WHILE CLIMBING DOWN A 980 YARD LOADER, ASSOCIATE SLIPPED ON OR MISSED THE LAST STEP. THIS CAUSED EE TO TWIST KNEE WHEN EE LANDED.

EE was filling the pug mill feeders and moving material away from the stacker. He noticed a customer truck on the yard. While attempting to leave the base pile he ran over a wind-row of material. This jarred the loader and transferred to his back, causing some lower back pain.

After checking screens, began to step down on what appeared to be snow, yet discovered there was ice under snow. As he slipped, one leg went out one way, while the other bent awkwardly underneath him.

Employee was using a hammer to remove a bolt that had been cut from a loader bucket. While hitting the bolt with an upward motion he sustained a sharp pain in his right wrist, resulting in a sprain. Associate waited to report the injury because he thought it would stop hurting.

Changing lagging on C-3 drive pulley, limited space and hot slag that fell into his glove from using oxygen/acetylene torch.

The body of a trespasser was found in the South Pit. The Fairfax County Police located the body. The victim was approximately 100 feet below the rim of the pit.

Associate was working on bin 3 crusher with use of a contract crane service. When the crane began lifting the box one side of the box became wedged on its seating surface. One associate tried to apply pressure to one end of the box using downward pressure by pulling on the box by hand. The result was the box shifted approximately 3 inch injuring the his right hand.

Associate was clearing snow from in front of door of loader, was kicking snow side to side with foot and foot caught on grip strut flooring and caused injury

Assoc. cutting bolts loose on deck plate at gyratory crusher. When cut the 1st bolt the deck plate sprang up and hit his right ankle throwing him into the dead pan of the crusher.

Associate was cleaning chunky material buildup from around cement silo. Associate completed cleanup, left to return to lab, stepped on piece of material turning ankle

Associate changing tooth on 988G loader bucket, was having trouble getting tooth on so hit it with hammer, piece of metal broke off and lodged in associates abdomen.

WHILE MOVING AN OLD IDLER THE EMPLOYEE'S FINGER WAS MASHED BETWEEN IDLER ROLLER AND IDLER FRAME.

EMPLOYEE WAS WORKING IN THE SHOP, HE WAS HOISTING THE SHORT & PUMP UP BY THE JOB CRAINE. HE FAILED TO NOTICE THAT THE HOOK ON THE CHAIN HAD GRABBED EXISTING RAIL ON LAODER. HIS FINGER WAS CA UGHT BETWEEN THE HANDRAIL & THE CHAIN HOOK.

EMPLOYEE STEPPED DOWN ONTO A CONCRETE SURFACE THAT HAD MUD ON IT. HE LOST HIS FOOTING IN THE MUD AND SLIPPED AND FELL CATCHING HIMSELF WITH HIS LEFT HAND.

EE WAS IN A HOPPER WELDING.

IN BUNDLING 8" DIA. STEEL PIPE, ONE PIPE SLIPPED FROM FORKS ON THE LOADER, STRIKING AND LANDING ON EE S RIGHT LEG AND FOOT, RESULTING IN A CLOSED FRACTURE OF THE TIBIA/FIBULA AND SMALL BONES I N FOOT

MECHANIC WAS TAPPING A HOLE IN THE BELLY PAN. THE TAP BROKE AND HIS LEFT FOREARM SLIPPED ACROSS THE TOP OF THE BROKEN TAP, LACERATING THE SHITE. LACERATION STITCHES CLOSED.

WHILE WELDING AT THE CRUSHER, THE INJURED PERSON TRIED TO GET INTO A BETTER POSITION. HIS SAFETY BELT RESTRICTED HIS MOVEMENT AND SO HE TWISTED HIS BODY TO GET A BETTER POSITION, THIS ACTION P UT A STRAIN ON HIS BACK. ON 06-11-90 HE REPORTED TO HIS SUPERVISOR THAT HIS BACK HURT AS A RESULT OF 06-08-90 STRAIN.

EMPLOYEE WAS GRINDING PAINT FROM SEATING AREA OF CRUSHER MANTLE TO BE REBUILT WHEN GRINDER KICKED BACK CAUSING LACERATION ON LEFT LEG JUST ABOVE KNEE

OPERATOR WAS SERVICING LOADER WENT TO STOP UP ON MADINE AND PULLED LOWER BACK.

EMPL WAS HELPING TO STRIP CONVEYOR BY USING A SLEDGE HAMMER AND WEDGE TO REMOVE THEM HE HIT A WEDGE AND A PIECE OF METAL SPLINTERED OFF AND HIT HIS L HAND CUTTING 2ND KNUCKLE

EMPLOYEE WAS PULLING CONVEYOR FROM CONVEYOR WITH WALDON LOADER STOPPED TO UNHOOK CHAIN FROM BELT AND MANUALLY PULLED ON BELT TO GET SLACK IN CHAIN FELT PAIN IN RT SIDE ONE FRACTURED RIB

TWO EM WERE DRAGGING CRATED OF MACHINE PARTS ON DECL INJ PULLED BACK MUSCLES

EMPLOYEE WAS H9LPING TO ADHUST 5 1/2 STD WHEN A PIECE OF GRIT GOT INTO HIS EYE AND SCRATCHED THE EYEBALL
