Antelope Coal Mine Coal

It had snowed on and off all day and the temperature was 16 degrees. There was snow on the ground where the employee parked their service truck. The employee exited the service truck and started to walk to the shop. The employee slipped and fell to the ground.

Chunks of coal were found smoking on an inaccessible bench in ramp 25.

While loading a train, and after sitting for a long period, team member's back started to spasm, which led to some pain.

EE was tightening a nut on the a frame of a haul truck with a high torque gun, when the torque gun came of the nut and struck EE's finger between the gun and the truck frame.

Employee fell while stepping out of a crew van onto frozen ground at the start of shift. The employee was diagnosed with a fractured fibula.

Employee was changing cutting edges on 24H grader, while using impact to remove hardware, a nut came out of socket, bounced off the ground and hit employee in the lower lip resulting in a chipped tooth.

While loosening lug nuts on a haul truck tire, an employee pinched their finger between a rad gun reaction arm and the tire rim. The employee was wearing gloves at the time of the injury.

Operator of haul truck noticed the GPS/Dispatch unit was not fastened securely. The employee decided to cut the zip tie to prevent damage to the unit, then call maintenance. The employee used a pocket knife to cut the zip tie, the knife went through the zip tie then into the thumb side of the palm of employee's left hand.

Hot coal was discovered on a highwall creating smoke. No flame observed. No miners were working in the area. The hot coal was covered with dirt eliminating the issue.

After dismounting a track hoe, the employee tripped on a chunk of dirt and fell (same level) onto EE's arm. The employee was evaluated and returned to work. The employee continued to have shoulder pain. On 7/31/20, EE was diagnosed with a rotator cuff tear that will require surgery.

Hot coal was observed at ramp 34 in the east side of the pit. This did not result in injury, damage or impairment.

While walking on the ground to access a pickup truck, the employee rolled foot. This caused the employee to lose balance and fall to the ground.

While conducting work place inspection smoke was seen coming from the Andeson Coal Seam in Ramp 50 East Pit. Have been experiencing raining conditions. There were no flames seen, only smoke. No miners working in this area.

While spreading rock on a road with a scraper, the machine hit a bump jolting the operator. The bodily motion caused a sprain/strain to EE's shoulder rotator cuff.

While on a break, an employee had symptoms of a serious medical issue not related to work. Approximately 3 hours later, while under the care of EMS, the employee was pronounced deceased.

A haul truck driver sustained an eye injury when dust entered the employee's eye after turning the air conditioning fan on high speed. The employee was evaluated by an eye doctor and prescribed medication. The employee was unable to drive the following shift, making this a lost-time injury.

The employee was working from a work platform, EE was trying to repair a hydraulic leak on a water truck. Another employee was moving the oil cart and struck the bottom of the work platform causing the employee on the work platform to fall.

A Haul truck operator has been diagnosed with a degenerative spine condition. Symptoms are aggravated by activities at work and at home. Progression of the disease is now interfering with the employee's ability to operate equipment. First documentation of the illness was on 1/20/2019; however, treatment did not become reportable until 2/21/2020.

While installing a cable crossing, employee's right foot was caught under the crossing while a Rubber Tire Dozer was pushing it into place. Within approximately 5 minutes the employee's foot was released. Due to the gravity of the injury, the employee underwent a partial amputation of the right foot after being evaluated at Wyoming Medical Center in Casper, WY.

The employee was assisting with a shovel move. The employee and three others were preparing to place the cable in the mule when the employee felt a pain and pop in right calf muscle.

Smoke was visualized in West Pit near the east end of ramp 33 in the Anderson seam (this in an inactive work area).No flames were visualized.

Smoke was noted in the West Pit ramp 35 area. The smoke could not be extinguished within 30 minutes so MSHA was notified. Measures were taken to protect employees and a plan was formulated to dump over the fire, but would wait for day shift so trucks dumping would have good visibility. The smoke was extinguished by 10am 09/23/2018.

Employee was drilling a hole in a metal bracket. The drill bit caught on a burr- twisting the employees right hand, resulting in a fracture to the 4th metacarpal of the right hand.

The shovel dipper would not trip so the shovel operator started to swing the dipper back to the dig face, the door opened over the tail of the truck. The bed came up jarring the haul truck driver. EE did have a seat belt on. Employee returned to work the next shift, worked the next 10 scheduled shifts. EE did not work on the scheduled shift 8/2/18 due to stiffness in neck.

