A cover on a breakerbox fell onto employee's middle finger. Employee got a cut.

Employee was checking screens when EE fell and scraped arm on grading.

Employee was locking up weed eater and it fell on their hand

Operator was cleaning an elevator chute when the left hand, ring finger was caught between a bucket and the elevator chute frame resulting in a cut from the finger tip to the first knuckle on that finger.

Sprained knee

The employee was using a knife to check the up and down motion of a sheave at the #2 booster pump. EE was spinning it by hand. The sheave hit the knife and cut the employee's right pointer finger.

An employee was tapping bolts into the 12" flange for the waste pump with the hammer in EE's right hand when the hammer slipped and EE hit left hand, fracturing EE's left middle finger.

Contractor was cutting a piece of tin with a grinder when a small piece of metal struck them in the eye.

Employee was assisting in the removal of the gear box from the #1 Sand Dryer. Two bolts that were holding the gear box in place broke, causing the gear box to fall onto the employee's leg and causing a left proximal fibula fracture.

A bucket line was being held up using a hoist and a crane. The hoist broke and struck employee on the arm.

A customer truck driver was rolling up the tarp when the customer fell through a wooden board on the trailer that was rotted.

Employee was bailing water out of a boat with a 3.5 gallon bucket. The employee twisted back causing a lumbosacral sprain.

There was an electrical fire at the variable frequency drive in the MCC room at #2 Wet Plant. EE began to extinguish the fire using a fire extinguisher. EE then ran to the main disconnect located at the transformer station to turn off the power. While running, ee stepped on uneven ground and injured knee.

There was an electrical fire at the variable frequency driver in the MCC room at the #2 Wet plant. While extinguishing the fire, ee inhaled the smoke and the extinguishing agent.

The employee was hand stacking 50 pound clay bag, and when ee turned to place a bag on the pallet, ee felt a pain in their back.

Employee was holding a driving pin to remove a shaft from a fan housing. Another employee was hammering the driving pin and struck the injured employee's finger with the hammer.

Raymond Roller Mill star feeder was blocked with large clay lumps. Employee shut the system down to remove the the blockage from the feeder. When he removed the lumps of clay causing the blockage, the feeder turned and rolled over his left hand causing severe lacerations to his ring and middle fingers.

Employee was moving a water hose and stepped on uneven packed sand. Pulled muscle in right calf.

Employee had been working outside in the heat all day. Approximately 3 hours after his shift was over and he had gone home, his wife observed that he was flushed and showing signs of heat stress. He was taken to an urgent care center where he was treated for dehydration.

Employee slipped and twisted his knee while stacking bags

Employee was lifting 50lb clay bags and felt a pop in his back.

Step off 6" concrete pad, twisted ankle.

Employee was cutting steel supports from a screening structure and came in contact with a hot piece of steel causing a burn to his upper left arm.

Employee was tightening a stainless steel banding strap which secures the float to the dredge pipeline. The strap slipped and the sharp edge of the strap lacerated the employee's thumb.

Employee of contractor was installing catwalk at a sand plant which is being rebuilt. He and his helper were attempting to lay a piece of catwalk in place and when he lifted his end to lay it on the support beam his helper pushed it into place before the employee moved his hand out of the way and his right hand was lacerated when pinned between the catwalk and support beam.

Employee stepped off the stacking platform onto the floor and lost his footing. In an effort to keep from falling, he grabbed the forklift which was parked nearby. He did keep from falling but he hurt his shoulder in the process.

Employee was assisting two other employees with the removal of a cleanout door on the discharge side of a 10" pump, in an attempt to unplug the line. They were not aware that there was a buildup of hot water and steam in the pipe behind the door. As he was removing the bolts, the door broke free and the sudden escape of steam and water burned the employee on the left forearm.

Maintenance workers were relocating the dredge pipeline and this employee was attempting to secure a lifting strap around the pipeline. He reached underwater to wrap the strap around the line and cut his right hand on a banding strap which holds the line to floats.

Employee was walking around a corner and hit his head on a low hanging brace

Employee was trying to locate a leak in the 8" vacuum line from the clay drum filters. When he ran his hand over the area of the pipe where he suspected the leak to be located, the vacuum sucked part of his left hand into a hole in the pipe. He subsequently pulled his hand out lacerating the middle finger of his left hand on the sharp edge of the rusty hole.

