THE INJURED WAS OPERATING A CHAINSAW CLEARING A DOWNED TREE THAT WAS LAYING AGAINST A SLOPE. WHEN THE EMPLOYEE CUT THE TREE AT ITS BASE, THE TOP OF THE TREE EXTENDING BEYOND THE CREST OF SLOPE FELL TO THE GROUND ALLOWING THE BASE OF THE TREE TO SWING UPWARD, STRIKING THE EMPLOYEE IN THE RIGHT SIDE OF THE FACE. THERE WERE NO IMMEDIATE WITNESSES. CAUSE OF ACCIDENT IS SPECULATIVE.

Employee pushed a large piece of clay off the conveyor and felt a pain in back.
Employee was tightening a jam nut on the crusher when the wrench slipped causing EE to fall backwards striking back on the steps behind EE.

Employee was picking clay chunks off the picker belt when he felt a sharp pain in his back and side.
The injured employee was walking to the plant at the beginning of the shift. A second employee pulled up beside him in his personal vehicle to offer the victim a ride when he ran over his left foot. Injury was a fracture of the left foot.

A large flat stone became lodged at the transfer point between the scalping screen chute & picker belt. EE attempted to dislodge the rock from the walkway beside picker but was unable to remove it. He moved around to the end of picker belt & used a spud bar to try to remove the rock. He jerked on it. The bar slipped, he lost his balance & fell to the concrete floor below.

SEVERAL EE WERE CHANGING SCREENS. THEY PLACED A NEW SCREEN ON TOP A HANDRAIL IN ORDER TO PUSH SCREEN OVER THE SIDE. NO ONE WAS OBSERVED IN THE AREA. HOWEVER THE INJURED PARTY CAME DOWN THE STE PS UNDER THE SCREEN & WAS STRUCK ON THE FOOT. PIECES OF BELTING LYING ON THE PLATFORM HAD OBSTRUCTED HIS VIEW OF THE AREA.

EMPLOYEE WAS WORKING ON PICKER CONVEYOR AT GRAVEL PLANT. A ROCK CAME DOWN THE CONVEYOR THAT NEEDED PUSHED OFF. HE TRIED TO PUSH IT OFF BUT COULD NOT MOVE IT AND IT PUSHED HIS RIGHT ARM INTO A STEEL PLATE. X-RAYS OF ARM AND WRIST SHOWED NO ABNORMALITIES.
