Employee stated EE knelt to measure coal. Upon rising, EE's R knee popped, and felt pain when EE put weight on it. EE finished shift. EE refused ambulance and medical attention at this time. EE sought medical attention on 11-23-19 and referred to orthopedic specialist. EE visited specialist on 12-2-19 and given time off for further evaluation.

Employee stated EE was crossing slope cable. EE stated the cable moved and struck EE in the leg. EE finished shift and did not seek medical attention at this time. EE first sought medical attention on 11-25-19. Upon exam, an x-ray was ordered which indicated a fracture. EE has been referred to Orthopedic specialist.

A non-injury accident occurred when the slope car malfunctioned. Production was halted and people withdrawn from the mine. Both escapeways were available at all times. NO persons were injured as a result of this mechanical malfunction. The malfunction was corrected at approximately 3:00pm.

A non injury incident occurred when the axle to the weight car broke. All personnel were withdrawn from the mine. Both the primary and alternate escapeways were available for use. NO PERSONS WERE INJURED. The weight car was removed and the slope car was returned to service approximately 50 minutes later.

Employee has been diagnosed with simple occupational pneumoconiosis

A malfunction of the slope car motor occurred. All workers were withdrawn from the mine. No injuries occurred during this event. The primary and secondary escapeways were both still available. The motor was changed and the car restored to service at approximately 8:00 pm on 8/9/19.

A non-injury event occurred when the slope car mechanically malfunctioned. Production was halted and men withdrawn from the face to effect repairs. Attempts to correct the malfunction were unsuccessful all personnel were removed from the mine until repairs are made. Both primary and alternate escapeways were available for use. NO PERSONS WERE INJURED DURING THIS EVENT.

A non-injury incident occurred when the brakes to the slope car malfunctioned. There was no production at the time. A personnel (fire bosses only) were notified and removed from the mine. The primary and alternate escapeways were still available. No one was injured during this event.

A non-injury fall of roof in outby area occurred in the intersection at spad #611 above the roof support anchorage. The area was dangered off until additional roof support was added during the following shift.

Employee stated ee was walking next to belt line when foot became stuck in the mud resulting in a twisting motion to back causing severe pain. Employee was transported outside on backboard to waiting ambulance. Employee was taken to hospital and was given time off until 9/10/18.

Employee stated ee was making a belt splice when ee missed a nail ee was pounding striking left index finger. Employee bandaged self and refused ambulance then traveled POV to hospital where ee received stitches. Employee did not miss any scheduled work

During inspection, a cable connection was found to be installed not according to manufacturer's recommendations on the Emergency Hoist. All EE's were withdrawn from the face area to outby the feeder dumping point while adjustments were made. No persons were injured or killed during this event and the alternate escapeway was still available. The repairs were completed in two hours.

Employee stated was walking around scoop when EE tripped on a box and fell across the bucket of a scoop striking EE's ribs. EE finished the shift and sought medical treatment and was given a full release but was diagnosed with a rib fracture. EE returned to work on EE's next scheduled shift and has not missed any work.

Employee stated was shoveling belt in sloppy conditions. As EE was dumping a shovel onto the belt, the mud adhered to the shovel causing it to contact the belt. When EE jerked the shovel back, EE felt a pain in lower left abdomen. EE exited mine and refused transportation. EE traveled to ER and was referred to specialist. EE is off work for unspecified amount of time.

Employee stated was assisting others installing a track switch. EE was standing between rails when EE was struck by a drifting mantrip. EE finished shift and did not seek medical treatment at that time. EE later visited the ER and was given time off work.

Employee stated the shuttle car canopy contacted a curtain board breaking it in half. The inby end of the board sprung under the canopy and as the SC moved forward, it forced the board toward the employee. The end of the board struck the employee under the jaw causing a laceration. EE controlled bleeding and traveled outside. EE waived amb and drove to hosp where EE recv'd sutures.

A mechanical failure of the slope car occurred resulting in the emergency braking system to lock up. A grounded cable to the brake coil was discovered and a new wire was installed. Production was halted, men withdrawn to outby the dumping point until repairs were completed at 11:17 am.

Employee stated was downstacking structure on rail flatcar when another structure rolled down & struck right hand causing a large knot on the back of hand. EE & co-worker were working on each side of the car. 48' structure fell about 12'18' hitting EE in right hand which was holding another piece of structure.

Employees stated was picking up a cinder block when another block fell off pallet striking the ring finger on left hand. EE refused first aid and exited mine. EE refused ambulance and traveled via POV to LRMC ER where EE had a X-ray and found that EE received a broken fourth distal digit. EE was given 3 days off work and referred to a specialist.

