Cordero Rojo Mine Coal

Employee was ascending a D11 Dozer and felt a pop in their knee. Employee was diagnosed with a partial tear of a ligament and was prescribed physical therapy on 1/9/25. Employee did return to work.

Employee was prepping a coal sample when felt sharp pain in left hip shooting down their leg.

Employee fell off boarding ladder of Haul Truck. Employee injured their shoulder and was unable to come back to work on 4/26/24.

D11 Track Dozer caught on fire while pushing in the Cast. No injuries. Fire was extinguished about the 30 min mark.

EE was ascending a ladder on a Haul Truck when EE felt a pop in their shoulder/arm. EE was evaluated by an Orthopedic, and was determined that EE tore the rotator cuff.

An employee dropped a small piece of AR plate on the side of their knee causing a laceration that required 6 stitches.

EE had just started inspecting down the belt tube when EE hit a patch of black ice. EE's left foot slid and then hit a dry patch in the metal grating causing both EE's tibia and fibula to break about 2 inches above the ankle.

Employee was tying in a coal blast pattern on very windy day. With Safety glasses being worn, a strong gust of wind blew coal debris in employees eye. Employee tried to flush out with eye drops from first aid kit. After shift employee went in with eye irritation and was prescribed Polytrim eye drops for minor corneal abrasion. Employee got full release with no lost time.

The employee used the platform ladder on the right shoe. They were unaware that it pivoted down. In the process of the ladder coming down, the employee was facing the ladder and the stop caused them to drop their knees into one of the steps on the ladder. The employee worked the rest of the shift, but was unable to return to work on 9-16-22, resulting in a Lost Time Incident.

Coal shot started a fire on an unoccupied Coal Drill. No injuries. Fire was put out in 40 minutes.

Employee was encompassed by fire suppression chemical. Employee had an allergic reaction to the chemical.

Employee was working in the engine compartment of a truck in the shop. Employee dropped an extension. While bending down to pick up the extension EE's hard hat fell off EE's head. Employee grabbed EE's hard hat and took a step forward hitting EE's head on the steering filter housing. Employee received a laceration on the top of EE's head that required 3 staples.

Employee was using a ground stake pounder to pound a stake in the ground. Employee got right ring finger in between the stake and the pounder, resulting in a laceration on the top of finger. Employee was taken to a local hospital and received 9 stitches.

Drill was drilling on top of coal when a small fire started on the machine at 4:00 pm. Operator used the fire suppression and a fire extinguisher to put out the fire. 6:20pm Fire Watch decided to leave and fill the truck back up with water. Day shift fire watch never returned to the drill. Around 7:30 pm smoke was noticed by an employee. Fire was put out around 8:30 pm.

Audiologist confirmed reportable hearing loss with secondary audiogram.

Employee was just beginning EE's shift when EE lost EE's balance in the men's locker room. In an attempt to catch self, employee reached out with EE's right arm and grabbed the top of the lockers. This resulted in a dislocation of EE's right shoulder.

While lowering a section of floor grating into a work platform frame in the shop, the Welder's finger was caught between the frame and the grate. The contact cut through Welder's leather gloves and resulted in a laceration requiring 6 stitches.

While loading under a shovel, the shovel bucket opened prematurely dropping material abruptly on the tail of the truck bed. The operator experienced back pain due to the impact. Treatment was limited to first aid and the employee continued to work without restrictions until 12-22-17, when the employee received light-duty restrictions from physician.

A Dragline Oiler was moving a toolbox and pinched the tip of his finger against the toolbox and the hard surface of the swing case. The contact resulted in a fracture on the tip of his finger. He was wearing gloves at the time of the accident.

An employee was walking from the parking lot to the mine office. He stepped off of a curb, lost his balance, and fell to the ground. An x-ray revealed a fractured bone in his arm.

A plant operator was installing a bullwheel on a crusher unit. The spreader bar that the bullwheel was mounted on swung back towards the employee. The employee's fifth (pinky) finger was pinched between the spreader bar and a bracket causing a fracture and a laceration that required sutures to close. He returned to work that same shift.

A mechanic was tightening a chain and device used to change out dozer cutting edges. An overhead crane was also used. He over-hoisted the crane which put too much tension on the device. The device failed and struck his wrist causing a laceration and fracture.

A dragline oiler was next to the tub unhooking a cable strap. The dragline rotated and he was struck in the shoulder with a hook shoe. A physician allowed him to work his normal duties until a surgery could be scheduled.

A haul truck operator was attempting to fuel her truck. She was pulling on the hose to straighten it when her foot got caught in the hose. She fell to the ground. After working her normal shifts for 3 weeks, she was unable to report to work on a night shift on 2/27/15 due to her injury.

A welder was working on a shovel dipper door which was laying flat on the shop floor. While he was crouched on top of the door, his foot slipped off the edge. He fell to the shop floor and fractured his left wrist.

A mechanic was using a torque wrench on a nut. The wrench failed causing the mechanic to fall to the shop floor. As a result of the fall, his right ankle was broken.

A series of thunderstorms passed over the three pit areas. The coal runs in the bottom were flooded and activity in these areas was halted. The dirt runs, three draglines, and facilities were not affected.

A mechanic was attempting to pull a gear across a work table toward a crane. The gear slipped off the table pinching his finger. The wound took 3 sutures to close and he was given a full duty release. He returned to work on that same shift.

A mechanic was cutting a section of heater hose with a razor knife. His other hand was stabilizing the hose. As he drew the knife toward himself, the blade contacted the top portion of his left hand above his thumb causing a two-inch laceration. The mechanic received sutures to close the wound and returned to full duty on the same shift.

A mechanic was removing a hydraulic hose from a hydraulic pump on a haul truck. He was using a socket and air gun. When he started the task, the socket came loose and struck him in the mouth. He sustained a crack to his right front tooth.

An employee was using a six foot pry bar when the bar slipped and hit him in the lower lip and jaw. He received a laceration to the inside lower portion of his lower lip. Sutures were required to close the wound. The employee was released to return to full duty that same day.

A dozer operator was pushing a cable tower that had cable strung on it. The cable became draped over the dozer arm. In attempting to lift the cable off the dozer arm, he felt a "pop" in his left arm. He continued to work his next shifts, per ER doctor release,until he could see an orthopedic surgeon.He was diagnosed with a torn bicep tendon.

Mechanic alleges that while he was washing a truck in the mine wash bay, his foot slipped as he took a side step causing him to lose his balance. He states he did not fall down but felt pain in his lower back as he regained his balance. This incident is still under investigation.

A partially loaded cast blast pattern detonated unexpectedly when an adjacent cast blast pattern was fired. The two patterns were separated by a shot break consisting of two rows of empty drill holes. The unplanned detonation caused damage to a small equipment trailer and broke several windows on an unmanned overburden drill.

Employee was working on equipment and the wind came up and blew a foreign body in his right eye. Foreign body was removed by a doctor.

A haul truck operator was walking behind a haul truck when she slipped on packed snow and fell injuring her right knee and back. She received first aid treatment and returned to work on the same shift. On April 27, 2012 a physician prescribed work restrictions.

Employee was pushing a wheeled dumpster through the shop when his left pinky finger was caught between the dumpster and the tire of a haul truck that was parked in the shop. The finger was lacerated and required sutures to close.

Employee states, "Tripped while returning to my truck after fueling, stepped wrong and twisted my knee." Walking surface was flat, clear of debris, dry and illuminated at the time of the incident.

