A roof fall was discovered approx 1200'-1500' from the faces of the 001-0 section. The fall occurred two breaks inby survey staion No.2874 and measured approx 70'-X 20' wide X 2' - 4' thick. The area had been bolted with 36" fully grouted resin roof bolts and some cribs had been installed in the area.
At the start of the shift, the EE was operating track mounted vehicle to his section. As the vehicle went through the 2nd set of airlock doors, an unsecured lid over the traction motor compartment slid out of place and contacted the right support of the airlock door. The lid sprung foward hitting EE and pushed his face into the cover over the contactor compartment of vehicle.
A roof fall was discovered one break inby survey station #6970 in the 16th developement during a weekley examination of the area. The fall measured approx. 20' long x 20' wide by 4'- 5' thick . The roof was composed of laminated sandy shale and had been supported with 36' fully grouted resin bolts.

Jacked JB Long three-wheeler up. He was letting jack back down; bar slipped out of his hand and came up, catching him under his chin cutting him.
A ROOF FALL OCCURRED IN THE NO.4 ENTRY (BELT) ENTRY JUST INBY SURVEY STATION NO 2863 AND APPROXIMATELY 1400 FEET FROM THE ACTIVE 001-0 (P") SECTION. THE FALL MEASURED APPROXIMATELY 45' LONG X 20' WIDE X 5'-6' THICK AND WAS COMPOSED OF LAMINATED SANDY SHALE. THE FALL CAUGHT THE 001-0 SECTION BELT LINE IN THE AREA OF THE FALL.
A roof fall occurred in the NO. 2 return entry in the NO. 2 heading approximately 500' outby the face of the 001-0 section. The fall, in a slip form measured approximately 25' long x 16' - 18' wide x 0 - 5' thick. The rock fell above the anchorage zone of the roof bolts installed in the area.
EMPLOYEE WAS TAKING SUPPLIES UNDERGROUND IN A UNIHAULER. APPROACHING THE VENTILATION DOORS HE GOT OUT AND OPENED THE 1ST SET OF AIRLOCK DOORS. HE MOVED THE UNIHAULER BETWEEN THE DOORS, GOT OFF TO CLOSE THE OUTBY DOORS. AS HE WAS TURNING TO CLOSE THE RIGHT SIDE DOOR, AN AIR CURRENT CAUGHT THE RIGHT SIDE DOOR CAUSING IT TO SHUT AND CAUGHT HIM BETWEEN THE TWO DOORS
A roof fall was discovered in the bleeder entries approx 1400' from the active 001-0 section. the fall measured 30'-40' long by 15'-18' wide by 4-5' thick. The fall occurred in the crosscut 1 break inby and 1 brk to the right of survey station No. 2331.

A ROOF FALL WAS DISCOVERED IN THE BLEEDER ENTRY, APPROX. 1300' FROM THE ACTIVE 001-0 SECTION. THE FALL MEASURED APPROX. 70'LONG X 14-16'WIDE X 0"-2' THICK AND WAS COMPOSED OF SANDSTONE. THE FALL BEGAN AT SURVEY STATION #2409 TO SURVEY STATION #2408.
A ROOF FALL WAS DISCOVERED IN THE BLEEDER ENTRIES APPROX. 1300' FROM THE ACTIVE 001-0 SECTION. THE FALL MEASURED APPROX. 80'-90' LONG X 15-18' WIDE X 4-5' THICK. THE FALL BEGAN AT SURVEY STATION #2421 & FELL THROUGH THE RIGHT CROSSCUT AND INTERSECTION OF THE NEXT HEADING.
EE WAS LAYING PVC PIPE (WATER LINE) OUTSIDE ON THE MINE SURFACE AND HURT HIS BACK. (EE DID NOT REPORT INJURY IMMEDIATELY - CONTINUED TO WORK UNTIL 5/25/05.)
The ee was installing roof bolts in the 1st cut that had been taken out of the No. 3 left crosscut. As the ee began walking to the head of the bolter from the tram station, he stepped upon a step up in the mine floor. As he stepped upon the 4" step up his foot slipped off and the ee twisted and hyperextended his knee.

A roof fall occurred between the No. 4 and No. 5 entries adjacent to S.S. #2227. The fall measured approximately 40'L x 18'W x 40"Thick. The area had previously been bolted with 36" mechanical roof bolts.
EMPLOYEE WAS USING A HAND HELD HYDRAULIC DRILL DRILLING A HOLE FOR AN ANCHOR PIN TO ANCHOR TAILPIECE, WHEN THE GASKET IN THE TOP OF THE DRILL BLEW OUT CAUSING HYDRAULIC OIL TO HIT HIM IN THE ARM.

EMPLOYEE WAS OPENING SOME AIRLOCK DOORS ON TRACKWAY, SLIPPED AND CAUGHT HIMSELF WITH HIS ARM ON THE BUS.
EE HAD TURNED BELT OFF AT TAILPIECE, GOT UP ON TAILPIECE AND WAS GOING TO CROSS OVER WHEN HIS FOOT SLIPPED AND HE STRATLED TAILPIECE HITTING HIM IN THE GROIN.

DURING THE SHIFT A BELT SPLICE HAD TO BE MADE INT HE NO.1 BELT, THE INJURED AND 2 OTHER EE WERE SENT TO MAKE A BELT SPLICE. AFTER ALIGNINING AND TYING OFF THE BELT, THE INJURED EE BEGAN CUTTIN G THE BELT TO MAKE A NEW SPLICE. THE TOP BELT BEING SPLICED WAS LYING ONT HE BOTTOM BELT AND THE EE WAS REACHING UNDER THE BELT RAILING TO CUT THE BELT. AS THE EE CUT THRU THE BELT WITH A UTI

LATER THE ROOF FELL FROM THE NO. 2 ENTRY OVER TO & INCLUDING THE INTERSECTION OF THE N O. 3 ENTRY AND FROM THE LAST OPEN XCUT OF THE NO. 1 ENTRY DOWN THE ENTRY APPROX. 40'. THE FALL MEASURED APPROX. 170' LONG BY 18-20' WIDE BY 5' THICK. ROOF IN THE AREA CONSISTED OF LAMINATED B
AS THE CONTINUOUS MINER ON THE SECTION WAS CHANGING PLACES, THE EE WAS HELPING MOVE THE MINER CABLE OUT OF THE ROADWAY. AS EE WAS PULLING CABLE, HE STRAIGHTENED UP & FELT A POP IN HIS BACK. EE CONTINUED TO WORK @ REGULAR JOB TILL 4/1/02 WHEN HE WENT TO A SPECIALIST DUE TO PERSISTENT & INCREASING PAIN. HAD BACK SURGERY 4/2/02. MINE WAS IDLE 4/1/02 & EE BEGAN MISSING WORK 4/2/02.

EE WAS HAULING ROCKDUST TO FACE ON UNIHAULER. ITGOT STUCK ON SLICK HILL. HE WENT TO THE BACK OF THE UNIHAULER TO GET ROCKDUST TO DRY UP HILL. HESLIPPED & FELL INTO SIDE OF UNIHAULER.
