The associate was moving punch plates off the pallet to take in the screenhouse. The associate was sliding one of the plates off the pallet to put a clevis on it so they would be able to hoist it into the screenhouse. While sliding it, thumb got caught in the holes and almost severed the tip of the thumb.

WHILE CLIMBING DOWN A 980 YARD LOADER, ASSOCIATE SLIPPED ON OR MISSED THE LAST STEP. THIS CAUSED EE TO TWIST KNEE WHEN EE LANDED.

EE was filling the pug mill feeders and moving material away from the stacker. He noticed a customer truck on the yard. While attempting to leave the base pile he ran over a wind-row of material. This jarred the loader and transferred to his back, causing some lower back pain.
After checking screens, began to step down on what appeared to be snow, yet discovered there was ice under snow. As he slipped, one leg went out one way, while the other bent awkwardly underneath him.
Employee was using a hammer to remove a bolt that had been cut from a loader bucket. While hitting the bolt with an upward motion he sustained a sharp pain in his right wrist, resulting in a sprain. Associate waited to report the injury because he thought it would stop hurting.

Associate was working on bin 3 crusher with use of a contract crane service. When the crane began lifting the box one side of the box became wedged on its seating surface. One associate tried to apply pressure to one end of the box using downward pressure by pulling on the box by hand. The result was the box shifted approximately 3 inch injuring the his right hand.