Employee was stepping onto the top of a hopper car to get clay sample and he hit his left shoulder on the handrail. He then lost his balance and grabbed the handrail with his left hand and felt a severe pain in his left shoulder.

Employee was moving a metal bottom elevator idler (spare part) in the Store Room and strained his lower back (right side). He did not report to Management until August 20. Doctor issued work restrictions. Employee continues to work on light duty.

Raymond Mill Operator had just finished help spot a railcar hopper underneath the loading shoot. He was traveling on a metal walkway (catwalk). He alleges his steel-toed boot (left foot) tread (lugs) hung on the metal walkway's grating openings. He caught himself (handrails) but strained his left leg. Doctor diagnosed a possible torn tendon resulting in lost time.

Dredge operator stood up in the john boat, lost his balance, fell, and hit his lower back on the john boat. Employee reported to management, appeared OK, & refused medical treatment. On Jan. 28 employee reported pain. On Jan. 29 employee was taken to a doctor. Doctor issued "no work" restrictions until Feb. 5. Although, employee is expected to return without restrictions.

Employee turned to leave the platform at the bottom of the stairs to the Scale House, missed the step, fell, & caught himself on the handrailing resulting in twisting his left knee (diagnosed knee sprain immobilized with knee brace).

Drilling bolt out of Raymond Mill when drill hung & twisted left wrist counter clockwise causing a wrist strain.

While attempting to open the discharge door to the No. 1 Dry Plant's hot tank with a 6-foot pry bar, the employee scrapped his left hand's knuckles on both the little & ring fingers. A deep laceration required staples and physician issued work restrictions.

While employee was descending the right side of the front-end loader he fell to the ground resulting in a left knee injury (inflammation). Doctor immobilized the left knee (knee brace) and issued work restrictions. Employee returned to work and was accomodated for his work restrictions. Employee will be re-examined on September 6 at 9:00 AM.

While turning a wheel valve, employee caught his right hand's ring finger. Employee continued working. Employee did not require medical treatment until July 26. Employee was able to return to work and his job functions allow him to continue working without violating his issued work restrictions. Treating physician did not immobilize the finger until August 3.

Injured employee was taking turns with another employee driving the hitch into a trailer's receiver with a 5-lb. hammer. While injured employee was standing behind and to the left of the other employee swinging the hammer, the hammer's wedge flew-out and struck the employee just below the chin causing a laceration that required sutures to close the wound.

Injured EE, #1 Loader Operator, was helping the Mechanic replace the #1 Loader's battery. Injured employee's right, little finger was accidentally caught between the new battery (weight about 50-lbs.) & the battery box on the loader. X-rays indicated fracture of employee's right, little finger and work restrictions were issued. Employee returned to work.

EE WAS WALKING ACROSS PARKING LOT AND FELT A SHARP PAIN IN HIS RIGHT KNEE. EE STATES THAT HE THINKS HE MIGHT HAVE HURT IT SEVERAL WEEKS EARLIER WHILE WORKING ON A FRONT-END LOADER, BUT HE DID NOT REPORT IT AT THE TIME.

EMPLOYEE WAS TRYING TO REINSTALL A WIRE ROPE, USED AS A SWING CABLE ON THE DREDGE, BACK ONTO THE GUIDE PULLEY, WHEN HE SLIPPED IN SOME OIL ON THE DECK AND FELL, HURTING HIS BACK.

EE WAS PULLING A 20' X 4" HOSE OFF THE BACK END ON A PICKUP TRUCK AND HE LOST HIS FOOTING SLIPPED AND FELL BACKWARD ONTO HIS BACK, ON THE GROUND.

EE WAS UNLOADING SAND SAMPLES FROM THE BED OF A PICK UP TRUCK. SHE LEAN OVER THE SIDE OF THE BED OF THE TRUCK TO LIFT A PALE OF SAND WEIGHING APPROX. 14 LBS. AS SHE LIFTED THE PALE, SHE SAID S HE FELT A SHARP PAIN IN HER LOWER BACK.

EE WAS CUTTING METAL WITH A GAS TORCH AND A PIECE OF HOT METAL FLEW INTO HIS EYE.

