Employee was starting a P-70 pump when the line pressurized the line kicked the employees feet out from under him causing him to fall. The employee tried to catch himself with his left hand which caused him to fracture his left wrist. Due to no orthopedic doctor on duty the employee was referred to his orthopedic for medical treatment.

Employee was operating scoop when a piece of draw rock approximately 16" in diameter & 1/2" thick struck him on the side of his head causing his head to strike the frame of the scoop deck.
While en-route to rock dust old works foreman identified a roof fall in the roadway at Brk77 1 Brk inby spad 193.Due to the belt line being removed the travel way was re-routed to the old belt entry. The fall was approx. 20'W X 20'L X 4'H the area was supported with 4' fully grouted resin bolts. The fall will not be cleaned up all approaches have been dangered off and timbers set
While working on his communication line an electrican identified a roof fall in the #4 entry near spad #2775. The fall was approx. 20'W X 30'L X 3'-4'H the area was mined approx. March 1992. The area was supported with 4' fully grouted resin bolts and will not be cleaned up. All approaches have been dangered off and timbers have been set.

Employee was attempting to pull a piece of rock from the mine roof. When the rock fell it landed on the slate bar that had slipped from the employees hand and the slate bar bounced back up striking the employee in the upper right thigh. This caused a laceration that required sutures.
While pulling slack to hang cable the employee felt pain in his lower abdomen. He continued to work until 11/11/13 when he was placed off work with an abdomen strain or sprain. The employee currently remains off.
While conducting his weekly examination a fireboss identified a roof fall in the Left return #1 entry of Detour Mains at brk #84 near spad #163.The fall measured approx. 120'L X 20'W X 4'H the area was supported with 4' fully grouted resin bolts and mined approx.7-2010. The fall will not be cleaned up and the area has been dangered off and timbers set.
While conducting his weekly airway examines the examiner identified a fall in old 3Lt.Panel. The fall was located outby spad 9895#2 intersection & fell up the #2 entry for approx.120'then Lt. handed thru CC into #1 & continued outby for 100'. The fall was 20'WX220'LX4-5'H supported w/4'fully grouted. Area will not be cleaned up has been dangered off and crib & timbers set.
Mantrip was traveling outside while going under the #9 belt cross under they ran over a rock. This caused the mantrip to raise up and the spare tire hit the cross under guard knocking it loose. The guard came down and hit the employee on the nose. The caused a fracture that required medical treatment. See Amended attached narr.
An apprentice electrician was watching repairs being made on the section feeder when a piece of metal chipped off & hit the employee in the LT.bicep.The emp. was treated & released from the hospital to restricted duty with a referral to a general surgeon to have the object removed.The employee continued to work until Apr.10 when he had the object removed and remains off.
Employee was welding on a scoop with a plasma welder. Later he noticed his eyes were burning and irritated and went to the doctor to get checked. He was treated and released for minor UV exposure to his eyes and referred to an eye doctor. This is to report medical treatment only NO days were missed.

While traveling the intake an examiner identified a roof fall in the #6 entry inby spad #1195. The fall measured approx. 20'W X 60'L X 4'-5'H and was bolted with 4' fully grouted resin bolts. The area was mined approx. 8/2/12 the area will not be cleaned up and the area has been dangered off and timbers set.
During his fire boss run the examiner identified a roof fall in the #3 neutral entry outby spad#1107. The fall measured approx.20'WX60'LX4'-5'Hthe area was mined approx.5/30/12. The area was supported with 4'fully grouted resin bolts the fall will not be cleaned up and timbers have been set and the area dangered off.
During his weekly examination a fireboss identified a roof fall in the left return of the 2nd Rt. section off of Detour Mains near spad #1155. The fall was approx. 20'W X 60'L X 5'-6'H and was supported by 4' fully grouted resin bolts the area was mined approx. 6/25/12 The fall will not be cleaned up and cribs have been built.

The fall was approx 20'X20'X4'-5'H and was supported with 4' fully grouted resin bolts the area was mined approx 8/1/11. During his weekly exam a fireboss identified a roof fall located at Spad #10726 in the return. The roof fall measured 20'Wx20'Lx3'to4'H. This area was supported by 4' glue bolts and was mined approximately 5-5-07. The roof fall will be cleaned up with a cleanup plan posted at the site. Currently the area is dangered off and timbers set.
On 9-14-12 the employee was struck by a piece of rock that fell out between the bolts on the outby RT hand corner of the 6 RT face. The rock struck the employee on the RT arm, RT elbow, and on the RT leg. The employee continued to work until an MRI showed a fractured RT knee on 9-27-12.

On 6-14-12 the employee was helping remove a bearing off a miner planetary when a piece of metal broke off striking him in his left forearm. The employee continued to work until 7-10-12 when he had the piece of metal removed. The employee received sutures after the metal was removed and remains off.