While attempting to reposition a small section of chute liner, the liner came to a stop and the mechanics gloved hand slid along the bottom edge. The liner had been worn down to a sharp edge and cut through the employees glove and hand resulting in a 2" laceration. The employee received stitches to repair the laceration.

While moving electric shovel cable, employee was struck by the cable in the left ankle. Employee was watching the cable and the pickup in front of EE when a section of cable contacted the employee from the side resulting in a fracture to the left ankle which required surgery to repair.

While grinding on a weld, employee got a small fleck of metal in left eye. The fleck of metal was not able to be removed on site so the employee was taken to the eye doctor to have it removed. The employee received a full duty unrestricted release back to work, but was prescribed two separate prescription eye drops to aid in the healing process, which makes this case reportable.

Foreign body in right eye.

Employee was using a ratchet to remove a coupling bolt when the socket that was connected slipped off of the nut. This caused the handle of the ratchet to make contact with his bottom lip and the top center tooth resulting in a fracture of the tooth.

On 6/15 another weather system went through the SMA Pit of the Antelope Mine. Miners and equipment were removed from the pit. No miners were trapped or injured. At approximately 9:20pm a call was made to MSHA 800 number to report the inundation of rain water and at 9:45 EE with MSHA called and placed a J-Order on the SMA pit.

While pressing in the master pin on the rail of a Komatsu Excavator the culet in the track press failed. The mechanic was standing behind the track with his hand in the air holding a flash light while directing the press operation when he was struck in the right middle finger by a culet fragment.

On 6/3 a weather system went through the Antelope Mine. At 8:00PM the SMA pit was evacuated due to lightening. A rain storm followed at 11:00PM. When the shift supervisor went back to the pit to inspect for hazards she noticed that the pit was taking on water. She immediately called the Safety Manager at 2:35AM & the MSHA 800 number to report the inundation of rain water at 2:42AM.

Employee tripped while walking down a flight of stairs. Employee was diagnosed by an Emergency Room physician as having a concussion from hitting his head during the fall, as well as a Distal Bicep Tear which will require surgery.

The employee was preparing a chain come-a-long that was going to be used. While pulling the chain slack, the dead end hook slipped off of a threaded stud. The come-a-long struck the employee in the face which caused the safety glasses that the employee was wearing to contact his left eyebrow. The result was a 2 inch laceration which required stitches.

While putting tools away into the lower drawer of his toolbox, the employee was kneeling down on his left knee. When he was done putting the tools away, the employee simply stood straight up. In doing so the employee felt a sharp pain in his left knee. Upon examination, X-Ray, and MRI it was determine that the employee had sustained a tear in the left meniscus.

Employee was mounting the right hand steer tire on a 24H blade. He bent over to stand the bead seat band up on end. As he began to stand back up straight, the employee felt a sharp pain in his lower back.

While unhooking the D-ring rope from the back deck of the Rubber Tire Dozer, the employee lost his grip on the grab bar and fell approximately 4' to the ground. The employee landed on his left arm when making contact with the ground, and reported pain in his left wrist.

Employee reached up to turn off the overhead mounted air conditioning unit in the Cat rubber tire dozer that he was operating. In doing so he felt and heard a pop in his right shoulder. This was followed by extreme pain.

Employee was pulling on a rope that was attached to a road sign, the employee was trying to place the rope over the ball hitch of his pick up truck so he could move the sign to another part of the mine. As the employee was pulling on the rope trying to get the sign close enough to the ball hitch of the pickup he slipped and his head struck the flat bed pickup.

After completing his shift, the injured miner was walking through the parking lot to get in his car pool. When he passed between two parked cars, he slipped and fell. The fall caused soft tissue injury to the employee's left knee and shoulder. The employee was given first aid treatment but was not able to return to his next scheduled shift on January 2, 2014.

The shop welder was attempting to straighten a metal step on the car body of an excavator. He was using a large pry bar and a propane torch to try and straighten the bent area. As he was applying pressure to the bar it rolled causing him to fall to the ground, when he fell the pry bar he was working with struck his gloved hand against the floor.

