EE was coming down the plant from cleaning the walkways. When EE got behind the old #1 screen EE slipped on ice. EE went to catch self and tore muscles in shoulder.

Employee was using haul truck to load barges. As employee was being loaded the truck in staging area began to back up and the rear right corner of the bed hit the cab of the employee being loaded truck door. The employee was able to maneuver through the cab & out the passenger side door. Lacerations & abrasions to the groin area were a result. Accident under investigation.

EE was in the adjustment area on the jaw crusher trying to slide the block behind the toggle plate cover to center it up. The cylinder on the porta-power was at an angle and when pressure was applied, it kicked out and hit EE in the face and left arm. This resulted in 3 stitches between the nose and mouth.

EE was running the water truck. When EE exited the water truck EE stepped on uneven ground and rolled right ankle. This resulted in EE seeking medical attention and was given restricted duty sitting work activity only.

EE was running the WA/700 in the pit when a piece that someone had added to the loader fell and hit EE's arm. The piece held the pre-shift tickets for the equipment. EE did not seek medical attention until 12/04/2017. EE received medical treatment at this time. Fluid was drained from the left elbow and returned to work at full duty.

EE was loosening the compressor fill cap of a Sandvick T35 with an adjustable wrench. When the cap let loose, the wrench pinned EE's middle finger against the site glass busting the finger. EE went to the doctor and received six stitches.

EE was checking the oil in a Komatsu WA600 when right hand struck the engine compartment door. 24-48 hours later EE was diagnosed with cellulitis of the hand. EE was prescribed Rx to treat the cellulitis.

EE came to the office and complained of eye irritation at 3:15 PM, and stated at some point in time he got something in his eye. He drives a truck stripping dirt with cab closed. Flushed, and went to Dr who prescribed antibiotic drops.

Employee was improperly dismounting his haul truck and bruised his back.

EE was climbing out of the skid steer when he missed the step and he twisted his leg when he fell.

Employee has experienced a MSHA reportable 25 decibel shift in hearing.

EE was installing a troughing roller and he smashed his fifth finger on right hand between frame of conveyor and roller. He had assistance available, but tried to perform job without assistance, resulting in a smashed finger. He is on light duty.

EE released the locking mechanism on the tag line for the cable crane bucket, and it uncoiled and it violently struck him in the left shoulder / shoulder blade a couple of revolutions. He was wearing all his equipment and a life jacket took a majority of the energy. He was sent to doctor for examination and treatment for some bruises/abrasions.

Victim was climbing out of a barge using a ladder. When he got to the barge landing he slipped and fell on his knees resulting in bruised knees.

Lifting wooden blocks to block up a truck that needed repair, felt a pop in his back and a pull in groin area resulting in a back strain and hernia.

Walking through surge tunnel preparing to change rollers on a conveyor, he stepped on an uneven spot in the floor causing a sprain to his right ankle.

Climbing out of a hopper barge using a rope to pull himself out. The rope broke and he fell back down into the barge causing a sprained ankle.

Using a cleanup loader to scrape the bottom of a coal barge. The loader bucket hit a burr on the barge floor causing the loader to stop. He hit the dash with his knee and leg resulting in a cracked bone in his leg. Did not go to the doctor until 8-13/07.

Replacing screen bearings, lifting and turning in an awkward position and strained his back.

EE PICKED UP AN EMPTY SPOOL FOR STORAGE OF CABLE TO THROW IT AWAY. A STAPLE IN THE SPOOL PUNCTURED EE'S LEG AND NEEDED TO BE REMOVED BY A DOCTOR.

Standing on the 2nd step of a steploadder, lost his balance and fell against a handrail causing a laceration to his upper lip requiring sutures.

CLEANING THE WINDSHIELD ON THE LOADER, SLIPPED AND FELL BACKWARDS LANDING ON WRIST CAUSING A STRAINED WRIST

Driving a pit truck back to the pit area. victim stated the steering did not work and he ran off the travel road to an uneven area causing his leg to strike shifting lever resulting in a laceration to his leg requiring sutures.

EMPLOYEE WAS STRUCK IN THE BACK BY A PIECE OF MATERIAL CAUSING A BRUISED MUSCLE IN HIS BACK.

INSTALLING AN OUTRIGGER CYLINDER ON A BACKHOE, WHILE LIFTING THE CYLINDER IT SLIPPED AND FELL ON HIS FINGER CAUSING A MASHED FINGER REQUIRING STITCHES.

VICTIM WAS STEPPING FROM HIS TRUCK TO A PLATFORM HE TRIPPED ON THE WALKWAY ON THE TRUCK CAUSING HIM TO FALL. HE STRUCK HIS HEAD ON THE PLATFORM CAUSING A LACERATION REQUIRING SUTURES TO HIS FO REHEAD.

