Arcosa Streetman Plant # 1605 Metal/Non-Metal

EE was repairing a hinge on a metal storage box and felt discomfort in left shoulder area. Clinic was called and EE opted for self-care. On 07/15/2024 EE was seen by a medical professional and was returned to work with no restrictions and continued self-care. On 07/22/2024 EE returned for a follow up visit and was given restrictions and scheduled for an MRI.

At appromently 8:30 an employee was using a chop saw in the maintenance shop when the ee notice a fire on the wall. The employee extinguished the the fire and continued to work, at 8:47 the employee noticed the fire had reingnited. MSHA was called at 9:12

Employee was driving John Deere Gator from parts room. EE said had a fitting in EE's right hand. The fitting fell and EE bent over to pick it up from the floor board while driving over covered drainage ditch. Per employee the "bump" from driving over drainage ditch cover and EE bending over caused EE's back to jar.

On 3/9/2021 the miner was prying a log out of the cooler discharge EE felt discomfort in EE's right lower back. The miner reported this to EE's supervisor and stated that EE didn't want to go and have it checked out. On 3/16/2021 the miner reported to EE's supervisor that the pain had increased over EE's long change off and would like medical attention.

While cleaning raw material build up from feed hopper skirt board with a scraping bar the employee attempted to strike an over size chunk of material and smashed right index finger and thumb on the corner post of the hand rail.

Employee was replacing an A/C blower motor at MCC-3. While removing electrical tape and zip ties with a knife the knife slipped lacerating the employee's left thumb.

While cleaning the kiln feed pipe a piece of material dislodged in the feed chute striking the EE on the left thumb causing a displaced fracture of distal phalanx of finger.

Operator was cleaning C-9 product chute. While chipping material with an air chisel, a rock fell onto the operators hand causing a laceration on the left thumb

Employee was pulling tray from material splitter. As ee was pulling the tray out, the right side hung up. EE then pulled with left hand only to dislodge it. This is when ee felt a pain in left shoulder.

Employee was removing five gallons sample buckets from a pickup when they felt a pain in the lower back.

Employee was attempting to break up compacted material inside extruder hopper. Employee used a large auger part to strike the side of the hopper. Employee smashed employee's finger between the auger part and the hopper.

Employee was walking to the employee building. Employee turned to left to go back inside the shop and slipped on some loose gravel spraining right hand.

On 1/12/16 employee was cleaning up a material spill. EE thought EE had pulled a muscle in EE's stomach. Employee reported the injury to EE's shift supervisor. However, the supervisor did not report the injury to the plant manager. On 1/25/16 employee went to EE's primary doctor. The doctor informed the employee that EE had a hernia.

Employee was using a cutting torch to cut a return roller bracket off the C-51 conveyor system. He moved his left hand in front of the torch, burning himself. Employee reported to leadman but the leadman did not report to plant manager. Therefore plant manager did not tell safety department until 3/16/15.

EE's were installing a dogbone, the dogbone slipped off the shaft smashing hand of other EE. The bone slipped of the cooler side shaft. The injured employee reached up to keep the bone from striking another employee in the head, when he did, it caught his hand in between dogbone and chain.

Employee was cleaning a chute on a conveyor system. He was in a squatted position for several minutes, when he tried to stand up he pulled a muscle in his lower back.

Employee was moving oil barrels in shop and pulled a muscle in his lower left back.

At 8:45am EE was remodeling the birdhouse (load out shack). He shot his left index finger with a staple gun.

EE was dismounting WA50 Wheel Loader and had just taken foot off of step and he took 1 or 2 steps back when he fell backwards on material or uneven ground. EE could not get up or move his right leg after he fell down.

Employee had just finished cleaning C-10 chute and he started to climb out of chute, bringing his left leg against belt to pull himself out, when felt a pain, but pain went away and he did not report to employer until it started hurting again, 2 days later.

Employee was lifting a heavy equipment tire that was mounted on a dayton rim from the ground. The tire was lying flat on hard ground. Employee was lifting one side of tire to get it vertical to roll to the mobile shop when he felt a twinge in his groin area.

Employee received a call that work on C-7 was completed. Due to excessive noise during the call, he moved to the platform on south side of cooler to hear better and due to poor lighting, tripped over the dam sections in center of platform, falling on his right shoulder.

Employee had his annual HCI hearing test here at the Plant on 4/26/12. The results showed a STS had occurred and that the STS met the criteria for a recordable shift on Form 300. We sent him for a retest to our company doctor on 10/2/12. We did not know that this needed to be filled out on 7000-1. This was informed to us on 11/7/12 during a MSHA Plant Inspection.

EE was reaching into tool box when lid fell striking his right hand, lacerating his knuckle.