A plant mechanic found an employee on the floor and unresponsive. The mechanic called a mine rescues squad member to come check on the unresponsive employee. Rescue measures were initiated, Mayday announced, and unresponsive employee was transported by mine ambulance. County ambulance intercepted the mine ambulance and patient care was transferred to paramedics.

A plant mechanic was standing on the walk way near the belt line assisting with the startup of the belt. As the mechanic turned to walk up the belt line walkway, he felt and heard a "pop" in the calf of his left leg. The mechanic also reported feeling extreme pain at the same time.

employee was assembling a new ladder. during the line up process, he was using a pry bar and a line up bar. another employee was assisting in the process and was using a hammer. when the ladder was struck, the line up bar was jarred loose causing the ladder shift. the end of the employees fingers were cut in the process.

The employee was placing poly pipe over the high wall when the pipe came loose from the chain connecting it to a pickup truck. When the pipe came loose it struck the employee on the ground and knocked him to the ground.

Employee was grinding in a confined space for most of his shift. At the end of the shift he felt a discomfort in his eye before he left the mine site. He did not report this incident until the following day. The hospital could not remove the embedded object and it was removed by an Eye Doctor on 4/1/2013.

Employee was walking on uneven terrain in muddy, wet, slick conditions. He slipped and fell on his hands and knees.

Employee was installing a belly pan on a track dozer. When he pulled back on the belly pan trying to get it in to place the belly pan pop up catching his thumb between the belly pan and the hinge on the body of the dozer.

Employee was working on a dozer in the shop, as he climbed off the dozer he felt his right knee lock up. He was unable to straighten his right knee.

An employee was working in the cab of a haul truck, he exited the cab and descended the ladder. He stepped on the grating on the lower deck platform. The grating moved and he rolled his ankle and fell into the handrail that he was holding onto.

A fuel truck driver was descending a ramp when the truck slid sideways on ice and then overturned. The operator received a laceration to his left elbow that required staples to close.

As the employee placed his safety glasses on, the plastic lens broke in the middle causing the left half to pivot inwards striking the employee's left eye. The style of glasses has a single plastic piece for both the left and right lenses and the bridge. The glasses broke in the center of the bridge. The injury was treated by application of prescription medication in the eye.

The dozer seat back fell off mounting bracket and was lodged down towards floor of operator cab. Operator tried to move it out of his way and return up to dragline pad. Operator had to use force to dislodge seat back to get it out of way, the seat came loose striking operator on upper lip with the mounting bracket.

The employee exited the cab of his haul truck, a strong gust of wind blew off his hard hat and safety glasses. The employee tried to catch his hat and glasses and a second gust of wind knocked the employee off balance, he then fell down the stairs of his haul truck hitting his right arm on the hand rail support causing a break in both bones of the right forearm.

Employee was walking to service truck when another employee unloaded and laid a long pry bar on his shoulder. The end of the pry bar struck the employee walking up from behind in the lower lip causing a laceration that required a doctor to close with glue.

The employee was on the bed of a lowboy assisting in transporting a track dozer. He slipped and fell to the ground. After a few days, he sought medical attention where it was found he had fractured his ankle.

A rubber tire dozer operator was setting at the shovel waiting to do some work. There was a ceiling mounted heater in the cab, with a defusser/guard missing. He reached his right hand into the hole to see if the filter was missing also. The tip of his middle finger contacted the fan and was lacerated. Six sutures were required to close the wound and he was released to full duty.

Mechanic was changing hoist ropes on a P&H 2300 shovel in cramped quarters. When he attempted to pull the bucket end out of the socket, the rope dropped down pinching his middle finger. He sustained a fracture to his fingertip.

A private citizen, non-miner, had a heart attack while getting public coal on our site. The area is in a non-active mining area. The person is not a miner and he was doing a non-mining activity. Our Mine Rescue team transported him to the local hospital and delivered him to Emergency Room personnel. The gentleman, as of this filing, is still living.

An employee was assisting another employee in removing a flange bolt. The wrench slipped on the bolt head causing it to contact the employee's left thumb tip. He received a laceration and fracture to the tip. Sutures were needed to close the wound and a splint was applied. The employee returned to full duty on his next shift.

A plant employee was helping lift an idler back into position on an overland conveyor. When it wouldn't fit, they lowered it back down. One employee dropped his end causing the other employee's finger to be caught between the idler and a railroad tie on the ground. His fingertip was fractured and lacerated. The finger was put in a splint and received 4 sutures.

