Miner was injured while using a scaling bar to remove loose rock while loading a header. The bar glanced off the rock and hit EE on EE's right thumb. EE was taken to be evaluated and was diagnosed with a contusion and placed on restricted duty. Restrictions do not prevent EE from driving a truck, which EE has been doing without incident.
EE lost EE's balance while loading a floor shot and hurt EE's knee. EE was taken to the Occupational clinic to be evaluated and was diagnosed with a left knee sprain. EE was placed on restricted duty until EE's follow up appointment on 9/5/25.
The miner was adjusting a conveyor belt. Miner was injured when the pipe wrench miner was using slipped & mashed finger. Miner was wearing ppe. Miner was taken for a medical exam & diagnosed w/ a contusion but only received 1st aid treatment. Miner was restricted from lifting weight with hand. On follow-up appt. miner was told to only work in a splint & would be reevaluated.
The miner was injured prior to loading explosives at a header. EE entered the basket on the powder truck and was manipulating the ANFO hose when the hose "whipped" and struck the face, causing small pieces of rock to break off. One piece struck EE in the right palm causing a laceration.

EE was cleaning EE's windows and mirrors on the HM300-5 articulating truck, when EE started to come back down the steps, EE missed a step causing EE to fall landing on EE's back and shoulder.

EE dropped EE's flashlight on the ground, when EE bent over to pick it up EE lost EE's balance and fell face first on the gravel.
EE was riding on the loader with EE's legs hanging off the front of the loader to the backside of the mine to get a pickup truck. The loader driver went to make a left turn and when EE did the step on the front of the loader caught EE's left leg.
EE WAS TAKING DRILL STEEL APART OFF THE FLOOR DRILL, THE STEEL SWUNG AROUND STRIKING THEM ON THE TOP OF THE LEFT HAND.
EE was climbing down off the WA 500. Misjudged the steps and fell hitting the back of head on the ground.

EE was weed eating the bank and there was loose rock in the area, EE was starting to walk up the bank and slipped on the loose rock, landing on their right knee.

Miner was operating a ridged frame haul truck. EE was being loaded at the mucking pile and was jostled in the cab bumping head. Miner said ee had seat belt on at the time.
EE was helping unload the crane off the lowboy being delivered to the Quarry.
Employee was loading plates onto the roof bolter, when they slipped and fell onto their pinky finger.
Employee was operating the excavator, when it became hung up on a large rock, the rock broke loose causing the excavator to drop back on the ground jarring the employee.
EE placed EE's hand on the tail pulley to turn it in order to access the set screws. Other employees were helping, and one employee turned the pulley at the same time resulting in EE's left hand being cut.
EE was running the anfo hose on the powder truck loading a shot, when EE felt a pain and a pop in EE's right forearm.
EE was hammering on a piece of metal, trying to straighten it out. EE missed swinging at the metal and strained EE's right wrist. An incident report was filed on but no medical attention was administered. EE aggravated the wrist again on 3/12/18, and sought medical attention on 3/13/18.

Employee was inspecting boom on a track hoe. While climbing down the ladder he lost his footing and fell 3-4 feet to ground. He landed on his back and left elbow. Fractured left elbow, contusions to left knee and lower back, Fractured tail bone.
Reaching to move one piece of sheet metal roofing material, a second piece slid into worker's safety glasses and cut his nose.

The roof bolter failed to install roof bolts properly along with bad ground conditions which contributed to this roof fall. This caused an area 30'X 40' X 12' deep to fall.The roof will have to be repaired or abandoned.
Ee was cleaning floor of screening plant, pushing dust through an open man door that had been opened. A CO-worker ask ee to hand him some screen panels. He walked toward co-worker with the screen panels. He forgot that the man door was open and he fell into the opening to catwalk about 6 ft below injuring his left knee.