Employee on 3/13/12 approximately at 08:00 AM was preparing ls160 bobcat tire and rim for repair. He lifted the rim on to work table and picked up a 4 lb. sledge hammer to repair bent rim when he felt a sharp pain in his mid back area.

Employee was checking the water pump for refueling. When he came up on the pump he noticed the gas cap was off. As he was trying to put the cap back on the pump caught fire. Employee ran to the nearest fire extinguisher and as he was running back up a sloped area he felt something give in his right calf.

IE was working along with another EE to increase production of our precoat material. While catching several samples, IE picked up bucket of material with both hands and set it down above the scales. He then picked bucket back up and started pouring the rock and felt a twitch in his left shoulder.

EE was rigging a chain to make a lift. EE's left ring finger was pinched by the other of two chains on the crane.

Employee was using his personal pocket knife to open a tube of silicon caulking. He was cutting toward his thumb when the knife slipped and cut his right thumb.

EE was attempting to remove the shear bolt & nut from the hinged mouthpiece on the extruder. EE had to pull upwards on the nut to loosen & remove. While pulling upwards EE felt something pull in his lower back.

EE was using a 4 lb. shop hammer and a 1 3/4" combination wrench to loosen nuts on coal mill bowl.EE struck the wrench with the hammer and this caused the wrench to bounce up and strike EE on top of his left hand.

EE's were cleaning the raw shale buildup from C-34 tripper chute. While cleaning the chute, IE was using a pneumatic chipping gun and when IE tried to change his footing he slipped and hit his left calf muscle on the inside of the chute.

EE was helping co-worker smooth concrete out by dragging a water hose across concrete after it had been floated. He tripped over a piece of 1" flat bar, causing him to fall on his right knee.

EE removed troughing idler from frame of C-13 and carried it about 40 feet. When he lifted the idler over inside of handrail to throw to the ground, he felt a tearing pain in his right shoulder.

At approximately 3:30 PM on 8/6/10 EE & co-worker were assigned the task of cleaning of SC#1 belly pan. EE was working at removing buildup of raw shale from the upper section of the belly pan with a pneumatic chipping gun when a large piece of raw shale broke loose and slid down the sloped belly pan landing on the top of his right foot.

At approximately 9:00 PM on 4/14/2010 while working at the emergency hopper, employee was using a 12 ft. long, 1/2" diameter air lance to help wet raw shale from sticking to the hopper walls. Employee was stretched out pulling on the lance when he felt a sharp pain in his lower back.

At approximately at 8:00 AM on 2/5/2010, Injury was pulling auger sections out of the extruder along with #1 EE, #2 EE and #3 EE. Injury was taking the augers and tossing them on the ground. He had tossed 3 augers and switched places with #2 EE. He started feeling discomfort after 10 minutes of switching with #2 EE.

Employee was in the quarry pit area setting off a shot and there was flying rock from the shot. EE was running to get away from flying rock when he slipped and fell due to the ground being wet from rainy conditions. There is usually never any flying rock from shots, but the wet conditions hindered the shot. EE suffered a strain in his left upper leg.

While dismounting skid steer loader, Injured caught his right hand in between the skid bar and the roll over cab. He sustained a sprain/strain to the right hand. He was working on the finish end inside of C-21 tunnel.

EE was air-arc gouging on frame damage to a scraper. The area was very small and at or above head level causing his head to be surrounded by billowing air, fumes dust and grit. EE felt that he had gotten metal in his eyelashes and then in to his left eye. The first real discomfort he felt was on Saturday, 5/2/09.

Injury was cleaning Screen #6 on a very windy afternoon. Material blew in his right eye. His eye began to bother him on his way home. He reported this the next morning on 4/7/09 to his supervisor. He was sent to company doctor and 1 stuck piece of material and 1 loose piece of material were removed.

Injury was at the coal unloader placing the vibrator on the railcar. He was using a ratchet and pipe to tighten up bolts, while performing this job, he complained that his right shoulder was hurting and could feel a burning sensation in it.

While dismounting Haul Unit #157 from the left side door, the wind closed the door on injured's right hand before he could remove his hand out of the way. The wind was gusting about 15-20 MPH from the north. When he caught his hand in the door it caused partial amputation of his right pinkie finger.(Per call-no bone that means no amputation)

EE was walking out of the loadout building when he stepped on a grain scoop with the front part of his right foot. Stepping on the scoop bent his toes and his foot had some pain. EE reported this incident to his supervisor the next morning, but thought that his foot would get better. EE came to supervisor on 7/17/08 and said he needed to have his foot checked by a physician.

Employee was loosening motor mounting bolts on Triple Roll Crusher using a socket and 3/4" ratchet. He exerted force and didn't remember any type of slipping or jerking, but began to feel a pressure in his back.