A shop attendant was securing an access ladder to an engine area after he had washed the area for a PM. He was in the process of pinning the ladder up. He released his grasp on the ladder to grab the pin. The ladder swung down and lacerated his right ear lobe. The laceration required 7 sutures to close the wound.

A haul truck operator let go of the steering wheel to adjust the seat head rest. The truck drifted off the road and climbed up on a bank, then turned on its side. The operator received a laceration on her forehead and soft tissue injuries. Due to pain medication and soreness, the operator was unable to report to her next scheduled shift.

Rubber Tire Dozer operator was reaching down to hook shovel cable up to his dozer in order to assist a shovel relocation. He rolled his right ankle over and fell. When he fell, his hard hat came off and his head struck the rear boarding ladder causing a laceration. The laceration required 7 staples to close the wound. His ankle was sprained and not broken.

An operator traveled off a highwall in a track dozer. The dozer rolled once and landed upright. The operator was wearing a seat belt. Initial medical assessment found no injury. Follow-up medical assessment found the operator had sustained a fracture to a spinal process on a thoracic vertebra. He was given a full duty release.

Employee was walking through the shop when a gust of wind blew a particle into his left eye. He was wearing his safety glasses at the time. He received a corneal abrasion and was given prescription medication by a physician. He returned to work on his next shift.

While moving a yield sign on a skid, the employee's right pinky finger became trapped between the skid and the ground. The employee jerked his finger free and in doing so caused a fracture to the tip of the finger and a laceration that took eleven sutures to close. After treatment, the employee was given a full duty release from the ER doctor.

A Maintenance Inspector falls off the back of a work vehicle. He lands on the ground, breaking one rib on his left side. He had stepped on an open tool box lid, which then shut, causing him to lose his balance.

A shop welder was needle scaling a welding bead on a haul truck. He felt something in his eye and rubbed it. He contacted mine rescue personnel. After attempts were made to flush the eye, the employee was taken to an optometrist. Extensive scratching on the eye surface was found. The employee was given a prescription antibiotic in the eye and a full duty release to return to work.

A rubber tire dozer operator was exiting the dozer cab at the end of the shift. When he shut the cab door, his left ring finger tip was pinched between the cab door and the door frame. The operator sustained fractures of the finger tip and a laceration of the finger tip, requiring six sutures to close.

A mechanic was working off the rear platform of an 830B Caterpillar Rubber Tire dozer. He was changing a light bulb. As he turned to his left to move toward the boarding ladder, his left foot contacted a tow hook that had been welded on top of the rear bumper. He fell approximately 4.5 feet, slightly fracturing his left elbow.

Employee was coming down the North silo in the elevator and half way down the elevator quit working then had to be lowered manually.

Driller stepped out of the cab to work outside. Location was on an upper bench in the cast. Due to windy conditions, a foreign particle got in his left eye. Attempts to flush it out on site were not successful. Employee was transported to an optometrist where the particle was removed and medicated eye drops were given and prescribed.

A maintenance worker and contractor were trapped in an exterior elevator on the silos when the elevator lost power. After reviewing emergency lowering procedures with the manufacturer, the elevator was lowered to the ground.

Certified electrician was turning a rotor on a haul truck wheel motor to start profiling. As he pulled down on the rotor he heard a "pop" sound in his right arm and immediately experienced pain in the area. X-rays revealed he had ruptured the distal tendon in his right upper arm.

A Haul truck operator parked her truck for fuel and service. She left the cab and was going to descend the truck stairway to isolate the truck. She stepped on a pile of coal on the edge of the deck and lost her balance. She fell forward onto the stairway landing rolling her right ankle over. In doing so, she fractured the distal end of her right fibula.

Employee caught left index finger between a pin and dipper bail support arm after the bail moved unexpectedly. He received a 1/2 inch laceration to the tip of his finger which required 7 sutures.

Employee slipped on last step hitting her head. Received a laceration on the right eyebrow about 1/2 inch long. Wound required 7 sutures to close it.

Employee receives wound on gloved left ring finger while pounding dozer tracks in place. Emergency personnel treated the wound and released the patient. Employee's wound became discolored over time and sought medical care. A metal sliver was removed and two sutures closed the incision.

