Employee was walking from the shop office back to the garage. There is a small depression where the concrete slabs meet, and the employee tripped and rolled their right ankle. Employee was immediately taken to ER and will be out of work due to restrictions.
While shoveling pit on VS2, employee felt a pain in EE's right elbow.
Lumbar spine discomfort.
While miner was working, miner tripped and fell on shoulder (left) and felt a pull Miner asked to go to Doctors on 9/4/2025 where EE was put on restricted duty and prescribed a sling.
While working on 1-1/2 # 2 screen. Employee slipped inside chute while unbolting old skirt. Employee used right arm to brace self causing injury to upper right arm/shoulder area. Employee did hit floor. No subsequent injuries due to hitting floor.
At approximately 2pm EE was in the process of releasing a barge from the south dock when EE's right knee struck the mooring peg
while fabricating a new scraper for the south dock stinger EE over reached while holding the scraper in place. EE went to set the bolt and felt a burning sensation in EE's right elbow. EE was taken to urgent care for evaluation. EE was placed in a sling and has a restriction of no usage of EE's right arm.
Miner injured their lower back while replacing return rollers on conveyor. Miner did not seek medical attention and documented purpose. Miner went to urgent care on 2/13/2025.

while in the process of putting the load on F-Belt employee 101439 went to put a screen in on a 57's feeder when EE felt a pop followed by a warming sensation in the joint of EE's left thumb
While clearing a rock jam in the crusher, the associate lost EE's footing, fell backward, and struck the side wall. EE then slid down to the entrance door of the crusher. After being assisted to EE's feet, EE walked approximately 40 feet and then rested on a walkway. EE expressed concerns about tingling in EE's feet and legs.
Employee 101408 was reaching down to pick up the cable for the north dock winch when they felt some discomfort in their lower back.
Miner was using a shovel to remove material from the tunnel at SP6. While removing material the miner's shovel slipped from miner's grip and went to push forward and had left thumb extend back.
A miner leaving the bull gang area noticed another miner on the ground. Miner on the ground was sitting upright and was coherent. Ambulance service arrived on site to assess the fallen miner. Miner had passed out and fell down. Miner was diagnosed with a broken Right Ankle.
Employee was welding inside a crusher. When employee stepped out of the crusher their left knee gave out. Ice was immediately applied to the left knee and employee requested medical treatment where they were signed out of work. No specific injury has been identified as of yet.
Employee needed knee surgery after 25+ years of maintenance work. Surgery was originally suggested at the end of 2022 when the employee had pain and swelling that didn't go away within a few days but the employee wanted to put it off as long as possible while trying other treatment options. After exhausting other options, surgery was scheduled for 1/2/2024.

Employee notified management of discomfort in their back on 9/12 alleging that on 9/8 EE's back started bothering EE but was common due to previous injuries at past job. EE went to the ER and was signed out of work starting on 9/15 due to lower back pain.
Employee felt a strain in right shoulder/elbow when picking up a piece of steel. Initial incident date 3/9/23 but employee did not want further attention at first. When the pain didn't go away they saw a doctor where they were prescribed a muscle relaxer.
While welding, employee stopped and lifted EE's helmet and itched EE's eye with EE's glove which must have had something on it. We flushed EE's eye with eye wash solution but after awhile EE said that it felt like there was something scratching EE' eye and ask to have it checked out. A small piece of metal was removed with a cotton swab but EE was given a precautionary antibiotic.
Employee was changing out a screen in the Dock Washroom when the screen snagged the employee's glove and pulled the employee's arm with it, tweaking the employee's right shoulder. When it didn't get better the employee saw a doctor three days later where employee was written out of work. X-rays were taken but nothing was identified.

The employee was using a 3 lb. lump hammer to hammer a steel wedge between steel plating and the 450 excavator bucket frame, when EE noticed a small laceration on EE's right index finger. The laceration had a small steel splinter in it that needed to be surgically removed.
Original incident - employee tripped over skid steer bucket and hurt their shoulder on 11/15/2021. No restrictions after this point until employee saw doctor 10/26/2022 where they were scheduled for surgery on 3/17/2023.
While grinding a bearing employee states something got past face shield. Eye was rinsed and they returned to work. Incident was reported 5/18 due to irritation. Employee saw a doctor on 5/24 where no foreign body was noticed but eye drops were given. Management notified 5/28 and this form was submitted the next working day 6/1.
While operating a 70 ton Terex haul truck the operator hit a divot bump at 10mph causing pain in lower back.
The employee was pulling a torch cart when EE slipped and fell. The torch cart handle landed on the back of EE's legs/knees. Knee swelled up but no damage was seen on x rays.

Employee was inspecting pipe. The pipe broke and water from the pipe hit the employee causing the employee to fall. The employee felt discomfort in the left knee.
Employee received a small laceration to thumb while cutting an avocado with a knife during a morning break. Employee received stitches at a local ER.