EE was helping Maintenance work on C-34 conveyor belting when his left hand was pulled in to pulling block between 3/8" cable and sheave. The cable tightened causing his left hand and fingers to be pulled in to block, lacerating and breaking his middle finger and parital amputation to his pointer finger. (first joint)

EE was working 12 hours on the loader & operator fatigue could have caused him to not have proper posture while running the loader. Cause of injury is unclassified because of insufficient data, but he said his right shoulder was hurting him. EE reported to supervisor and decided on 7/14/08 that he needed to go to doctor to see what was wrong with his shoulder.

Employee was working with several others in the loadout front silo. He began to experience discomfort in his right eye. After some days with no improvement, he went to the eye doctor who found an imbedded fragment in his right eye.

EE was welding a patch above the jaw crusher and he drove a piece of 4" channel iron into the gap between the jaw Pittman and the hopper above the serve as a seat. When repositioning his body, the channel slipped out allowing him to fall into the jaw crusher, catching his fall with his left hand, wrenching his shoulder.

Employee was working the the Plant mud pumps. He went to shut off agitator & lowered it to stand on while accessing the pump pipe and checking valve. While manipulating valve cable clamp, pipe fell pulling him off balance, causing him to fall.

Employee was working in the maintenance shop on the 2" pipe assembly, which was in a vice. He was using a 24" pipe wrench and the wrench slipped striking his left knee, causing his knee to swell with extensive pain and bruising.

Employee was trying to unstop dust tank and was beating on the tank with a piece of 2" pipe instead of a sledge hammer. He beat on the tank for a total of 4 hours to keep it feeding. Kenny came in on 2/8/07 around 1:30 P.M. for his next scheduled work time and reported to his supervisor that he hurt his shoulder.

EE was cleaning C-9 chute that feeds raw material that drops in to the Kiln. A big chunk hit his right thumb, causing his thumb to be smashed by the material.

EE told his supervisor on Friday morning that he got something in his eye on Wednesday. It had bothered him on Thursday and by Friday morning it seemed to be getting worse. He couldn't recollect any specific moment when he knew something got in his left eye, just that it began to bother him.

Employee was installing a transmission. Laying on his back & pushing against the transmission with his feet, he felt a pain through his leg to his toes. He reported his injury on 6/23/06 but declined medical treatment. EE aggravated his back last week, 10/5/06, while working in a cramped & awkward position causing his pain to flare up & on 10/9/06 he asked to seek medical help.

Employee had just reported to the railcars to unload coal. He climbed down from his hauler and had both feet on the ground when he began to turn as he released his handhold on the hauler. His left foot slipped into a washout in the ground. He fell to the ground feeling a pain in his left leg.

EE was standing to the south side of C-1 counterweight discussing maintenance repairs with Production Supv. when he turned around pointing out something and a 3 lb. shale chunk fell from C-1 conveyor hitting him on top of his hard hat, knocking him unconcious for several minutes. The impact knocked his hard hat off and knocked him down to the ground.

EE was working inside the Kiln Feed Tank removng build-up. He was trying to reposition himself to begin removing build-up in another area & when trying to reposition himself the material slid downward allowing his left leg to slide with material while his right leg remained. This cause his right foot to fold sideways causing the sprain to his ankle.

While inspecting roll shells on RC#1 and went to close the expanded metal guard that he had opened. His right pinkie finger got caught under the guard when it closed.

Employee had installed the engine and hydraulic pump assembly in the 463 Bobcat and was lifting the assembly with a pry bar to slip the engine mounts into place. His fingers were under the mount when the pry bar slipped, dropping the assembly on his finger, amputating the last joint on his left index finger.

EE had just completed cleaning C-35 chute and had gone inside Substation #1 to remove his lock-out devices from C-35 breaker. EE stated while reaching up and removing his lock-out devices he felt a sharp pain in his left hip and as the night progressed, the pain intensified and moved down his leg.

EE was cutting a section of the dump hopper out when the plate was cut loose. It flipped over & came out of another opening in the hopper & struck EE on the left index finger, which was secured on the handrail of snorkel lift.

EE was loading trucks at loadout when he pulled a sample out of a truck to test. After filling his bucket with a scoop, he twisted his body to pick the bucket up. As he did so, he felt a twinge in his lower right side. When he went to pick the bucket up he realized he had strained his back.

EE was on top of kiln removing shims from under pads on #3 tire. After old shims were removed, compressed air was used to clean rust, scale, slag, etc. from under pads before installing new shim. Wind was blowing and something got in EE eye. Minor irritation persisted until medical treatment was sought.

EE was removing bearing housing from screen and shaft. Housing was hanging from a come-along, secured to skirt board. EE was pulling and balancing housing. The housing shifted and EE tried to clear his hands, but was caught by the end of his little finger.