Employee was cutting belting with a utility knife. He was cutting towards himself when the knife slipped and left a 1/2" laceration in his right thumb requiring 3 stitches.

The employe was crossing in front of the cab of an Komatsu 830-E haul truck and struck his left leg against a piece of metal under the windshield causing a cut that requiered five sutures to close.

The employee was removing an inspection plate of a D-11 dozer to change transmission fluid. The plate would not come off so the employee was going to get a pry bar. He placed his hand on the floor to scoot out from under the machine when the plate fell from the dozer onto his right hand.

The employee was loosening a bolt using a hammer wrench when the wrench slipped and caught his left ring finger between the wrench and the motor casing. The wound required stiches.

The NEMA baghouse had an unplanned ignition resulting from smoldering coal being sucked into the baghouse as the baghouse fan was turned on after a pro-longed outage. The ignition resulted in one of the explosion doors being blown open. The system was shut down, MSHA was notified and the hot spot was eliminated by the use of water.

Water truck parked at end of shift. During am hours, short occured at battery cable. spark ignited grease on frame grease dripped onto fuel line, melting small hole in line. Diesel dripped onto ground. Sparks ignited diesel on ground. Passing coal train saw smoke underneath truck @ 5:30 am called wright fire dept. fire dept arrived @ 6 am. extinguished fire @ 6:03 am.

Grease and coal dust around a bearing located on a McLanahan crusher caught fire which ignited suspended coal dust. This resulted in hot embers being drawn into the dust collector system causing an ignition of coal dust and a fire in a bag house.

The employee sustained an injury to his right knee due as a result of slipping on a snow covered surface. The employee was walking from the administration building to the shop ready line.

The employee was operating a 240 Ton haul truck hauling interburden material. While driving the loaded truck to the dump location, the employee drove through a rough spot in the road and the seat bottomed out. The employee felt pain in their lower back.

Employee was positioned inside of a manlift installing a pivot arm cap on a tailgate pivot arm of an 830E Komatso haul truck. The employee placed two bolts in the cap and was reaching down to pick up another bolt when the cap came loose stiking the little finger on the right hand which was positioned on the handrail of the manlift.

The employee was using a wrench to loosen an injector hold down clamp on the engine of a 637E scraper engine. The clamp broke loose and the employee struck his tooth on the air cleaner mounting bracket breaking a tooth.

The employee was working on D11R dozer at a height of 11'1" while standing on a step located on the left side of the dozer cab. The employee was removing a chain which was supporting a left ripper tilt cylinder. The step broke and the employee fell to the ground causing a compound fracture below the knee to the right leg.

THE EE HAD COMPLETED A PRE-SHIFT WALK AROUND ON A 240 TON HAUL TRUCK & PICKED UP A CHOCK BLOCK. THE EE THEN SWUNG THE CHOCK BLOCK IN AN UPWARD MOTION TO PLACE IT IN A CHOCK BLOCK HOLDER AND FELT A SHARP PAIN IN THE LOWER RIGHT SIDE OF BACK.

Laborer and an operator were removing pipe from a truck when operator closed the tailgate and pinned the laborer.

AT THE TIME OF THE INCIDENT EE WAS PREPARING AN ENGINE FOR INSTALLATION IN A D11 DOZER. AS THE ENGINE WAS BEING LOWERED TO THE GROUND HIM WENT TO PLACE A BLOCK OF WOOD UNDER IT. WHEN HE THREW THE BLOCK UNDER THE FRONT OF THE ENGINE THE BLOCK CAUGHT ON THE GLOVE HE WAS WEARING AND PULLED HIS FINGER UNDER THE ENGINE WHERE IT WAS PINCHED BETWEEN THE BLOCK AND THE ENGINE.

Employee was delivering gas cylinders. While unloading one of those cylinders it encountered some ice which caused the cylinder to change direction and the ee's hand got caught between the cylinder and the rack. the resulting injury was a fractured bone in his right hand.

CARRYING CONVEYOR IDLER DOWN INCLINE CONVEYOR, TRIPPED & FELL FORWARD TRIED TO CATCH HIS FALL, CAUSING FRACTURE IN RIGHT HAND.