While moving material in a haul truck employee drove over a bump in the haul road and experienced pain in tail bone/lower back.
while employee was being loaded in truck, claims experienced lower back discomfort, however, injury leading to lost time was applying direct frozen bottle of water to skin causing frostbite
While clearing plug in #4 screen of the 1-1/2" section, associate cut right pinky finger.
While operating portable plant employee pinched finger on grizzly bar leading to a minor fracture and 6 stitches on right index finger. Contributing factor was not following the standard operating procedures set (task training).

Haul truck driver felt discomfort in neck and lower back after being loaded. It is believed a large stone was in bucket and when loaded into bed it made bed shift side to side.
Employee was using heat to warm slow moving feeder belt and left jacket cuff caught in pinch point of troughing roller and belt and resulted in a left wrist fracture

Replacing old drill pipe from drill rig, rigging on pipe became slack when one end of pipe rested on ground and other end pipe released and fell from strap striking employee on hardhat.
It is unknown at this time but cause of death appears to be a heart related. Employee was operating a Cat 777F haul truck and struck berm and truck came to rest on berm approx 400 feet from initial contact.
Employee standing on lowboy trailer working remote control for overhead crane to adjust 7 foot mainframe crusher on trailer and stepped back into opening in center and dropped 2 feet striking lower back/tailbone on trailer cross member resulting in bruising, no fracture. Prescription meds required and restricted work.

Two employees carrying a 4'x 8' 60 lb plastic liner sheet on second floor of sand plant tower and strong wind gust required them to control the sheet and the action caused a left bicep tendon tear in one employee. Recordable was not known until MRI result on 11/10/14. Employee will not be a lost time.
Employee was rotating a centrifuge drum to wash with hose and felt strain in bicep on 10/17/14. Did not notify supervisor until 10/20/14 and continued to work until MRI revealed a bicep tear on 10/27/14. Employee runs control room and will not be losing time from work.
Employee went to climb up the fixed ladder on the Cat 992 Front End Loader and felt pain in his left shoulder. MRI result on 10/27/14 revealed a rotator cuff tear. Employee will not lose time from work even with current restrictions. Employee is a plant leadman.

Employee standing on one end of stone barge tied to the dock where loading takes place was struck by the winch cable that moves the barge when the cable clamps were unable to hold cable at the cable eye. Employee sustained a simple fracture of the fibula and swelling of area due to trauma requiring a procedure to relieve pressure.

Employee was attempting to remove a hatch from a barge with a prybar and prybar slipped, causing employee to receive a laceration to right pointer finger.
Employee turning conveyor drive belts by hand on spare conveyor is storage yard and finger tip pinched bewteen sheave and drive belt
EE was working in the shop and stepped on a 1.5" nut on the floor and strained his right ankle. EE continued to work normal duties but the pain persisted. Intially the doctor thought it was a non-work related bone spur, however in May x-rays showed a stress (minor) fracture. EE continues performing normal functions, no lost work days.

Employee states while operating water truck he struck a rut/pot hole in roadway causing him to be jarred in the seat. Employee was returned to work and placed out of work by his personal physician on 12/8/09.
Working on the pan feeder, the employee tried to position a lever hoist inside the framework of the feeder. After attaching the hook to the framework, he released the body of the lever hoist causing it to rotate downward, pinching his finger between the hoist and link to be installed.
Employee was removing an expanded metal guard on the head pulley for the stacker conveyor to the crusher. The guard slipped off the last remaining bolt and caused a tear in EE's shirt and an abrasion to his right pectoral muscle. He received a tetnus shot from local clinic.
Employee was closing the door of a 100 ton haul truck, when he pinched his finger between the door and frame.
Employee was holding a carriage bolt from spinning with the head of a hammer. Another employee engaged a 1" impact gun and the bolt spun causing the hammer head to slip and the employee's finger was struck by the bolt.

Employee was cutting rubber belting with a knife for use in the 1 1/2" section of the mill, when the knife slippled and struck him in the leg.
Employee was trimming a sticking drawer on his desk with a pocket knife when the blade folded up and cut into his thumb.
Employee was climbing down from 100 ton haul truck when his hand slipped from handrail resulting in employee falling to the ground.
Employee was lifting a wire cable off of a loaded scow when he felt a pull in his shoulder. Employee was seen by doctor and returned to work in modified duty capacity. He continued to have pain and went for further testing which showed slight tear in the shoulder. Doctor has placed him out of work until further evaluation and treatment.
Carrying a part from the storeroom to dumpster the employee was walking on uneven ground a felt a strain in his knee.
Employee lost his footing on wet steps of ladder and broke his fall by grabbing on a railing. He continued to have some pain in arm and had arthroscopic surgery on 1 23/2009.

Employee went to pick up a piece of steel, lost his grip, he tried to catch the steel and it struck his finger against a rock.
Using a knife to cut a box, employee struck his finger.
When crawling backwards after installing part in feeder, employee felt a strain in his back.
While walking through the plant yard the employee slipped in mud causing him to fall. Originally diagnosed with hip strain, an MRI performed in mid-April showed some bone fragmentation, which was removed laparoscopically on 5/15/2007.

Cleaning rock jam with an excavator, employee was dumping a bucket of stone, when a large stone fell and struck stairway. The rock then shattered and a piece of stone struck the windshield of the excavator and then struck the employee's hand.
