EE was working on a CAT 990H attempting to remove the loader arms. In the process of removing the pin between the Zbar and hydraulic cylinder, the Zbar pivoted momentarily pinching EE's left upper thigh between the Zbar and the cylinder.

Employee was in the process of changing skirt rubber on a conveyor when EE received a laceration to EE's forearm. Employee received 5 sutures and was released to full duty

Employee was opening a metal gate on a chute. The top hinge bolt was missing and as the door opened, it fell forward and pinned EE's finger between the door and the handrail. The employee sustained an injury to EE's right index finger requiring stitches.

EE scraped his right shin while entering cab of drill when his right foot slipped on step. This happened approx.2 months ago but employee didn't seek medical attention until 12/8/11. He was treated by a doctor on 12/8 for an infection. His wound was cleaned and dressed. No restrictions or lost time. Incident is being reported within 10 days of seeking medical attention.
Heat Stroke
Employee was working outside, disassembling an H4000 crusher. While carrying 5 gallon buckets of oil, he became overheated. He was taken to the doctor and was diagnosed as having heat exhaustion.