Employee was hitting on a chisel when it broke and a piece flew back and hit him in the forehead causing a laceration that required sutures. This is to report medical treatment only No days were missed.
While the operator was attempting to line the bolt head up with the drill chuck, the joystick rotation control was accidentally hit causing his right hand and arm to be jerked. This resulted in a strain and sprain injury to his right hand and arm. This is to report medical treatment, employee missed NO day but remains on restricted duty.
At approximately 11:45am, a roof fall was identified on the 3 left section ( 3 ) breaks inby 8c Head in the #4 neutral entry. The fall was approximately,20'W x 20'L X 5'-6'H. After loading wire rope into a scoop bucket the employee turned and felt a sharp pain in his left knee. The employee was later diagnosed with a sprain and remains off.
On 04/10/2012 approximately 7:00a.m. a fireboss identified a roof fall in the number 4 entry located at 9 break on 6 belt at SPAD number 3512.The roof fall was 20'Wx80'Lx5'to6'H.Area was supported with 4 foot fully grouted resin bolts and the original mining date is unknown. The roof fall will not be cleaned up and timbers have been set and the area has been dangered off.
On March 26th a section boss was putting up a center line when he twisted his left knee. He continued to work until May 21st when he had surgery, the employee missed 5 days but is now on restricted duty.
A roof fall was identified at approx. 6:00 P.M. on the 3 Lt. Section the section was idle at this time. The fall was in the #5 entry at spad #9150 and measured approx. 70'L X 20'W X 6-7'H. The area was supported with 4' fully grouted glue bolts and 5' torque tension bolts. The area has been dangered off and cribs set. The are will not be cleaned up.
Employee was supplying bolt machine when he turned to place the bolts in the operator tray he felt pain in his lower back. This is to report restricted duty only No days were missed.
While attempting to lift a belt rail into place with a com-a-long the belt chain broke caused the belt rail to fall and strike the employee in the right knee. This resulted in a fracture of the right Tibia. This is to report medical treatment only.
At approx. 11:45 employee informed his section boss that he had been involved in an accident. Apparently the employee was walking past the #1CM in the #2 entry inby spad #2326 when the miner pinched the employee against the coal rib.

Employee was riding on passenger side of a Damascus permissible 2-man ride when the ride malfunctioned causing the ride to abruptly turn into the belt line. This jolted the employee into the control panel in the center of the ride. This resulted in a few fractured ribs the employee was kept for observation and released and remains off.
Employee was going through a mandoor at #38 break when the mandoor fell on his left hand cutting his middle finger. This caused a laceration that required sutures. This is to report medical treatment only NO days were missed.
On 11/14/11 EE informed safety he had went to seek medical attention from an incident on 11/11/11.While walking down the belt line he ran his head into a brow causing him to lose his balance and fall into the belt structure. It was determined that the employee had a fractured rib. This is to report medical treatment only NO days were missed.
While attempting to scotch a wheel on a shuttle car with a crib block employee caught his left middle finger between the crib block and the frame of the shuttle car. This caused a laceration to the end of his left middle finger that required sutures. This is to report medical treatment only NO days were missed.
Employee turned to walk inby in the belt entry and tripped over a piece of belt structure causing him to fall striking his left eye brow and the bridge of his nose. This caused a laceration that required sutures. This is to report medical treatment only NO days were missed.
While making a belt splice on the #3 belt the belt foreman was pulling the splice pin with a pair of channel locks, the pin hung and the channel locks slipped off the pin and he struck his right hand against the belt rail and chain hoist. While advancing the roof bolter up for the next row of roof bolts a piece of draw rock fell striking EE on the left little finger. This caused a fracture of the second metacarpal joint on the left little finger that required medical treatment. This is to report medical treatment only NO days were missed.
Miner operator was reaching up to take a piece of curtain down when he hit right index finger on a roof bolt plate. This caused a laceration that required sutures. This is to report medical treatment only NO days were missed.
While backing a mantrip up the left front tire struck a rock in the roadway causing the steering wheel to spin quickly. EE left hand was struck by the webbing in the steering wheel causing a small laceration that required sutures. This is to report medical treatment only no days were lost.
A roof fall occurred on the 3Lt.Mains Section in the #7R crosscut 1 break inby spad #422 at approx 7A.M.The fall continued outby into the intersection of #7 entry causing 3 rows of bolts to be pulled.The fall measured 25'L X 15'W X 8'H. The area was bolted with 4' fully grouted glue bolts. The area has been cleaned up and rebolted with 5' torque tension and 9' cable bolts.
While pulling a belt rail from the scoop the operator caught his right little finger against an offset in the rib. This avulsed the tissue of his finger down to the proximal joint of the middle phalange. This required amputation of his middle phalange. (100 days) Employee has been on restricted duty since 2/24/11.
Electrician was changing take-up jack on joy miner. While lowering the jack he caught the ring finger of his right hand in the mat chain lacerating it. This required seven sutures. No days were lost, this is to report medical treatment only.

