Mechanic employee was using a 30mm wrench to tighten a jam nut on the drill when the wrench slipped off the nut and struck them in the face causing 2 lacerations on their lip and breaking 3 caps on teeth.

Employee was about to throw trash from back of pickup truck. When EE lowered the tailgate there is a battery terminal underneath where the tail gate comes down, and EE's left middle finger got pinched between the tailgate and a battery. EE decided to be seen on 7/29/24 and was placed on restrictions.
Employee was checking a battery to a welder while standing on the back step of a Mechanic Truck. When EE leaned the step moved because the pin was not in place to secure it causing EE to fall landing on right side. Seen in ER 4/23/24 no restrictions. Seen again 4/26/24 placed on restrictions.

While lifting the cutting edge to put back onto the loader bucket, the chain supporting the cutting edge broke. The cutting edge came down onto EE's thumb smashing it.
Employee was attempting to operate a 980 loader. As employee was walking around the loader they tripped on a rock causing them to fall on left knee.
Employee was leaving work for the day and was looking up at the hill while walking to car. Employee tripped on a concrete barrier, falling to hands and knees.

Employee had a prior surgery done years back. While lifting 5 gallon buckets the scar where employee had surgery hurt and was red and swollen.
Employee was cutting crown bar rubber with a utility knife and they cut themselves on the upper left leg.

Employee stepped off the bumper of pick up truck to unlevel ground. When EE's right foot came in contact with the ground, EE twisted the ankle and fell on knee.
Employee was helping secure the screen from inside the screen deck. The screen was turned on and began vibrating material onto EE's screen. EE scraped and bruised EE's back as EE was exiting the screen.
Shoveling when ee twisted and heard a pop in back.
Employee was going to go clean up around wash plant, when EE stepped in an area of mud and EE's foot sank about 6-8 inches in the mud, twisting the ankle.

EE WAS TRYING TO OPEN PRIMARY JAW WITH BORROWED PORTER POWER WHEN ADJUSTER BOLT HOLDING TWO SUPPRESSED SPRINGS BROKE AND THE PLATE HOLDING THE SPRINGS HIT HIS RIGHT LEG. WAS GIVEN A RESTRICTE D DUTY RELEASE ON 2/4/04 WHICH WAS WITHIN HIS REGULAR JOB FUNCTIONS. HOWEVER, HE RETURNED TO THE DOCTOR ON 2/10/04 AND WAS GIVEN A RELEASE THAT WAS NOT WITHIN HIS NORMAL DUTIES.