AT APPROX. 11 AM, A CONTRACT EE & A MILLWRIGHT EE WERE IN A MAN LIFT BASKET EXTENDED INSIDE OF THE SMA HOPPER-STILLING SHED. THE CONTRACT EE WAS WORKING ON CUTTING A WEAR PLATE LOOSE IN THE HOPPER. HE WAS HOLDING A TORCH IN ONE HAND & USED A HAMMER WITH THE OTHER HAND TO KNOCK A SECTION OF THE PLATE LOOSE. THE IMPACT OF HAMMER BLOW VIBRATED. THE TORCH IGNITED THE COAL DUST.

THE EE WAS ASSISTING IN UNCOUPLING HYDRAULIC LINES FROM A CABLE REELER ATTACHED TO A CAT LOADER.IN THE PROCESS OF RELEASING THE STORED PRESSURE OF THE HYDRAULIC LINES, THE LOADER BOOM ATTACHME NT SHIFTED AND PINCHED THE EE'S FOOT BETWEEN THE BOOM AND GROUND.

EE WAS CLEANING GREASE ON FLOOR OF 2300 P&H ELECTRICAL SHOVEL. THE EE WAS UTILIZING A METAL ROD APPROX. 3' IN LENGTH. THE METAL ROD WAS BEING USED TO PUSH RAG ALONG THE FLOOR OF SHOVEL BY THE HOIST DRUM. WHILE PUSHING THE RAG ACROSS THE FLOOR THE STICK WENT INTO A HOLE APPROX. 6" IN DIAMETER LOCATED ON SHOVEL FLOOR. STICK CAME IN CONTACT W/SHOVEL COLLECTER RING CAUSING ELECTRICAL F

THE INJURED EE WAS ASSEMBLING A THREE PIECE RIM WHEN THE LOCK RING AND FLANGE BLEW OFF THE TIRE. THE EE INFLATED THE TIRE TO 15PSI AND WAS TADING ON THE LOCK RING TO MAKE SURE RETIRE WAS SEAKE R PROPERLY WHILE TAPING THE COCK RING IT CAME OFTHE TIRE STRIKING THE EE ON THE LEFT MIDDLE FINGER CAUSING LACERATION. THE EE REIEVED FOUR SUTURES.

EMPLOYEE WAS ASSISTING NORTH ANTELOPE COAL EMPLOYEES CHANGE THE ROPES ON A 4100 XPB SHOVEL. IN THE PROCESS ONE OF THE ROPES SLIPPED AND HIT THE EMPLOYEE IN THE BACK. THE EMPLOYEE DID NOT GO TO THE DR UNTIL JULY 17, 03. FURTHER EXAMINATIONS DETECTED A CRACKED LUMBAR.

EMPLOYEE WAS WELDING ON A HAUL TRUCK WHEN HE NOTICED SOMETHING IN HIS LEFT EYE. HE SOUGHT MEDICAL ATTENTION AT THE EMERGENCY ROOM WHERE A PIECE OF METAL WAS REMOVED FROM HIS LEFT EYE. A RUST R ING WAS STARTING TO FORM AND HAD TO BE REMOVED.

VERY WINDY - EMPLOYEE WAS WALKING INTO THE SHOP WHEN A FOREIGN OBJECT BLEW INTO HIS EYE, SCRATCHING HIS CORNEA THUS HAVING TO GO TO THE HOSPITAL AND HAVE THE PARTICLE REMOVED AND A PRESCRIPTIO N EYE DROP GIVEN.

THE 6-7 TRANFER BAGHOUSE CAUGHT FIRE AND BLEW THE EXPLOSION DOORS OFF THE BAGHOUSE. THE FIRE WASEXTINGUISHED WITHIN TWENTY MINUTES.

EMPLOYEE WAS DISMOUNTING CAT 426 BACK HOE AND STEPPED ONTO UNEVEN GROUND, TWISTING ANKLE.

THE EE WAS USING A HOSTSEY STEAM CLEANER TO WASH UNDER THE HOOD OF A SERVICE LUBE TRUCK. THE EE HAD COMPLETED WASHING UNDER THE HOOD AND WAS WORKING ON WASHING THE REMAINDER OF THE TRUCK WHEN THE EE WAS WALKING BACKWARDS AWAY FROM THE SERVIVICE TRUCK AND SLIPPED AND FELL ON A CHOCK BLOCK ON THE WASH BAY FLOOR. AN EE WAS USING A POCKET KNIFE TO CUT A TIE WRAP FROM A HYDRAULIC HOSE WHICH AHD BEEN COILED FOR SHIPPING. THE EE WAS USING THE RIGHT HAND TO HOLD THE KNIFE AND THE LEFT HAND WAS HOLDING ONTO THE HYDRAULIC HOSE. IN CUTTING THE TIEWRAP, THE EE CUT BACK TOWARDS TO LEFT HAND RESULTING IN A LACERATION TO THE LEFT INDEX FINGER. FIVE STITCHES WERE REQUIRED TO CLOSE THE LACERATION.