Employee states that she was closing the cab door on a drill as she exited when her left thumb was pinched between the door latch and the latch striker mechanism. The thumb sustained fractures, lacerations and an avulsed nail.

Employee was welding in the drum of a dragline. He just completed his job picked up his tools and was walking out of the dragline to go to lunch . He was wearing his safety glasses with side shields. As he stepped out into the light he felt something in his eye. He was taken to the eye doctor where a small piece of metal and a rust ring was removed from his eye.

While walking to the crew bus at the end of her shift, haul truck operator slipped and fell fracturing a bone in right wrist. Area had been bladed prior to the incident.

Employee was installing a 480 volt breaker when a short circuit occurred. Prescription ointment was applied to the employee's eyes for treatment of the flash injury that resulted.

On 10/30/2011 an employee exited a crew bus and was walking towards their haul truck. When they slipped on a patch of ice and fell to the ground landing on their right shoulder. On 10/30/2011 the injury was not reportable. However on December 15, 2011 they employee went in to see a doctor about the injury and was prescribed physical therapy for the injury.

A mechanic was attempting to move a retainer bolt from a hardnose pivot pin. The socket slipped off the bolt head and his face came in contact with the stabilizer bar. This caused a laceration to his chin and a chipped/cracked tooth. The laceration required four sutures and the tooth required crowning procedures.

The employee was cleaning out a bed of a pick up truck after completing a task. In the process he pinched his left forearm between a thirty pound transition pipe and the truck bed. This caused a laceration to his left forearm which required five sutures to close.

Employee was descending the ladder of a Caterpillar 16G blade. As he reached the ground, he twisted his left ankle resulting in a sprain/strain injury to the ankle.

EE was installing the double winch bracket onto the boom tip of the digger derrick. He was pushing the bracket down so it could swing over the boom tip. When the bracket went EE fell forward causing his left ankle to twist resulting in a fracture to the left fibula. The tight fitting bracket and the terrain were contributing factors.

Employee's hand was pinched between lower pin on tow hauler grab hook and a jackstand. When the employee was removing the pin, the grab hook unexpectedly rotated causing the pinchpoint. The employee's hand received a two inch laceration that required five sutures to close.

Employee was repositioning a hand cart loaded with two brake calipers in the warehouse, when it overturned striking the employee's right thumb, causing the thumb to be fractured.

Employee carried a floor jack to a concrete apron outside a utility shed to jack up a pickup. When he reached the pickup, he dropped the jack appoximately 1 1/2 feet to the ground. The jack handle sprung upward striking the employee's left ear. The blow caused a laceration that required sutures to close.

Employee filed a worker's compensation claim for alleged carpal tunnel syndrome on 12/27/2010.

Employee was welding cracks on a 8750 revolving frame in inclement weather, after the shift was over and he had left the mine site. An hour and half after the shift ended he reported to a shop supervisor that he had a foreign object in his (L)eye.

While walking in pit area, employee slipped and fell down causing a sprain to his left ankle.

Employee was attempting to raise the mast on a stadium light plant. When the employee got the mast vertical, he then attempted to extend the mast to raise the lights to desired height. At this time the dead end of the cable bracket broke free striking employee on the chin causing a laceration requiring 10 sutures to close.

A D-10 track dozer started pushing chunks off of the east side of the cut between the two shovels. The dozer operator started working back and forth to try to get back up their push. The dozer at that time started sliding forward on the material that had built up, and fell off of the bench.

Employee was working in the hell hole of a D-11 caterpillar dozer. To remove a breather from the transmission. When the breather broke loose from the transmission. It caused the employees right arm to move up and back striking a fire suppersion nozzle. Causing a fracture to the right elbow.

Employee was air arcing a heel band off of a 4100 shovel bucket. He was a making a cut with the air arc when the band fell from the bucket landing on his right wrist and hand.

Employee was in the process of changing a tire on a 1 ton pickup. The tire iron slipped off the lug nut and cuased the employee to fall backwards. During this, the employee's left ankle rolled, resulting in a fracture.

Employee experienced a flare up of arthritis of the right shoulder due to motion of operating a haul truck over rough roads.

Employee was lining up a lift cylinder to a D-block on a D-11 Cat dozer. During lining up of the cylinder and the D-block. The employee's left hand was caught between the D-block and the gusset on the dozer blade resulting in fractures to the left middle and ring finger.

Employee was removing a lip shroud from a shovel bucket. A hammer was used to loosen the shroud. This caused a piece of metal from the bucket to strike the employee between the eyes causing a laceration.

Employee states that he bent over to wash under a conveyor and when he stood up his knee "popped" and he felt pain.

After securing the rope on a cable tree rope brake, the employee pulled sharply on the free end of the rope which caused the rope brake to swivel towards the employee. The handle on the rope brake struck the employee on the right eye causing ocular fractures. The employee was wearing safety glasses at the time of the incident.

A haul truck driver states she was injured when the truck she was driving traveled over rough roads. The motion of the truck caused back pain. As a result, a physician prescribed pain medication, physical therapy, and time away from work.

While using a hydraulic cylinder to straighten bent flanges on a dragline mast, the cylinder dislocated, sprung from position and struck 2 employees approx. 8 feet away from the cylinder in their protective safety headgear. Employee 1 recieved a minor contusion to the forehead and Employee 2 recieved contusions, cuts and fracture from impact of the cylinder to his safety glasses

Employee was reaching to mouse a crane hook when the load of door panels slipped off a 6 x 8 x 48 1/2" wooden block and slid down to 2.5 feet - temporarily pinning his foor between the door panels and the floor.

Employee was checking the water level in a 12 volt battery. He was wearing safety glasses and a face shield. When he removed the cap, using a scraper, the cap flipped up and battery acid that was inside the cap splashed up under the face shield and safety glasses into his left eye.

Employee jumped from a rear ladder of a scraper while starting the rear engine resulting in a suspected injury to left knee.

While repairing a crack in the revolving frame on the dragline a contractor accidently cut through the floor. Sparks ignited grease below. He attempted to extinguish the fire but was unsuccessful. It was reported to MSHA at 1:45 am. Due to the inaccessibility of the fire, it was declared extinguished at 5:00 am. No injuries resulted from the incident and fire damage was minimal.

Mechanic was loosening a hydralic hose fitting on a Caterpillar 16G blade. Using a 1/2 inch ratchet, six inch extension, and a crows foot open end wrench. When the hydralic fitting came loose. It caused the ratchet to come free from the extension. Which struck the employee in the left lateral tooth causing a break in the tooth.

Employee was prying on a tie-rod end of 240 ton dresser haul truck. The pry bar slipped off the tie-rod, and employee lost his footing and fell from a standing postion to the ground. The resulting fall caused a fracture of the tibia and fibula of the left leg.

Associate was unreeling washbay hose, turned to move forward associate boot hooked on a metal rail that was not flush with concrete and associate twisted left knee.

Installing radio equipment, was removing modulator from headliner, module came loose quickly causing his left thumb to hit the radio security cage/bracket.

Employee was descending from the chain case of 16G blade. The employee lost his footing, and fell to the shop floor fracturing his left humerus. Amendment: This incident was originally classified as a medical reportable but has since become a lost time injury.

Employee was using a chipping hammer to remove a stuck bolt. He struck the chipping hammer with another hammer and a chip was propelled into the employee's left hand, dorsal side between the second and third metacarpal bones.

