Employee was reeling in a water hose on a water truck and sprained their shoulder.

Employee states their foot slipped off first rung of haul truck ladder while boarding and ankle got progressively worse throughout shift. The employee reported the injury by note to the supervisor who was out running for parts. The employee is on modified work duty and in an ankle brace.

An employee was descending down the 740B articulating truck ladder when ee slipped and fell. The employee fell to the ground and in the process injured self. Urgent care facility diagnosed employee with a sprain to the back and left knee. The employee was required to see a specialist before returning to work.

Employee was stepping out of a John Deere 326E skid steer, when left boot sole got caught underneath one of the steps, causing loss of balance and a fall. While falling, left leg twisted. Incident originally diagnosed as a sprain, and OTC drugs were provided. On June 2, diagnosis was revised as a partial MCL tear, and the employee was referred to a physical therapist.

While discharging a 6-inch pump line to fill the water truck, a hose fitting failed, causing the water line to whip toward the employee, knocking him off his feet and onto a concrete block where he was struck by the hose and pinned against the block. The miner suffered five or six broken ribs and a punctured lung.
Employee was adding guarding to the surface portable plant. As he went to reach for additional tools, he lost his balance on loose material on the catwalk. The employee consequently went to regain balance and in doing so his left arm came down on the side of the guarding. This resulted in a laceration which required 6 stitches.
Employee two years experience in Maintenance on the plant was dismantling a conveyor. Wearing the proper PPE (leather welding gloves) the employee was driving a pin out with a punch and hammer. He then missed the punch when he swung and struck his finger with the hammer on his left index finger.

Employee was using a cutting torch for a structure brace, and a piece of slag came in contact with employee's right elbow, causing first and second degree burns. Employee was not wearing all company-required PPE, which was available. Employee was prescribed a small dosage of painkillers and returned to work the following shift on modified duty. Released for full duty on 8-26-14.
Inspection/access door with bottom hinges not supported after being unlatched. Damaged portion of hinges not visible with door in closed position. Miner was positioned to the side of the door, hands resting on solid surface. Weight of door broke the hinges after being opened and fell on top of miner's fingers. Result: two broken fingertips of left hand (index and middle fingers).

Employee called into work on October 3rd, stating that he had hurt his foot and would not be coming in. On 10/11 he called his boss and stated that he had to go for additional testing to confirm if his injury may be related to work. On 10/17/2011 he was seen by the Workmen's comp physician who ordered and MRI. On 10/20/2011 results of MRI confirm work related injury.

While working in a man-basket; EE had reached for a drill and while doing such EE struck his mouth on the gate that had rotated out of postion because the top hinge pin had come out of the hole that it was mounted through. EE did not go to doctor until either 3/14 or 3/17. EE did see his family dentist for care of the tooth during the evening of 3/11/2008.

EE was attempting to reposition a metal section of the vent tubing so that the tube mated better with the section already mounted in the back. EE was wearing belt & lanyard but when attempted to reach a clamp he loosen his lanyard & he stepped on the rail to reach the clamp; when he reached the clamp & cable, the tubing gave away & fell. The EE fell on vent tubing and then ground.