THE DRILLER WAS REMOVING A DRILL STEEL FROM THE DRILL. HE WAS ATTEMPTING TO PLACE A STRAP AROUND ONE END OF THE STEEL WHEN HIS LEFT HAND WAS CAUGHT BETWEEN THE STEEL AND A FORK ON A FORKLIFT. THIS RESULTED IN A LOT OF SWELLING ON HIS LEFT HAND MOSTLY IN THE THUMB AREA GIVING HIM LIMITED USE OF HIS HAND.

THE INCIDENT OCCURRED WHEN AN O&K DIRT SHOVEL WAS LOADING A LARGE ROCK INTO THE BED OF A 190 TON CAT HAUL TRUCK. THE SHOVEL OPERATOR PICKED THE ROCK UP WITH THE SHOVEL BUCKET & PLACED THE BUCK ET OVER THE TRUCK BED. THE OPERATOR THEN CURLED THE BUCKET & THE ROCK FELL ONTO THE DUCKBILL OF THE HAUL TRUCK. THE TRUCK OPERATOR'S NECK BECAME SORE LATER, DIAGNOSED AS NECK STRAIN.

EN EE WAS REPAIRING A CRACK ON THE FRAME OF A 190-TON HAUL TRUCK. HE HAD POSITIONED HIMSELF BELOW THE CRACK WELDING OVERHEAD. HE HAD COMPLETED A PASS AND USED A METAL CHIPPED TO REMOVE SLAG. H IS WELDING HOOD WAS DOWN. HE FLIPPED HIS HOOD UP TO INSPECT THE WELD. HE BEGAN CHIPPING SLAG WITH HIS HOOD UP AND HIS SAFETY GLASSES ON WHILE CHIPPING. A PIECE OF SLAG IMPACTED HIS RIGHT EYE.

TWO DAY SHIFT MAINTENANCE EES WITNESSED A VISIBLE FLAME BETWEEN THE SKIRT BOARD & THE CONVEYOR BELT AT THE #1 CRUSHER. THE FLAME WAS CONTRIBUTED TO AN UNPLANNED IGNITION OF COAL DUST IN THE HO PPER.

OPERATOR OF CAT 16G MOTOR GRADER WAS EXITING THE CAB AND DESCENDING DOWN THE LADDER. THE EE'S RIGHT FOOT SLIPPED FROM THE LADDER CAUSING LOSS OF FOOTING. OPERATOR FELL TO THE GROUND STRIKING O N OLD GUARD WITH BUTTOCK.

AN OILER WAS DRIVING A LUBE TRUCK DOWN A PIT ACCESS ROAD. THE TRUCK SLID DOWN A PIT ACCESS ROAD.THE TRUCK SLID SIDEWAYS, IMPACTED A BERM WITHT HE FRONT BUMPER AND TIPPED ONTO THE PASSENGERS SI DE OF THE TRUCK. EE WAS DIAGNOSED WITH SLIGHT KNEE STRAIN AND PLACED ON RESTRICTED DUTY.

HE TORQUING DRUM BOLTS, SOCKET BECAME STUCK, WHEN REMOVE, IT SMASHED FINGER AGAINST WALL OF DRUM.

A MAINTENANCE EE WAS TAKING A DRIVELINE OUT OF A 789 CAT HAUL TRUCK. THE EE STEPPED FROM THE CENTER CROSS FRAME MEMBER TO ONE OF THE BRAKE CANS WITH HIS RIGHT FOOT. EE WAS PULLING CABLES IN A STEEL CABLE BELT WITH A HOOK KNIVE. THE KNIFE SLIPPED AND CAT OTHER IN THE RIGHT HAND.