Smouldering coal by the shovel flared up and increased rapidly. When it was apparent that it could not be exinguished in under 30 minutes, MSHA was notified. Mine and MSHA personnel agreed on a plan and the fire was extinguished. Very hot, dry weather that had prevailed for 4 days contributed to the flare-up. No injuries and no damage resulted from the fire.

Employee was using a long bar to remove a rock out of crusher cone when he felt pain in his right shoulder.

While laying a dozer blade pin down, the pin shifted in the cardboard box containing the pin. This caused the pin to slide out of the box pinching an employee's finger causing a laceration to the end of the finger. In addition, the employee experienced a tuft fracture to the end of the finger. The employee was wearing gloves at the time of the accident.

During a sig. winter weather event it appears snow and/or moisture entered the busway between the transformer and the switchhouse on a stationary 69kv to 4160v substation. A short-circuit resulted and the arcing caused a fire that caused damage to the substation. The fire burned for approximately 38 minutes after discovery and MSHA was notified at 37 minutes after discovery.

While lifting a second intermediate propel shaft, the chain sling being used broke striking the employee on the forehead. The emplyee was wearing safety glasses and a hard hat at the time of the incident. The wound required sutures to close.

While removing a hydraulic cylinder from the boom of a coal unloader, a come-a-long was used to secure the cylinder to the boom as the cylinder pins were removed. After the pins were removed, the cylinder unexpectedly moved striking the employee on the left cheek. The employee received sutures to close the wound.

While employee was cleaning the windows on the haul truck, the wind blew dirt into the operator's left eye. The eye was flushed. The employee still felt something and was subsequently seen by an optometrist. A one time application of antibiotic ointment was applied. NOTE: Employee was wearing safety glasses with side shields at the time of the incident.

While entering the cab of a blade, the operator felt a"pop" in her right knee accompanied by minor pain. At shift change the pain increased significantly and the injury was evaluated by a physician who prescribed medication for a sprain/strain injury of the knee.

While monitoring ground conditions in the pit area, employee began to feel dizzy, light headed and nauseous. Employee was seen by a physician who diagnosed heat exhaustion/dehydration.

EMPLOYEE WAS GRINDING OVERHEAD INSIDE THE DRAGLINE TUB EVEN THOUGH HE WAS WEARING A FACE SHIELD HE THOUGHT THAT HE HAD GOTTEN SOMETHING IN HIS EYE. HE USED FIRST AID PROCEDURE TO REMOVE IT. LATER THAT DAY HIS EYE STARTED TO BOTHER HIM & HE WAS TAKEN TO AN OPTOMETRIST WHERE A RUST RING WAS REMOVED.

Employee was cleaning a shelf in the warehouse with a cloth (wiping forward on shelf). Employee turned her head as she was wiping and dust particles came down behind her safety glasses and got in her eye.

Right index finger caught in pinch point while adjusting shims on a shovel bucket. Tip of right index finger was crushed.

On January 13, 2006, employee began missing time from work due to an injury to the right knee. Injury was originally thought to be a strain, but testing revealed damage to tendon that requires surgical repair. Injury occurred when the employee stumbled on a shovel catwalk.

While trying to remove a wire choker from a haul truck the employee's left middle finger was lacerated by a sharp broken wire strand which cut thru the leather glove. The injury required stitches.

Employee was in the process of greasing a scraper. He was standing on the small platform on the gooseneck preparing to grease the gooseneck. As he squated down to reach the grease fittings he lost his balance and fell aprox. 4 feet to the ground. His outstretched right arm resulted in a fracture to the right radius at the wrist.

As Employee was grinding welds off a roller frame, a metal fragment was dislodged and thrown back at his face, where it entered his eye.

2nd degree burn to right thumb, and small 2nd degree burn on neck below right jaw line, caused by a DC volt arc between a junction box and a hoist motor on a dragline.

Employee was welding on a haul truck bed. She was positioning a piece of stock when she noticed something in her right eye. Examination found a small cut to the cornea.

Employee was working on heater system on a Rubber Tire Dozer. His left middle finger came into contact with the fan blade of this system while the fan was running. He received 14 sutures to repair the laceration.

EE WAS FABRICATING A TAP BLOCK. EE WAS DRILLING A 9/16" HOLE IN THE BLOCK USING A DRILL PRESS. AS EE WAS PULLING DRILL SHAVINGS AWAY FROM THE BIT, SHAVINGS SLICED THROUGH RIGHT GLOVE AND CUT RIGHT INDEX FINGER.

EMPLOYEE WAS STEPPING OFF OF A LADDER ONTO GROUND LEVEL. WHEN HE TOOK THE LAST STEP DOWN, HE STEPPED ON A PIECE OF PIPE HE WAS GOING TO INSTALL FOR THE FOGGER SYSTEM. WHEN HE STEPPED ON THE PIPE, HE ROLLED HIS LEFT ANKLE AND HEARD A POP.

EE WAS USING HIS FOOT TO HOLD DOWN A CRIB TIE THAT WAS STUCK UNDER A SWING ARM THAT WAS BEING DELIVERED TO THE MINE. ANOTHER PERSON STRUCK THE CRIB TIE WITH A SLEDGE TO DISLODGE IT. WHEN THE TIE RELEASED, THE EE LOST HIS BALANCE AND FELL DOWN. HE STRUCK HIS LEFT ELBOW AS HE LANDED. THE DISTANCE OF THE FALL WAS ABOUT 3 FEET.

While lifting a sample the technician said she injured her elbow. This was diagnosed as tennis elbow and became reportable 3/15/05 due to lifting restrictions given during a follow up appointment. Workers Compensation has been filed, but company is contesting the case and this report will be reviewed for further action pending outcome of the appeal.

EE WAS INVOLVED IN INSTALLING A SET OF DUMP ROPES ON A DRAGLINE BUCKET. AS THE NEW ROPES WERE BEING REMOVED FROM THE REEL ONE SET OF ROPE ENDS RELEASED TOWARD THE EE UNEXPECTEDLY. HIS LEFT INDEX FINGER WAS CAUGHT BETWEEN THE ROPE END & SERVICE TRUCK. COMPOUND FRACTURE TO THE LEFT INDEX FINGER.

Employee drilling overhead into iron. A chard of metal got past his safety glasses.

THE EE WAS IN THE PROCESS OF MOVING THE BELT SPLICE MAGNET. HE SET THE CONTROLLER DOWN & WAS RUNNING THE CONTROL BUTTONS AS WAS TRAMMING THE MAGNET. THE DOWN BUTTON STUCK & LOWERED THE MAGNET ONTO HIS MIDDLE FINGER WHICH WAS STILL ON THE CONTROLLER. THE EE PULLED HIS HAND OUT FROM UNDERNEATH THE MAGNET CAUSING THE AVULSION TO THE RIGHT MIDDLE FINGER.

EE WAS FORMING 1"ROUND STACK METAL KEEPER PINS TO A SHOVEL BUCKET WHEN HE FELT A SHARP PAIN IN HIS LEFT SHOULDER/PECTORAL AREA.

EMPLOYEE WAS REMOVING SHEATHING FROM A PIECE OF 1" CABLE WITH A UTILITY KNIFE AND CUT THE MIDDLE FINGER OF HIS LEFT HAND REQUIRING 5 SUTURES. HE CAME BACK TO WORK THE SAME SHIFT WITH A FULL DR S. RELEASE.

