EE was cleaning EE's windows and mirrors on the HM300-5 articulating truck, when EE started to come back down the steps, EE missed a step causing EE to fall landing on EE's back and shoulder.

EE dropped EE's flashlight on the ground, when EE bent over to pick it up EE lost EE's balance and fell face first on the gravel.

EE was riding on the loader with EE's legs hanging off the front of the loader to the backside of the mine to get a pickup truck. The loader driver went to make a left turn and when EE did the step on the front of the loader caught EE's left leg.

EE WAS TAKING DRILL STEEL APART OFF THE FLOOR DRILL, THE STEEL SWUNG AROUND STRIKING THEM ON THE TOP OF THE LEFT HAND.

EE was climbing down off the WA 500. Misjudged the steps and fell hitting the back of head on the ground.

The employee was removing the 4" dust suppression hose that was stopped up on HD-943 drill.

EE was weed eating the bank and there was loose rock in the area, EE was starting to walk up the bank and slipped on the loose rock, landing on their right knee.

Employee had lost the tooth off the pick and was trying to recover it from near the face. While picking up the scaler tooth a rock fell off and struck the employee in the right shoulder.

EE was helping carrying a screen to be changed out, and stepped into a 5" opening at the edge of the plywood and screen.

Miner was operating a ridged frame haul truck. EE was being loaded at the mucking pile and was jostled in the cab bumping head. Miner said ee had seat belt on at the time.

EE was helping unload the crane off the lowboy being delivered to the Quarry.

Employee was loading plates onto the roof bolter, when they slipped and fell onto their pinky finger.

Employee was operating the excavator, when it became hung up on a large rock, the rock broke loose causing the excavator to drop back on the ground jarring the employee.

EE placed EE's hand on the tail pulley to turn it in order to access the set screws. Other employees were helping, and one employee turned the pulley at the same time resulting in EE's left hand being cut.

EE was running the anfo hose on the powder truck loading a shot, when EE felt a pain and a pop in EE's right forearm.

EE was hammering on a piece of metal, trying to straighten it out. EE missed swinging at the metal and strained EE's right wrist. An incident report was filed on but no medical attention was administered. EE aggravated the wrist again on 3/12/18, and sought medical attention on 3/13/18.

Miner was welding on a piece of steel in the shop. EE removed gloves and walked around the welding table to reposition leads. Then went back to the steel to slide it around and burned hand on a hot weld.

EE was complaining of chest pains

EE was driving when he started feeling light headed and dizzy.

EE was raising manlift to get to feeder pan when he caught his right index finger between structure and basket. EE stated that manlift traveling after he let off of controls.

Roof fall no injuries

Roof fall over the weekend - no one on site.

EE was on drill to remove radiator when he slipped and fell approximately 6-7 feet EE broke both wrists

EE was cutting light gauge stainless steel with a grinder, EE held the material improperly grider kicked up and the blade hit the inside of his forearm.

Wrecking forms away from concrete, crow bar slipped and just had a light sprain to left hand wrist.

Employee was inspecting boom on a track hoe. While climbing down the ladder he lost his footing and fell 3-4 feet to ground. He landed on his back and left elbow. Fractured left elbow, contusions to left knee and lower back, Fractured tail bone.

Reaching to move one piece of sheet metal roofing material, a second piece slid into worker's safety glasses and cut his nose.

N/A Abandoned Area Not Maintained.

The roof bolter failed to install roof bolts properly along with bad ground conditions which contributed to this roof fall. This caused an area 30'X 40' X 12' deep to fall.The roof will have to be repaired or abandoned.

Ee was cleaning floor of screening plant, pushing dust through an open man door that had been opened. A CO-worker ask ee to hand him some screen panels. He walked toward co-worker with the screen panels. He forgot that the man door was open and he fell into the opening to catwalk about 6 ft below injuring his left knee.

EE was performing maintenance on equipment. EE used pocket knife to cut tie wrap. Knife slipped and punctured ee's left wrist. Laceration required four stitches.