Employee stated was assisting setting the feeder. When the feeder was properly positioned the coworker lowered the feeder onto the tailpiece. At this time, the tailpiece chute suddenly moved catching the right thumb between the tailpiece and the chute. The EE applied a dressing to the wound and finished the shift. After work EE received 9 sutures and was released to return to work.

Employee stated was in the process of hanging fly pads between #4 and #5 Entry 1 bk outby last open line. EE stated went to tear down fly pads outby in #5 Entry, when EE turned to take a step after tearing the fly pad down, EE said EE stepped in a hole and turned ankle. EE stated that it was wet and muddy in this Entry. Employee was transported to RGH by ambulance.

Employee stated ee had already installed 2 bolts in row and decided to install a spot bolt next to the rib. After installing the spot bolt, ee swung in and a rock 48" x 32" x 4-5" fell out between bolts hitting ee in the back. EE went to Princeton Community Hosp. ER. X-ray indicated possible fracture of C-7 and ee was given time away from work and referred to specialist

Employee was assisting 2 others in rotating the drums of the CM when they felt pain in the lower back/hip. EE refused 1st aid, waited for a short time but then exited the mine to recuperate. EE traveled home but on 2/10/18 at 4:45 pm stated they were going to RGH ER to be checked. After examination, EE was give time away until 02/16/18 or sees a PCP. EE remains off work.

A roof fall was discovered at apx. 7:00am on the 3 south mains panel, 7c/c, #6 entry between spad #4128 & #4179. The fall measured apx. 74 in length, 20 in width and 2 to 8 in thickness. There were no injuries or interruption in ventilation. The area was dangered off, the HV cable was re-routed and timbers were set preventing access to all approaches of the area.

Employee stated was cleaning sprays on CM. As EE stepped to the back of the CM to help a coworker, EE turned back to the head and struck the back of left hand on a sharp piece of metal causing a laceration. EE was transported to ER and received 6 stitches. EE returned to work on next scheduled shift.

A contractor Employee stated was sliding track ties under the rail when the rail dropped pinching left ring finger. EE applied a rag to the wound and refused further 1st aid. EE was transported to the surface to waiting ambulance and then to Raleigh Gen. Hosp ER where EE received stitches. EE remains off work. EE was not wearing gloves at the time.

Employee stated was drilling hole in 4L cc. While installing finishing steel in the hole, a piece of rock fell pinching right thumb between the rock and pusher steel causing a crushing injury. EE was transported to Welch hospital. EE was transferred to RGH where EE was admitted overnight. EE underwent surgery and was released the next day. EE remains off work at this time.

Employee has been diagnosed with Occupational Pneumoconiosis.

Employee was pulling CM slack into heading for left crosscut when the slack fouled causing them to be pulled backward onto their left knee. After throwing the cable down EE tried to stand and felt a pain in the left knee. EE was transported to surface to ambulance and to hospital. EE was referred to ortho dr and remains off work at this time.

A CM cut the fiber optic cable used for tracking. The dispatcher notified the RP and all personnel inby to begin manual tracking procedures. The repairman was summoned to make repairs. Approaching the 12hr limit mentioned in the ERP, the hotline was notified at 12:46pm Ref#00793039. No persons were injured or trapped. The cable was repaired and tracking restored at 3:00pm.

Employee has repeated trauma disorder (osteoarthritis).

Employee stated EE was walking across bridge and stepped into hole which resulted in EE twisting left knee. Employee was transported to emergency room via ambulance. X-rays indicated no fracture but employee was referred to PCP for further evaluation.

Employee stated ee was installing inside bolt in first row when draw rock fell inby ATRS which caused ee to react in a twisting motion resulting in a strain to lower back. EE was transported to RGH ER and released to return to work 2 days from 09/28/2017.

Employee was squaring end of belt for splice when a piece of rock fell from the top striking them on the left arm. EE was administered 1st aid and transported to surface and then to hospital ER. Diagnosis was broken left ulna and laceration. EE fracture & wound was treated in surgery and was admitted to hospital.

An electrical malfunction caused fuses to blow to the emergency hoist. Production was halted, personnel were withdrawn to the feeder line until repairs could be made. The back up power supply was initiated at 10:04am and the hoist returned to service. No persons were injured or ill during this incident

Employee stated was stepping off an I beam on the #1 belt drive landing on left leg causing trailing right knee to twist. Employee finished shift and did not seek medical treatment at that time. EE sought medical treatment on 6/16/17 and began missing work on 6/18/17.