Employee was helping another change a flat tire on the maintainer. While rotating tire to get the valve stem in correct position, employee's fore finger on his right hand was smashed between the rim and a wheel stud.

EE WAS CLEANING BUILDUP INSIDE OF C-40 CHUTE WITH THE ENTRANCE DOOR RAISED UP INTO A VERTICAL POSITION. EE WAS GRIPPING A PRY BAR, APPROX. 4 FT. LONG WITH BOTH HANDS ON THE PRY BAR. THE SOUTH WIND BLEW THE DOOR SHUT FALLING AGAINST HIS LEFT HAND, MIDDLE FINGER. EE REPORTED THE ACCIDENT ON 9/13/04, BUT SAID HE WAS FINE. MONDAY, 10/11/04, EE REQUESTED MEDICAL EVAL.

Employee was descending the 500 Loader at the pugmill area of our finished end of the Plant & placing both hands on the ladder to descend the loader. When stepping on 2nd rung of the ladder, which had mud on it, lost his footage, slipping & fell to the ground, approximately 6 feet. Landing in an awkward postion, falling on his right ankle and knee.

Ee was shoveling on Monday morning, June 7th, 2004 around 9:00 A.M. at the pugmill area of our finished end area of the Plant. Ee was shoveling around concrete area when the shovel got caught under a holding clip at base of concrete wall. When he lifted up the shovel, the shovel did not release and ee got a sharp pain in his lower back.

A storm with high winds hit the Plant around 10:45 P.M. on June 1, 2004 and EE went to put the Kiln on the pony motor because of loss of power in the Plant. While walking on the catwalk a gust of wind caught EE and pushed him across the catwalk, bending his right hand back double by the handrail. EE heard a pop when this happened causing a strain to his right hand.

EMPLOYEE WAS CLEANING UP MATERIAL SPILLAGE AROUND THE COAL MILL AND SHOVELING THE SPILLAGE INTO A WHEELBARROW, THEN DUMPING THE WHEELBARROW INTO A BOBCAT BUCKET. HE FELT SOMETHING PULL IN HIS LEFT SIDE OF HIS BACK.

EE WAS WALKING BACK INTO OUR 12 FT DIAMETER KILN AND WAS APPROXIMATELY 100 FT INSIDE KILN WHEN HE SLIPPED ON SOME LOOSE CASTABLE AND FINE DEBRIS BUILTUP ON THE SIDES OF THE KILM. WHEN HE SLIPP ED HE STOPPED HIMSELF FROM FALLING WITH HIS RIGHT HAND.

EE WAS WORKING THE NIGHT SHIFT (20:00 TO 08:00) ARC-GOUGHING 4 INCH S.S. ANCHORS FROM INSIDE OUR 12 FT DIAMETER KILN WITH 3 CO-WORKERS. DURING THE PROCESS ALL WELDERS WERE ARC-GOUGING AT THE S AME TIME CREATING A FLASHBACK ON THE BACK SIDE OF THEIR WELDING HELMETS. HE RECEIVED FLASH BURN TO BOTH EYES.

EE WAS FABRICATING A MACHINE GUARD AND HAD DRILLED AND TAPPED HOLES FOR SEVERAL BOLTS TO ASSEMBLE THE GUARD. WHEN TAKING THE BOLTS OUT TO OPEN THE GUARD FOR FURTHER WORK, HE STRUCK HIS LEFT MI DDLE FINGER KNUCKLE ON A SHARP METAL EDGE, CUTTING HIS KNUCKLE AND REQURING 5 STITCHES INT HE KNUCKLE AREA.

CONTRACT HAULER LOADED TRUCK AND WAS LEAVING THE STREETMAN PLANT. THE TRUCK DRIVER PULLED TO THE RIGHT AND TRAILER GOT OFF IN GRASS. THE DRIVER TRIED TO PULL IT BACK ON THE ROAD AND THE TRUCK ROLLED OVER. HE TRUCK DRIVER RECEIVED A LACERATION TO HIS LEG.

EE WAS LEAVING LOAD OUT TO RELIEVE ANOTHER AS DISPATCHER, WHEN CLIMBING TWO STEPS, HIS TOE ON HIS RIGHT BOOT CAUGHT UNDER TOP STEP CAUSING HIM TO TRIP. WHEN HE STARTED TO FALL, HE CAUGHT THE H ANDRAIL AND IT SPUN HIM AROUND CAUSING HIS RIGHTLEG TO HIT THE ANGLE IRON ON THE HANDRAIL.

WHILE WORKING AT THE RIM GRINDER, THE WIND GOT UP AND BLEW MATERIAL IN HIS LEFT EYE.

