Employee was attempting to locate frozen spot on 10" pipe while on a ladder. The pipe broke and struck EE off the ladder resulting in a broken foot and wrist.
The rock truck operator had just dumped a load of refuse on the boney dump. As EE descended down the hill, EE realized EE did not have brake pressure. The employee then drove the truck through a sump to bring it to a stop. EE struck EE's head in the cab which resulted in a laceration which required 12 staples. EE was wearing EE's seatbelt.

Installing a 2000 amp breaker. When placing the breaker into its location it fell on EE's right index finger. Causing a laceration and getting five stitches.

They were removing a steel pipe at the garage. While moving the pipe it struck the employee on the top of foot causing a bruise.
Employee was trouble-shooting a pump to see if motor was bad or the bearing housing. When they shut off pump when it slowed down and stopped they grabbed the belts and it started to drain the water out of the line which started coasting in reverse and took their thumb through the sheave cutting the thumb.
Changing tire on equipment. The tire fell striking employees foot.

Contractor was cutting I Beam to remove from plant, when spark got in between building tin and began to smoke. Plant workers and contractor began putting water between tin and extinguished smoke.

Employee was drilling hole on a motor base when the drill bit jammed in hole. He reversed the drill and while pulling back out on the drill the drill struck employee's mouth thus breaking his front teeth. Employee is getting necessary dental work done.

Employee was in the process of putting his left foot on the first rung of the ladder to enter loader. The right foot had been on uneven bottom when his foot rooled to the side.
Employee was attempting to clean material that had built up between the head roller and the chute on the no. 11 belt with a roof bolt when the roof bolt got up against the belt thus causing an avulsion of the left index finger.
Over the road truck driver pulled in to get loaded with coal. Driver was doing his truck inspection. He bent over to check air hoses. When he stood back up he got dizzy, lost balance, fell and hurt his ankle.
Employee had just placed rub rail onto frame of continuous miner & needed to move the cover plate in order to permit the pin to pass through the rub rail to secure to the miner, when the rail fell off the frame of the miner onto the top of employee's feet.

Employee was checking breaker in Motor Control Center Building. As he went to step up on a bucket with screwdriver in right hand he apparently lost his balance and as he went to catch himself he accidently contacted the top side of breaker with screwdriver thus receiving flash burns.

A COWORKER WAS DRIVING A BOLT OUT OF PIPE FLANGE WITH ANOTHER BOLT AND HAMMER. THE INJURED EE TRIED TO CATCH THE BOLT THAT WAS BEING DRIVEN OUT AND AS HE REACHED FOR THE BOLT, THE BOLT POPPED THRU, STRIKING FINGER CAUSING A LACERATION THAT REQUIRED STITCHES.
EE WAS PUTTING VESSEL BACK TOGETHER WHEN THE PADDLE SWUNG STRIKING HIS LEFT KNEE. EE DID NOT NOTIFY US UNTIL 2-9-01 AT WHICH TIME HE REQUESTED A REPORT BE COMPLETED. ON 2-12-01, EE SAW A PANEL PHYSICIAN AND COURSE OF TREATMENT MAKES THIS REPORT A MEDICAL.