Employee stated while loading structure, ee strained left groin area. Employee finished shift and did not seek medical treatment at this time. After arriving at home, employee called wishing to seek treatment. Employee visited ER and received X-rays which were negative and discharged. EE visited PCP on 6/7/17 and was given time off to assess possible pulled muscle.

Employee stated while riding a Mac8 to the section, the driver backed into and dislodged a timber which fell striking ee on the hardhat. EE did not loose consciousness, refused backboard and was transported to the surface by EMT with a cervical collar in place. EE was transported to hospital via ambulance and diagnosed with a concussion and given one week off per protocol.

Employee has been diagnosed with Occupational Pneumoconiosis

A wheel on the weight car attached to the slope car broke off. Production was stopped and all miners withdrawn to outby the feeder. The primary escapeway is still available and the alternate escapeway is still travelable. The weight car was removed from the slope car and the slope car was available at 4:12pm. No persons were injured or killed or trapped during this incident.

The slope car used for hoisting men in/out of the mine had an electrical malfunction. A production was stopped and miners withdrawn to outby the feeder. The primary escapeway is still available and the alternate is still travelable. No persons were injured or killed. The slope car was repaired at 10:55am

The slope hoisting car that hauls men in and out of the mine went down. A bad battery was found to be the problem. The slope car was fixed at approximately 12:30am. Men were still able to get out of the mine and no portals were blocked.

Employee has been diagnosed with Occupational Pneumoconiosis

Employee stated was assisting persons build a stopping when air pressure pushed out the stopping. A block fell striking EE on the hand. EE exited the mine and sought medical treatment. EE was referred to a specialist and was released back to work. EE worked several shifts and returned back to the doctor and began missing work 02/02/2017.

Employee stated was cutting bands from around a pallet of cinder blocks when the cut end of the band flew up and struck employee on the face under safety glasses. Employee exited the mine, sought medical treatment and was referred to a specialist. The specialist prescribed the employee with eye drops and fully released the employee to return to work.

This is a non-injury incident. The slope car had an electrical malfunction at 3:59am and was temporarily out of order. All underground personnel were notified with section crews withdrawn to the fresh air congregating point. An electrician was sent and a transformer was replaced. The car was running again at 4:21am

Employee was checking fire extinguisher indicator when handle got pushed, causing it to discharge in right eye. Employee had to get prescription eye drops for right eye.

Offside roof bolt operator was installing supplemental roof bolts when right hand/arm was caught by the turning drill steel.

An electrical malfunction in the slope car control system caused a fuse to blow. Electricians were dispatched immediately to resolve the issue. The mine was not producing at the time. Only 10 employees were underground at the time and they were evacuated until the fuse was replaced at 3:00pm.

Employee has been diagnosed with Coal Workers Pneumoconiosis.

Employee stated, was roof bolting in 5R c/c when struck by loose rock which fell during the bolting cycle. Rock impacted personal canopy and then pushed employee to ground. EE was transported to surface by EMTs and taken to RGH ER. EE was treated and released but returned to doctor 2 days later and is away from work until 6-3-16

Employee stated that EE and another employee were lifting a crossover shaft for a Fletcher RB. While lifting EE felt a pain in lower back, neck, and groin

Employee stated was lifting a CM cable to drag another cable underneath. EE finished their shift and did not seek medical attention at that time. On 5/13/16, EE visited ER for evaluation and was released at own discretion. EE continued to work until 5/20/16 when EE returned to doctor. EE was given time away from job until 6/1/16.

Employee was entering a scoop canopy. When EE stepped into the deck, right foot slipped causing EE to twist right knee. EE finished shift and sought medical treatment later that day. EE visited LRMC ER and was referred to Ortho doctor on Monday. Ortho doctor has excused employee from work through 07/02/2016. EE remains away from work.

Employee stated was throwing CM cable out of roadway when felt a pain in right shoulder and bicep. Employee refused first aid and finished shift and sought medical treatment the following morning. Employee visited RGH ER and referred to ortho doctor. Employee was excused from work through 5/6/2016 and remains away from work.

An electronic malfunction of the slope car occurred. No persons were injured or trapped during this event. All personnel underground were notified and informed and advised that the primary escapeway (and emergency hoist) and alternate escapeway were still available in case of emergency.

Employee stated they were on a ladder cleaning lights when the ladder tipped causing employee to fall to the floor. EE was transported to ER and received a sling.

