Employee was using a razor knife to remove old lagging from a conveyor tail pulley when the knife slipped and cut employee's left bicep. Employee sought medical attention, received 7 sutures and was released to full duty.

Employee was cutting an area of steel hopper with a torch when a piece of hot steel slag fell into clothing and burned left thigh and genital area. Employee sought medical attention and was placed on restricted work duty.
Employee was operating a pneumatic impact wrench from the sitting position. The 1/2 inch air line KC nipple came loose from the quick connect fitting. The air line struck the employee in the groin area.

Employee was changing wires on the middle deck of a screen. He turned to exit and grabbed the hand rail when it gave way, resulting in him falling and striking his head. He sought medical attention and received sutures in his head. He was released to restricted duty
Employee was removing screen cloth from a delivery pallet to a screen rack when his arm came in contact with an adjacent wire with a raw edge, cutting his arm. He received stitches and was released to full duty the same day.

Employees were attempting to clean rocks from punch plate holes, striking rock with a hammer causing fly rock. A piece of rock struck injured on nose. He did seek medical attention; however, when he began having difficulty breathing decided to be checked by the company Occupational Health doctor. Forwarded to an ENT for further evaluation. Surgery 2/11/2009.
Employee and coworker were trying to bolt holes on the ridged plastic discharge pipe from the quarry pump and connect to the steel discharge manafold on the main pipe. Employee alleges he jammed his left shoulder. After investigation was conducted, claim has now been accepted as a compensable claim.

Employee was climbing stairs on loader after fueling. His foot hung on top stair; he tripped and lost balance, falling into handrail.
Employee was putting a section of new belting into a conveyor belt. While attempting to cut the old belt using a straight edge and razor knife along the straight edge, it slipped and cut his right index finger.

Employee was lifting idlers into place on top section tail pulley of conveyor. Injured his back, resulting in back/leg pain, day following the incident. Didn't realize injury immediately and did not notify supervisor. On vacation 5/17-5/23 went to personal physician; had an epidural. Risk Mgmt conducted investigation; accepted claim 6/08/06.

2 employees were attempting to load troughing rollers onto a service truck for repairs. Co-worker was placing the roller onto the truck, the injured employee yelled to hold up caused the injured employee to get his left ring finger caught. He pulled his finger out; peeling his nail off and received a fracture and laceration to end of finger.

EE ALLEGES THAT CO-WORKER ENGAGED EMER STOP ON CONVEYER BY ACCIDENT, WHERE UPON EE WALKED ON CONVEYER TO RESET EMER STOP, EE ALLEGES THAT CONVEYER STARTED UP AND HE WAS THROWN FROM HIS FEET IN TO GUARD ON BOX INTERSECTION. EE DID NOT DE-ENERGIZE EQUIPMENT BEOFRE RESETING.

EMPLOYEE'S HAND WAS PINNED BETWEEN METAL SCREEN DECK & SCREEN SIDE PLATE AS HE WAS ATTEMPTING TO MANUALLY POSITION THE DECK FOR INSTALLATION. THE DECK LODGED AGAINST MATERIAL ON THE WALKWAY WH ILE SUSPENDED BY HOIST & COME-ALONG.