EE WAS OVERHAULING THE 990 LOADER ENGINE. EE WORKED A 10 HR. DAY AND LATER THAT NIGHT HIS RIGHT WRIST BEGAN TO ACHE. HE HAS CONTINUED TO WORK IN THE HOPE THAT HIS WRIST WOULD IMPROVE, BUT IT C ONTINUES TO BOTHER HIM.

EE WAS HELPING TO REPLACE A BEARING ON THE JAW CRUSHER. HIS RIGHT FOOT SLIPPED AND HE CAUGHT HIS RIGHT HAND FINGERS ALONG WITH A SCREW DRIVER BETWEEN MOTOR SHEAVE AND DRIVE BELTS CAUSING LACERATIONS AND BRUISES TO MIDDLE FINGER AND RING FINGER. THE MIDDLE FINGER WAS CUT BADLY AT THE END OF HIS FINGER.

EE WAS CLEANING C-34 TRIPPER CHUTE @ APPROX 1:30 A.M. HE HAD BOTH FEET LOCATED AT THE BOTTOM OF THE CHUTE OPENING WITH HIS TOES PROTRUDING INSIDE THE CHUTE & EXPOSED TO FALLING MATERIAL. HE HA D BEGUN CHIPPING ON THE BUILDUP IN THE CHUTE WHEN A LARGE CHUNK FELL ACROSS HIS RIGHT FOOT.

EE COMPLAINED OF BACK PAIN WHEN OPERATING 980G LOADER ON WED., MAY1, 2002. COMPANY DOCTOR DIAGNOSED EE'S CONDITION AS ACUTE INFLAMMATION OF LUMBAR DISC. EE DID NO KNOW WHEN IT REALLY HAPPENED; THEREFORE, WE DO NOT HAVE A DATE OR TIME.

EE WAS ASSISTING MAINTENANCE REMOVING COVER (MANHOLE) ON SCRUBBER #1. EE PULLED OUT ON THE BOTTOM OF THE DOOR WHILE MAINTENANCE LOWERED IT WITH A CABLE COME-ALONG. THE COME-ALONG CAME UNHOOKED FROM THE CABLE, PINCHING HIS RT. INDEX FINGER INTHE DOOR.

EE WAS LIFTING AIR FILTERS FROM A ROCAK AND FELT A POP AND PULL WITH A BURNING SENSATION IN HIS UPPER AND LOWER RIGHT ARM.

EE WAS ATTACHING BLADE TO BOBCAT WHILE HE WAS INSIDE C-21. WHEN EE PICKED UP ARM OF BLADE A SMALL 10-12 INCH IN LENGTH, COPPERHEAD SNAKE STRUCK HIM ON HIS RIGHT HAND BY WRIST JUST ABOVE HIS GL OVE.

EMPLOEE WAS SAMPLING MATERIAL FROM BEHIND COOLER. EMPLOYEE'S EYE BECAME IRRITATED. EMPLOYEE WASHED EYE WITH SOLUTION. THE CONDITION DID NOT IMPROVE. HAD TO GO TO DOCTOR TO GET DUST REMOVED.

TWO EE'S WERE WELDING INSIDE THE KILN FEED TANK DURING OUR SCHEDULED SHUTDOWN. MATERIAL STARTED FALLING FROM THE CHUTE STRIKING BOTH EE'S. ONE EXITED THE TANK ON THE SOUTHSIDE, THEN REACHED IN TO PULL INJURED FROM THE TANK. HE WAS STRUCK ON THE LEGS AS HE WAS HALFWAY OUT.

EE WAS JACK HAMMERING OUT 6" CASTABLE LINING WITHIN THE KILN, USING A 60 LB. JACKHAMMER. A PIECE OF CASTABLE BROKE LOOSE CAUSING EE TO LOSE HIS FOOTING AND FALL FORWARD, CATCHING HIS FINGER BE TWEEN THE JACK HAMMER HANDLE AND CASTIRON LIFTER

WHILE WORKING ON A ROLL CRUSHER, HE NEEDED TO REMOVE ROLL CRUSHER TIRE FROM HUB SO HE COULD WELD ON IT, WHILE PULLING TIRE OFF BY HAND, THE TIRE CAME LOOSE FROM HUB, HITTING THE FLOOR IN MAINT ENANCE SHOP, THE TIRE BOUNCED BACK AND CAUGHT HIS LEFT MIDDLE FINGER BETWEEN HUB AND TIRE.