EE stated EE was installing the 1st rib bolt on the opr side. After drilling hole, EE inserted top of the bolt into hole and the plate became stuck in gob. EE jerked the bolt & felt a pain in EE's left shoulder. EE was transported outside & refused ambulance. At Bluefield Reg. Hosp ER EE rec'd X ray. EE was restricted from work until visit to specialist. Started missing on 3/17/16.

An electrical malfunction of the slope car occurred when two fuses blew on the electrical circuit. NO persons were injured or trapped as a result of this incident. All personnel were notified of the situation and both the primary and alternate escapeways were still available. Electricians were immediately dispatched and the car was repaired within 2 hours.

Employee stated EE was driving a rubber tire ride @#17c/c when EE's tire hit a rock jerking the steering wheel striking EE's right wrist. Employee has X-ray which was inconclusive. Employee continued to work until 03/11/2016 and had a follow up exam. X-ray indicated a fracture. Employee was referred to orthopedic dr but continues to work at full duties.

Employee was nailing up a fly pad with back to the rib when a piece measuring 66"x40"x22" fell striking employee and coming to rest on the left foot.

After parking shuttle car, employee walked to the power center and stepped in a mud hole, twisting employee's left knee. Employee was not given time off of work until 2-4-16.

Employee stated EE was attempting to straighten and align a roof bolt into the drill pot chuck using both hands. When EE tried to straighten the bolt, it suddenly spun around causing EE to awkwardly lunge forward. This sudden motion caused a sharp pain in EE's right shoulder. Employee was transported to the doctor where X rays indicated negative for any fractures.

Employee stated after T bar was set, EE walked forward to swing boom outward to install EE's rib bolt. A piece of draw rock 6'L X 3'W X 2"T fell on EE's back forcing EE to the ground. Once EE caught self with EE's hands, the rock slid down EE's back striking EE's right hand causing bruising. Employee was transported to doctor for X-rays which were negative for any fractures.

Employee stated EE was operating CM when a piece of brow rock fell out striking EE across the shoulder driving EE to the ground. Employee finished EE's shift and did not seek medical treatment at this time. EE continued to work and sought medical treatment on 10/26/17 and given time off. EE began missing work on 10/27/17

During routine examination of the hoist, a condition was observed with the brake system which required the car to be taken out of service for repairs. No persons were injured or trapped. All production was halted and personnel withdrawn outby the feeder until the repairs were completed 48 minutes later at 11:18am.

Employer was notified on 1/29/16 that the employee had a pneumoconiosis diagnosis on 8/29/15.

Employer was notified on 1/26/16 by the compensation administrator that the employee received a diagnosis of pneumoconiosis on 8/25/15.

Employee stated he picked up a can of oil, turned and ran into a roof bolt. Employee recv'd 1st aid and transported to a waiting ambulance. Employee sought treatment at RGH ER and was treated and released but did not return to work. Employee sought additional medical attention but results were not shared with this department. Employee remains away from work

AN ELECT MALFUNCTION OF THE SLOPE CAR OCCURRED AT 10:45PM. NO PERSONS WERE INJURED BECAUSE OF THIS EVENT. THIS IS A NON PROD DAY SO ONLY EXAMINERS WERE UNDERGROUND. THEY WERE NOTIFIED OF THE MALFUNCTION AND AVAILABILITY OF THE PRIMARY ESCAPEWAY HOIST IF NEEDED. ELECTRICIANS IMMEDIATELY BEGAN REPAIRS AND REPLACED A DEFECTIVE SCR AND A BLOWN FUSE BY 12:30AM (7/27/15)

Employer was notified by compensation administrator on 6/25/15 that the employee was diagnosed with pneumoconiosis on 6/15/15.

A fall of roof occurred on an overcast at spad643 interrupting proper airflow. Power was removed from all affected areas, all employees were immediately withdrawn from the mine. Air quality or volume to employees was not affected.

The brakes locked up on the Slope Hoist Car preventing it from moving. Employees were brought out until car was repaired. Slope Hoist Car was reparied at 6:00pm.

The Slope Hoist Car was found with a ground fault. No one was in the mine at the time of the incident. It was repaired before the next oncoming shift.

The Emergency Hoist Car was found inoperable. The Coal Mine was evacuated until the Emergency Hoist was returned to normal service.

Employee stated his 4 wheeler bounced him into top striking his head against the roof. Emp refused 1st aid and ambulance. Emp had family drive him home but then decided en-route to go to ER. Emp walked unassisted with no visible injuries into the ER. He was assessed and received full release. Emp returned to doctor on 5/22/15 and was granted time off until 5/30/15.