EE WAS WALKING ACROSS THE ASPHALT PARKING LOT ON HER WAY TO THE ENTRANCE OF THE WOMENS CHANGE ROOM WHEN SHE SLIPPED ON SOME ICE & FELL TO THE GROUND. SHE STUCK HER RIGHT ARM OUT TO CATCH HER F ALL & SUSTAINED A FRACUTRE TO HER RIGHT WRIST.

EE WAS DISMOUNTING A TRACK DOZER. AS SHE STEPPED ONTO THE LAST STEP, HER FOOT SLIPPED OFF. SHE FELL ONTO RIGHT SIDE. HER HAND & WRIST WERE PUSHED INTO HER CHEST AS SHE LANDED ON HER RIGHT SIDE . SHE FRACTURED HER RIGHT WRIST.

GRINDING ON I-BEAM IN 150 DRAGLINE ROOF BEAMS AND METAL WENT INTO RIGHT EYE.

PUT HIS RIGHT KNEE DOWN ON A PIECE OF ROCK.

TRAVELING TOO FAST FOR ROAD CONDITIONS RESULTED IN VEHICLE ROLLOVER. DRIVER SUSTAINED FRACTURED RIGHT WRIST.

TRAVELING TO FAST FOR CONDITIONS RESULTED IN VEHICLE ROLL OVER. PASSENGER CHRISTI TAYLOR, SUSTAINED FRACTURED LEFT FEMUR AND RIGHT ANKLE.

TWO MECHANICS WERE TESTING THE TILT RELIEF PRESSURE ON THE LEFT CYLINDER. ONE WAS HOLDING A 38# PIN (AS A BLOCK) BETWEEN THE DOZER BLADE AND CYLINDER HOUSING. WHEN PRESSURE WAS APPLIED, THE PI N STRUCK HIM ON THE LEFT SIDE OF THE FACE. HE RECEIVED THREE FRACTURES.

CUTTING IN A TIGHT PLACE WITH AN AIR ARC. BLEW DUST UNDER WELDING HELMET AND SAFETY GLASSES INTO LEFT EYE.

THE LOSE TO THE POWER WASH GOT CAUGHT ON THE CATWALK. HE PULLED THE HOSE & THE WAND WENT OVER HIS LT. FOOT. THE POWER WASH NOZZLE WAS STILL ON &THE WATER WENT OVER HIS STEEL TOE TO THE LACE AR EA. IT TORE HIS BOOT & CREATED A SMALL PUNCTURE TO HIS LT. FOOT. IT REQUIRED 2 SUTURES TO CLOSE AFTER CLEANING.

EMPLOYEE WAS REMOVING THE NIPPER CYLINDER ATTACHMENT BUCKET ON A D-10 DOZER WITH AIR IMPACT TORQUE MULTIPLIER WRENCH. THE IMPACT WRENCH ENGAGED DUE TO A LOOSE PLANETARY GEARING HOUSING AND PIN CHED THE EMPLOYEES LEFT THUMB BETWEEN THE BRACKET AND THE WRENCH SOCKET. THE EMPLOYEE RECEIVED 6 SUTURES AT THE E.R. ROOM & CAME BACK TO WORK WITHOUT ANY RESTRICTIONS.

HE WASHED A MET AL SPECK OUT AND THEN WENT TO THE HOSPITAL AS ITWAS STILL BOTHERING HIM. NOTHING ELSE WAS FOUND IN HIS EYE BUT THEY ADMINISTERED PRESCRIPTION EYE DROPS.

EE WAS IN THE PROCESS OF SWITCHING TANKS DURING A BATCH LOADING PROCESS & HAD BLOWN OUT THE LINE TO SWITCH TANKS & HAD LOOSENED THE EARS ON A CAMLOCK SITTING ON THE HOSE WHEN THE HOSE BLEW OFF DUE TO PRESSURE REMAINING IN THE LINE SPLASHINGHOT EMULSION PRODUCT ON EE'S FACE & INTO HIS EYES. EE REC'D 1ST DEGREE BURN & SOME EYE IRRITATION & WAS GIVEN A PRESCRIPTION FOR PAIN BY ER DR.

EMPLOYEE WAS PULLING PIPE OFF TRUCK, PULLED SLING AWAY FROM PIPE AND IT HIT EMPLOYEE'S LEFT SHOULDER, WHICH IN TURN TWISTED HIS BACK CAUSING SPINAL PAIN.

EE WAS USING A RATCHET AND SOCKET, TURNING THE ENGINE OVER IN NET 590. WHILE PULLING THE RATCHET, HE DROPPED SOMETHING IN HIS RIGHT WRIST. IT STARTED TO SWELL. HE WAS SEEN BY A DOCTOR. HE WAS DIAGNOSED WITH A SPRAINED WRIST.

EE WAS USING A RATCHET TO TIGHTEN A CLAMP FOR THE EXHAUST ELBOW INSIDE THE INGIME COMPARTMENT. WHILE TURNING THE RATCHET, 1 IS FINGER GOT TOO NOSE THE METAL EDGE AND HE REC'D LACERATION, REQUI RING 4 SUTURES.

EE WAS CHANGING THE DUMP BED ON A HAUL TRUCK. HE WAS IN THE PROCESS OF PUTTING THE BED PIN OUT OF THE BORE HOLE WHEN THE PIN CAME LOOSE & BOUNCED OFF THE TRUCK & STRUCK HIS LEFT PINKY. HE WAS TAKEN TO THE HOSPITAL FOR X-RAYS WHICH REVEALED FRACTURE TO HIS BACK & MIDDLE KNUCKLES OF HIS PINKY FINGER.

OPERATOR LOST CONTROL OF SCRAPER, ROLLED OVER 1 TIME. AT THIS POINT IN TIME REASONS ARE UNKNOWN.OPERATOR RECEIVED A CONCUSSION & LACERATIONS TO THE TOP FRONT OF HIS HEAD.

ON 12/25/01 EE WAS JARRED IN HER HAUL TRUCK WHILE BEING LOADED BY A SHOVEL. SHE WENT TO SEE DR & RET'D TO WORK WITH FULL RELEASE. SHE CON'T WORKING BUT WE (THE EMPLOYER) REQUESTED A 2ND OPINION & MRI. THE MRI REVEALED SEVERE DETERATION OF LOW CERVICAL AREA & IT WAS RECOMMENDED OF THE ORTHAPEDIC SURGEON THAT SHE NOT RUN EQUIPMENTAS SHE COULD BE PARALZYED IF THE RIGHT CONDITION

A CONTRACTOR WAS SETTING UP A 50' EXTENSION LADDER. IT STARTED TO FALL AS VICTIM WAS WALKING BY.HE REACHED OUT TO GRAB IT. HE OVER-ESTENDED HIS LEFT SHOULDER. HE FELT FINE FOR SEVERAL WEEKS, THEN STARTED HAVING PAIN. HE HAD SURGERY ON HIS SHOULDER ON 2/6/02.

EE WAS HOLDING A PIN DRIVER ON A RIPPER POINT ON A TRACK DOZER. THE DOZER OPERATOR WAS SWINGING THE SLEDGE HAMMER. ON THE SECOND HIT, A PIECE OF THE DRIVER BROKE OFF, HITTING EE ON THE RIGHT S IDE OFHIS NOSE. THE PIECE OF METAL WAS REMOVED & HE RECEIVED 6 SUTURES.

