Employee reported to supervisor on 1/4/2025 that employee allegedly hurt wrist on 12/4/2024, approximately one month prior, during a maintenance project while using a hammer. Employee sought medical treatment on 1/4/2025, and advised management of being prescribed a wrist brace and prescription.
Employee reported that they were removing rocks from a catwalk in November and later alleged soreness in their left hamstring. Employee thought they would be fine, but approximately a month later, was still feeling pain in the left hamstring and sought medical attention on 12/09/2024. On 12/09/2024, management was made aware of employee being prescribed physical therapy.
Employee pulled away from office in service truck and was struck in passenger door by customer truck. On 12/12/2024, employee sought medical attention where management became aware EE was given physical therapy order.
Employee was cutting a water hose toward the barbed end of the nozzle when the knife slipped and cut the top of EE's left hand between the thumb and index finger.

Employee was in the process of reinstalling a roller frame on a dozer. As the employee was sliding the frame back on the dozer a bogie slid and fell onto the employee's left hand.

Dozer became stuck in wet material. After employee hooked cable to dozer ee turned and stepped away to go back to operator's cab ee felt a pop and sharp pain to the front side of right knee.
Employee was climbing up ladder on 773 Haul Truck and felt a pop in right knee. Employee asked to seek medical treatment on 02/07/18 and was diagnosed with a strain.

WAS INSTALLING SHUT OFF VALVE IN SPRAY BAR, WAS STEPPING FROM CONVEYER ONTO CATWALK, WHEN HIS FOOT CAUGHT ON PULL CORD, CAUSING HIM TO FALL.
LIFTING AND CARRYING REBAR ALL DAY CAUSED BACK STRAIN.
EE WAS WALKING DOWN CATWALK WHEN HE TOUCHED THE HANDRAIL WITH HIS LT. HAND. THE HANDRAIL HAD JUST BEEN WELDED AND WAS HOT. THIS BURNED HIS LT. HAND.
EE WAS ERECTING STEEL FOR FEEDER SUPPORT. HE WAS TRYING TO SET A KNEE BRACE UNDER AN I-BEAM BY HAND. HE TRIED TO HOLD IT WITH HIS SHOULDER AND BOLT IT UP AT THE SAME TIME. BY PUSHING ON THE KNEE BRACE WITH HIS SHOULDER. HE FELT PAIN IN BACK RIGHT SIDE.