A malfunction of the hoisting car occurred at 5:22pm caused by two fuses blown in the control panel.All sections and personnel were notified at 5:25pm, production was halted and employees withdrawn to outby the loading point.The hoisting car was repaired at 6:39pm and crews were notified they could return to work.No employees were injured or killed as a result of this incident.

An malfunction of the hoist car occurred at 3:24pm caused by the brakes being unable to be released. All personnel were notified at 3:25pm, production was halted and employees withdrawn outby the loading point. The hoist car was quickly repaired at 3:35pm and crews were notified at 3:36pm to return to work. No persons were injured or killed as a result of this incident.

An unplanned roof fall occurred near spad 3168 in area mined by previous operator. It measured 80'LX15'WX6'H. No persons were injured. No ventilation affected. No escapeways affected. All three approaches have been cribbed and dangered off and no further actions will be taken

A non-injury incident occurred when the automated hoist car mechanism malfunctioned at 12:15am. Electrician was immediately dispatched. Both section crews were withdrawn to point outby the loading point until repairs were completed. Electrician arrived at 12:20am and repaired hoist car for manual operation by 12:30am (15 mim). Complete automated repairs were completed at 12:53am.

Employee stated he was driving a 4 wheeler when it hit a pot hole and bounced him into the top. Employee did not miss work until 09/21/15. Employee received therapy after the injury and had neck surgery on 9/21/15.

The (Bonnie Car) had a wheel that broke off. No injuries occurred from this. Production was halted until Bonnie Car was removed from service. Primary escape way and emergency hoist were fully functional during this time.

Employee received a notification from the compensation provider on 10/10/14 that the employee was diagnosed with hearing loss on 8/6/14. Employee terminated employment at this mine on 4/13/14.

The Hoist slope car used to haul men in and out of the mine was down. It faulted out with no one to fix it at the time. No one was working underground at time of incident. Hoist car was repaired prior to men enter mine at 10:48 am

The Hoist car used to haul men in and out of the mine went down. There was no one working at the mine due to the Labor Day Holiday. It was repaired prior to men entering the mine.

The hoist car became inoperative due to electrical/electronic malfunction. All back-up safety equipment performed as designed. No persons were injured or affected. NO production taking place at the time. All other persons were withdrawn from the mine until repairs could be made. The E-hoist remained fully functional during the period.

A rib roll occurred in the track entry 1 brk outby spad 63 measuring apx 50'L X 16'H X 6'T. No persons were involved or injured. Production was stopped and men were withdrawn outby the affected area. The alternate escape way was immediately rerouted. All approaches were dangered off.

Accident occurred at the bottom of the slope at 6 break. Slick conditions and wet, muddy gloves. Co-worker had a slate bar in the bolt hole, rolling track toward EE. Slate bar slipped from Co-worker's hands when the track rolled forward and the slate bar hit EE in the hard hat and slid down his ear requiring 7 stitches.

An unplanned roof fall occurred between spads 313 and 325. The fall measured 19'W X 45'L X 8'H. No injuries occurred and passage of men and airflow were not affected. This fall was in an entry not normally traveled by the weekly examiner.

An unplanned roof fall occurred along the 2B belt area, at 17 cross cut, between #6 & #7 heading. The fall measured 12'W X 20'L X 5'H. No injuries occurred and passage of men and airflow were not affected. This is an area not normally traveled by the weekly examiner.

An unplanned roof fall occurred beginning inby spad 348 and extending up to the next crosscut then continuing through the crosscut between 1 & 2 entries just short of spad 347. The fall measured 19'W X 120'L by 8'H. No injuries occurred and passage of men and airflow were not affected. This fall was in an entry not normally traveled by the weekly examiner.

An unplanned roof fall occurred between spads 312 and 308. The fall measured 19'W X 90'L X 8'H. No injuries occurred and passage of men and airflow were not affected. This is an area not normally traveled by the weekly examiner.

An unplanned roof fall occurred between spads 283 and 284. The fall measured 19'W X 40'L X 8'H. No injuries occurred and passage of men and airflow were not affected. This is an area not normally traveled by the weekly examiner.

An unplanned roof fall occurred between spads 290 and 291. The fall measured 19'W X 70'L X 8'H. No injuries occurred and passage of men and airflow were not affected. This is an area not normally traveled by the weekly examiner.

