This accident is still under investigation and my be revised at a later date. Employee had parked rock truck to help crusher crew. When he stepped on the second to last step he slipped and fell to the ground into his back. He did not wish for medical attention at that time. On 1/20/12 he decided to see a doctor.

After the morning walk around with safety chare and tool box meeting the crew then completed stretch and flex. The crew was then instructed to change out used screens on the screen deck. When the last bolt was being tightened is when employee felt a sharp pain in his lower back.

This is an "Alleged Injury", this report may be amended upon further investigation. EE was changing bearing on conveyor, feeder was locked out, taged out, along with conveyor. EE was crawling out of feeder, when he took off hard hat and a rock flew from secondary feeder and hit him in the head.
This is an "Alleged Injury", this report may be amended upon further investigation. Injured was removing a pin from a tooth on a trackhoe bucket. Hitting the pin with a sledge hammer a small piece broke off and hit him in the left inner thigh. Causing a small cut/burn. Did not report or ask to be taken to a doctor until 2/2/09.

Employee went under screen deck to stop a leak of conveyor belt and removed shovel by kicking it. Employee's right leg got caught up in roller.
Employee was walking to the control van to get some tools because a conveyor had stopped. While returning to the conveyor he was hurrying and stepped on a large rock spraining his right ankle.
Employee was trying to remove an electrical cable from under the conveyor belt while lowering the belt to the ground and set the conveyor on his right foot.

Employee started up the stockpile in a loader and stopped short of top then started forward again to dump and felt the loader begin to tilt forward and go over the edge of stockpile. When the loader stopped suddenly, the employee fell onto the windshield hitting his right shoulder.

PERSONNEL WERE CHANGING A WEAR PLATE ON A CONVEYOR EE CRAWLED UNDER TO OTHER CARRIAGE BOLTS FRO LAGGING. EE DROPPED A WEAR PLATE STRIKING EE BELOW. APPROX. 50 LBS DROPPED 3 FT.
EMPLOYEE WAS EXITING A CASE SKID LOADER AND STEPPED ON THE EDGE OF THE BUCKET, TWISTING HIS LEFT ANKLE.
EMPLOYEES WERE ATTEMPTING TO SWING THE LID CLOSED ON A ROCKEATER USING A ROCKBAR. THEY HAD BEEN INSTRUCTED TO CLOSE IT BY MECHANICAL MEANS ONLY. WHILE PUSHING ON THE BAR, IT BROKE AND BOTH EMP LOYEES FELL APPROXIMATELY 10' TO THE GROUND. ONE LANDED ON HIS FEET AND HAD NO COMPLAINTS OF ANY INJURY. THE OTHER SCRATCHED THE LEFT SIDE OF HIS FACE AND UPPER BACK STRAIN.
EE OPERATING A CAT LOADER PULLED UP TO THE FEEDER BIN & DUMPED. BEHIND HIM GOING TO HIS LEFT WAS A 2ND LOADER, WHICH PULLED IN TO THE BASE COURSE PILE. THE INJURED OPERATOR SAW THE OTHER LOADE R START TO BACK UP & APPLIED HIS HORN.THE OTHER LOADER STOPPED QUICKLY BUT BUMPED THE LOADER AT HIS RIGHT REAR STEPS, BENDING THE BRACKET. INITIALLY DENIED TREATMENT BUT LATER REQ. MED.9/25/0
EE WAS ATTEMPTING TO UNCLOG/DISLODGE A LARGE CLUMP OF BASE COURSE IN A FEEDER BIN. HE WAS SUING A PRY BAR AND WAS PULLING AND PUSHING AGGRESSIVELY WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING A DOZER AND GOT A LITTLE EXUBERANT. IN HIS HASTE HE DID NOT TAKE BREAKS TO REST HIS MUSCLES AND THE NEXT DAY DISCOVERED HE WAS SORE. HE DID NOT NOTIFY EMPLOYER UNTIL 7-2 9-03 AND WAS TAKEN TO SEE A PHYSICIAN ON 7-31-03.
EE WAS SERVICING EQUIPMENT AND DUST/ DEBRIS IN HIS LEFT EYE. SUPERVISOR FLUSHED EYE ON 7-1-03. ON 7-2-03 THE EYE WAS STILL BOTHERING EE AND WE TOOK HIM IN FOR TREATMENT 7-2-03.

EE WAS LAYING ON HIS BACK REMOVING BOLTS FROM SKHAKER SCREENS USING AN AIR IMPACT WRENCH. THE EE HAD ON RX GLASSES WITH SIDE SHIELDS. DIRT FELL ONTO HIS FACE AND GLASSES AND EYES. HE ROLLED OV ER TO CLEAR THEM & WENT BACK TO WORK. CAME IN THE NEXT DAY WITH HIS LEFT EYE SWOLLEN, RED AND TEARING.
AFTER LOGOUT/TAGOUT THE GUARD WAS REMOVED TO GET TO ROLLER. A COMEALONG WAS USED TO HOLD THE BELT IN PLACE. WHEN USING AN IMPACT TO REMOVE A BOLT, THE BOLT SUDDENLY CAME LOOSE AND THE TENSION ON THE BELT CAUSED IT TO JUMP, SLAMMING THE EE'S HAND BETWEEN THE IMPACT AND THE BOTTOM OF THE FEEDER.
EE WAS STANDING ON A ROCK TRYING TO DISLODGE SOME ROCKS FROM THE CONE, THE ROCK HE WAS STANDING ON ROLLED CAUSING HIM TO FALL TO HIS RIGHT KNEE.
LANDING AREA NOT CLEAR, DOOR HEAVY AND DIFFICULT FOR ONE PERSON. RETAINING CHAINS DIFFICULT TO FASTEN WITH ONE HAND.