EE WAS SHOVELING CLAY FROM UNDER THE CLAY DRYER EXTRUDER AND PULLED A MUSCLE IN HIS BACK.

EMPLOYEE SLIPPED WHILE WALKING FROM SLOPE SCREENS TO OPERATOR'S SHACK LANDING ON HIS BUTTOCKS.

EMPLOYEE WAS CLEANING SCREENS AND LOST HIS BALANCE, STRAINING HIS BACK.

EMPLOYEE WAS WORKING ON PIPELINE WHEN HE STARTEDFEELING PAIN IN HIS CHEST. HE CAME TO THE SHOP AND 911 WAS CALLED TO THE PLANT TO CHECK HIM OUTEMPLOYEE WAS ADMITTED TO HOSPITAL OVERNIGHT FOR TESTS.

EE STEPPED ON PILE OF CLAY AND TWISTED RIGHT ANKLE.

EMPLOYEE WAS LOADING CONTAINER BY HAND, LIFTED 50 LB. BAG ABOVE HEAD AND STRAINED LOWER BACK.

EE WAS AT THE TIME CLOCK PUNCHING OUT. HE TURNED, STUMBLED & FELL TO THE FLOOR, INJURING HIS LEFT ARM, GROIN, & LEFT SHOULDER.

EE FILED ACCIDENT REPORT ON 11/20/97, NO MED TRETMENT REQUIRED AT THAT TIME. 1/21/98 EE WENT TO HIS PERSONAL DR. FOR A BLOOD PRESSURE CHECK & TOLD HIM HIS BACK HURT. 1/26/98 AGAIN WENT TO PERS ONAL DR & TOLD HIM HIS BACK WAS STILL HURTING. DR TOLD HIM TO REST FROM WORK 1 WEEK & BE REEVALUATED. 1/30/98 CO FILED COMP CLAIM & A COMP DR SAW HIM 2/2/98 & PLACED HIM ON RESTRICTED DUTY.

EE WAS INSPECTING TOP OF RAILCAR WHEN ANOTHER EE TURNED ON HI PRESSURE PUMP WHICH CAUSED HOSE TO WHIP AROUND AND SPRAY RT. EYE W/HI-PRESSURE WATER.

EMP WAS HELPING MAINTENANCE ADJUST GEAR REDUCER AND WAS PULLING ON IT TO ADJUST.

EMP WAS STRAINING, PULLING ELEVATOR CHAIN STRAINED LOWER BACK.

ADJUSTING VALVE. TURNED TO WALK AND RIGHT KNEE POPPED OUT.

EE WAS ADJUSTING IDLE CONTROL ON WELDER ENGINE W/O PROPER GUARD IN PIKE IDLE SPRING PINCHED FINGER CAUSING EE TO JERK HAND, WHICH HIT ROTATING FAN BLADE, CUT RIGHT INDEX FINGER.

EMPLOYEE WAS REACHING FOR PULL CABLE FOOT SLIPPED LOST BALANCE HEAD STRUCK RR CAR LADDER CAUSING APP 3" GASH REQUIRING 7 STITCHES

PULLING CABLE ACROSS FRONT OF WORK BARGE, CABLE HAD SPLICE THAT CAUGHT ON BARGE CAUSING SUDDEN STOP. EMPLOYEE FELL BACKWARDS AGAINST WELDING MACHINE CAUSING MULTIPLE CONTUSIONS ON BACK & PELVI S.

DREW SPIKE HAMMER BACK TO SWING, HEAD FELL OFF HANDLE AND STRUCK EMPLOYEE IN THE SMALL OF BACK CAUSING TRAUMA TO LOWER LUMBAR REGION.

EMPLOYEE WAS BAGGING SAND AND LOADING ONTO PALLET WHEN HE HAD A SEVERE PAIN IN THE UPPER MIDDLE P0ART OF HIS BACK. EMPLOYEE COMPLAINED OF FEELING SICK TO STOMACH AND WAS HAVING TROUBLE BREATHI NG ALONG WITH THE PAIN IN HIS BACK.

WAS PULLING WASH HOSE WHEN HOSE CAUGHT ON SOMETHING STOPPING FORWARD MOVEMENT SUDDENLY WHICH PULLED MUSCLES IN SHOULDER BLADES AND NECK.