An unplanned roof fall occurred between spads 262 and 292. The fall measured 19'W X 45'L X 8'H. No injuries occurred and passage of men and airflow were not affected. This is an area not normally traveled by the weekly examiner.

An unplanned roof fall occurred between spads 324 and 336. The fall measured 19'W X 50'L X 8'H. No injuries occurred and passage of men and airflow were not affected. This fall was in an entry not normally traveled by the weekly examiner.

An unintentional roof fall in inactive works occurred on the 2nd mains, at 6c/c, between #8 & #9 entry at spad #813. The fall was 20'W X 60'L X 6-8'H. No persons were injured and ventilation was not disrupted. The area will not be recovered. The area was breakered off and barriers erected to prevent entry into the area.

Employee stated he tripped over cable falling into shuttle car striking ribs. Employee finished his shift. He sought medical treatment on 1/27/14 and x-rays were negative. Employee continued to work until he sought medical treatment for continued pain 2/20/2014 and CT indicated a fracture of 9L rib. Employee continues work at full capicity and has not missed any work.

Witnesses stated,when walking down slope belt, the slope car cable was observed to have a damaged place. The slope car was not in use. It was immediately locked and tagged out. All personnel underground were notified of the situtation and the availability of the emergency hoist. MSHA and State officials were notified within 15 minutes. No persons were injured in any way.

An unplanned roof fall had occurred at the Road Fork #51 mine 1 break to the north of spad #220 that measured approximately 20 feet by 20 feet by 5 feet thick. No injuries were reported and additional supplemental support was added to help make the area safe.

Employee was loading garbage on 1 section. While dragging trash to the track to load flats he felt a sharp pain in his lower back. Note: Employee began missing work due to this injury on 9-5-13

Assembling water line when he stepped on the water line caused him to loose his balance. As he caught himself heard and felt a pop in his right knee. EE was splinted and trans.to the surface to a waiting ambulance. EE was treated, discharged and referred to specialist.EE is a contractor that is under mine supervision. It is intended for this incident to be filed under the mine ID#

Employee was walking over to assist another employee when he slipped on slick bottom and fell hitting his right hand on the mine floor. He visited a doctors office the next morning and was referred to another physician at a later date to be determined for follow up. The employee returned to work on his next scheduled shift on 06/27/2013.

Employee was dragging a 6" piece of water line when the other end of the water line dropped to the ground causing the end he was carrying to jerk out of his grip. The water line struck his SCSR in a downward motion causing increased stress load on his left knee causing it to buckle toward the middle.

Employee was entering bath house to begin his shift when he brushed against a pair of coveralls hanging from another employee's basket. Dirt was dislodged and struck the employee in the left eye.

Employee was loading pie pans into the scoop bucket when he stepped on a rock and twisted his left knee and lower back as he fell against the coal rib.

EE was unloading a bucket of miner bits from the top of a scoop when he complained of pain in his right shoulder. He was taken to Hospital where x-rays were taken and the physician recommended he take 2 weeks off and follow up with his doctor on 4/1/13. Location 6 ti 7 c.c. - spad 3002

An unplanned roof fall occurred at the Road Fork 51 mine that measured approximately 20' long by 20' wide and 8 feet in height. No injuries reported. This area was developed in December 2009. The location of the fall was 1 break inby spad #975.

EE was behind a line curtain on the outby corner of the last open break of No.5 entry. He was reaching under the curtain to get a pogo stick to rehang a partially down tail curtain. A shuttle car was tramming from the face and to avoid hitting the miner slack, he trammed close to the curtain and mashed the employee's right hand between the offside dump end tire and the mine floor.

Employee was jacking up a belt rail to put it back in the stand when the jack slipped out causing the belt rail to catch his left thumb between the rail and the stand.

An unplanned roof fall occurred at the road Fork #51 mine that measured approx.15' L. X 6' H.and 20' W. No injuries were reported. The area was developed in November 2009. (See attached Map)

The brake system on the hoist car at Roadfork #51 mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found that the contactor that goes to the hydraulic pump on the brake in the bottom part of the hoist house on the master control panel was in to.

EE was stepping through the threshold of a doorway and caught the toe of his right boot on the door jam causing him to trip and caught himself and felt pain in his right knee. EE stated missing work on 1/21/13 due to having surgery to repair a torn meniscus. EE is off until further notice.

The hoist car at Roadfork #51 mine was taken out of service for a period of time that exceeded 30 minutes. When x-rayed it was determined that the hoist cable had deterioration that caused it to be changed out with a new cable.

