EE was Hammering a head pulley bearing Gear box on west screen feed, when a small sliver piece of metal broke off and lodged into the right side of EE's chin.

EE was exiting surge tunnel and took off hard hat to scratch head and while turning, struck a piece of metal located above, causing a small laceration requiring 4 stitches to close.

Employee was dismounting a conveyor frame and stepped onto a rock twisting EE's ankle.

Employee was using a 5# sledge hammer to pound on a steel pin and a shard of steel came off the pin striking the employee in the stomach.

Employee was cutting conveyor belt with razor knife. The employee was cutting the belt perpendicular to himself and as the employee was cutting the belt, the knife slipped while applying pressure. Puncturing his lower right thigh. Resulting in 8 stitches to close.

Employee was forming urethane flashing for a belt conveyor with a hand grinder and started not feeling very good during the task. It was thought the employee may have been feeling ill from some of the fumes from the cutting process, but when brought to the clinic it was diagnosed with the flu and fumes may have contributed to accelerating his flu symptoms.

One of our employees approached a vehicle that was parked just inside our entrance, he found an unknown female (trespasser) that had committed suicide with a handgun inside of her vehicle. She had drove her vehicle just inside of our entrance gate located off of SR-92. 911 was called immediately and Alpine Police Dept responded to the scene.

Scooping up rip rap on unstable and unleveled ground, caused loader to move side to side, causing it to injure his back. Did not inform Supervisor about injury until 8-15-12.

Miner was moving fence posts. While pounding a post in the ground. The post pounder came off the top of the post, the miner fell back and the pounder hit him on the top of his head.

Changing a screen and pounding it into place, he smashed his thumb between hammer and screen.

EE leaned on the hand rail of the dry plant. The rail broke and he fell about 4 feet into the hopper of the under belt. His hard hat came off and the rail hit him in the head.

CUTTING PLASTIC SCREEN, BLADE SLIPPED, CUT LEFT ARM (KNIFE). (DOCTOR CLOSED 5 SIMPLE SUTURES.)

ROCK FELL AND HIT HAND, DIAGNOSIS CONTUSION AND ABRASION TO RIGHT THUMB.

EMPLOYEE MISSED HAND RAIL ON LOADER WHILE CLIMBING INTO MACHINE,AND FELL OVER BACKWARDS ON TO THE GROUND.

ROCK FELL OFF A BELT & HIT EE ON THE TOP PART OF THE FOOT.

EE BACKED OVER A WINDOW & HURT HIS LOWER BACK AS THE DOZER BOUNCED DOWN.

ROCK FELL OFF CONVEYOR, AND HIT EE ON THE LEFT HAND.

LABOR WS ADJUSTING AND CLEANING ROLLERS WHEN THE HAMMER HE WS USING GOT PULLED INTO BELT. CAUGHT HIS ARM BETWEEN RETURN ROLLER AND FRAME OF CONVEYOR BELT SCRAPED AND BRUISED LEFT ARM AND SHOUL DER. NO BROKEN BONES.

ROCK CAME OFF CONE CRUSHER AND HIT ME IN THE MOUTH BROKE ONE TOOTH OFF AND KNOCKED 2 LOOSE.

HELPING THE WELDER, AND GOT A WELDING FLASH. SHOULD HAVE TURNED HIS HEAD, AND IT WOULDN'T HAVE HAPPENED. KIND OF A DUMM MOVE.

TRIPPED ON USED SCREEN CLOTH LAYING ON GROUND.
