Employee was fueling a track hoe. Employee was climbing down from the track hoe and slipped. He grabbed a handle with his right hand to break his fall. He began experiencing pain in his right shoulder.

Employee was stepping down from a ladder on the screen deck. He sprained his left ankle when his left foot came down onto a drag instead of the ground.

Employee was replacing anvils for an impact crusher. He lifted an anvil to replace on the ring and began experiencing pain in his lower back.

Employee was using a pry bar on a belt wiper to get the wiper to work properly. He began experiencing a burning sensation in his neck along with muscle spasms in the neck and back.

Employee was climbing down the steps of his loader. He slipped off the last step and felt pain in his back

Employee was using an impact wrench to loosen the bolts on the cutting edge of a loader bucket. He reached under the edge of the loader bucket to locate the bolt head. He then engaged the impact wrench. The bolt head on the underside of the loader bucket spun, caught the employee's glove, and lacerated his left middle finger.

Employee was pushing a suction hose onto a pump. He began experiencing pains in his lower back

Employee was installing pipe and slipped on frost covered pit run. He began experiencing pain in his lower back.

EE WAS LIFTING A CO2 BOTTLE. HE BEGAN EXPERIENCING PAIN IN HIS LOWER BACK.

EE WAS USING A RAZOR KNIFE TO CUT THE COVER OF A WIPER. THE KNIFE SLIPPED & LACERATED THE PALM OF EE'S LEFT HAND.

EE WAS CLIMBING ONTO A VSI CRUSHER TO CHANGE THE SHOES. HE HIT HIS LEFT KNEE ON THE EDGE OF THE VSI.

EE WAS BACKING A WHEEL LOADER FROM A STOCKPILE. THE RT. REAR TIRE ROLLED INTO A GROUND DEPRESSION. THIS CAUSED THE WHEEL LOADER TO ROCK HARSHLY FROM SIDE TO SIDE. EE EXPERIENCED PAIN IN LOWER BACK IMMEDIATELY AND PAIN WORSENED OVER THE WEEKEND.

EE WAS OPERATING A 980F CAT LOADER OVER BUMPY TERRAIN. THE LOADER BOUNCED AND HE WAS PUSHED UPWARD BY THE LOADER SEAT VERY QUICKLY.

EMPLOYEE'S LEFT INDEX FINGER WAS AMPUTATED. THE FINGER WAS PINCHED BETWEEN A CONVEYOR BELT AND A WIPER BLADE.

EE WAS ASSISTING DIESEL FUEL SUPPLIER BY HOLDING NOZZLE INTO TANK AIR PRESSURE BUILT UP IN THE TANK AND BLEW THE NOZZLE FROM THE TANK BEFORE EE RELEASED THE NOZZLE HIS ARM WAS JERKED AND THEN HE EXPERIENCED PAIN IN HIS SHOULDER.

EE WAS PULLING A METAL STAKE FROM THE GROUND. THE EE EXPERIENCED SHARP PAINS IN HIS LOWER BACK.

EMPLOYEE WAS USING A SLEDGE HAMMER TO REMOVE THE LINING OF THE CONE CRUSHER. EMPLOYEE BEGAN EXPERIENCING A PAIN IN HIS NECK.

EE WAS REMOVING SCREENS FROM THE WASH PLANT. HE TRIPPED & FELL ONTO A STEEL PEG & LACERATED HIS LOWER RIGHT LEG NEAR THE SHIN.

EE WAS OPERTING A CUTTING TORCH & HOLDING IT IN HIS RIGHT HAND. THE CUTTING TORCH FELL FROM HIS HAND AND THE FLAME FROM THE CUTTING TORCH CAME IN CONTACT WITH THE RIGHT SIDE OF HIS LEFT INDEX FINGER.

EE WAS USING A CRESCENT WRENCH TO TIGHTEN BOLTS ON JAW CRUSHER. THE WRENCH SLIPED AND HIS RIGHT MIDDLE FINGER WAS MASHED BETWEEN THE WRENCH HANDLE AND THE FRAME OF THE CRUSHER.

EE WAS TRYING TO ALIGN A BOLT HOLE SO HE COULD INSTALL A BOLT. EE WAS USING A PRY BAR TO PERFORM THIS TASK. PRY BAR SLIPPED AND END OF PRY BAR HIT EE'S MOUTH. EE'S TOP FRONT TOOTH WAS CHIPPED AND 1/3 OF HIS BOTTOM FRONT TOOTH WAS BROKEN.

EE WAS INSTALLING 6" STEEL PIPE TO THE WASH PLANT. EE PLACED HIS HANDS BETWEE THE ENDS OF TWO PIECE OF PIPE.THE TWO PIECES OF PIPE WERE PUSHED TOGETHER AT THAT TIME BY A FELLOW WORKER. THE EE' S FINGERS WERE MASHED BETWEEN THE TWO PIPE ENDS.

EE WAS CLIMBING DOWN FROM THE CAB OF A FRONT END LOADER. HE MISSED THE SECOND STEP OF THE LOADDER, FELL LANDED ON HIS FEET. EE REPORTED HIS KNEE POPPED AND HE HAD CONTINUOUS PAIN IN HIS KNEE.

EE WAS WORKING ON A HYDRAULIC MOTOR REQUIRING BENOING OVER AND LIFTING. EE EXPERIENCED PAIN IN THE LOWER BACK.

EMPLOYEE WAS WELDING. HE ACCIDENTLY TOUCHED THE HOLT WELDING ROD TO INSIDE OF LEFT UPPER FOREARM, CAUSING A BURN. 2ND DEGREE BURN.

EMPLOYEE WAS USING A HAND SHOVEL. EMPLOYEE TWISTED AT THE WAIST TO THROW DIRT FROM THE SHOVEL AND EXPERIENCED PAIN IN THE LOWER BACK.

EMPLOYEE WAS CUTTING A PIECE OF WEAR RUBBER WITH A BELT KNIFE. THE KNIFE SLIPPED CUTTING HIS RIGHT THIGH.

EE WAS OBSERVING A FASULTY ROLLER ON A CONVEYOR. THE ROLLER FELL ONTO THE CONVEYOR BELT AND THE EE TRIED TO GRAB THE ROLLER OFF THE BELT. EE'S FINGER CAUGH BETWEEN THE ROLLER AND THE BELT. EE' S FINGER WAS AMPUTATED ABOVE THE FIRST JOINT FROM THE END OF THE RIGHT INDEX FINGER.

EE STRAINED BACK MUSCLES WHEN LOADING A CONVEEYOR CHUTE INTO A PICK TRUCK.

EE BENT DOWN TO PICK UP A CHAIN HOOK. EXPEREINECED LOWER BACK PAIN.

EMPLOYEE WAS USING A HAMMER & CHISEL TO REMOVE BEARING A PIECE OF STEEL CHIPPED FROM THE CHISEL WHEN HIT BY THE HAMMER THIS PIECE OF STEEL LOOCED IN EMPLOYEES RT HAND

I.NAME WAS ONA WORK BREAK AND DECIDED TO GET A CANDY BAR FROM HIS LUNCH PAIL IN THE CAB OF THE LOADER. AS EMPLOYEE WAS CLIMBING LADDER TO THE CAB HIS HAND MISSED THE LADDER. I.NAME HELD ONTO T HE LADDER WITH OTHER HAND BUT SPUN OFF THE LADDER WHEN HE LOST HIS BALANCE. WHEN HE SPUN OFF THE LADDER HE COLLIDED WITH THE FUEL TANK, BREAKING RIBS ON LOWER RIGHT SIDE.