The hoist car malfunctioned for a period of time that exceeded 30 minutes. When checked the wires going to the motor speed sensor was grounded to each other. The hoist car was fully operational at approx. 1:30 p.m. No injuries were reported.

Employee filed a pneumoconiosis claim based on a diagnosis of 8/15/12.

EE had a bundle of timbers in scoop bucket. He was standing in front of bucket and cut the band and a timber rolled from the bundle onto his left foot and caused a fracture to the tarsal of his big toe.

Employee stated he had an "Old Back Injury" and while he was shoveling, he felt a pain in his back.

Employee was cutting a box of splices open and knife slipped and cut palm of right hand just below thumb.

An unplanned roof fall occurred in Roadfork #51 mine, near spad #2399 that measured approximately 35'long x 20'wide x 8-12'high. No injuries were reported. The area was developed on 6/11/12.

The slope car at Roadfork #51 mine malfunctioned for a period of time that exceeded thirty minutes. When checked it was found that a hall switch had malfunctioned that affected the braking system. No injuries were reported.

Employee stated he was lifting water line and his back gave out. Employee was taken to ER. Employee returned to work on 7/11/12 and continued to work until 7/17/12 except for his scheduled day off Sunday 7/15/12.

On 6/2/12 the hoist car malfunctioned for a period of time that exceeded 30 minutes. While attempting to pull feeder and flat car out of slope portal, (2) 8" pieces of channel broke loose from the rear of the car as well as some damage to the coupling. No injuries were reported. The car was put back in service on 6/4/12.

An unplanned roof fall occurred in the Roadfork #51 mine, 1 brk. outby spad #303, that measured approximately 60' long X 20' wide x 5-6' high. No injuries were reported. The area was developed on 6/5/08.

EE was riding the hoist car down the sloper when the brakes locked up and jammed his back against the seat. He went to Logan General where x-rays were negative and was released to work. Due to continuing pain, EE visited Dr on 5/16/12 and was put off work until 5/23/12. On 5/23/12 EE visited Dr again and was excused from work until 6/1/12.

EE was cutting a piece of curtain. Holding the curtain with his left hand, he cut through the curtain and the knife made a laceration in his left hand just behind his thumb.

Employee was lifting rock on belt line and felt lower abdominal pain.

An unplanned ignition occurred at the Roadfork #51 Mine in the face of the #3 right. The accident was investigated by Federal and State agencies. Modifications were made to the CH4 & Dust Control Plan. No injuries were reported.

The brake system on the hoist car at the mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found the 2 wires had grounded together. No injuries were reported.

Employee set cable bolt and was turning to see where the control side operator was. When he turned he struck his cheek on side of the canopy causing a laceration

An unplanned roof fall occurred in the Roadfork #51 mine, 1 break inby spad #770 that measured approx. 30' L. X 17' W. X 6' H. No injuries were reported. The area was developed on 11-30-09

Employee was assisting scoop man in scooping up a pallet of Block in a scoop bucket, employee was beside the operators deck. Witness stated employee tripped/stumbled over a block and fell onto scoop canopy and caught his head between mine roof and canopy of scoop.

EE was holding cable in left hand and cutting it with a razor knife. The knife slipped off cable causing a laceration to the left ring and pinky finger.

EE was climbing down off the dozer when he missed a step and fell pinning his left leg between the tracks and lifting arm.

EE was installing a cable bolt in the roof. The plate came in contact with the bottom of his wrist causing a laceration.

EE was holding fork by the end to lock bottom into position on the forklift. Fork slipped off and fell onto his foot causing a fracture to the big toe.

EE was holding a wash hose washing off a scoop. The pressure pump kicked on and the nozzle was pulled from his hand causing a laceration.

EE was checking a set of batteries that were being taken underground when he shut the lid his left middle finger was caught underneath it causing a laceration.

Member was stepping off motor, He stepped on a rock which rolled his ankle to the inside.

EE was standing approx 4 feet from scoop bucket while the forklift operator was picking up a pallet of rock dust to roll it over lathe scoop bucket. As the forklift operator picked up on the pallet it became fouled on the side of the scoop bucket and a piece broke off and struck ee above his left eye causing a laceration.

EE was measuring the stop blocks on the ripper head. He grabbed hold of one of the bit lugs with his right hand and felt pain in his bicep when he pulled himself to his feet. EE went to family health care where he was placed off work pending the results of an MRI.

Employee was attempting to lift Joy 21 shuttle car tire up to roll it into a scoop bucket. While lifting the tire he felt a sharp pain in his lower back.