MAINTENANCE DEPT. HAD JUST COMPLETED INSTALLING A 6" CAST IRON GATE VALVE INTO THE 6" STEEL OVERHEAD WATERLINE THAT IS USED TO FILL THE WATER TRUCK. THE WATER TURCK WAS BACKED UNDER THE SPOUT TO BE FILLED. AFTER FILLING EE CLOSED THE 6" VALVE AND COWORKER STARTED TO DRIVE OFF WHEN EE NOTICED THAT METAL SCREEN COVERING MANHOLE HAD HUNG ON FILLER SPOUT CAUSING THE 6" DOWN SPOUT TO BR

EE WAS PUTTING A DISCHARGE HOSE ON A WATER PUMP.THE HOSE WAS HELD UP BY A LOADER BUCKET. EE WAS TRYING TO PUSH THE END OF THE HOSE INTO PLACE TOBOLT TO THE WATER PUMP. THE HOSE SLIPPED OFF THE LOADER BUCKET AND CO-WORKER CAUGHT IT & HELD IT UP CAUSING THE HOSE TO PUSH AGAINST THE BACK OF EMPLOYEE'S NECK.

EMPLOYEE WAS INSTALLING LINERS IN THE COAL MILL FAN HOUSING. THESE LINERS ARE HEAVY AND BEING SET IN PLACE WITH OVERHEAD CRANE. EMPLOYEE PICKED UP ON ONE END AND INJURED HIS BACK.

EE WAS JACK HAMMERING INSIDE THE KILN. HE WAS REMOVING A 6" CASTABLE LINING INSIDE A 12' X 250' KILN WHEN A SLAB OF CASABLE FELL FROMT THE ROOF OF THE KILN, HITTING THE EDGE OF THE SCAFFOLDING FALLING OVER ONTO EE HITTING HIS LEFT SIDE AND ARM.

EE WAS TRAINING CONVEYOR BELT AT THE PUGMILL, HITTING TROUGHING ROLLERS WITH METAL HAMMER. A PIECE OF METAL FELW BEHIND SAFETY GLASSES AND STUCK IN HIS RIGHT EYE.

EE HAD BEEN SHOVELING RAW MATERIAL SPILLAGE IN C-35 TUNNEL. HE BENT DOWN TO LIFT THE SHOVEL UP WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.

EE OBSERVED A BROKEN SENSOR AND REACHED DOWN TO INSPECT THE DAMAGE. THE BOTTOM GATE VALVE ON DUST COLLECTOR #3 PEA TANK AUTOMATICALLY OPENED CATCHING HIS HAND BETWEEN AIR CYLINDER AND THE PROX IMITY SENSOR.

EE WAS DISMOUNTING 988-B WHEEL LOADER WHILE HE HAD BOTH HANDS ON THE LADDER HANDRAILS. WHILE STEPPING DOWN ONTO THE 2ND RUNG HIS FOOT SLIPPED AND HE STRUCK HIS CHEST ON THE UPPER PLATFORM DURI NG THE FALL.

EE TURNED HIS ANKLE & FELL.

EE WAS WORKING AT BIG JOHN CRUSHER, PULLING ROCKS FROM ROLL CRUSHER WHILE THE ROLL CRUSHER WAS LOCKED-OUT. EE WAS LIFTING A HEAVY ROCK FORM CATWALK ONTO CONCRETE BY HIMSELF, WHEN HE FELT A S HARP PAIN IN HIS LOWER BACK.

EE WAS ATTEMPTING TO REMOVE COVER FROM COAL MILL FAN HOUSING FOR BLADE REPLACEMENT. EE WAS USING A 1 TON COME-A-LONG TO REMOVE COMVER. EE WAS USING HIS RIGHT ARM & BRACED ON CHAIR WITH LEFT AR M. THE DOOR CAME LOOSE WITH SUCH QUICKNESS THAT IT PULLED ON HIS LEFT SHOULDER, CAUSING THE STRAIN TO HIS SHOULDER.

EMPLOYEE WAS CHECKING ON THE KILN ON THE BURNER FLOOR AND THEN WALKED OVER TO CHECK THE COAL BIN AND THEN WALKED OVER TO THE C-11 TO CATCH A WEIGHT. HE THEN WENT BACK INSIDE THE CONTROL ROOM A ND NOTICED HIS UPPER PART OF HIS LEFT ARM WAS HURTING FROM WHERE SOMETHING HAD STUNG OR BITTEN HIM.

AGGREVATION OF A PRE-EXISTING GANGLION CYST. SEE ATTACHED DR.S LETTER PERTAINING TO EE'S WRIST.

EE WAS HELPING OTHER WELD UP THE CRACK IN THE KILN AND HE WAS TOO CLOSE TO THE RAY FROM THE WELDS. HE WAS NOT PAYING CLOSE ENOUGHT ATTENTION TO THE WELDING THAT WAS GOING ON INSIDE THE KILN AN D HE STATED THAT HE BURNED BOTH HIS EYES. EE WASGIVEN A PRESCRIPTION FOR HIS EYES FOR THE BURN IN HIS EYES.