EMPLOYEE WAS DOZING ALONG THE HIGHWALL AND DROPPED INTO A HOLE THAT WAS 2-3 FEET DEEP. THE INJURY RESULTED FROM HIS HOSE PUSHING AGAINST THE DOZER CONTROL HANDLE WHICH PINNED HIS FINGER AGAINS T THE TOGGLE SWITCH FROM THE IMPACT OF THE DOZERSETTING IN THE HOLE. HE RECEIVED A SMALL PUNCTURE LACERATION WHICH REQUIRED 5 SUTURES TO CLOSE THE WOUND.

EE WAS POSITIONING A METAL CABLE CHOCKER TO FREEA DOZER THAT WAS STUCK. THERE WAS A SHARP BURR OR WIRES STICKING OUT FROM THE CABLE WHICH PENETRATED HIS OVERALLS CAUSING THE LACERATION. HE REC EIVED 3 SUTURES TO CLOSE THE WOUND AND RELEASED TO RETURN TO WORK.

EE HAS BEEN AN EQUIP OPERATOR FOR OVER 10 YEARS. SHE HAS BEEN HAVING TROUBLE WITH HER R HAND FALLING ASLEEP. SHE WNET TO THE DOCTOR AND HE DIAGNOSED CARPAL TUNNEL SYNDROME. NO WORK MISSED YET BUT WITH HAND SURGERY IN THE NEAR FUTURE.

HE WAS STEPPING OFF A FLAT BED TRAILER. HIS FOOT SLIPPED OFF THE STEP & HE FELL TO THE GROUND. HE LANDED ON HIS RT. SIDE. HE RECEIVED A BONE CHIP TO HIS L. WRIST & BRUISED HIS L. SHOULDER.

EE PULLED HIS TRUCK UNDERNEATH THE SHOVEL TO GETIT LOADED. THE SHOVEL LOST POWER AS IT WAS SWINGING INTO HIS TRUCK AND THE BUCKET HIT THE TRUCK BED, JARRING THE OPERATOR. HIS NECK WAS SORE AND STIFF. HE CONTINUED WORKING.

THE GD70 DRILL IS EQUIPPED WITH A SUB SAVER. IT WAS BEING REPAIRED BY A WELDER. AS HE WAS PREPARING TO MAKE THE FIRST WELD, THE PINUND OF THE STEEL BROKE AT THE THIRD THREAD VALLEY. A 1500 LB LIFT SECTION FELL AGAINST HIM BRUISING RIGHT ARM

WORKING OUTDOORS IN DUSTY, WINDY ENVIROMENT. RIGHT EYE BECAME IRRITATED FELT LIKE SOMETHING WAS IN IT. FLUSHED EYE WITH SOLUTION AT SITE, DID NOT IMPROVE, WENT TO E.R. AT HOSPITAL. COAL DUST P ARTICLES WERE REMOVED AND ANTIBIOTICS WERE ADMINISTERED.

EMPLOYEE WAS PUTTING SCORIA DOWN ON THE ROADS WITH A SCRAPER AND HIT A ROUGH SPOT IN THE ROAD. SHE WAS SEEN AT THE HOSPITAL AND DIAGNOSED WITH A BACK STRAIN. SHE CONTINUED TO WORK HER FULL DUT IES, BUT HER BACK CONTINUED TO CAUSE HER PAIN. ON 8-1-01 SHE WENT BACK TO SPECIALIST AND WAS TOLD TO STAY OFF EQUIPMENT IF HER BACK WAS TO HEAL. SHE BECAME A LOST-TIME AS OF 8-02-01.

FAILED LUG PLATE WAS ON THE GROUND, WHILE LIFTING PLATE, ON END WITH FORKLIFT, EE WAS POSITIONING BLOCKS UNDER THE LUG. THE LUG SLIPPED OFF THE FORK AND PINCHED OR SMASHED EE'S THUMB BETWEEN T HE LUG PLATE AND THE BLOCK OF WOOD.

EE HAD THE LOAD HE WAS WORKING ON SUSPENDED ABOUT 2" OFF THE CONCRETE FLOOR. THE RIGGING SLIPPED JUST ENOUGH TO CAUSE THE LOAD TO COME DOWN ON HIS FINGER WHICH WAS UNDERNEATH THE FRAME AND FRA CTURED THE TIP OF IT WHILE SMASHING THE OUTER EDGE.

THE JUG TO THE # SWINGER HAD TO BE RE-ALIGNED AND REPAIRED. THE JACK WAS PLACED BETWEEN THE WALL AND THE SWINGER. WHILE EE WAS NEAR, THE JACK SLIPPED OFF THE WALK, STRIKING HIM IN THE LEFT LEG AND LEFT HAND.

EE DID NOT REPORT HIS ACCIDENT UNTIL 2/12/01. HEINFORMED SITE SUPERVISOR THAT HE HURT HIS BACK. NO FURTHER INFORMATION AVAILABLE AT THIS TIME.

WHILE WALKING DOWN A SLOPE TO THE CRUSHER, THE EMPLOYEE SLIPPED ON ICE. HIS LEAD HIT A CONVEYOR BRACE AND HIS R EAR WAS LACERATED. HE REC'D 9 SUTURES.

EE WAS ENTERING PENTHOUSE AT CORDERO SILOS. WASHDOWN PROCESS WAS TAKING PLACE CAUSING FLOORS TO BE WET. EMPLOYEE SLIPPED ON WET, ICY FLOOR. SHE TWISTED HER RIGHT KNEE CAUSING PATELLAR DISLOCAT ION.

EE & COWORKER WERE CARRYING BOTTLES OF OXYGEN WHEN EE FELT PAIN IN HIS LOWER BACK.

EE WAS TRYING TO BREAK A PLUG OUT OF A "Y" STRAINER WITH A BREAKER BAR & FELT A POP IN HIS RIGHTELBOW. HE DIDN'T FEEL ANY FURTHER PAIN SO HE FINISHED HIS SHIFT. THE NEXT MORNING HE HAD SWELLIN G IN HIS ELBOW. HE WENT TO THE DOCTOR WHO DIAGNOSED A HAIRLINE FRACTURE.

DURING THE SHIFT EMPLOYEE GOT A FOREIGN OBJECT IN HIS EYE. IT WAS SCRATCHY THE NEXT DAY SO HE HAD IT LOOKED AT. AN EYE DOCTOR PUT ANTIBIOTIC DROPS IN THE RIGHT EYE FOR THE CORNEAL SCRATCH.

EMPLOYEE WAS CLOSING THE TRUCK SIDE DOOR. HE WAS HOLDING THE WINDOW WITH RIGHT HAND AND PULLING DOOR WITH LEFT HAND. WHEN IT CLOSED AND MASHED KNUCKLE AREA, RESULTING IN A LACERATION REQUIRING SUTURES.

EE WAS SECURING HER LUNCH BUCKET WITH A BUNGEE CRD. SHE HOOKED IT TO A METAL SHELF. AS SHE WAS STRETCHING IT TO HOOK THE OTHER SIDE, THE END CAME UNHOOKED, SPRANG LOOSE AND HIT HER IN THE LEFT EYE. SHE RECEIVED A CORNEAL ABRASION AND LACERATION.

EE GOT OFF THE ELEVATOR AT THE PREP PLANT & A GUST OF WIND CAME UP & BLEW DIRT & DUST IN HIS FACE & EYES. EE HAD A RESCUE TEAM MEMBER FLUSH EYES OUT BUT IT DIDN'T HELP. WE TOOL EE TO THE EYE D OCTOR WHERE A METAL SPEC WAS REMOVED FROM HIS CORNEA. THE EE RETURNED TO WORK THE FOLLOWING SHIFT WITHOUT ANY PROBLEMS.