An unplanned roof fall occurred at one break inby spad #1086 that measured approx. 50' L. X 20' W. X 12' H. No injuries were reported. The area was developed on 6/1/10.

Employee was riding mantrip to section and noticed his rescuer was hanging outside of the mantrip. He grabbed a hold of his rescuer to pull it back into the mantrip when his finger came into contact with a rock and caused a laceration.

Member was picking up a solid block he was using to build a stopping. A member that was assisting him picked one up and tossed it toward him. The block struck EE's right ring finger causing a fracture to the tip end.

Member was tramming miner out of #3 entry on #1 section. He turned and was walking away from miner, when he stepped in a hole, hyper-extending his left knee. Member did not start missing work until 11/15/10.

Member was walking down the slope belt on the track side, when he slid in some mud and rock dust. Member grabbed a hold of the slope tail piece to stop himself from sliding when he felt a pain in his left shoulder.

EE was struck by a piece of cinder block hanging from the top, from an old stopping. This was a 1st aid case only. No medical treatment. No days away from work. This form is being submitted to abate a violation that stated the operator did not file a report of this accident within ten days. The accident does not meet the reporting requirements of Part50. EE only had an x-ray.

Employees in process of breaking shaft form. Left hand bearing beam shifted to hoist house side causing 8" steel beam fastened between bearing beams to fall in shaft causing injuries to 2 employees.

Member was walking between the rib and a supply motor when he slipped and fell on his left shoulder. Member was not taken off work until he was seen by a specialist on 12/31/2008.

Member was trying to remove a bolt from the conveyor motor on the miner when the ratchet slipped and hit his right index finger on the pan of the miner causing a laceration.

Member was installing rib bolt when a rock fell striking his right hand resulting in a contusion. Rock measured 10" long x 3" thick.

Injured was installing a fly board & got his left hand caught between the roof & the board, causing a fracture to his left hand.

Injured stated he was jacking a flat car back on track when the jack bar slipped out striking his finger. Injured went to Dr. on 19th of June & reported to work with a release on 6-21-08 stating he fractured his finger.

EE was pulling a cable and the strap broke and hit EE in the hand causing a fracture on his 2nd and 3rd fingers on his right hand.

EE stated a rock fell and injured his left foot. EE continued to work untill 6/2/08 before he decided to seek medical treatment, at that point he returned to work with a light duty slip.

EE was assembling steps at overcast, when he hit his arm on a rail. It had a burr, that caused a laceration, that required 4 stiches.

EE was walking up the # 2 entry.he was moving a block out of the road way a Fairchild scoop loaded with a slinger type duster came up the entry the operator was unaware of him, presence and the duster struck and pushed him to the ground injuring his leg.It was not determined until 4-26-08 that he was a lost time accident. He missed work due to attending therpy.

While trying to pull a piece of rock from the roof with a piece of drill steel, employee got into the wrong position. The rock hit the steel springing it into his chin causing a laceration that required 7 stiches.

Employee was found by the Hoist House dazed and covered from head to toe in mud. No evidence of what caused the injury. He stated he doesn't remember what happened to him. There were no witnesses to this event.

Employee was shoveling on the slope belt and stated he was walking and slipped. He stated he fell and twisted his back. He walked app. 300 ft up the slope to the surface to report the injury.

Member was behind ventilation curtain. Shuttle car was behind miner in the #3 heading. The shuttle car backed out of #3 into crosscut between #2 & #3 entries. The shuttle car caught member between the shuttle car and the rib causing injuries.

Employee cut his finger on a splice plate (received 7 sutures).

EE was exiting a 10 sc shuttle car and lost his footing and went to catch himself and injured right hand.

Member was operating a remote controled continuous miner cutting up a roof fall. A piece of rock fell on to the miner measuring 68" long x 33" wide x 18" to 7" thick as this rock was loaded into a shuttle car the rock fell off the miner onto theminer operators foot resulting in a severe contusion with swelling.

Miner operator was striking a miner bit with a hammer and hit his left thumb, causing an abrasion.

Member was conducting a a preshift of the pumps. Operation is in rehab of the slope. EE stated in his normal activity he experienced unbearable pain in his lower left back. He states he had no incident or accident. EE stopped work 4/4/06 for further medical test. He has been under doctor prescribed medication for the past year with back pain.

INJURED ASSISTED THREE CO-WORKERS INSTALLING A 6' X 7' METAL DOOR AT THE PORTAL ENTRANCE. WEIGHT SHIFTED AND EXTRA EXERTION CAUSED THE BACK INJURY.