In the course of performing his normal duties, employee thinks he twisted his left hip while ascending and descending the equipment.

Employee was climbing down truck ladder and fell hitting the ground on right side and back.

EMPLOYEE STATES THAT HE LOST CONTROL OF THE EMPTY CAT 773 DUMP TRUCK HE WAS DRIVING. THE TRUCK VEERED, RAN THROUGH THE PROTECTIVE BERM AND OFF A 70'-HIGH WALL. (EMPLOYEE: NEW HIRE IN TRAINING. TRUCK LANDED ON ITS TOP. RESULTED IN PASSENGER FATALITY - BLUNT FORCE TRAUMA INSIDE CAB OF TRUCK; NEITHER WAS THROWN FROM CAB.)

EE WAS SCALING WHEN EXCAVATOR SLIPPED OFF ROCK, JARRING EXCAVATOR AND THROWING EE ONTO THE FLOOR, INJURING HIS BACK.

EE WAS HAND SCALING WHEN HE FELT PAIN IN HIS RT. SHOULDER.

EE STEPPED ROM TRUCK ONTO CAT WALK & SLIPPED ON LOOSE ROCK. HE STRUCK HIS HEAD ON A SUPPORT POSTEE RECD BRUISES ON HIS HEAD

EE WAS USING A SAW WHEN HE DEVELOPED A PAIN IN HIS NECK.NECK LATER BECAME SORE & STIFF

EE WAS HELPING ANOTHER EE TO LIFT A SET OF STEELCONVEYOR LEGS. EE STRAINED BACK & GROIN ARA

EE WAS STEAM CLEANING PARTS, WHEN HE TRIPPED ONTHE STEAM JENNY HOSE. EE RECEIVED A FRACTURE TO HIS LEFT ANKLE.

EE WAS CLIMBING ONTO RUNNING CONVEYOR TO DISLODGE ROCK WHEN HIS FOOT SLIPPED AND BECAME CAUGHT IN CONVEYOR. EE RECD CONTUSIONS TO HIS RIGHT FOOT.

EMPLOYEE WAS REMOVING AN OLD SHAFT FROM A SPROCKET WHEN A CHIP OF STEEL FROM A PUNCH STRUCK HIS FINGER AND BROKE IT.

EE WAS SHOVELING ROCK FROM CRUSHER WHEN HE HURT HIS BACK.

EMPLOYE EWAS REPLACING COIL SPRINGS ON PRIMARY FEEDER. HE WAS COMPRESSING AND BANDING COIL SPRINGS WHEN HIS HAND SLIPPED RELEASING SPRINGS STRIKING EMPLOYEE IN FACE.

EMPLOYEE WAS UNSCREWING DRILL STEELS AND IT FELL ON FINGER.

EE WAS WORKING ON TOP OF ENGINE COMPARTMENT OF AN EUCLID WHEN HE TRIPPED AND FELL OFF TRUCK.

EMPLOYEE WAS HELPING MECHANIC LIFT PLANTARY ASSEMBLY ONTO TRUCK WHEN HE PULLED MUSCLE IN LEFT SIDE.

EMPLOYEE WAS MOVING WIRES IN AN ELECTRICAL PANEL WHILE IT WAS ENERGIZED WHEN ONE WIRE WENT TO GROUND SHORTING PANEL BOX OUT. EMPLOYEE WAS THROWN OR FELL FROM STEP LADDER HE WAS STANDING ON.

EMPLOYEE STATES IT WAS FOGGY IN MINE AND HE DID NOT SEE THE PILLAR THAT HE HIT WITH THE VECHICLE HE WAS DRIVING.

EMPLOYEE STATES HE MIGHT HAVE FELT A CRAMP OR STING WHEN HE GOT DOWN FROM HIS TRUCK TO GO HOME ON MARCH 25,1991.HE HAD SEVERE BACK PAIN WHILE DRIVING HOME FROM WORK.HIS BACK HURT ALL NIGHT.
