Employee was working on a crusher. A bucket tooth was wedged in the crusher. The crusher was jacked up. The jack kicked out and employee was struck in the head by an unknown object and knocked him into the bed of the truck where he struck his head on an unknown object. Employee received head trauma.

EE was carrying a piece of metal less than 50lbs to service truck when EE lost EE's balance and fell to the ground landing awkwardly on right hand resulting in a broken finger. Area was free of debris and level.

A miner was conducting EE's morning pre-shift on the Cat Loader EE was operating, while walking around the equipment the operator stated EE stepped wrong twisting EE's ankle and then stepped again twisting it again. The miner was taken to the medical clinic and issued a walking boot to wear for 1 week or less if pain goes away. Miner returned to work next day.

A miner was walking out to the dumpster from the Shop and attempted to kick a rock out by the dumpster and the rock was froze to the ground causing miner to fall down. When miner fell the miner landed on face and knocked a crown off of a tooth. The crown had to be glued back on by a dentist.

EE was using a water hose washing off the concrete pad under the Plant. Employee stepped backwards off the concrete pad and felt knee pop. Employee continued to conduct regular activities while being monitored but pain has not went away. Physical Therapy was prescribed on 7/14/2022 making it reportable at this time.

Employee was filling the hydraulic system of the 992 C Cat Loader. Employee finished filling the loader and was coming down the ladder. When employee got to the 3rd step from the top his left foot slipped off and he began to fall. While falling, employee's right elbow struck the ladder rungs causing a contusion to his right elbow. Employee was off work and restricted for 4 days.

Employee was heating up catch-alls with propane tank & wood burner under Conveyor #25. As employee ducked to go under the conveyor his right foot slipped and he fell. As he fell his right leg twisted - felt pain at inside of right knee. Employee was carrying a propane tank & weed burner so he couldn't catch himself from falling. Went to clinic on 3-11. His right knee was sprained.

Employee was going to change a screen and was looking for the correct size of screen at the Secondary Wash Plant. The screen was in a stack and 2 of the screens fell and smashed his left hand into an I-beam breaking left index finger & cutting left thumb. Was given a splint and is restricted to "no use" of left hand.

Employee states while driving a loader, the loader rocked causing him to hit his elbow on console causing pain in his right shoulder. Reported it to his supervisor. Didn't go to Doctor until 11-30-11.(No medical treatment)Thought he would be o.k. 3-28-12 still having problems with his shoulder. Sent to Co. Clinic on 4-6-12, received P.T. Claim became recordable.

Employee got out of his personal vehicle to go to work. Employee fell on ice as he was going to start his loader. Employee received pain in his back, neck, head and left shoulder. Employee treated at ER. Was to return to work immediately. Then he went to a chiropractor on his own. Chiropractor said he was to be off work from 1-25 to 1-31-11. Employee returned to work on 1-31-11.

Employee was kneeling down to look at the Grizzly shaker and his wrist scraped a sharp rock that was on the beam that bordered the walkway. Employee received a laceration to his left wrist. Stitches were required. Super glue was used to close the laceration. He was restricted with limited use of his left hand.

Having asthma attack, because he couldn't get to inhaler, had a panic attack. No injury from fall. Asthma attack not work related. Document to follow.

Employee was using a chipping hammer to knock slag off the weld. A piece of hot slag flew up and went under employee's safety glasses and hit his left eyeball. Employee received a foreign body in the Cornea. Employee was restricted at first but later had to be off work.

EE was operating the Hitachi Shovel & digging in tight rocks all day. EE was jarred around while runing the shovel all day & felt pain in his right shoulder. Pain got worse when he sat down to rest at home that evening. He went to work on Tuesday & was sent to the clinic. Diagnosed as cervical strain; EE was put on restricted duties, but no work was available.

EE had been tightening & loosening bolts and nuts with an air impact tool almost all week. EE complains of swelling that began Monday 2/14/05 and the numbness in his right thumb began on 2/17/05. EE was sent to the company clinic on 2/17. He received a strain to his right hand/thumb. He is to wear a splint. EE HAD CARPAL TUNNEL SURGERY 4/4/05 (RIGHT HAND).

Employee was inside the shaker replacing main support beams. Employee was welding, grinding and cutting to replace the beams. Foreign debris got into his left eye. He had his welding shield on. Later that evening his eye started to hurt and the next morning he woke up with a lot of pain and matting in his eye. He received a severe abrasion.

Employee was greasing the tail pulley of Conveyor #10 at primary plant. Pulling grease hose with both hands, lost his balance, tripped and fell against guard which was at waist height. Employee was close to guard and although he put his hands out, he hit the guard right at his mouth and broke 5 teeth and has to have extensive dental repair.

EE WAS ON HIS KNEES AND WAS HITTING THE PINCH BAR WITH A SLEDGE HAMMER AND ALSO PRYING WITH THE PINCH BAR TO ATTEMPT TO REMOVE ICE AND STRAINED HIS LOWER BACK. EE WAS RESTRICTED BUT NO WORK WA S AVAILABLE UNDER THE RESTRICTIONS. EE WAS OFF WORK.