INJURED WAS REPAIRING AND REPLACING SECTIONS OF CATWALK AT #1 SAND PLANT. HE HAD JUST REMOVED A 3'X3' SECTION ON THE UPPER LEVEL OF THE SPIRAL PLANT, HE CARRIED THE REMOVED SECTION AROUND THE CORNER AND CAME BACK AND STEPPED THROUGH THE HOLE AND FELL APPROX 14' TO THE LOWER LEVEL.

EE STEPPED ON WATER HOSE TWISTING HIS LEFT ANKLE.

EE WAS REACHING DOWN TO HOOK COME A LONG TO PONTOON WHEN BACK POPPED.

GEAR REDUCER FELL WHILE BEING REMOVED, PINNING EMPLOYEES LEFT HAND AGAINST HARD SURFACE.

EE WAS PULLING ON A DREDGE CABLE AND STRAINED HIS LEFT KNEE.

EMPL MOVING MOTOR ON J#1 VACUUM PUMP INTO POSITION WITH PRY BAR AND FELT A KINK IN HIS NECK.

WHILE LOADING BULK BAGS THE ROPE TO STOP THE FLOW OF SAND TO BAGS BROKE EE LOST HIS BALANCE AND WENT OVER BACKWARD OFF THE RAMP AND HIT A TRUCK WHICH WAS BULK LOADING HE HIT BACK OF HEAD AND S HOULDER AND LANDED ON HIS TAILBONE.

EMPL WAS MOVING HEAVY FILTERHEAD INTO POSITION TO HOIST UP STAIRWAY FOOT SLIPPED ON WET FLOOR CAUSING FILTRHEAD TO BECOME UNBALANCED FALLING & CATCHING LEFT HAND BETWEEN FILTERHEAD & FLOOR.

EMPLOYEE WAS OPENING A VALVE & LOST H IS FOOTING SLIPPED & PULLED A MUSCLE IN HIS LOWER RT BACK

THE ACCIDENT WAS CAUSED BY A COMBINATION OF A FRESHLY WAXED FLOOR AND A PAIR OF SLIPPERY SOLED SHOES.INJURED HAD GOTTEN UP FROM HER DESK TO DISTRIBUTE THE MORNING MAIL AND SLIPPED LOST HER FOO TING AND FELL FRACTURING HER UPPER LEFT ARM.

EMPL WALKING BY BAGGING MACHINE AND STRUCK LEFT KNEE ON IT CAUSING CONTUSION.

ON 8-25-89, THE EMPLOYEE WAS LIFTING 50 LB. CLAY BAGS FROM A CONVEYOR BELT AND STACKING THEM IN A RAILCAR, WHEN HE EXPERIENCED A PAIN IN HIS LOWER BACK. ON 12-2-89 HE WENT TO A DOCTOR AND IT WAS DETERMINED THAT HE SHOULD NOT RETURN TO WORK FOR SEVERAL WEEKS.

EMPLOYEE WAS CLEANING SAND FROM RAIL SLIDING TRACKS WITH THE BUCKET ON A FRONT END LOADER. THE LOADER WAS GOING IN A FOREWARD DIRECTION WITH THE BUCKET ON THE RAILS. THE EMPLOYEE WAS LEANING FORWARD WATCHING THE TRACKS ND THE BUCKET CAUGHT ON THE RAILS,THE LOADER STOPPED, SUDDENLY AND THE EMPLOYEE WAS THROWN FORWARD INTO THE WINDSHIELD.

EMPLOYEE WAS OPENING THE UPPER DOORS ON A COVERED HOPPER PREPARING TO LOAD THE CAR WITH SAND AT #1 SAND PLANT HE WAS APPARENTLY USING IMPROPER LIFTING TECHNIQUES AND STRAINED HIS BACK WHILE L IFTING ONE OF THE DOORS

TRASH CLEANED FROM SUCTION WAS LAYING ON DECK EMPLOYEE STEPPED OVER PIPE AND STEPPED ON THE EDGE OF A 4" X 8" BOARD AND SEVERLY SPRAINED HIS ANKLE EMPLOYEE TRIED TO CONVINCE DR HE COULD RETURN TO WORK BUT DR WOULD NOT AGREE