EE WAS CLEANING CHUNKS OFF THE COAL FLOOR. EE THEN PROCEEDED TO MOVE A 55 GALLON BARREL WITH MUD CHUNKS USING A DRUM DOLLIE TO MOVE THE BARREL. EE PULLED BACK ON THE DOLLEY HARD AND THEN DIS COVERED THE MATERIAL CONTAINED IN THE BARREL WAS LIGHTER THAN HE THOUGHT. WHEN EE JERKED BACK WITH DOLLY, HE SAID HE FELT A SHARP PAIN IN HIS BACK AND LEGS.

EE WAS EXITING WORK AREA FROM TOP OF LOADOUT SILO WHEN HE DROPPED HAND WRENCH AND HE WENT TO PICK IT UP, WHEN RAISING UP HE STRUCK HIS HEAD ON A3" ANGLE IRON SUPPORT BEAM, DRIVING HIS NECK DOW NWARD INTO HIS SHOULDER AND STRAINED A NECK MUSCLE.

EE WAS GOING DOWN C-21 TUNNEL TO CHECK ROLLER ON C-32 WHEN HE STEPPED ON SOME AGGREGATE AND LOST HIS FOOTAGE AND SLIPPED. EE GRABBED CHAIN ON C-32 TO CATCH HIMSELF AND TO TRY & KEEP FROM FALLI NG, TWISTING HIS LOWER BACK.

EMP WAS HOISTING IN A KILN TRUNION WITH A TWO TON COME A LONG. WHILE PULLING ON THE COME A LONG IN AN AWKWARD POSITION, HE FELT A TWINGE IN HIS LOWER BACK.

EMP HAD GONE DOWN TO BOOST OFF THE 988 LOADER WITH THE 980 LOADER AFTER HOOKING UP THE BOOSTER CABLES. TRIED TO START THE 988B AND THE CABLES WERE NOT MAKING GOOD CONNECTION, SO EMP WAS MOVING CLAMPS AROUND TO GET BETTER CONNECTION, CREATINGA SPARK WHICH CAUSED THE BATTERY TO EXPLODE.

EE WAS USING A PUNCH AND HAMMER TO REMOVE A BEARING FROM A SHAFT WHILE WORKING IN THE MOBILE SHOP. THE BLOW FROM ON THE PUNCH CAUSED A PIECE OF METAL OFF OF THE PUNCH SHOOTING THE PICE OF MET AL INTO MY FOREARM AND CAUSING A SMALL LACERATION AND A RISE IN THE SKIN DUE TO THE METAL FRAGMENT.

EMPLOYEE WAS BENDING AND LIFTING A 4" PIPE WHILE WORKING ON A SLOPE AND STRAINED HIS BACK.

EE HAD OPENED THE DOOR ON C-10 HEADPULLEY TO CHECK HEADPULLEY SHAFT. THE WIND WAS BLOWING 20-30 MPH. THE WIND CAUGHT DOOR AND BLEW IT OVER, CATCHING EE RIGHT HAND AND THUMB BETWEEN DOOR AND CH UTE.

EE TRIPPED ON STAIRS GOING TO THE BURNER FLOOR AND MISSED THE TOP STEP, AND FELL INTO HANDRAIL HITTING HIS RIGHT RIB CAGE AREA.

EE WAS BENDING OVER WHILE WORKING AT SCRUBBER 1 TO PICK UP WELDING LEAD AND HE COULD NT BEND BACK UP STRAIGHT WITHOUT ALOT OF PAIN.

EE WAS RETURNING FROM THE DUMP HOPPER TO THE QUARRY WHEN HE HIT SLICK AREA OF THE HAUL ROAD WHERE THE WATER TRUCK HAD BEEN RUNNING TO CONTROL THE DUST. HE SLID SIDEWAYS IN THE HAUL ROAD HITTIN G A DRY AREA OF THE HAUL ROAD FLIPPING THE EMPTYTRUCK AND THROWING HIM OUT OF THE DRIVER SIDE DOOR.

EE WAS REPLACING EXPANDED METALS IN SCRUBBER TOWER. COWORKER WAS HELPING EE AND COWORKER'S HAND LET THE METAL SLIP CAUSING EE TO BE PULLED TO ONE SIDE. THIS CAUSED EE TO PULL MUSCLE IN HIS LOW ER RIGHT SIDE OF BACK.

INJURED WAS UNLOADING COAL FROM A RAILROAD CAR. HIS SKIN BECAME IRRITATED AND HE WENT HOME TO TAKE A SHOWER. HE DECIDED TO GO TO THE DR. AND THE DR. GAVE HIM AN ALLERGY SHOT.