EMPLOYEE NOTICED PAIN IN HIS RIGHT EYE AT BREAK AT 10:30 P.M. HE HAD IT LOOKED AT BY A RESCUE TEAM MEMBER AT 3 A.M. AS HE WAS STILL IN PAIN. NOTHING WAS FOUND IN THE EYE AT THIS TIME. HE WE NT HOME FOLLOWING HIS SHIFT & STILL HAD PAIN THE NEXT DAY SO HE WENT TO DR. A PIECE OF METAL SHAVING WAS REMOVED FROM HIS RIGHT EYE; HE REPORTED BACK TO WORK WITH NO FURTHER PROBLEMS.

INSTALLING HYDRAULIC TENSION BLADE.

EE WAS ATTEMPTING TO OPEN EAST DOOR TO LAB OFFICE. THE DOOR DID NOT UNLATCH AND THE MOMENTUM OF HER BODY RAN INTO THE DOORS WINDOW. THE WINDOW BROKE AND EE GOT SMALL PARTICLES OF GLASS IN BOTH EYES. RESULTED IN CONJUNCTIVITIS AND FOREIGN PARTICLES IN BOTH EYES.

EE WAS BENDING DOWN TO PUT A SPACER IN THE LEFT SIDE OF A NOSE CONE ON A HAUL TRUCK. AN ANCHOR CHAIN FAILED AND A 3 TON COME-ALONG SWUNG DOWN AND THE CHAIN OR PART OF THE COME-ALONG STRUCK HIM ON THE LEFT SIDE OF HIS HEAD CAUSING A LACERATION WHICH REQUIRED SUTURES.

EMPLOYEE WAS DRIVING TO BLAST PATTERN IN HIS VEHICLE AT THE BULK STORAGE SITE. THE VEHICLE OVERTURNED ON THE DRIVER SIDE OF THE ROADWAY APPROX 1050 FT FROM THE BULK STORAGE AREA. EMPLOYEE HAD NO RECOLLECTION OF HOW THIS HAPPENED AND THERE WERE NO WITTNESSES. EMPLOYEE WAS IN COMPLIANCE WITH RULES AND REGULATIONS. ACCIDENT DID NOT INVOLVE MINING EQUIPMENT.

EE WAS UNLOADING A SMALL POWER POLE, AND HIS HAND SLIPPED OFF. THIS CAUSED A TEETER TOTTER EFFECT WITH THE POLE. THE POLE WAS SUPPORTED AT THE CENTER POINT WITH A SET OF POLE TONGS. AS THE TOP OF THE POLE CAME IN CONTACT WITH THE GROUND IT FORCED THE POLE TONGS TO RELEASE. THE POLE HIT THE GROUND AND ON THE BOUNCE IT HIT HIM IN THE LEG.

EE WAS DRIVING A HAUL TRUCK, HAULING DIRT. HE BACKED UP TO DUMPS THE LOAD. HE RAISED THE BED AND PULLED FORWARD AS THE DIRT YEARD THE BID, HE STOPPED. THE FRONT THE TRUCK CAME DOWN WITH FORCE, JARRING JIM IN HIS SEAT HE REACH A COMPRESSION FRACTURE TO HIS BACK.

EE WAS WORKING ON REMOVING A PARK BRAKE ASSEMBLY ON A 220 TON HAUL TRUCK. ONE OF THE BOLTS HOLDING THE ASSEMBLY IN PLACE BROKE OFF DUE TO BACK PRESSURE AND STRUCK THE EE ON THE TOP OF THE LITT LE FINGER OF HIS RIGHT HAND. HE RECEIVED A DEEP LACERATION REQUIRING SEVERAL SUTURES.

EE WAS IN THE PARKING LOT CLEANING THE WINDSHIELD ON HER VEHICLE. SHE WAS STRUCK IN THE L EYE WITH A SNOWBALL

EMPLOYEE SAT DOWN TO EAT HIS LUNCH AND BRUISED SOME METAL SHAVINGS OUT OF HIS HAIR WITH HIS HAND. THE ONLY THING HE CAN THINK OF IS HE MUST HAVE GOT A PIECE OF THE METAL SHAVING IN HIS EYE DURING HTE TIME.

EE WAS FRAMING A POLE. THE HOIST CAME LOOSE FROM ITS ATTACHMENT POIT STRIKING HIM IN THE RIGHT SIDE OF THE HEAD. HE WAS TRANSPORTED TO CCMH BY CORDERO PERSONAL AND RECEIVED SIX STITCHES 2ND RE LEASED.

EE WAS WORKING ON AN AIR HOSE VALVE. HE TURNED THE PRESSURE BACK ON TO THE HOSE AND THE FITTING HAD NOT SEATED ALL THE WAY IN. THE HOSE BLEW OUT OF THE VALVE, WHIPPING AROUND AND STRUCK HIM IN THE UPPER LIP CAUSING A LACERATION. THE EE WAS TAKING TO THE E.R. ROOM WHERE HE RECERRED 4 SUTURES AND RETURN TO WORK.

EMPLOYEE WAS LAYING DOWN A PUMP DRIVE GEARBOX ONTO A WORK TABLE. IT WEIGHS APPROX 300 LBS. AS ITGOT TO LAYING ON THE TABLE, THE BOTTOM KICKED OUT CAUSING IT TO FALL QUIKLY. HIS FINGER WAS PINC HED BETWEEN THE WORK TABLE AND THE GEARBOX.

JUSTIN AND JEREMY WERE MOVING MG MOTORS FROM THESOUTHEND OF THE SHOP. AT THE NORTHEND OF THE SHOP, ON THE FLOOR WAS AN "I" BEAM AND CRANE HOIST WITH A HOOK. JUSTIN STEPPED BETWEEN THE "I" BEAM AND HOOK, THEN TOOK A STEP BACK. THAT IS WHEN HEMUST HAVE STEPPED ON OR HIT THE HOOK, CAUSING ITTO SLIDE CATCHING JUSTIN'S ANKLE, CAUSING A FRACTURED ANKLE BONE.

EMPLOYEE WAS TRYING TO LOOSEN A NUT ON A HYDRAULIC FILTER HOUSING ON A WATER TRUCK WHEN THE NUT DIDN'T LOOSEN, HE USED A "CHEATER BAR" TO HIT WRENCH HANDLE AND STRUCK HIS LEFT WRIST.

SCRAPING GREASE AND DUST.

WHILE TRYING TO REMOVE A PIN FROM THE BLADE OF A DOZER, EE WAS HOLDING A PIN WHILE ANOTHER WAS HITTING IT WITH A HAMMER. A GLANCING BLOW HIT THE PIN DRIVER & A SMALL PIECE OF METAL CAME OFF TH E DRIVER HITTING EE IN THE THIGH.

IT WAS AN EXTREMELY WINDY DAY. EE WAS WORKING ON A TRACK DOZER BY THE 153 DRAGLINE. SOMETIME DURING THIS TIME A SMALL PIECE OF METAL WENT INTO HIS EYE. WASN'T NOTICED TIL LATER IN THE SHIFT.

EE WAS DOING A WELDING PROJECT ON SWING SHIFT 10-24-98. ON THE NEXT SHIFT, 10-25-98, HE NOTICED PAIN AND SWELLING. UPON EXAMINATION, A SMALL BURN WAS NOTICED BELOW HIS RT KNEE CAP, APPROX 1/16 OF AN INCH. HE WENT TO CCMH E ROOM, WHERE HE WAS TREATED FOR INFECTION.