PULLING WELDING LINES INTO THE SHOP AND STRAINED HIS BACK.

PULLING ON CABLES TO THE OFF A BARGE ON THE RIVER AND STRAINED HIS BACK. DID NOT REPORT THE INCIDENT UNTIL 3-12-02.

WALKING ON A CATWALK STEPPED ON AN UNEVEN PLACE IN THE CATWALK AND TWISTED HIS ANKLE RESULTING IN A SPRAINED ANKLE.

VICTIM WAS DRIVING A 6X6 VOLVO TRUCK HAULING DIRT FROM A STRIPPING AREA. HE DROVE OVER AN UNEVEN AREA IN THE ROAD CUAISNG THE TRUCK TO BOUNCE AND ROCK. THE BOUNCING ACTION JARRED HIS BACK CAUS ING A BACK STRAIN. SUPERVISOR NOTIFIED

VICTIM WAS OPERATING THE DOZER WHEN THE SEAT BROKE, CAUSING HIM TO FALL BACKWARDS, HITTING HIS HEAD ON THE BACK GLASS, RESULTING IN A STRAIN TO HIS NECK.

VICTIM WAS CLIMBING STEPS WHEN HIS CLOTHING GOT CAUGHT ON ONE OF THE STEPS CAUSING HIM TO TRIP AND FALL STRIKING HIS KNEE ON ANOTHER STEP RESULTING IN A BRUISED KNEE.

INSTALLING A ROLLER ON A CONVEYOR. THE ROLLER SLIPPED AND CAUGHT HIS FINGER BETWEEN THE ROLLER & CONVEYOR FRAME CAUSING A LACERATION TO A FINGER ON HIS RIGHT HAND REQUIRING SUTURES.

VICTIM WAS DRIVING THE TRUCK HAULING A LOAD OF RIP RAP & HIT A BUMP IN THE ROAD. THE TRUCK BOUNCED & CAUSED HER LEFT ELBOW TO HIT THE DOOR HANDLE RESULTING IN A BRUISED ELBOW.

VICTIM WAS GREASING A GREASE FITTING WHEN THE GREASE NOZZLE CAME OFF THE GREASE FITTING CAUSING THE GREASE TO SPLASH IN HIS EYES REQUIRING HIM TO GO TO THE DOCTOR TO HAVE THE GREASE REMOVED FR OM HIS EYES.

VICTIM WAS USING A RATCHET TO LOOSEN A BOLT WHENTHE RATCHET SLIPPED AND HE MASHED HIS FINGER BETWEEN THE RATCHET AND A SCREEN CAUSING A LACERATION TO HIS FINGER.

VICTIM WAS OPERATING A CLEAN-UP TRACTOR INSIDE A BARGE WHEN HE WAS STRUCK BY A CLAMSHELL BUCKET CAUSING CRACKED RIBS AND A BROKEN COLLAR BONE.

VICTIM WAS REMOVING A VERTICAL SHAFT FROM A CRANE USING A PORT A POWER. HE DROPPED THE PORT A POWER ON HIS LEFT INDEX FINGER CAUSING A FRACTURE TO HIS FINGER.

VICTIM GRABBED A PIECE OF HOT METAL, CAUSING BURNS TO HIS FINGERS AND PALM OF HIS LEFT HAND.

VICTIM WAS CLIMBING OUTF A BOBCAT LOADER AND SLIPPED AND SCRAPED HIS LEG ON THE LOADER BUCKET CAUSING A BRUISED AND SCRAPED LEFT SHIN.

VICTIM WAS CLIMBING ON A SCREEN WHEN HIS FOOT SLIPPED AND HE FELL ON HIS RIGHT WRIST CAUSING A SPRAIN TO HIS RIGHT WRIST.

VICTIM WAS DRIVING A WEDGE FOR A SCREEN WHEN THE WEDGE POPPED OUT AND STRUCK HIM ON THE UPPER LIP CAUSING A LACERATION REQUIRING SUTURES.

EE WAS CLOSING CROSS BAR ON BOBCAT WHEN HE PINCHED HIS LEFT THUMB BETWEEN CROSS BAR & FRAME CAUSING A MASHED THUMB

VICTIM WAS WALKING OVER A PILE OF STONE WHEN HE TRIPPED CAUSING A SPRAINED ANKLE.

VICTIM HAD HOOKED UP CABLES TO A BOBCATE LOADER FOR HOISTING. WHEN THE BOBCAT WAS HOISTED IT SWUNG INTO THE VICTIM PINCHING HIM BETWEEN THE BOBCAT AND A COLUMN CAUSING A STRAINED BACK.
