finger. Resulted in a laceration requiring glue to close the wound.

A non-injury roof fall occurred on the No. 4 South Section one break inby the section feeder, 100 feet inby spad 8615. The fall measured approximately 17 feet long x 20 feet wide x 12-14 feet high.

Employee stated EE was removing a line curtain from a board. When EE lifted upward, EE felt a pain in right shoulder and neck. EE continued work for 30 mins, then asked to exit the mine for medical examination. EE refused 1st aid on the section and was transported outside to ambulance which EE also refused. EE was examined at ER and given time off to see a specialist.

A non-injury accident occurred on 3A Panel in #6 entry at 25c/c. A fall of roof occurred blocking one entry of the intake and the primary escapeway. When discovered, all personnel inby affected were withdrawn outby the location. Propsets & cribs were installed around the affected area and dangered off. The lifeline was rerouted and all personnel trained on the new route of escape.

Employee stated while carrying roof bolts, EE stepped in a hole and felt a pain in EE's back and groin. EE was transported via ambulance to hospital where EE was given time off work and referred to a specialist.

Employee stated while exiting a scoop, a rock fell from the top between rib and roof, striking EE on the left foot. EE was immediately transported outside and taken by ambulance to hospital. There EE received X-rays and was diagnosed with left 5th toe intra-articular fracture medial base. EE was referred to specialist and given time off work.

Employee stated while grinding a weld during a repair, the grinder fouled causing the tool to kickback contacting EE's left gloved hand resulting in a laceration. EE refused 1st aid and traveled to medical facility where EE received sutures. EE returned to work on the next scheduled shift.

Employee stated, a strapped kettle bottom fell from between the strap and the rib deflecting off the rib and striking EE's right lower leg. EE was transported outside by EMT to waiting ambulance which EE refused. EE traveled POV to medical center where x-ray determined a fracture of the fibula. EE has been granted time off work and remains off work at this time.

During a routine required check, the emergency hoist did not operate properly due to a malfunction of a limit switch. All personnel were immediately evacuated from the mine until repairs were made. NO persons were injured or trapped during this event. The alternate escapeway was still available. The repairs were completed that evening, the hoist tested and performed as required.

Employee has been diagnosed with Occupational Pneumoconiosis.

Employee stated was assisting electricians repair broken CM conveyor chain. The chain had been lifted with a chain ratchet approx. 24" to access a drive cog when it fell striking EE on the left hand & arm. 1st aid was rendered, and EE was escorted outside by EMT to waiting ambulance. EE traveled to ER, treated, and fully released. EE returned to work on their next scheduled shift.

A non-injury fire occurred when acetylene gas leaking from a cylinder was accidently ignited. The fire was contained within the metal cylinder storage box. Fire extinguishers, rock dust and water were used to extinguish the fire in about 13 min. The mine was evacuated until area was deemed safe by State, Federal and Company officials. The damaged equip was removed from the mine.

Miner was working on battery forks on the underground scoop when EE's right arm became caught between the scoop frame and battery fork. Miner was released to work after the accident and has continued to work until 01/31/2024 as a result of a required surgery.

Employee was loading supplies on bolter. EE was placing a bundle of plates on the bolter and caught a gloved right ring finger between the bundle of plates and a shield. EE was transported outside to waiting ambulance, examined, and refused service. EE traveled by POV to medical facility for examination. EE was released to return to work on the next scheduled shift.

Employee stated was swinging boom to install bolt and was resting a hand on the tray to steady self. A piece of coal fell on left index finger against the tray. EE was transported outside to waiting ambulance. EE was examined by ambulance crew and refused service. EE traveled by POV to ER, was examined and released to return to work on EE's next scheduled shift.

A noninjury roof fall occurred outby 1 section, 60 ft inby spad 4124 in #3 entry approximately 20' wide, 40' long by 10' thick. The crew was withdrawn outby the area until the alt escapeway could be re-established, a ventilation change was approved and ventilation controls adjusted. The affected area had breaker timbers, barriers and danger signs erected at all approaches.

Employee stated was pulling belt slack when the gripper detached, recoiled and struck EE in the face.

Employee stated EE was swinging the DDR drill pot over to install a roof bolt when a portion of draw rock measuring 14" wide, 24" long, 1.5" in thickness fell from the mine roof and struck EE's left forearm causing a 3 inch laceration. EE was taken to a hospital where EE received medical treatment.

Employee stated, while making a belt splice on the stacker belt, the take up cradle rolled back mashing EE's left foot between the cradle and the frame. EE was transported via ambulance to ER where X-rays revealed broken bones in EE's left foot. EE was referred to a specialist. Specialist has removed from work and EE remains off work.

Employee stated, while drilling hole, the finisher steel broke. EE lowered the pot to change steel. As EE reached for the pusher, the finisher dropped out of hole, striking EE's L, 5th finger between steel and pot. EE finished shift and sought treatment. EE was treated and released to return to work. On 12/28/21, EE returned to doctor and received 3 stitches on this date.

Employee stated that the dust hose to the bolter was stopped and as EE was removing the hose from the back of the dust collection system, EE's left arm came in contact with a breeze band causing a laceration.

Employee stated while EE was trouble shooting a 4-wheeler control panel, EE was using a utility knife to cut a zip tie that was holding the leads together. EE was holding the leads with the left hand and as EE was cutting the zip tie, the utility knife slipped causing a laceration between EE's left index finger and thumb.

Employee stated employee was operating a scoop near spad #2683 on the intake airway when a cable bolt that was buried with mud was flipped up striking the employee's left ear. Employee continued working the rest of the shift and did not receive medical attention until 10/17/2021.

Employee stated while installing a 5" resin bolt into the inside hole, the 6" wrench they were using to push the bolt, broke and the upper half struck them on the safety glasses causing a small laceration to left eyelid. EE exited mine and sought medical treatment, received 1 suture and was fully released to return to work. Employee returned to work on next scheduled shift.

EE was manually rolling de-energized miner drum using knee on the miner head when knee slipped off the drum. EE landed on the mine floor and was struck by the turning CM drumhead with a bit lug to right knee. Employee finished shift and did not seek medical treatment until 12 hrs later. EE returned to work for next scheduled shift.

Employee stated while drilling a cable bolt hole, the drill steel became lodged. EE was jerking on the steel when EE felt a pop in the left shoulder. EE exited mine to waiting ambulance but refused service. EE drove self POV to Hospital where EE was give time off work and referred to specialist. EE remains off working pending determination of specialist.

Employee was installing a roof bolt into the mine roof at which time a piece of rock fell from the mine roof striking the employee in the left bicep area causing a laceration and left palm/thumb area causing a laceration.

A guide cable on the emergency hoist was discovered loose during a fire drill. Upon discovery, both sections were notified and production was immediately halted. EE's were withdrawn from the mine until repairs were made. No persons were injured or killed as a result of this incident. The repair was completed at approximately 4:10am the following morning.

Employee stated while drilling hole for outside bolt in first row of #8 entry, a rock 27"x8"x3" fell from the top onto the canopy, slid off canopy and fell to the drill steel tray. EE stated the rock struck and injured EE's Rt 3rd & 4th finger. EE was transported to medical facility where EE was treated and released to full duty.

While surveying locations for conveyor belt support installation, a section of 4 inch water line that was supported by chains fell striking the individual in the head and back.

Employee was driving mantrip, steering wheel jerked causing sprain to left hand

Employee was climbing onto loader, hand slipped and EE fell to ground causing a back strain to upper and lower back.