THE EMPLOYEE WAS PULLING THE REAR PLANETARY OFF THE LEFT SIDE OF A WATER TRUCK. AS THE COVER CAME OFF, IT SLIPPED OUT OF HIS HANDS AND THE SHIM CAUGHT HIS LEFT THUMB AND CUT IT. THE LACERATION REQUIRED SIX STITCHES.

EMPLOYEE'S RIGHT INDEX FINGER GOT CAUGHT ON A BROKEN STRAND ON A CHOKER CABLE, WHEN THE OTHER EE LET GO OF THE CHOKER AND IT FELL TO THE GROUND. THE CHOKER WAS BEING USED TO PULL A HAUL TRUCK THAT HAD GOTTEN STUCK. THE EE RECEIVED FOUR STITCHES IN HIS RIGHT INDEX FINGER.

EE WAS STEPPING FROM ONE CUBICLE TO ANOTHER CUBICLE IN THE REVOLVING FRAME OF AN 8750 DRAGLINE AND STEPPED ONTO A GRATING COVERING A MANHOLE AND THE COVER COLLAPSED AS THE WELD ON THE GRATING BROKE WHEN THE EE STEPPED ON IT. THE EE LEFT LEGTHROUGH THE HOLE CAUSING A LACERATION ON HIS SHIN.

WHILE WORKING AS A MAINTENANCE LEAD PERSON, THE EE GOT OUT OF A PICK UP TRUCK AND FELT PAIN IN HIS BACK. HE BENT OVER TO MEASURE A RADIUS AND SUFFERED EXTREME BACK PAIN. THE EE WAS TRANSPORTED TO THE CHIROPRACTOR WHO ADMINSTERTED ELECTRICALSTIMULUS TO RELAX THE MUSCLE AND PHYSICAL ADJUSTMENT. THE DRDIAGNOSED A PINCHED NERVE.

EE PICKED UP A GROUND WHEEL PIN OFF OF A P&H SHOVEL AND ATTEMPTED TO CARRY IT ACROSS THE SHOP. HE LOST HIS GRIP DUE TO GREASY PIN AND MASHED RIGHT RING FINGER BETWEEN PIN AND CONCRETE FLOOR. E E IS A CONTRACTOR OF SKV WHO WAS WORKING FOR CORDERO MINING CO.

EE WAS CLIMBING DOWN OFF OF UNIT #188 HAULTRUCK TO GET TO THE GROUND. AS THE EE WAS CLIMBING DOWN THE LADDER TO GET OFF THE HAUL TRUCK, THE BOTTOM RUNG OF THE LADDER SNAPPED BACK AND HIT THE EE IN THE NOSE.

THE EE WAS CLIMBING THE BOARDING LADDER ON UNIT #590 EUCLID HAULER WHEN HIS FOOT SLIPPED OFF THE TOP STEP, CAUSING HIS LEFT SHIN TO STRIKE THE NEXT STEP, CAUSING A LACERATION TO HIS LEFT SHIN, APPROX 2"LONG. TREATMENT REQUIRED 4 STAPLES TO CLOSE THE LACERATION.

WHILE WORKING AS GROUNDPERSON AT 157 DRAGLINE, THE EMPLOYEE GOT SOMETHING IN HIS RIGHT EYE AND HIS EYE EVENTUALLY BECAME IRRITATED. HIS EYE WAS FLUSHED OUT AT THE MINE AND ALSO AT THE ER. NO F OREIGN BODY WAS REMOVED FROM EYE, BUT DR. DID DIAGNOSE AN ABRASION TO THE RIGHT CORNEA AND PRESCRIPTION MEDICATION WAS GIVEN.

WHILE REMOVING THE DRIVE LINE GUARD FROM A CATERPILLAR D11N, THE 9/16" WRENCH EMPLOYEE WAS USING SLIPPED PINCHING HIS THUMB BETWEEN THE WRENCH AND THE GUARD.

EE HAD SHARP PAIN IN BACK WHILE WALKING ON JOB SITE. SAID IT WAS FROM LIFTING AND CARRYING 2" PLASTIC PIPE ON PRIOR DAY.

EMPLOYEE WAS WASHING COAL DUST FROM STEEL IN SILO. HE COMPLAINED OF CHEST PAINS. TESTS SHOW PAIN WAS CAUSED BY CLOGGED ARTERIES AROUND HEART.

EMPLOYEE WAS WELDING A SUBCON TRACTOR FOR KG CONSTRUCTION.HE WAS HIRED TO REPLACE A SPINDLE ON A SCRAPER.THE OWNER OF WALTERS WELDING WAS WORKING ON THE OLD SPINDLE WHICH WAS STILL ON THE SCRP ER. THE OLD SPINDLE FELL FROM THE SCRAPER,APPROXIMATELY 15 INCHES TO THE GROUND,AND ROLLED OVER THE INJUREDS FOOT. EMPLOYEE WAS GRINDING ON THE NEW SPINDLE RIGHT BY THE OLD SPINDLE.

EE WAS REMOVING THE LID ON A PULL-TOP CAN OF TUNA FISH WITH HIS RT HAND, AND HOLDING THE CAN IN PLACE WITH HIS LEFT HAND. AS THE EE PULLED THE TOP BACK, THE LID POPPED OFF AND CUT HIS LEFT HAN D CREATING A LACERATION APPROX TWO CENTIMETERS IN LENGTH BETWEEN THE INDEX FINGER AND THUMB. THE LACERATION REQUIRED FOUR SUTURED TO CLOSE.

EE WAS CLIMBING OFF THE LOWERED MAST OF THE 393 DRILL WHEN HE SLIPPED AND FELL APPROX 5 FT ONTO THE CATWALK, LANDING ON HIS FEET, BUT STRIKING HIS RIBS AGAINST THE HANDRAIL. THE EE DID NOT GO TO THE DR UNTIL 9-24-97, AT WHICH TIME X-RAYS SHOWED THE EE'S 7TH RIB WAS FRACTURED.

EE WAS AIR-ARC WELDING ON THE #3 SWINGER OF THE 150 DRAGLINE, WITH HIS PROPER PERSONAL EQUIPMENTDONNED. LATER ON N THE SHIFT, THE EE'S LEFT EYE CONT' TO IRRITATE HIM. AFTER WORK, THE EE VISITE D HIS EYE DR WHO REMOVED THE SMALL STEEL PARTICLFROM THE EE'S EYE AND ISSUED A RX.

EMP WAS INSTALLING HANDRAIL SECITON ON ELEVATED DECK AT CORDERO MINE SILO. EMP LOST CONTROL OF HANDRAIN. HANDRAIL FELL APPROX 18' STRIKING INJURED EMP.

EE WAS OPERATIG A LAMINA MAGNET DRILL, REAMING ASWING RACK DOWEL HOLE. THE DRILL BOUND UP IN THEHOLE AND THE MAGNET LET LOOSE, ALLOWING THE DRILL TO SPIN. THE DRILL SPUN AROUND HITTING THE EE ON THE RIGHT SIDE OF HIS FACE & THROWING HIM DOWN ON THE CRAWLER FRAME ABOUT 4-41/2 FT BELOW

WHILE GRINDING IN THE DRAGLINE TUB A PARTICLE OF METAL GOT INTO EE'S EYE. HE SOUGHT MEDICAL ATTENTION AND THE PARTICLE WAS REMOVED.EE WAS WEARING SAFETY GLASSES AND A FACE SHIELD.