EE WAS RETURNING A LADDER TO REAR OF SERV TRK INDOING SO HE STEPPED ON REAR BUMPER OF TRK TO REPOSITION THE LADDER & HIS FOOT SLIPPED OFF THE BUMPER STRIKING THE GROUND CAUSING A FRACTURE TO L EFT HEEL

WHILE CHANGING HOIST ROPES ON A SHOVEL THE EE WAS STRUCK ON THE SIDE OF THE HEAD BY THE ROPE AS IT CAME AROUND THE SPOOL. THE EE RECEIVED A LACERATION TO THE HEAD WHICH LED TO SUTURES IN THE W OUND.

WHILE CHANGING TIRE ON HAYUL TRUCK EE WAS USING AIR IMPACT WRENCH WHEN A METAL SLIVER CAME OFF OF NUT OR SOCKET AND HIT EE IN LEFT EYE. THE CORNEAL FOREIGN BODY WAS REMOVED BY DR. IN TOWN. EE REUTRNED TO WORK ON HIS NEXT SCHEDULED REGULAR SHIFT WITH NO RESTRICTIONS.

EMPLOYEE WAS MOVING SHOVEL BUCKET PINS WHEN A PIN SLIPPED AND PINCHED HIS FINGER. THIS BROKE THE END OF THE FINGER AND CAUSED A LACERATION.

CONTRACTOR WAS AIR ARCING ON GEAR CASE AND SPARKS IGNITED GREASE AROUND GEAR AREAS. FIRE BURNED 60 MINUTES DUE TO DIFFICULTY GETTING WATER DOWN IN TO THE CASING. NO INJURIES. DAMAGE WAS MIN IMAL.

EMPLOYEE OPENED DRAGLINE CAB DOOR TO THROW OUT ABAG OF TRASH. THE WIND BLEW THE DOOR SHUT ON HISLEFT HAND CAUSING A LACERATON TO HIS RING FINGER

EE LIFTED METAL LID WITH HIS LEFT HAND TO LOOK INSIDE A HATCH AT THE COAL LEVEL. THE METAL LID SLIPED OUT OF HIS LEFT HAND AND FELL BACK 12" ONTO THE EE'S RIGHT HAND. WHEN THE LID HIT HIS RIGH T HAND IT CAUSED A HAIRLINE FRACTURE OF THE 5TH METACARPAL.

EE WAS OBSERVING MAINTENANCE TO FREE STUCK BELT FEEDER TO CRUSHER. WHEN MACHINE STARTED, A BOLT SHEARED OFF AND STRUCK HIM IN THE RIGHT SIDE OF HIS MOUTH. THE RESULTING LACERATION REQUIRED 10 SUTURES TO REPAIR.

WHILE LOADING THE TRAILER AT THE END OF THEIR SHIFT, THESE EE NOTICED THE WIRING WAS DAMAGED. NO HAVING A SMALL SCREWDRIVER TO REPAIR THE WIRING, THEY USED A POCKET KNIFE. THE POCKET KNIFE SLI PPED CAUSING A LACERATION TO TAY'S LEFT INDEX FINGER THAT REQUIRED SUTURES.

EMPLOYEE WAS WALKING NEAR A THREE AND A HALF INCH DRAGLINE CABLE AS IT WAS BEING REPOSITIONED. ALOOP SNAPPED OVER, HITTING EMPLOYEE'S LEFT ANKLECAUSING A SMALL FRACTURE OF A NON-LOAD BEARING B ONE IN THE LEFT ANKLE.

PRYBAR SLIPPED WHILE CLOSING RAILCAR DOOR. FELL AND BRUISED BACK.

DURING WALK AROUND INSPECTION TRIED TO STRAIGHTEN OUT REAR CABLE STEP ON 834 DOZER. AS IT STRAIGHTENED OUT AND WENT FORWARD FELL INTO MACHINE AND CUT FOREHEAD ON PIPE OF UPPER STEP SECTION. 2 CM LACERATION WAS RECEIVED WHICH REQUIRED 6 SUTURES.

WHEN SETTING A WEDGE IN A DRAG SOCKET ON THE 8200 DRAGLINE USING A 20# SLEDGE HAMMER, BEATING ON THE WEDGE PART OF THE SOCKET, A PIECE OF THE HAMMER BROKE OFF STRIKING HIM IN THE RIGHT LEG. HE WAS STANDING ABOUT 15 FEET AWAY FROM THE SOCKET AT THE TIME.