EE WAS HELPING TO PUSH A WHEEL BARREL WITH A DAMM SECTION INSIDE OF IT. EE HAD JUST PASSED THE FIRST DAMM SECITON AND WAS MANUEVERING THROUGH THE FIRST LIFTER SECTIN. EE WA ON THE PUSH SIDE OF THE WHEEL BARREL WHEN THE DAMM SECTION FELL BACKWARDS AND PINCHED HIS RIGHT THUMB BETWEEN THE WHEEL BARREL AND DAMM.

EE COMPLAINED OF A SORE BACK WHILE CATCHING SAMPLES FROM TRUCK BEDS.

EMPLOYEE WAS SWEEPING SHOP FLOOR WHILE ANOTHER WAS GRINDING A PIECE OF METAL OBJECT, WHEN A FOREIGN OBJECT FROM THE GRINDER CAME OFF AND WENT IN EMPLOYEE'S RIGHT EYE.

HE WAS WELDING ON CHUTE. GATE FLOPPED AND CAUGHT HIS LEFT HAND AND PINNED HIS LITTLE FINGER AGAINST IRON AND BROKE IT.

HE DID NOT SECURELY WELD HEAVY BUCKET EDGE ON END OF BUCKET. WHEN RELEASING OVER HEAD CRANE AND STARTING TO WELD ON EDGE, IT FELL ON HIS BIG TOE.

INJURED ALLEGES LIGHTENING STRUCK NEAR HIS WORK AREA DURING AN ELECTRICLA STORM. AFTER RUNNING TO THE OFFICE EMPLOYEE STARTED COMPLAINING OF CHEST PAIN. HE WAS TRANSPORTED TO HOSPITAL WHERE HE WAS KEPT FOR OBSERVATION.

EMPLOYEE WENT TO DISMOUNT THE LOADER ON THE RIGHT H AND SIDE OF THE LOADER NOT KNOWING THE BOTTOM STEP WAS MISSING. EMPLOYEE SLIPPED AND PULLED A GROIN MUSCLE IN HIS LEG.

EE WAS STANDING NEAR GRINDING PLATFORM & FOREIGN MATTER WAS BLOWN INTO EE LEFT EYE A VERY TINY SPECK OF MATERIAL WAS REMOVED BY EYE DOC

CRAIG STRAINED MUSCLE IN BACK THROWING CHUNKS OUT OF SCREEN TOWER #2. CHUNKS SHOULD HAVE BEEN CUT INTO SMALLER PIECES.

INJURED WAS BEING TAIGHT HOW TO OPERATE 769 HAUL UNIT. HE WAS RIDINGOLN THE RIGHT SID EPASSENGER SEAT. THE RIGHT DOOR WAS TIED OPEN ON THE HUAL UNIT WITH A PIECE OF CORD TO ALLOW VENTILATION . HE AND ANOTHER EMPLOYEE WERE HAULING FINES TO THE BIG SHED. THE CORD BROKE CAUSING THE DOOR TO SLAM ON EE'S RIGHT THUMB.

EE ATEMPTED TO DISMOUNT A 973 TRACK LOADER TO TURN OFF A CONVEYOR BELT THAT WAS SPILLING. EE STATED THAT HE WAS IN A HURRY AND SLIPPED ON THE TRACK AS HE WAS REACHING FOR THE HAND HELD AT END OF THE TRACK. EE FELL CHIPPING HIS TOOTH.

INJURED PICKED UP A SECTION OF EXPANDED METAL.

EMPLOYEE WAS HELPING TO CARRY A SCREEN CLOTH APPROXIMATELY 48" X 68" WEIGHING APPROXIMATELY 50 LBS. WHILE LEAVING AN ENCLOSED BUILDING AND WALKING OUTSIDE A GUST OF WIND BLEW THE SCREEN CLOTH AND CAUSE HIM TO HIT HIS BACK AGAINST A HANDRAIL

EE WAS INSIDE BUILDING STANDING ON DRY CLEAN CONCRETE FLOOR AND ATTEMPTED TO LIFT ONE SIDE OF HEAVY GEAR TO LOOP CHAIN AROUND FOR LIFTING WITH OVERHEAD CRANE AND SUSTAINED A STRAINED MUSCLE IN UPPER LEFT BACK REGION.

EMPLOYEE WAS INSTRUCTED TO HELP REPAIR CARBON BLOCK SEAL RING LOCATED AT DISCHARGE END OF KILN, AT THIS LOCATION THERE ARE SEVERAL COOLING FANS BLOWING UNDERNEATH TRYING TO COOL DOWN HOT MA TERIAL BEING DISCHARGED FROM KILN; DUE TO THE CONDITION OF THE SEAL RING AND AIR BLOWING FROM THE COOLING FANS THERE WERE A LOT OF DEBRIS IN THE AIR GETTING INTO THE EMPLOYEE'S RIGHT EYE.