EE WAS GRINDING ON A DRAGLINE TUB AND A PARTICLE OF METAL GOT IN HIS RT EYE. ORIGINALLY EE THOUGHT THAT THE EYE WAS JUST SCRATCHED BUT THE IRRITATION CONTINUED. EE REPORTED THIS ACCIDENT ON 6- 2-97. EE WAS WEARING SAFETY GLASSES AND A FACE SHIELD.

EE WAS TIGHTENING BOLTS ON A HAUL TRUCK MIRROR AND FELT SOMETHING PULL IN HIS BACK. HE DID NOT SEEK REPORTABLE MEDICAL ATTENTION UNTIL 1/19/98 AT WHICH TIME A CHIRPORATOR TREATMENT WAS GIVEN TO THE EE'S BACK WHICK INVOLVED ELECTRICAL IMPULSE MUSCLE MANIPULATION.

EE WAS REPARING A 480 VOLT ELECTRICAL CABLE. HE WAS CUTTING AWAY INSULATION ON CABLE WITH A RAZOR KNIFE. THE KNIFE SLIPPED AND HE CUT THE BACK OF HIS LEFT INDEX FINGER. THE RESULTING LACERATION REQUIRED 4 SUTURES TO CLOSE

EE WAS GRINDING WELD ON REPAIR PROJECT IN THE DRAGLINE REVOLVING FRAME. HE WAS WEARING SAFETY GLASSES AND A FACESHIELD. AT THE END OF HIS SHIFT HE DID NOT EXPERIENCE ANY PROBLEMS. LATER WHEN H E WAS CLEANING UP, HE FELT SOME PARTICLES ON HISEYELASHES, BUT FELT HE WAS ABLE TO WASH THEM AWAY. THE NEXT MORN HE FELT A PARTICLE IN HIS EYE. ATTEMPTS TO FLUSH THE PART OUT WERE UNSUCCESSFU

EMP WAS CLIMBING UP ON DOZER ARM TO CHECK ENGINEOIL AND COOLANT. FOOT SLIPPED ON DOZER ARM CAUSING HIM TO FALL LANDING ON RIB CAGE AND STRIKING JAW ON DOZER ARM.

THE EE WAS ATTEMPTING TO MOVE A TRAILER MOUNTED WATER PUMP. THE HITCH ON THE TRAILER WOULD NOT RAISE HIGH ENOUGH TO HOOK ON THE RECIEVER OF THE TRUCK. THE EE USED A HANDYMAN JACK TO RAISE THE HITCH. AS THE EE WAS JACKING THE TRAILER UP, THEHANDLE SLIPPED FROM HIS GRASP AND STRUCK THE EE IN THE CORNER OF THE MOUTH. THE WOUND REQUIRED 5 SUTURES TO CLOSE.

THE INJURED EE WAS PUTTING A PIN IN THE BACK OF A RUBBER TIRED DOZER. THE CHAIN ATTAACHED TO THE TOP OF THE PIN WAS NOT IN THE PROPER POSITION AS THE EE MOVED THE CHAIN THE PIN DROPPED, SMASHI NG THE END OF THE LONG FINGER ON HIS RIGHT HAND.

EE STRUCK THE DOOR TO THE LADIES RESTROOM HARD WITH HER HANDS. THE RESULTING IMPACT CAUSED A HAIRLINE FRACTURE OF THE RT WRIST.

WAS LOSENING BOOSTERS DOWN HOLE, SO HE GOT OUT OF THE TRUCK, BENT OVER TO GRAB STAKE AND TWISTED OR STRAINED LOWER BACK MUSCLE.

THE EMPLOYEE STATED THAT HE DID NOT KNOW HOW THE INJURY OCCURRED. HE INDICATED THAT HIS RIGHT SHOULDER STARTED HURTING EARLY IN THE SHIFT AND GOT PROGRESSIVELY WORSE THROUGHOUT THE DAY. THE EMPLOYEE CANNOT ATTRIBUTE THE INJURY TO ANY SPECIFIC ACTIVITY OR ACCIDENT.

EMPLOYEE WAS CLEANING A PLUGGED BAGHOUSE WHEN HIS WATER STREAM HIT SOME HOT COAL DUST WHEN THE DUST BECAME AIRBORNE & ENTERED THE OUTSIDE ATMOSPHERE IT IGNITED & CAUSED A FLASH FIRE WHERE THE EMPLOYEE WAS STANDING

AFTER WELDING A MUD FLAP BRACKET ON A WABCO 170 TON TRUCK BED, THE BOOM ON THE JLG MANLIFT GOT CAUGHT ON A HAUL TRUCK TIRE. EE LOOKED OVER THE SIDE OF THE BASKET TO SEE WHAT IT WAS CAUGHT ON W HEN IT SPRUNG FREE. THIS ACTION CAUSED THE BASKET TO STRIKE EE IN THE FACE WHICH CAUSED A LACERATION REQUIRING 4 SUTURES.

INJURED WAS DISMOUNTING A CAT D10 TRACK DOZER DURING WET AND RAINY CONDITIONS. HE SLIPPED AND TRIPPED OVER SOME DIRT CHUNKS AND FELL BACKWARDSONTO HIS RIGHT SHOULDER. THIS CAUSED A TEAR TO THE RIGHT ROTATOR CUFF WHICH WILL BE SCHEDULED TO BE SURGICALLY REPAIRED ON 8-2-95. THE INJURED WILL REMAIN WORKING IN HIS NORMAL JOB DUTIES DURING NORMAL WORK WEEK UNTIL SURGERY WHEN HE

EMPLOYEE STATED THAT AS HE ATTEMPTED TO BREAK HIS FALL WITH HIS LEFT ARM, HE INJURED HIS LEFT SHOULDER. IT WAS DIAGNOSED AS A POSSIBLE MUSCLE SPRAIN ON 4-28-95, UPON FURTHER MEDICAL EVALUATIO N ON 5-17-95, AN MRI INDICATED THAT THE LEFT ROTATOR CUFFWAS TORN.

EE STATED THAT WHEN LIFTING A SHORT RAIL UP OVER A SWITCH STAND, APPROX. 7', THE BOTTOM OF RAIL BUMPED THE TOP OF SWITCH STAND WHICH JARRED THE RAIL LOOSE FROM THE RAIL TONGS. AS A RESULT THE RAIL FELL AND STRUCK THE EE.

WHILE DRIVING A DUMP ROPE WEDGE OUT OF SOCKET WITH 20# SLEDGEHAMMER, A FRAGMENT OF THE WEDGE STRIKING PLATED BROKE OFF AND IMBEDDED INTO LEFT THIGH.

EE WAS STANDING AT REAR OF FLATBED WELDING TRK. GRINDING ON METAL PLATES. WINDY AND RAINY. LARGER BOOM TRK. BACKING UP AND STRUCK EE AND PINNED HIM. BACKUP ALARM WAS OPERATING. WIND, RAIN, AND GRINDING COMBINED SO EE DID NOT HEAR ALARM. EE SUSTAINED BRUISES.

WHILE CHECKING THE TOP OF THE WATER TRUCK TANK IN RELATION TO TEH LOADER SPOUT AT THE WATER STAND. DUST SUPPRESSANT CHEMICAL, CHEM-LOC 411 SPLASHED IN RIGHT EYE.
