ROCK FELL 3 BREAKS OVER FROM THE PREVIOUS PILLARSECTION IN THE #3 ENTRY. IT LOOKED LIKE A SLIP. IT FELL 50' - 60' LONG, 20' WIDE, 3' THICK. IT WAS BOLTED WITH 3' BOLTS.
DRAW ROCK FELL FROM THE #3 BOLT INSTALLED TOWARD THE RIGHT SIDE OF THE PLACE WHILE THE ROOF BOLT OPERATOR WAS MARKING OFF THE PLACE TO INSTALL MORE BOLTS.

I FOUND THE ROCK FALL WHILE I WAS MAKING MY WEEKLY EXAMINATIONS IN THE RETURN AIRCOURSE. I DON'T KNOW WHEN IT FELL. IT WAS WITHIN A 7 DAY PERIOD. THE FALL WAS A CONTINUING OF ANOTHER ROCK FALL . IT WAS BOLTED WITH 4' CONVENTIONAL BOLTS. IT FELL ABOUT 10' THICK, FOR 120' LONG.
ROCK FALL ABOUT 6 FT THICK AND ABOUT 45 FT LONG. FELL BETWEEN SPAD 890-891 IN THE RETURN ON AN OLD SECTION. IT WAS CRIBBED OFF ON EACH SIDE. IT WAS BOLTED WITH 5 FT BOLTS.
ROCK FELL IN THE FIRST RIGHT BLEEDER FROM OUTSIDE. THE ROCKFELL BETWEEN THE BLOCKS OF COAL 2 BREAKS ABOVE SPAD 20. IT WAS ABOUT 30" THICK AND 20' WIDE. IT DIDN'T BLOCK THE HAULWAY I TRAVEL. IT WAS BETWEEN THE 2 TRAVELWAYS UP THE BLEEDER.

EE PUT HIS HAND UNDER THE RIPPER HEAD WHILE CO-WORKER WAS TRAMING THE MINER TO SET BITS. HE SAID HE WAS REACHING FOR SOME BITS UNDER THE HEAD OF THE MINER. IT RAN A BIT TRHOUGH HIS LEFT HAND, IN THE PALM.
THE BELT GOBED OFF AND BURNT THE BELT INTO. THE VICTIM IS HALED THE BELT SMOKE.
THE MAIN ROOF FELL AT SURVEY STATION 630 IN INTERSECTION. 3' CONV. BOLTS WAS USIED IN THIS AREA. THE FALL WAS APPROX. 20' L, 20' W, AND 3' H.
BOLTING TOP IN 6 HEADING. HE POSITIONED ROOF BOLTER TO PUT IN THE DIRST BOLT IN THE FIRST ROW. A PIECE OF ROCK FELL OUT BETWEEN THE RIB AND THE ATRS ON THE ROOF BOLTER. THE ROCK WAS ABOUT 18 I NCHES WIDE AND 18 INCHES LONG. IT WAS 2" THICK IN THE MIDDLE AND SHARP AROUND EDGES. IT BROKE 2 SMALL BONES IN LEFT FOOT.

THE FALL IS CONTINUATION OF A FALL THAT OCCURRED YEARS AGO AND INVOLVED ONLY THE INTERSECTION.

A ROOF FALL OCCURRED IN NO1 HEADING APPROX 2 1/2 BREAKS INBY SURVEY STATION #337.THE FALL MEASURED APPROX 18'X20'LX3-4' THICK AND FELL ABOVE THE ANCHORAGE ZONE OF THE ROOF SUPPORTS.THE SECTION HAD BEEN MINED TO THE OUTCROP IN THE AREA AND THE SECTION HAD BEEN MIMNED TO THE OUTCROP IN THE AREA AND THE SECTION EQUIPMENT MOVED TO ANOTHER LOCATION PRIOR TO THE FALL.THE AREA THAT FELL H
THE EMPLOYEE WAS OPERATING ROOF BOLTER MACHINE IN THE BREAK AND HAD INSTALLED THREE BOLTS IN THE 4TH ROW OF THE PLACE BEING BOLTED. AS HE MOVED THE BOLTING MACHINE OVER TO BEGIN DRILLING FOR FOURTH BOLT AND HAD JUST SET ATRS SYSTEM, A ROCK FELL FROM BETWEEN BOLTS AND STRUCK HIS RIGHT LEG.
EMPLOYEE WAS USING A HAND GRINDER TO KNOCK OFF THE SHARP EDGES OF A PIECE OF METAL. GRINDER KICKED BACK AND CUT TOP OF LEFT HAND AND BETWEEN RING AND MIDDLE FINGER. THIS REQUIRED FOUR STITCHES TO THE TOP OF THE HAND AND TWO STITCHES BETWEEN THE FINGERS.
EMPLOYEE WAS LOADING BOLTS AT SUPPLY HOLE INTO A SCOOP WHEN HE PULLED A MUSCLE IN HIS BACK ENTIRE CREW WAS LAID OFF ON 010589 EMPLOYEE THEREFORE LOST NO TIME ALTHOUGH HE WOULD HAVE HAD THE CR EW WORKED ANY MORE THE FOLLOWING WEEK

A ROOF FALL IN SLIP FROM FALL ON #4 BELT 2 CROSSCUTS INBY SPAD #15. THE FALL WAS 20 FEET LONG, 12 FEET WIDE, AT WIDEST POINT, AVERING 30 FEET THICH UP TO 3 FEET AT THICKEST POINT. THE BELT W AS NOT DAMAOGED. THE ROCK WAS MOVED AND BELT RESTARTED IN APPROX. 2 HOURS.

EMPLOYEE CAME UP BEHIND MINER TO TELL OPERATOR SOMETHING AND LAID HAND ON BOOM NEAR TURN TABLE ON THE BOOM. THE MINER OPERATOR LET THE BOOM DOWN EMPLOYEE'S ARM DOWN BETWEEN BOOM AND FRAME. AME.
AS EMPLOYEE STARTED TO MOVE A BAG OF ROCK DUST HE FELT A SHARP PAIN IN HIS LOWER ABCK.NOTE:EMPLOYEE WAS ISSUED APRESCRIPTION BY THE ATTENDING PHYSICIAN AND HAD TO FILLED THAT DAY.
STATES HE WAS RUNNING A SHUTTLE CAR DURING THE REGULAR OPERATORS LUNCH PERIOD AND WAS RUNNING OVER SOME ROUGH BOTTOM WHEN THE SHUTTLE CAR BOUND.CED AND CAUSED HIM TO HIT HIS BACK ON THE SEAT E MPLOYEE ORIGINALLY TURNED HIS INJURY IN ON HIS INSURANCE AND DID NOT REPORT THE ACCIDENT UNTIL 9-19-88 AFTER HE CONTINUED TO HAVE BACK PAIN THE EMPLOYEE DID NOT LOSE ANY TIME UNTIL 9-26-88
