While attempting to put a drive belt on a clean coal screen the employee's finger was pinched between the belt and the pulley. This caused a laceration on the ring finger of the left hand that required sutures. This is to report medical treatment only NO days were missed.
Employee was standing in the doorway when a gust of wind blew the door shut hitting the employee in the forehead. This caused a laceration that required sutures. This is to report medical treatment NO days were missed.
While tramming the backhoe across the railroad crossing the employee inadvertently hit the bucket lever. The caused the bucket to drop and throw the employee into the windshield. This caused him to hit his head and nose on the windshield. He was later diagnosed with a small fracture to his nose. This is to report medical treatment only NO days were missed.
Employee was walking across floor where old screen was taken out and some old bolts were sticking up. Employee caught his right foot between the old bolts this caused him to lose his balance twisting his ankle and making him fall. This resulted in a fracture of the right foot. He started off 1/31/12.

While carrying a 1/4" x 4'x8' piece of sheet metal across the 2nd floor in the breaker building the foreman's feet slipped in loose material on the floor. He fell backwards and struck the back of his head on a 4" pipe then stuck a 4" steel angle beam beside the wall before landing on the floor on his back.
While attempting to cut a zip tie used as a support, EE's hand slipped causing him to cut his right index finger, which required sutures. This is to report medical treatment only NO days were missed.

Stepping off last rung of 980G, left leg buckled and knee popped. Shook off and could walk on it. Friday night moved left leg slightly and knee popped and couldn't bend knee.
EE was dismounting a dozer when his foot slipped in mud on the track. This caused EE's foot to fall between the track and the dozer frame, causing him to lose his balance and fall forward. This caused a fracture of the lower Fibula. This is to report medical treatment only, no days were missed.

Employee injured his right knee while descending a stairway near the pre-wet screen. He continued to work and started off work on 9/10/09.
Setting left lift jack in place, strap slipped off knocking employee back causing him to fall cutting right forearm. Jack fell on left shoulder.
Employee was working off of a ladder and his boots were muddy causing him to lose traction and slip. He fell off of the ladder and landed on his elbow resulting in two fractured bones.

While cutting a piece of belt to make a splice, the utility knife blade came out of the knife, causing EE to fall forward. When EE attempted to catch himself, he accidentally fell forward on the knife blade that was still stuck in the belt, lacerating his left hand which required several sutures. Employee was laying on back under beltline lifting with legs. Lifted wrong.
An employee of Superior Steam Cleaning & Sandblasting was on a small section of roof. He was removing sheeting on the side of East Gulf Prep Plant. As he was tying off his safety harness he slipped and fell though a skylight. He fell and cut his head on the right side.

While pushing loaded wheelbarrow full of wet coal over raised concrete, employee lost his balance, tried to hold it from spilling and twisted lower back which started hurting instantly. He stopped work and used radio to inform supervisor. Taken by ambulance to E.R. Injuries severe lower back sprain and possible soft tissue injury.
Injured was positioning the pulping column in to the primary coarse sump. Column was supported by cum-a-longs. The column shifted as it was being lowered and the injured's little finger on his left hand was caught between and fractured. No days were lost, this is to report medical treatment only.

Employee was attempting to move a piece of grating by dragging it and strained a muscle in low back.
Employee was mixing stone laid concrete and got some on his right hand.
Employee was trying to free stuck coal by pounding on the truck dump bin with a sledge hammer. While swinging the sledge hammer his left hand was caught between the hammer and the side of the bin. This resulted in a laceration on the outside of his left palm that required 6 sutures. The employee was wearing gloves. This is to report medical treatment only.
The plant electrician received arc/flash burns to his eyes while attempting an energjued breaker 480 V while wearing high voltage gloves. After he removed the first phase he began to remove the second phase at this time it went to ground and resulting arc burndt his eyes and face. He has been suspended from work for failure to lock and tag.