While bolting in the #4 entry ee attempted to put a bolt in a newly drilled hole when a piece of draw rock fell out. The rock hit ee right hand pinching it against the drill pot. This caused two lacerations that required sutures. This is to report medical treatment only No days were missed.
Employee was working on a cable splice when his knife slipped and cut his inner left thigh which required medical attention. This is to report medical treatment only.
After turning a right handed crosscut, EE was backing out of the 4RT cut when a piece of draw rock fell. The draw rock rode back along the rib. The rock was approx. 12' L X 3'-4' W X 6" Thick. The rock pinned his right foot to the mine floor.
While examining an area on Old Zippo Mains where a preposed bleeder was to punch through a roof fall was identified. The fall was approx. 65' L X 20' W X 4'-5' H. The area was mined approx. 1/11/07 and was supported with 4' fully grouted resin bolts. The area has been dangered off and all approaches have been timbered.
While building a crib under the feeder the miner caught his right thumb between a crib and the feeder. This resulted in a fracture to the distal tip of his right thumb. This is to report medical care only, no days were lost.

On 9/23/09 EE was getting off of a 2-man battery ride when his foot slipped twisting his right knee. He continued to work until December 16th,2009 when he had Arthroscopic Surgery on his meniscus. He was released to return to work on full duty December 17th, 2009. No days were lost, this is to report medical treatment only.
While conducting his fireboss run the mine examiner identified a roof fall in the old Zippo Mains #7 entry near spad #10587. The fall was approx. 60'L X 20'W X 4' High. The area was supported with 4' fully grouted resin bolts. The fall will not be cleaned up and the area has been dangered off and timbers set.
While hooking a steel cable hook to the forklift the scoop operator pinched his left ring finger between the steel hook and the fork lift lacerating it. He received (3) sutures. This is to report medical treatment only.
Electrician was changing the bearing on the #1 taipiece. When he went to grab the shaft a piece of metal went through his glove and stuck in his right index finger. When he removed it some was left in the wound. The metal was removed by the Dr. and (5) sutures were required. This is to report medical treatment only. No days were lost.

The fall was approx. 40' long X 20' wide and approx.4' high. The area contained 4' resin and 4' dbl. lock bolts. The area was mined approx. 11/93. The area has been dangered off and timbers have been set.

A roof fall was discovered in the return near spad #10363. The fall originated in 5Rt. and extended thru 6 into 7Rt and ended near spad 10368. The fall was approx. 4' high 18' wide and 100' long.
A fire was found in the #10 Belt Drive 300 KVA Belt Power center located 2 Breaks outby #10 Belt Drive. Fire was contained in the transformer and was extinguished in 15 minutes. No injuries resulted from this incident.
The electrician was standing in a high place on the 1st left section 1 break inby spad 10608. When he turned to pick up a cm remote box he struck the left side of his head against a roof bolt plate along the brow. This caused a laceration to his left ear requiring (7) sutures. 1st left section #7 Entry 1-Break inby spad 10543. Rock caught CM in face cut. Area started working, crew retreated. A roof fall occurred and 3 rows of roof bolts were lost. The area was dangered off, fall reported & additional roof supported added.The CM was recovered with a hyd. retreiver. The fall was approximately 16'W X 35'L X 5'H. The bolted portion contained 4'resin bolts.
While opening the airlock door near the mine drift, the scoop operator let the door he was attempting to open swing back catching his left little finger between the doors, lacerating it. Sutures were needed to close the wound he received. Medical Treatment Only-No days were lost.
Employee was helping electrican change torque shaft on CM. Turned to crawl back to mobile bridge carrier and heard a popping sound and had pain in his right knee. Employee continued to work until 01/24/2007 when diagnosis was made that surgery was needed due to cartilage damage.
The electrician and outside utility man were carrying cribs in preparation to work on a scoop in the supply yard. The utility man let a crib slip off his shoulder and it struck the electrician on his left hand. This resulting in fractures to his left distal middle and index finger. While rock dusting was in progress the electrician was traveling (crawling) up #6 entry on the 001-0 Unit Supersection and caught his left ear on a belt hanger that was in the top for a shuttle car anchor. This resulted in a laceration that required (6) sutures. This is to report medical treatment only. No days were lost.
Employee traveled to the continuous miner to talk to the operator about the next move. When the employee turned to go back to the roof bolter he struck a roof bolt that was bent down but hidden by the line curtain. He received a laceration on his posterior left side about kidney level that required 6 sutures to close. No days were lost; this is to report medical treatment only.
EE was helping to get an imbeam out of sled when i-beam shifted smashing left forefinger between i beam and sled.
Employee was hooking up torches at #7 belt drive. He was between a discharge roller & a rubber tired mantrip (2-man). A scoop backed into the mantrip & he was caught between the mantrip & the discharge roller. He was transported to the hospital, kept overnight for observation & discharged with a diagnosis of bruised kidneys. Equipment inspected, no defects observed.
